Carbon fiber-reinforced PEEK implants in oncologic spine surgery: a multicenter experience on implications for postoperative patient management.

OBJECTIVE: Carbon fiber-reinforced polyetheretherketone (CFRP) implants have been used safely in treating spinal oncological disease and promise improved imaging follow-up and radiotherapy planning. However, data on the implant's routine use in the clinical setting with implications for postoperative management are scarce. The aim of this observational study was to provide real-world insight into the current use of CFRP instrumentations in spinal oncological disease and to define their actual relevance for postoperative imaging follow-up and adjuvant treatment planning. METHODS: Datasets of p

Maintenance of construct integrity with subaxial cervical pedicle screws when crossing the cervicothoracic junction in complex pediatric deformity: an international multicenter study.

OBJECTIVE: In cases of complex pediatric spinal deformity, posterior spinal instrumentation crossing the cervicothoracic junction (CTJ) may be required. This is most frequently encountered for revision surgery to address proximal junctional kyphosis (PJK). In the cervical spine, lateral mass screws are most commonly used, although they may result in biomechanically weak proximal constructs and predispose to instrumentation failure. The use of supplemental anterior constructs has recently been reported with promising results, but this necessitates an additional surgical procedure. Subaxial cerv

Patients' perception of recovery after third molar surgery.

PURPOSE: This study evaluated patients' perceptions of recovery after third molar surgery. METHODS: Two hundred forty-nine patients (age 13 to 37 years) at 2 clinical centers were enrolled in a prospective study before the surgical removal of third molars. Each patient was given a 21-item Health-Related Quality of Life instrument (HRQOL) to be completed each postoperative day (POD) for 14 days. The instrument was designed to assess patients' perception of recovery: pain, oral function, general activity measures, and other symptoms. Pain dimensions were recorded with a 7-point Likert-type scal

Operative Approach for Robotic Descending Colon Cancer Requiring Bidirectional Mobilization of the Splenic Flexure and Sigmoid-Descending Junction.

INTRODUCTION: Robotic Descending Colectomy Occasionally Requires Splenic Flexure (SF) and Sigmoid-Descending Junction (SDJ) Mobilization. In Multiport Robotic Surgery, This Bidirectional Mobilization Can Be Technically Demanding Because the Operative Field Extends in Two Directions, and Conventional Curved-Line or Stepped-Line Configurations May Limit Operative Envelope or Cause Arm Interference. MATERIALS AND SURGICAL TECHNIQUE: For lesions requiring continuous SF-SDJ access, an oblique-line configuration using the da Vinci Xi (DVXi) or the da Vinci SP (DVSP) system is typically required for

Delayed neurological deficit due to a medially misplaced thoracic pedicle screw during adolescent idiopathic scoliosis correction: a complication 6 years in the making.

PURPOSE: Neurological deficits developing years after pedicle screw misplacement is a rare phenomenon. Here, we report level IV evidence of a previously asymptomatic medial thoracic pedicle screw resulting in paraparesis after a motor vehicle accident. METHODS: A 21-year-old male presented with acute onset of paraparesis following a motor vehicle collision. Six years prior this incident, the patient underwent a thoracolumbar fusion T4-L4 for AIS performed by an outside orthopedic surgeon. CT scan and CT myelogram illustrated decreased spinal canal diameter and cord compression from a medial T

A NEW TECHNIQUE OF STERILE POLYETHYLENE INSTRUMENT COVER AS TEMPORARY CORNEAL PATCH TO RECONSTRUCT ANTERIOR CHAMBER IN PENETRATING KERATOPLASTY COMBINED WITH INTRAOCULAR SURGERY.

PURPOSE: Complications such as explosive choroidal hemorrhage, residual cortex, and capsule rupture often occur during intraocular surgery combined with penetrating keratoplasty (PKP) due to poor maintenance of the anterior chamber. To address these challenges, the authors have innovatively used a sterile polyethylene instrument cover to temporarily reconstruct the anterior chamber. METHODS: In this report, the authors describe a technique where a "temporary corneal patch" was created from a sterile instrument cover, using a trephine to ensure a diameter approximately 1 mm to 2 mm wider than

Cutaneous Fistula Caused by Metallosis Following Rigid Spinal Fixation: A Rare but Important Diagnostic Consideration.

Metallosis is a rare complication of spinal instrumentation, typically associated with joint arthroplasty or fusionless constructs. Fistula formation due to metallosis is exceptionally uncommon, particularly in cases involving rigid spinal fixation. We report a case of a 48-year-old female patient who developed a cutaneous fistula at the site of a previous thoracic spinal fixation performed for a metastatic spinal tumor. Clinical examination revealed a small granulomatous mass with a fistula. Although dynamic radiographs showed no apparent instability, computed tomography revealed radiolucent

Advanced olfactory neuroblastoma in a teenager: a clinical case and short review of literature.

BACKGROUND: Olfactory neuroblastoma, also called esthesioneuroblastoma (ONB), is a rare neuroectodermal neoplasm that originates from the olfactory epithelium of the nose-sinus tract. It generally occurs with epistaxis, nasal obstruction, diplopia, and anosmia. METHODS: A 16-year-old female was admitted to our Unit with a complaint of nasal obstruction, recurrent epistaxis, anosmia, and intermittent headache of sixth month's duration. After the ENT consultation, physical examination, endonasal endoscopy, and multiple biopsies were performed. Instrumental images (CT, MRI) have been requested t

Transient Dorsal Column Dysfunction After Extreme Cervical Deformity Correction: Report of 3 Cases.

BACKGROUND: Chin-on-chest kyphotic cervical deformity can be debilitating. Surgical deformity correction serves to decompress neural elements and restore lordosis. This can be achieved through multiple osteotomies with instrumentation and fusion, sometimes requiring a staged approach. Such procedures carry a high risk of neurologic injury. Here we present examples of transient neurologic dysfunction not previously reported in the common literature. CASE DESCRIPTION: The authors present 3 patients who underwent extreme cervical deformity correction for chin-on-chest deformity. Deformity correc

Velopharyngeal insufficiency after maxillary advancement in patients with cleft palate - a survey of risk assessment in the United Kingdom and Ireland.

Patients with cleft lip and palate may require orthognathic surgery to correct severe impairments in midfacial growth. Maxillary advancement in this group, however, is linked to deterioration in velopharyngeal function (VPF), and it is not clear how cleft teams assess this risk. We therefore surveyed surgeons from 15 cleft units who provide orthognathic treatment, to gain an understanding of current practice in the UK and Ireland. A total of 16/21 surgeons from 14/15 units responded. While 14/16 surgeons agreed that these patients are at risk of a deterioration in VPF after maxillary advanceme

One-stage posterior approach for treating multilevel noncontiguous thoracic and lumbar spinal tuberculosis.

OBJECTIVES: Multilevel noncontiguous thoracic and lumbar spinal tuberculosis (MNST) is a relatively rare entity. The objective of this retrospective study was to investigate whether a technique involving a one-stage posterior debridement and decompression, combined with an intervertebral fusion and posterior instrumentation, is effective for treating MNST. METHODS: Thirteen patients, with an average age of 40.69 (18-67) years, who had MNST and were surgically treated in our department from January 2008 to October 2013, were reviewed. RESULTS: The average follow-up time was 37.54 ± 10.49 (19-

Sublaminar wiring for odontoid synchondrotic fracture stabilization in a 4-year-old: a case report.

BACKGROUND: The pediatric age group presents a challenge in diagnosis and management of upper cervical injuries. Cervical spine injuries are relatively rare in children as compared to adults and C2 vertebra is the commonly affected site for fracture and injury. METHODS: In our case, a 4-year-old female child was brought to the emergency after being hit over the head and neck by a falling wooden beam. Patient had associated minimal neurological deficits. Urgent CT scan of the head and cervical spine were done. CT spine suggested instability as there was anterior angulation of the odontoid proc

Cerebral autoregulation is minimally influenced by the superior cervical ganglion in two- week-old lambs, and absent in preterm lambs immediately following delivery.

Cerebral vessels in the premature newborn brain are well supplied with adrenergic nerves, stemming from the superior cervical ganglia (SCG), but their role in regulation of blood flow remains uncertain. To test this function twelve premature or two-week-old lambs were instrumented with laser Doppler flow probes in the parietal cortices to measure changes in blood flow during changes in systemic blood pressure and electrical stimulation of the SCG. In lambs delivered prematurely at ∼129 days gestation cerebral perfusion and driving pressure demonstrated a direct linear relationship throughout t

The unicompartmental knee: design and technical considerations in minimizing wear.

Unicompartmental knee arthroplasty is an alternative to total knee arthroplasty for patients with unicompartmental tibiofemoral noninflammatory disease. Careful patient selection and newer instrumentation has reduced the progression of arthrosis in the other compartment and tibial loosening, leaving polyethylene wear as the predominant failure mechanism in more contemporary designs. Increased wear increases the debris volume at the bone-implant interface, and wear particles will generate osteolysis leading to component loosening and unreplaced compartment degeneration. The design-related facto

Holmium: yttrium aluminum garnet laser-assisted endoscopic sinus surgery: laboratory experience.

Endoscopic sinus surgery has gained wide acceptance since its introduction into the United States. Complex sinus anatomy and troublesome bleeding have been associated with complications, which vary in severity from synechia to blindness and leakage of cerebrospinal fluid. Endoscopic sinus surgery using a holmium: yttrium aluminum garnet pulsed solid-state laser oscillating at 2.1 microns with fiberoptic delivery was performed in the laboratory, and the results were compared with those of conventional endoscopic sinus surgery. Three beagle dogs, six human cadaver heads, and one calf head were u
