Reduced white matter integrity in borderline personality disorder: A diffusion tensor imaging study.

BACKGROUND: Borderline personality disorder (BPD) has a pervasive pattern of instability in interpersonal relationships, self-image, and emotions. BPD may be linked to an abnormal brain anatomy, but little is known about possible impairments of the white matter microstructure in BPD or their relationship with impulsivity or risky behaviors. The aims of the present study were to explore the relationship between BPD and diffusion tensor imaging (DTI) parameters and psychological tests. METHODS: We evaluated 35 un-medicated BPD patients in a medication-free state and 50 healthy controls (HCs). W

Comparison of methadone and buprenorphine for opiate detoxification (LEEDS trial): a randomised controlled trial.

BACKGROUND: Many opiate users require prescribed medication to help them achieve abstinence, commonly taking the form of a detoxification regime. In UK prisons, drug users are nearly universally treated for their opiate use by primary care clinicians, and once released access GP services where 40% of practices now treat drug users. There is a paucity of evidence evaluating methadone and buprenorphine (the two most commonly prescribed agents in the UK) for opiate detoxification. AIM: To evaluate whether buprenorphine or methadone help to achieve drug abstinence at completion of a reducing regi

Sexual dysfunction in an Internet sample of U.S. men who have sex with men.

INTRODUCTION: Relatively little is known about sexual dysfunction (SD) in men who have sex with men (MSM). AIM: In order to better understand SD symptoms in MSM, we assessed self-reported SD symptoms, individually and by latent class analysis (LCA). METHODS: In 2004-2005 an Internet sample of U.S. MSM was recruited from gay-oriented sexual networking, chat and news websites. The analytic sample comprised 7,001 men aged 18 or older who reported lifetime male sex partners and oral or anal sex with a male partner in their most recent encounter within the past year. MAIN OUTCOME MEASURES: Seven

Dependency in older people recently admitted to care homes.

OBJECTIVE: to investigate dependency and general health status of a cohort of older people admitted to residential or nursing homes for long-term care. METHOD: we assessed 308 people aged over 65 years within 2 weeks of admission for long-term care to one of 30 nursing or residential homes in north-west England. Dependency was assessed using the Barthel activities of daily living index and the Crichton Royal Behaviour Rating Scale. We collected information from the homes' records on diagnosed conditions and current medication. RESULTS: 50% of the cohort were in a 'low dependency' band (Barth

Olanzapine for chronic, stereotypic self-injurious behaviour: a pilot study in seven adults with intellectual disability.

Dopamine one (D1) receptor supersensitvity in the corpus striatum is said to be the primary mechanism within the dopamine model proposed for chronic, refractory self-injurious behaviour (SIB), which may explain why conventional neuroleptics have proven largely ineffective. In common with other atypical antipsychotic agents, olanzapine has more affinity for the D1 receptor. The present study explored whether olanzapine could reduce rates of the stereotypic form of chronic SIB, a subtype where dopamine dysfunction is the most likely underlying mechanism. A clinical sample of seven patients with

Evaluation of EEG cyclic alternating pattern during sleep in insomniacs and controls under placebo and acute treatment with zolpidem.

In nonrapid eye movement (NREM) sleep, electroencephalographic cyclic alternating patterns (CAPS) express the organized complexity of arousal-related phasic events. As a translation of sustained instability of the arousal level, CAPS increase under perturbation and decrease under sleep-promoting conditions. After adaptation to the sleep lab, 18 subjects (12 with persistent psychophysiological insomnia and 6 without sleep complaints), all aged 40-60 years, underwent a random sequence of two nonconsecutive nocturnal recordings, one under placebo and one under an imidazopyridine hypnotic agent (z

Risk of mortality after emergency department presentation for self-harm among children and adolescents in Wales: a population-based cohort study.

BACKGROUND: Self-harm in childhood and adolescence is associated with substantial morbidity and premature mortality, particularly from suicide. We aimed to quantify the risk of cause-specific and all-cause mortality following emergency department (ED) presentations for self-harm. METHODS: In this retrospective population-based cohort study, we used anonymised, linked electronic health records from the Secure Anonymised Information Linkage (SAIL) Databank. We included children and adolescents aged 5-18 years who presented to EDs in Wales between 2008 and 2023. Individuals with at least one ED

Sexual Function Is Correlated With Body Image and Partnership Quality in Female University Students.

INTRODUCTION: According to the World Health Organization definition, sexual health is more than mere physical sexual function; it also encompasses emotional, mental, and social well-being in relation to sexuality and is not merely the absence of dysfunction or disease. In line with this definition, various studies have reported that female sexual function is associated with partnership quality, body image, and body self-acceptance. AIM: To investigate whether female sexual function is influenced by (i) body self-acceptance and (ii) partnership quality, as important factors in psychosocial wel

Feasibility of Implementing a Clinical Integration Package to Support Self-Measured Blood Pressure Monitoring in Primary Care: A Qualitative Study.

BACKGROUND: Despite strong evidence, self-measured blood pressure (SMBP) remains underutilized in under-resourced primary care settings. We evaluated barriers and facilitators to implementing a multicomponent SMBP delivery package in a diverse community clinic. METHODS: We examined barriers and facilitators to implementing the ASPIRE (Adapting Self-Measured Blood Pressure Monitoring among Underserved Communities) package, designed to support SMBP adoption in primary care. After an ASPIRE pilot conducted at a Chicago South Side clinic serving a racially diverse population with high social need

Resident-side health information for identifying community noise risks in digital public health governance.

INTRODUCTION: Digital public health governance needs to identify community-level health risks beyond data generated after residents enter medical-service settings. In this study, resident-side health information refers to resident-reported information on residential noise perception, sleep disturbance, and health behavior that can supplement medical-service data. Noise pollution is a frequently perceived environmental problem in everyday life, and China's source-based noise governance framework needs resident-side evidence on whether different noise sources correspond to different sleep-relate

Self-Reported Insomnia and Poor Sleep Quality Are Associated with Self-Reported Cognitive Changes in Older Adults.

Older adults are vulnerable to changes in sleep with age. Poor sleep quality is associated with self-reported cognitive changes, which can occur before the onset of objective cognitive decline associated with Mild Cognitive Impairment and Alzheimer's disease. The objective of this study was to examine associations between self-reported sleep complaints, objective sleep quality, and self-reported cognitive changes and their relations to symptoms of depression and anxiety in a group of community-dwelling older adults. Adults aged ≥ 50 without dementia (n = 45) were recruited and completed 1-2 we

Positive health during the COVID-19 pandemic: a survey among community-dwelling older individuals in the Netherlands.

BACKGROUND: Coronavirus Disease 2019 (COVID-19) reached the Netherlands in February 2020. To minimize the spread of the virus, the Dutch government announced an "intelligent lockdown". Older individuals were urged to socially isolate completely, because they are at risk of a severe disease course. Although isolation reduces the medical impact of the virus, the non-medical impact should also be considered. AIM: To investigate the impact of COVID-19 pandemic and associated restrictive measures on the six dimensions of Positive Health in community-dwelling older individuals living in the Netherl

Predictors of Imminent Risk of Nonvertebral Fracture in Older, High-Risk Women: The Framingham Osteoporosis Study.

Osteoporosis treatment decisions are often based solely on BMD or on 10-year fracture risk; little is known about factors increasing imminent fracture risk. Understanding factors contributing to imminent risk of fracture is potentially useful for personalizing therapy, especially among those at high risk. Our aim was to identify predictors of nonvertebral fracture for 1- and 2-year periods in women at high risk for fracture. The Framingham Osteoporosis Study cohort included 1470 women (contributing 2778 observations), aged ≥65 years with BMD hip T-score ≤ -1.0, or history of fragility fracture
