Magnetic resonance imaging of tendon and ligament abnormalities: Part I. Spine and upper extremities.

Magnetic resonance imaging (MRI) has proven to be an ideal technique for evaluating tendons and ligaments, which, because of their fibrous structure, have a low signal intensity on all sequences. Trauma and a wide variety of other conditions cause aberrations in the normal appearance of these structures, ranging from minor alterations in shape, size, appearance, and continuity to various changes in signal intensity within the tendon, tendon sheath, and ligament on different sequences. The purpose of this paper is to review MRI findings in patients with abnormalities of tendons and ligaments in

The thermoplastic Minerva body jacket: a clinical comparison with other cervical spine splinting techniques.

A retrospective analysis of the efficacy of a variety of external stabilization techniques used in 155 cases of unstable cervical spine injuries is presented. The movement at each intervertebral level was evaluated during thermoplastic Minerva body jacket stabilization in 18 additional patients. Many currently available approaches to external stabilization of the cervical spine were, thus, assessed. Thermoplastic Minerva body jacket stabilization offered superior segmental immobilization compared with published data for the halo. It is concluded that some unstable injuries to the high cervical

Reliability of interpretation of plain lumbar spine radiographs in benign, mechanical low-back pain.

A study was conducted to investigate the variability of interpretation of plain lumbar spine radiographs by rheumatologists in benign low-back pain (LBP). Intra- and interobserver agreement in classifying the presence of primary radiologic abnormalities according to pre-established criteria was assessed by the Kappa statistic in 115 anteroposterior and lateral radiograms. A significant variability of interpretation was observed for many findings often considered important in benign LBP. Particularly, low levels of agreement were observed for apophyseal joint abnormalities. Schmorl's nodes, spo

Mobility of the lower lumbar spine after posterolateral fusion determined by roentgen stereophotogrammetric analysis.

To determine the time-table for intervertebral stabilization after posterolateral fusion without osteosynthesis in the lower lumbar spine, 11 patients with no previous spinal surgery and with fusion due to spondylolysisolisthesis Grade 1 to 2 or lumbar disc disorder/facet joint arthrosis were examined by roentgen stereophotogrammetric analysis (RSA) in supine and erect positions, and by conventional radiography for 1 year after surgery. In eight patients with osseous fusion radiographically, the sagittal and the vertical translations between the fused vertebral segments began to decrease after

[Primary tumors of the spine. Initial oncologic aspects: epidemiology, anatomo-prognostic and therapeutic classification].

Primary tumors of the vertebral column are rare: 20 per cent of all primary tumors of the spinal column. The distribution by type of tumor shows that the three most frequent primary tumors considered to be "radio and/or chemo resistant lesions" are the chordoma (15.5 to 24.5%), the chondrosarcoma (20%) and the giant cell tumor (10%). A second group with "chemo and/or radiosensitive lesions" include the Ewing sarcoma, primary lymphoma and plasmocytoma (5%). We consider a third group with the benign tumor: osteochondroma, chondroma, osteoid osteoma, osteoblastoma, aneurysmal bone cyst, hemangiom

Lumbar disc degeneration: correlation with age, sex, and spine level in 600 autopsy specimens.

Using data from 16 published reports, the authors correlated macroscopic disc degeneration grades with age, sex, and spine level in 600 lumbar intervertebral discs from 273 cadavers (ages: 0-96 years). Male discs were more degenerated than female discs at most ages; significantly so in the second, fifth, sixth, and seventh decades. On average, L4-L5 and L3-L4 level discs showed more degeneration than discs at other lumbar levels. These macroscopic findings corroborate radiographic data from epidemiologic studies. The calculations suggest that higher mechanical stress, perhaps combined with lon

Mechanical behavior of the human lumbar spine. II. Fatigue strength during dynamic compressive loading.

Seventeen cadaveric human lumbar motion segments from eight spines were cyclically loaded in vitro under axial compression. Loading frequency and magnitude were chosen to simulate rigorous activity within an in vivo physiological level. The load magnitude was determined as a percentage of the ultimate compressive load, the latter estimated from the bone mineral content (BMC) of lumbar vertebrae determined by dual-photon absorptiometry. Following testing, the degree of macroscopic disc degeneration was assessed and the type of fracture in each specimen was determined from serial sagittal sectio

High resolution surface coil magnetic resonance imaging of the spine: normal and pathologic anatomy.

In a brief two year period, magnetic resonance imaging of the spine has developed from a single slice, low signal/noise ratio, extremely time consuming technique to a multiplanar, high resolution examination that can be performed in a time frame similar to that of CT imaging. MR has already assumed a primary imaging role in myelopathic states. It is premature to evaluate its place vis-a-vis CT and myelography in many other disease states of the spine, but clearly, MR is assuming an ever expanding role. As technical improvements continue, it is perhaps realistic to predict that magnetic resonan

[Axis deviation of lumbar spine due to abdominal expanding lesions in children (author's transl)].

The deviation of the physiologic lordosis of the lumbar spine is described in 40 cases of abdominal expanding lesions. One can find a lumbar kyphosis between L2 and L4 in retroperitoneal and a straightening between Th12 and L2 in intraperitoneal lesions. The evidence of a deviation of the lumbar axis is an indirect sign for further localisation of the tumor.

Traumatic single facet subluxation of cervical spine without neurological damage. A new clinical sign.

The vast majority of injuries to the cervical spine, unaccompanied by neurological deficit, escape diagnosis because of the minimal clinical symptoms associated with such injuries. For the same reason many of those who consult their doctors following such injuries, are not adequately investigated. This article presents a new clinical sign to recognise a certain type of cervical spine injury, and a simple way of documenting the observations. It is proposed that in the presence of this sign, adequate radiological investigations must be deemed mandatory to exclude bony injury to the cervical spin

Scheuermann's kyphosis. Results of surgical treatment by posterior spine arthrodesis in twenty-two patients.

Twenty-two patients with Scheuermann's kyphosis were treated with correction, Harrington instrumentation (in twenty-one), and posterior spine fusion. This procedure relieved pain and corrected deformity in all patients but loss of correction of 5 degrees or more occurred in sixteen of the twenty-two patients, including all those whose initial kyphosis was greater than 70 degrees. The incidence of complications was so high that this procedure should ordinarily be performed only for disabling pain or spinal cord compression.

Linking functional and structural dendritic spine remodeling during fear learning and extinction in vivo.

Structural plasticity of dendritic spines has been observed during different learning paradigms, but how the functional dynamics of dendritic spines changes with memory processing and how these patterns relate to structural plasticity and dendritic integration remain unclear. Here, we perform longitudinal functional and structural in vivo imaging of the frontal association cortex in mice subject to fear conditioning and extinction over several days. We show that fear learning induced responsive spines that are more likely to be synchronous and clustered, which are consolidated over the followi

Comparative Cost Analysis: Uniportal Endoscopy, Biportal Endoscopy, Microtubular, and Open Spine Surgery [Retrospective Review].

STUDY DESIGN: A retrospective review. OBJECTIVE: To perform a comparative cost analysis of unilateral biportal endoscopic surgery (UBE), uniportal endoscopic surgery, microtubular surgery, and open surgery in outpatient and inpatient settings. SUMMARY OF BACKGROUND DATA: Endoscopic spine surgery techniques have gained traction for their benefits. However, the relative costs of biportal and uniportal endoscopic approaches compared with traditional methods, such as microtubular and open spine surgery, remains a debated topic. An evaluation of cost implications is critical for health care provi

Stop guessing: A clinician's guide to demystifying spine-related arm pain in primary care.

Pain in the arm could arise from a wide range of conditions, from non-specific musculoskeletal disorders to serious spinal pathology. Spine-related arm pain (SAP), i.e., pain that is best explained by processes in the spine or spinal nerves, poses a particular diagnostic challenge in primary care due to its diverse overlapping symptoms and evolving presentations. This paper proposes a structured, literature-informed, consensus-based guide to assist clinicians in classifying SAP according to predominant pain mechanisms and stratifying the level of clinical concern. The guide takes the form of a

The Utility of Routine Postoperative Radiographs Following Surgical Treatment of Traumatic Cervical Spine Injuries.

Background/Objectives: Postoperative cervical spine radiographs are routinely obtained during in-hospital and follow-up period. We aim to evaluate the utility of postoperative radiographs for identifying instrumentation failure and the subsequent need for revision surgery in patients with traumatic cervical spine injuries. Materials and Methods: A retrospective chart review of patients who had surgical treatment for traumatic cervical spine injury was conducted. Clinical notes and radiographic reports were evaluated. Postoperative radiographs were obtained prior to discharge from the hospital,

Diagnostic Value of Flow-Void Sign for Localizing Ventral Dural Defects in Spine MRI.

BACKGROUND AND PURPOSE: Ultrafast dynamic CT myelography and digital subtraction myelography are used to localize dural defects in spontaneous intracranial hypotension; however, their invasiveness and technical demands limit widespread availability. The "flow-void sign" on T2-weighted spine MRI has been proposed as a potential non-invasive marker of the ventral dural defect, but its clinical value remains unclear. This study aimed to evaluate the utility of the flow-void sign for predicting dural defect sites in ventral cerebrospinal fluid leaks. METHODS: This retrospective cohort study, usin

Anterior Odontoid Screw Fixation: A Surgical Technique Guide By the Cervical Spine Research Society.

Odontoid fractures comprise 10%-15% of all cervical fractures in adults, of which Anderson and D'Alonzo type II odontoid fractures are the most common subtype. Anterior screw fixation for type IIB odontoid fractures is a safe and effective treatment option in the properly selected patient. This less invasive technique preserves the C1-C2 motion while stabilizing the dens and yields a high union rate with minimum morbidity in appropriately indicated patients. Younger patients and nonosteoporotic patients with anterior superior to posterior inferior (type IIB) fracture patterns are ideal candida
