Study on chitosan/carrageenan core-shell nanoparticles for oral co-administration of insulin/exenatide enhance insulin secretion in type 2 diabetes.

This study developed and evaluated the feasibility of using core-shell chitosan and carrageenan nanoparticles for insulin and exenatide dual-delivery to control blood glucose levels in type 2 diabetes model. Core-shell nanoparticles were fabricated using the coaxial electrospraying technique, exploiting the electrostatic interactions between components to achieve controlled drug release in physicological media. Experimental results indicated that the prepared nanoparticles demonstrated proper physicochemical properties of an oral delivery system, including a hydrodynamic diameter around 300 nm

Glucagon-Like Peptide-1 Receptor Agonists in Cocaine Use Disorder: Clinical Observations and Underlying Mechanisms.

BACKGROUND: Cocaine use disorder (CUD) remains a major public health concern, characterized by high relapse rates and the absence of approved pharmacological treatments. Glucagon-like peptide-1 receptor agonists (GLP-1RA), widely prescribed for metabolic disorders, modulate mesolimbic reward pathways and have been proposed as potential modulators of addictive behaviors. OBJECTIVES: To describe clinical observations of GLP-1RA exposure in individuals with CUD identified through the French national vigilance system and to contextualize these findings through a structured narrative review of pre

Calcium-permeable AMPA receptors contribute to the accelerated development of enhanced motivation for cocaine in female rats.

RATIONALE: The telescoping effect, where females progress more rapidly than males from initial drug use to developing a substance use disorder, is a widely cited sex/gender difference, yet its molecular basis remains unknown. OBJECTIVES: To test whether calcium-permeable AMPA receptors (CP-AMPARs) in the nucleus accumbens (NAc) contribute to the development of enhanced motivation for cocaine, a defining feature of addiction, which emerges earlier in females than in males. METHODS: Motivation for cocaine was measured under a progressive-ratio schedule before and after extended-access self-adm

Can the incentive-sensitization theory of addiction incorporate addiction to opioid drugs?

The Incentive Sensitization Theory (IST) of addiction posits that repeated intermittent exposure to potentially addictive drugs can sensitize brain mesolimbic dopamine systems. Those systems normally attribute incentive salience to rewards and their cues, but when sensitized may produce compulsive cue-triggered 'wanting' for drugs that can persist long after the discontinuation of drug use and the cessation of withdrawal symptoms, thus contributing to an enduring propensity to relapse. Much of the original evidence for IST came from studies on psychostimulant drugs, such as amphetamine and coc

Use of rhythm control therapy for atrial fibrillation among veterans in the Southeastern United States by rurality.

BACKGROUND: Atrial fibrillation (AF) affects up to 6 million adults, approximately 1 million of whom receive care through the Veterans Health Administration (VA). Considering advanced rhythm control interventions such as catheter ablation are often limited to higher-resourced urban care settings, disparities may disproportionately affect the VA patient population given the 4.6 million veterans who live in rural communities. OBJECTIVE: This study aimed to assess differences in the use of or time to rhythm control therapy based on rurality. METHODS: This was a retrospective study of 21,600 vet

The nonhallucinogenic ketamine metabolite (2R,6R)-hydroxynorketamine is a novel analgesic in animal models of pain.

Current treatment options for acute and chronic pain provide limited efficacy and safety. There is an urgent need to develop drugs with new, non-opioid treatment strategies that produce fewer adverse consequences. Preclinical evidence across multiple models of acute and chronic pain demonstrate that (2R,6R)-Hydroxynorketamine [(2R,6R)-HNK], a nonhallucinogenic metabolite of ketamine, promotes potent and long-lasting analgesic effects. This review summarizes the growing evidence for the analgesic action of (2R,6R)-HNK in rodent models of acute and chronic pain. (2R,6R)-HNK produces antinocicept

Adjunctive D-cycloserine to intermittent theta-burst transcranial magnetic stimulation in fibromyalgia: a randomized placebo-controlled trial.

Fibromyalgia is a chronic condition with a substantial unmet treatment need. Repetitive Transcranial Magnetic Stimulation (rTMS) has shown promise in decreasing fibromyalgia symptom severity. Emerging data suggest that adjunctive administration of the N-methyl-D-aspartate receptor partial agonist, D-Cycloserine, with rTMS can significantly improve clinical outcomes for other rTMS indications. Accordingly, this trial investigated adjunctive D-Cycloserine to intermittent Theta-Burst Stimulation (iTBS) of the dorsolateral prefrontal cortex (DLPFC) in adults with fibromyalgia. All participants rec

Rumination Out Loud? Linguistic, Neural, and Psychophysiological Correlates of the Think-Aloud Paradigm.

Stress and rumination are closely linked and contribute to the development and maintenance of mental disorders, yet assessing rumination in an ecologically valid way remains challenging. Conducting the think aloud paradigm (TAP) following the Trier Social Stress Test (TSST) allows for the real-time evaluation of ruminative responses, providing insights beyond traditional self-report measures. This study aimed to investigate ruminative responses to the TSST using the TAP, while simultaneously assessing psychological (stress, affect, state rumination), physiological (heart rate), and neural data

Effects of intermittent theta-burst stimulation (iTBS) on post-stroke depression (PSD): A study protocol for a single-center randomized controlled trial.

INTRODUCTION: Intermittent theta-burst stimulation (iTBS) is a specific type of repetitive transcranial magnetic stimulation (rTMS) paradigm with long-lasting effects and a short session duration. Although randomized controlled trials have suggested that iTBS is effective for treating depression, few studies have investigated the effects of iTBS in treating post-stroke depression (PSD). The purpose of the present study is to investigate whether iTBS can be used to treat PSD. We will also investigate the corresponding mechanisms of iTBS via functional magnetic resonance imaging (fMRI). METHODS

Adeno-associated virus gene therapy-mediated CCR5 blockade suppresses virus replication long term in SHIV-infected macaques.

Adeno-associated virus (AAV) vectors are promising vehicles for the delivery and long-term expression of antibodies for treatment of chronic diseases. Given the strong link between the C-C chemokine receptor 5 (CCR5)-Δ32/Δ32 genotype and protection from HIV, we explored the ability of AAV vectors expressing the CCR5-blocking antibody leronlimab to mediate a functional cure in simian-human immunodeficiency virus (SHIV)-infected rhesus macaques by interrupting viral access to the viral entry co-receptor CCR5. Delivery of AAV-leronlimab to rhesus macaques elicited antidrug antibodies (ADA) and ra

Combination of adenovirus vector and subunit protein elicits a robust immune response against the SARS-CoV-2 Omicron variant.

The rapid and ongoing emergence of SARS-CoV-2 variants has progressively reduced the protection offered by current vaccines, underscoring the urgent need for novel vaccine strategies. In this study, we designed a two-component vaccine in which one component is an adenoviral vector encoding the Omicron XBB.1.5 spike protein (Ad5XBB.1.5), and the other component is a self-assembling trimeric receptor-binding domain protein (RBDXBB.1.5-HR or RBDJN.1-HR). This platform integrates the complementary advantages of subunit protein and adenoviral vector vaccines, elicits robust humoral and cellular imm

Impact of cannabis use on presentation and treatment response in eosinophilic esophagitis.

Cannabis use is becoming increasingly common, both for recreational and medical purposes. However, there is a paucity of data regarding cannabis use in the context of eosinophilic esophagitis (EoE). We aimed to determine the impact of cannabis use on presentation and treatment response in EoE. To this end, we conducted a retrospective cohort study at a large academic medical center of newly diagnosed EoE patients age ≥ 12 years. Self-reported cannabis use status, baseline characteristics, and treatment response to topical corticosteroids and dietary therapy data were extracted. Bivariate and m

The Epidemiology of Recreational Use and Availability of DOC and DOI in the United States.

The U.S. Drug Enforcement Administration (DEA) proposed a rule in which they intend to place the psychedelic phenethylamines 2,5-dimethoxy-4-chloroamphetamine (DOC) and 2,5-dimethoxy-4-iodoamphetamine (DOI) into Schedule I of the Controlled Substances Act. We investigated the epidemiology of use and availability of these substances. We investigated national trends in seizures of these compounds (which indicate availability) using DEA National Forensic Laboratory Information System (NFLIS) and High Intensity Drug Trafficking Areas (HIDTA) data. Acute Effects of Cannabis on Alcohol Craving and Consumption: A Randomized Controlled Crossover Trial.

OBJECTIVE: Cannabis use is strongly linked with heavy drinking and worse alcohol treatment outcomes; however, it may also contribute to decreased alcohol consumption. To date, no human studies have established a causal effect of cannabis on alcohol motivation. The aim of this double-blind crossover randomized clinical trial was to examine dose-dependent acute effects of delta-9-tetrahydrocannabinol (THC) on alcohol craving and consumption. METHODS: Across three experimental days, 157 participants reporting heavy alcohol use and cannabis use two or more times weekly were randomized to smoke ca
