Anterior minimally invasive extrapleural retroperitoneal approach to the thoraco-lumbar junction of the spine.

BACKGROUND: The anterior approach to the thoraco-lumbar junction of the spine allows therapeutic interventions on post-traumatic, infectious, and neoplastic vertebral lesions from T11 to L2 combining spinal cord decompression, corporectomy, and vertebral body fusion. However, this approach also has a reputation for damaging the intervening anatomic structures (lungs, peritoneum, and diaphragm). The objective of this study was to show that both nervous structure decompression and anterior vertebral reconstruction can be achieved via an anterior minimally invasive extrapleural retroperitoneal (A

Acute posttraumatic spinal cord herniation. Case report and review of the literature.

Transdural herniations of the spinal cord are rare, and those occurring acutely after a spinal cord injury (SCI) are particularly unusual. In this report, the authors present the case of acute posttraumatic spinal cord herniation in a patient who sustained severe polytraumatic injuries. The clinical manifestations were acute flaccid paralysis of the right leg and rapidly progressive sensorimotor deficits of the contralateral leg. The herniation was surgically reduced. Postoperatively left leg paralysis was completely resolved. The authors review the pertinent literature, and suggest that, with

Management of Acute Subaxial Trauma and Spinal Cord Injury in Professional Collision Athletes.

Sports-related acute cervical trauma and spinal cord injury (SCI) represent a rare but devastating potential complication of collision sport injuries. Currently, there is debate on appropriate management protocols and return-to-play guidelines in professional collision athletes following cervical trauma. While cervical muscle strains and sprains are among the most common injuries sustained by collision athletes, the life-changing effects of severe neurological sequelae (ie, quadriplegia and paraplegia) from fractures and SCIs require increased attention and care. Appropriate on-field managemen

Seatbelt Syndrome in Children.

The seatbelt syndrome describes an injury pattern infrequently seen in restrained passengers in motor vehicle collisions. It occurs when sudden deceleration forces coupled with compression of the lap belt around the abdomen causes abdominal wall bruising, intra-abdominal injuries, and spinal fractures. Infrequent and improper use of appropriate belt restraints in children has led to high risks for injury in this population. We describe a case of the seatbelt syndrome with the uncommon finding of an associated posttraumatic intestinal obstruction. We also review the literature on the prevalence,

[Drug-induced osteonecrosis of the jaw - not just bisphosphonates].

Metastatic bone disease and osteoporosis have a large impact on quality of life and are associated with the development of skeletal-related events (SREs), such as fractures and spinal cord compression. Pharmacologic managing of metastatic bone disease and osteoporosis typically involves antiresorptive agents such as bisphosphonates and RANKL inhibitors. Undesired adverse effects resulting from the use of these drugs include osteonecrosis of the jaw (ONJ). Dentoalveolar surgery, particularly tooth extraction, appears to increase the risk of ONJ. However, spontaneous events were also documented.

Bone And Soft Tissue Changes In Patients With Spinal Cord Injury And Multiple Sclerosis.

In patients with spinal cord injury and multiple sclerosis, deterioration of body composition (changes in bone, fat and muscle mass) is associated with increased risk for diseases such as coronary artery heart disease, non-insulin dependent diabetes mellitus, lipid metabolism abnormalities, and osteoporotic fractures in these patients. Immobility leads to a changing pattern of loading in the paralyzed areas, and secondary alteration in structure. However, bone and soft tissue changes in these patients are usually neglected. The purpose of this article is to update on the pathophysiological mec

Spinal cord compression secondary to kyphosis associated with radiation therapy for metastatic disease.

Three patients were treated for pathologic fractures of the thoracolumbar spine causing progressive neurologic deficit. An anterior decompression with partial removal of the diseased vertebra was performed with improvement of neurologic function in each patient. Active tumor was not found, but necrotic bone with collapse and secondary kyphosis were the causes of anterior compression of the spinal cord. In two patients, the spine was stabilized with an anterior fibular strut graft that initially provided stability; later collapse progressed and deformity recurred. In the third patient, stabiliz

Accuracy and clinical outcomes of TiRobot-assisted pedicle screw fixation for thoracolumbar fractures: a meta-analysis.

Accurate pedicle screw placement is critical for achieving optimal stability and reducing procedure-related complications in thoracolumbar fracture surgery. Robotic navigation systems, such as TiRobot, have been increasingly adopted to improve surgical precision; however, their overall clinical value remains to be clarified. Following rigorous study selection, data extraction, and methodological quality assessment, quantitative synthesis was performed using RevMan 5.4. Primary outcomes covered screw placement accuracy and intraoperative parameters, while secondary outcomes included radiologica

Biomechanical optimization of iliolumbar fixation strategies for unilateral vertical sacral fractures: Prioritizing stability-mobility balance via finite element analysis.

OBJECTIVE: This study aims to optimize iliolumbar fixation strategies for unilateral vertical sacral fractures via finite element analysis, by comparing stability, implant stress, and lumbar mobility to identify the optimal clinical option. METHODS: A finite element model of the pelvis and L3-L5 lumbar spine was constructed to simulate four fixation models (L4L5IS, L5S1IS, L5IS, S1IS) under a 600 N vertical load. Sacral vertical displacement, implant stress, and fracture line separation were analyzed. RESULTS: Double-segment fixation (L4 + L5 + iliac screw and L5 + S1 + iliac screw) demonstr

Is There a Difference in Postoperative Outcomes Between Kyphoplasty and Vertebroplasty in the Management of Vertebral Compression Fractures?: A Meta-Analysis of Randomized Controlled Trials.

BACKGROUND: Cement augmentation using vertebroplasty (VP) or kyphoplasty (KP) can be employed to manage vertebral compression fractures (VCFs). Randomized controlled trials (RCTs) have disagreed about the superiority of one technique over the other. Therefore, a meta-analysis of RCTs is warranted. METHODS: PubMed, Cochrane, Embase, and Google Scholar were searched for articles from database inception to July 15, 2024. The inclusion criteria consisted of English and non-English-language RCTs comparing KP to VP in the management of VCFs. The studied outcomes were the risks of cement leakage and
