Oral surgery II: Part 2. The maxillary sinus (antrum) and oral surgery.

The maxillary sinus is the largest of the four paranasal sinuses and, being anatomically adjacent to the dentate region of the maxilla, is commonly a source of problems - not simply in terms of conditions affecting the sinus but also in establishing an accurate diagnosis. As anyone who has suffered both sinusitis and a dental abscess in the posterior maxilla will tell you, the symptoms are almost indistinguishable. For this reason, a sound understanding of the maxillary sinus is an essential requisite for all dentists.

Non-imaging techniques for the study of swallowing.

A number of non-imaging procedures have been used to study various aspects of swallow physiology including electromyography, electroglottography, manometry, the sounds of swallow, and the blue dye test. Each of these techniques is described according to the typical procedures used, the types of information produced and advantages and limitations when applied to the study of oropharyngeal swallow. For most accurate interpretation, the majority of these techniques are used concurrently with imaging procedures such as videofluorography.

Neonatal hyperekplexia: a case report.

We report the case of a baby with transient generalized stiffness noticeable from the first days of life, hyperreflexia, massive jerks in response to sudden tactile and acoustic stimuli, and long-lasting myoclonic jerks closely resembling epileptic seizures. The father and paternal grandfather both had hyperekplexia. At age 3 years, the child had normal psychomotor development and persistent abnormal startle response to unexpected sounds or touch.

A new medication to control patulous eustachian tube disorders.

The disorder of hyperpatent eustachian tube is often unrecognized or misdiagnosed because of the ambiguous symptom of a "plugged ear." But when the patient also complains of autophony and/or hearing amphoric sounds, a patulous tube should be suspected. Persistent symptoms are distressing to most patients. This preliminary report evaluates a new nasal medication that produces closure of the tube and controls or eliminates these symptoms. It is demonstrated to be safe, predictable, and effective with little or no side effects.

Fatal xylenol self-poisoning.

A case of fatal xylenol ingestion by a long-stay mental hospital patient is described. The clinical course was similar to that observed in other phenolic poisonings with active bowel sounds, nausea and vomiting, severe metabolic acidosis, hypotension and cardiac and renal failure. The formulation of xylenol ingested contains alcohol which would facilitate absorption; due to the dangers of such poisonings care must be exercised as to access and exposure to xylenol sterilising agents.

Pacemaker-induced endocardial friction rub.

Two patients had a typical sounding cardiac friction rub after placement of a temporary transvenous pacemaker. Absence of myocardial perforation was documented in one patient during thoracotomy for placement of an epicardial electrode and in the other with an electrogram recorded from the pacemaker as it was being withdrawn. The rubs disappeared during pacemaker removal. These cases are believed to represent endocardial friction rubs resulting from contact of the pacing wire with the endocardium.

Ear-specific selective speech adaptation.

Prolonged exposure to specific speech sounds can induce selective speech adaptation (SSA), which is driven by an early ear-specific/peripheral and a later binaural/central mechanism. We investigated the peripheral/ear-specific SSA component while minimizing involvement of the binaural/central mechanism. In Experiment 1, we exposed participants to monaural left-right alternating Dutch vowels /ø/ and /e/ via headphones (HP), followed by monoaural testing with sounds from an /ø/-/e/ continuum. This procedure induced substantial and long-lasting ear-specific/peripheral SSA. In Experiment 2, we pre

Intratracheal Leiomyosarcoma in a 13-Year-Old German Riding Pony Mare.

A 13-year-old German Riding Pony mare was referred for evaluation of progressive respiratory noise and mixed dyspnoea, which rapidly deteriorated to respiratory distress. Clinical examination revealed marked rattling inspiratory and expiratory tracheal sounds and severe, rattling, vesicular lung sounds. Arterial blood gas analysis demonstrated moderate hypoxaemia (partial pressure of arterial oxygen 76 mmHg at rest). Upper airway endoscopy identified a pink, cauliflower-like pedunculated mass originating from the dorsal tracheal wall approximately 15 cm cranial to the tracheal bifurcation, cau

Evaluation of the ultrasonography-guidance for botulinum toxin type a injection into lateral pterygoid muscle in patients with TMJ anterior disc displacement with reduction.

Anterior disc displacement with reduction (ADDwR) is a common temporomandibular joint disorder associated with pain, joint sounds, and limited jaw movement. Conservative treatments are considered first-line but often yield unsatisfactory results. Botulinum toxin type A (BTX-A) injection into the lateral pterygoid muscle (LPM) offers a minimally invasive alternative, and ultrasonography enhances safety and accuracy through real-time, radiation-free guidance. This single-arm clinical trial included ten patients (20 TMJs), aged 19-38 years, diagnosed with ADDwR. Each patient received 25 units of

External validation of cough-based algorithms for pulmonary tuberculosis screening from the CODA TB DREAM challenge using cough data from Peru.

The COugh Diagnostic Algorithm for Tuberculosis (CODA TB) DREAM Challenge recently evaluated the performance of artificial intelligence (AI) algorithms for tuberculosis (TB) screening using cough sounds. Eleven AI models were developed using a dataset of 733,756 cough sounds collected from 2143 adults from seven countries. This study evaluates the CODA Challenge AI models with an external independent cough dataset from Peru. Cough recordings from 303 coughing adults were collected from health facilities in Lima, Peru. The AUCs of the models ranged from 0.480 to 0.615, showing a decrease in per

A study on the perception, learning experiences, and preferences in music among hard-of hearing adults.

The purpose of this study was to investigate the music perception, learning experiences, and preferences of adults with hard-of-hearing and to examine how these factors vary depending on the type of rehabilitative assistive devices they use. A total of 107 adults with hard-of-hearing participated in a questionnaire consisting of 25 items related to music perception, learning experiences, and preferences with desires for learning music. The participants demonstrated a relatively high ability to perceive and identify music sounds, although they experienced difficulties in fully appreciating musi

Cortical tracking of hierarchical rhythms orchestrates the multisensory processing of biological motion.

When observing others' behaviors, we continuously integrate their movements with the corresponding sounds to enhance perception and develop adaptive responses. However, how the human brain integrates these complex audiovisual cues based on their natural temporal correspondence remains unclear. Using electroencephalogram (EEG), we demonstrated that rhythmic cortical activity tracked the hierarchical rhythmic structures in audiovisually congruent human walking movements and footstep sounds. Remarkably, the cortical tracking effects exhibit distinct multisensory integration modes at two temporal

Neural mechanisms contributing to increased acoustic startle reactivity in Cntnap2 knock-out rats.

Rats with a loss-of-function mutation in the contactin-associated protein-like 2 (Cntnap2) gene exhibit increased acoustic startle response magnitudes, paralleling the acoustic hyperreactivity that is observed in CNTNAP2-associated disorders in humans, including autism spectrum disorder. Previous studies have found increased neural activity in the brainstem region that mediates the acoustic startle response, the caudal pontine reticular nucleus (PnC). This increased neural activity was predominantly caused by increased PnC firing rates in female Cntnap2 KO rats, whereas in male Cntnap2 KO rats

Electromyographic Assessment of the Extrinsic Laryngeal Muscles: Pilot and Descriptive Study of a Vocal Function Assessment Protocol.

AIM: The aim of this study was to develop and test a surface electromyography (sEMG) assessment protocol to characterise the activity of the extrinsic laryngeal muscles (suprahyoid and infrahyoid) during phonatory tasks and vocal techniques. METHODOLOGY: The protocol of assessment was based on electromyographic assessment guidelines and on clinical voice evaluation needs and was tested in six healthy adults with no vocal disorders. Surface electromyographic activity of suprahyoid and infrahyoid muscles was acquired during different reference tasks (rest, reading, maximum contractions) and six

Melodic and articulatory development is delayed in deaf infants aged 2-4 months.

There is a paucity of knowledge about the early vocal development of infants with profound sensorineural hearing loss. In a search for potential differences in cooing vocalisations between hearing impaired (n = 12) and healthy infants (n = 12) aged 2-4 months, more than 2300 sounds were quantitatively analysed. Using objective criteria, laryngeal activity was analysed. Vocalisation melodies (time function of fundamental frequency) were recorded and grouped into simple (single arc) or complex (multiple arc) melody pattern. Occurrence of supra-laryngeal (articulatory) activity was also analysed

The Effect of Speech Masking on the Human Subcortical Response to Continuous Speech.

Auditory masking-the interference of the encoding and processing of an acoustic stimulus imposed by one or more competing stimuli-is nearly omnipresent in daily life and presents a critical barrier to many listeners, including people with hearing loss, users of hearing aids and cochlear implants, and people with auditory processing disorders. The perceptual aspects of masking have been actively studied for several decades, and particular emphasis has been placed on masking of speech by other speech sounds. The neural effects of such masking, especially at the subcortical level, have been much

The effectiveness of brain wave synchronization in the theta band on depression and anxiety in opioid-dependent patients.

BACKGROUND: Addiction is one of the most common psychiatric disorders and is closely related to depression and anxiety disorders. The presence of these disorders makes the process of addiction treatment difficult. Binaural beats are the representation and perception of a stimulus that does not exist in the outside world but is the brain's mental perception error of a virtual third sound, that is, the slight frequency difference between the two sounds. METHODS: The present study was conducted to assess the effectiveness of brain synchronization using binaural beats in the theta band on depress

Longstanding Auditory Sensory and Semantic Differences in Preterm Born Children.

More than 10% of births are preterm, and the long-term consequences on sensory and semantic processing of non-linguistic information remain poorly understood. 17 very preterm-born children (born at < 33 weeks gestational age) and 15 full-term controls were tested at 10 years old with an auditory object recognition task, while 64-channel auditory evoked potentials (AEPs) were recorded. Sounds consisted of living (animal and human vocalizations) and manmade objects (e.g. household objects, instruments, and tools). Despite similar recognition behavior, AEPs strikingly differed between full-term a
