Subarachnoid butorphanol augments lidocaine sensory anaesthesia in calves.

To determine the efficacy and safety of subarachnoid butorphanol combined with lidocaine, six calves were studied. Each calf underwent two treatments, at least one week apart, via subarachnoid injection: (1) butorphanol (0.03 mg/kg) plus 2% lidocaine (4 mg/kg) and (2) 2% lidocaine (4 mg/kg) alone. Subarachnoid injections were performed at the lumbosacral space. Analgesia, motor block, sedation, heart rate, arterial blood pressure, respiratory rate, arterial oxygen saturation as measured by pulse oximetry, and rectal temperature were compared before and after subarachnoid administration of drug

Morbidity and cost-effectiveness analysis of outpatient analgesia versus general anaesthesia for testicular sperm extraction in men with azoospermia due to defects in spermatogenesis.

The outcome and costs of testicular sperm extraction under outpatient local analgesia or general anaesthesia were compared in men with non-obstructive azoospermia. Nineteen consecutive patients were allocated to receive general anaesthesia, while the subsequent 21 consecutive patients received outpatient analgesia in the form of i.v. midazolam sedation, lignocaine spray, scrotal infiltration with local anaesthetic and spermatic cord block. Blood pressure, pulse rate and respiratory rate were determined. Sedation and testicular pain were assessed by subjective scoring. Both groups showed haemod

Extracorporeal membrane oxygenation in patients with congenital diaphragmatic hernia.

Since the introduction of extracorporeal membrane oxygenation (ECMO) support for neonatal respiratory failure, the use of ECMO for infants with congenital diaphragmatic hernia has increased significantly. ECMO is offered to infants with a high risk of dying (with reported survival rates of 38% to 65%). Unstable infants can be placed on ECMO with subsequent repair on ECMO or after weaning from support. The complication rate can be high with repair on ECMO, but changes in operative techniques have decreased the risk. Most centers use venoarterial ECMO in patients with congenital diaphragmatic he

Effects of velar resistance on speech aerodynamics.

The level of intelligibility attained by speakers with cleft palate reflects not only the ability to achieve adequate velopharyngeal closure but other complex variables as well. When closure is inadequate, performance is influenced by compensatory responses of the tongue, vocal folds, respiratory muscles and nasal valve. The purpose of the present study was to determine how a loss of velar resistance associated with velopharyngeal inadequacy affects speech pressures and airflow. The pressure-flow technique (Warren, 1979) was used to assess mean airflow rate, mean intraoral and nasal pressures

Prospective clinical comparison of two methods for mechanical ventilation of neonates: rapid rate and short inspiratory time versus slow rate and long inspiratory time.

A prospective comparison was made of the clinical courses of two groups of neonates ventilated according to different protocols: one group at rates of 20 to 40/minute with a one-second IT, and the other at a rate of 60/minute and 0.5 second IT. Other ventilator settings were adjusted within protocol limits to maintain desired blood gas values. Mean starting and highest PIP were lower in the rapid rate group. The results showed no difference in mortality, failure to remain within protocol limits, time requiring respiratory treatment or FiO2 more than 0.6, and incidence of PDA or chronic lung di

Some pharmacological properties of althesin (CT1341) in man.

Althesin in dose ranges of 40, 60 and 80 mul/Kg was administered to six healthy unpremedicated volunteers according to a balanced incomplete block design. Both subjective and objective changes resulting from the administration of the agent were studied and analyzed. In the dose ranges used, Althesin was found to have no deleterious effects on heart rate, respiratory rate or blood pressure and apnoea only occurred in one patient who had an end-expiratory CO2 level lower than normal, due to preanaesthetic hyperventilation. The onset of sleep is prolonged with Althesin at the injection rate of 1

Prediction of risk factors for respiratory failure in children with mycoplasma pneumoniae pneumonia based on clinical pulmonary infection scores.

BACKGROUND: Mycoplasma pneumoniae pneumonia (MPP) remains a leading cause of community-acquired pneumonia in children, with respiratory failure occurring in 15%-20% of hospitalized cases. Despite advances in understanding MPP pathogenesis, validated tools for early prediction of respiratory failure are lacking. This study aimed to develop and validate a nomogram based on Clinical Pulmonary Infection Score (CPIS) for individualized risk stratification in pediatric MPP. METHODS: We conducted a retrospective cohort study of 305 children with confirmed MPP admitted to our hospital between July 20

Effective Use of the Thoracolumbar Interfascial Plane Block With Total IV Anesthesia for Laminoplasty in a Patient With Myasthenia Gravis: A Case Report.

Myasthenia gravis (MG) is an autoimmune disorder of the neuromuscular junction that causes fluctuating muscle weakness and fatigue. The development of myasthenic crisis and respiratory compromise presents significant challenges in the perioperative management of these patients. Surgical pain-induced stress is a substantial risk factor for myasthenic crisis. A 68-year-old man with MG underwent L3-L5 lumbar laminoplasty under total IV anesthesia. A bilateral ultrasound-guided thoracolumbar interfascial plane block (TLIPB) was performed at the L4 level following anesthesia induction. Intraoperati

Establishment of a Cynomolgus Macaque Model for Human Adenovirus Type 55-Induced Respiratory Disease.

BACKGROUND: Human adenovirus type 55 (HAdV-55) is an emerging respiratory pathogen associated with severe pneumonia outbreaks, particularly in military populations and community settings. The lack of a suitable nonhuman primate model has limited the study of viral pathogenesis and the evaluation of vaccines and therapeutics. METHODS: Cynomolgus macaques were first administered phosphate-buffered saline intranasally and intratracheally as a negative control and allowed to recover for 20 days. The same animals were subsequently infected intranasally and intratracheally with HAdV-55. Clinical si

Impact of AKI on metabolic compensation for respiratory acidosis in ICU patients with AECOPD.

PURPOSE: Acute exacerbation of chronic obstructive pulmonary disease (AECOPD) can result in severe respiratory acidosis. Metabolic compensation is primarily achieved by renal retention of bicarbonate. The extent to which acute kidney injury (AKI) impairs the kidney's capacity to compensate for respiratory acidosis remains unclear. MATERIALS AND METHODS: This retrospective analysis covers clinical data between January 2009 and December 2021 for 498 ICU patients with AECOPD and need for respiratory support. RESULTS: 278 patients (55.8%) presented with or developed AKI. Patients with AKI exhibi

mRNA-based vaccines - global approach, challenges, and could be a promising wayout for future pandemics.

mRNA-based vaccines are assured to significantly boost biopharmaceuticals since outbreak of coronavirus disease- 2019. Respiratory infections, such as influenza, SARS, MERS, COVID-19, and respiratory syncytial virus, often have high transmission rates due to their airborne spread. Respiratory infections can lead to severe illness and death. These outbreaks can cause substantial economic and social disruption, as seen with the COVID-19 pandemic. In our interconnected world, respiratory diseases can spread rapidly across borders. mRNA-based vaccines (e.g. mRNA-1283) can reduce the transmission b

[Clinical research progress in non-invasive positive pressure ventilation from 2020 to 2021].

Non-invasive positive pressure ventilation (NPPV), an essential respiratory support method, is widely used in acute/chronic respiratory failure and assisting rehabilitation in patients with chronic obstructive pulmonary disease (COPD). We searched the relevant research articles about NPPV published from 1st October 2020 to 30th September 2021 through Medline. Researches focusing on the clinical application and viral transmission protection during high-flow nasal cannula oxygen and NPPV in COVID-19, were mainly retrospective and of small sample size. It demonstrated that high-flow nasal cannula
