Lateral Extra-articular Tenodesis With Proximal Staple Fixation.

A technique for lateral extra-articular tenodesis using proximal staple fixation is described as an adjunct to anterior cruciate ligament (ACL) reconstruction. Lateral extra-articular tenodesis has been used in an effort to decrease failure rates in ACL-deficient patients with ligamentous laxity, prior failed ACL surgery, or grade 3 pivot-shift findings. Numerous surgeons have described combining ACL reconstruction with extra-articular surgery. The approach described in this article is easy and cost-effective. Moreover, because this technique uses a staple instead of a SwiveLock (Arthrex, Napl

Total extracardiac cavopulmonary connection: an alternative technique of fenestration.

Fenestrating an extracardiac conduit used for total cavopulmonary connection normally requires an additional incision on the right-sided atrium and is time consuming. Herein we describe an alternative technique that may be used to facilitate this process, which consists of creating the fenestration by using part of the atrial incision resulting from the disconnection of the inferior vena cava from the right atrium. The advantages of this technique are avoidance of an extra incision and suture line on the atrium, and the ease of construction. This may be especially useful in patients with heter

[Transesophageal gastrostomy].

For gastrostomy after thoracic esophagus extirpation its distal stump was used. It is confirmed, that transesophageal gastrostomy has a number of advantages. Pezzer's catheter can be used as gastrostomic tube which self-fixes in given position and secures reliable gastrostoma sealing. Suturing of the stomach to parietal peritoneum around gastrostoma is not more necessary. Deformation and reduction of the stomach size are excluded and stomach is kept ready for subsequent plastic replacement of the oesophagus. The operation gets oncologically more radical as a result of removal of paraesophageal

Laparoscopic management of complete rectal prolapse: description of technique.

Laparoscopic surgery has been successfully employed for many years by gynecological surgeons. Following the widespread success of laparoscopic cholecystectomy, minimally invasive surgery has expanded to be adapted to all general surgical procedures including hernia repair, oesophagectomy, appendicectomy, Meckel's diverticulectomy, and Nissen fundoplication. Laparoscopic colonic surgery has not developed at the same rate as other procedures because of the requirement for advanced laparoscopic surgical skills, deficiencies in instrumentation and concerns about the potential risks of laparoscopic

Senile entropion.

A new surgical procedure has been developed for the correction of senile entropion of the lower lid. The procedure consists of horizontal shortening of the lower lid by a pentagonal wedge resection combined with an infratarsal eyelid suture technique. Twenty eyelids of 13 patients with entropion have been operated on over the last 4 1/2 years with use of various modificatons of this procedure. The average follow-up period is two years three months, with the longest being 4 1/2 years and the shortest being three months. There has only been one temporary recurrence and no overcorrections. The te

Arthroscopic Remplissage Using a Double-Pulley Technique.

BACKGROUND: Anterior shoulder instability is associated with Hill-Sachs lesions (HSLs) in 40% to 90% of cases. When addressing anterior shoulder instability, unaddressed engaging or "off-track" HSL have a recurrence rate of 75%. Remplissage is a known technique to address recurrent instability in the setting of an engaging HSL. In this video, we demonstrate that a double-anchor-pulley technique may be used to address recurrent instability in the setting of engaging HSL. INDICATIONS: Patients with recurrent anterior shoulder instability with off-track HSL in patients with glenoid bone loss <20

Cartilage-sparing otoplasty - 288 ears in 3 years.

BACKGROUND: Standard methods of otoplasty may risk anatomical distortion. Previous work showcased a novel, cartilage-sparing technique. This study aimed to confirm the safety and effectiveness of the method in a larger cohort. METHODS: This retrospective study included patients undergoing otoplasty by a single surgeon using a single technique between January 2021 and December 2023. Eligible patients were aged >5 years and had prominent or constricted or cup ears, forming a 2 cm distance from the mastoid. Surgical techniques included novel key-point sutures. Minimum follow-up was 6 months. Dat

Subpectoral Biceps Tenodesis Using an All-Suture Anchor.

BACKGROUND: The long head of the biceps tendon (LHBT) has long been considered an intra-articular pain generator in the shoulder. While nonoperative treatment modalities can be acceptable for less severe presentations of LHBT pathology, surgical treatment options such as tenotomy and tenodesis remain controversial for recalcitrant cases. Open subpectoral biceps tenodesis allows for removal of all pathologic tissue from the bicipital groove, and all-suture anchor fixation utilizes a small caliber unicortical drill hole, which can potentially reduce the risk of a stress riser and iatrogenic frac

The feasibility of debranching aortic arch and visceral arteries with sutureless telescoping anastomoses during open aortic aneurysm repair.

BACKGROUND: Open repair of aortic aneurysms frequently requires reimplantation of major aortic vessels. Traditional techniques can be time consuming, require meticulous hemostasis, and risk aneurysmal patch degeneration, which can require a challenging reoperation. We describe our experience using a stent graft to create a sutureless anastomosis that obviates these drawbacks. METHODS: Between April 2018 and March 2021, all consecutive adult patients who underwent open repair of the aorta with at least one supra-aortic trunk or visceral vessel reimplanted using the sutureless anastomotic techn

Fetoscopic two-layer closure of open neural tube defects.

OBJECTIVE: Fetoscopy for closure of open neural tube defects (NTD) remains controversial, as the use of patches or single-layer closure is not considered to meet the standards of good neurosurgical reconstruction. In this study, we describe a fetoscopic two-layer (myofascial and skin) closure technique for the treatment of NTD in five patients and report the preliminary anatomical outcome at birth. METHODS: From February to September 2017, five pregnant women with a fetus with a NTD, including three cases of myelomeningocele and two cases of myelocele, were operated on using a fetoscopic two-

Laparoscopic minimally invasive total gastrectomy with linear stapled oesophagojejunostomy--experience from the first thirty procedures.

BACKGROUND: There are only few reports on total gastrectomy by a laparoscopic surgical approach. One explanation is the fear of complications due to anastomotic dehiscence in oesophagojejunal anastomosis known to carry high morbidity and mortality. The introduction of staplers have contributed to making anastomosis safer and easier to perform and has facilitated more advanced laparoscopic surgery. In open surgery, most surgeons use a circular stapler for oesophagojejunal anastomosis or a hand sutured technique. Both techniques are difficult to use in laparoscopic surgery, especially if the oes

Role of the robot in totally laparoscopic aortic repair for occlusive and aneurysmal disease.

INTRODUCTION: The feasibility of robotically-assisted laparoscopic aortic surgery has been adequately demonstrated. The authors report on their clinical experience with robot-assisted aortoiliac reconstruction for aorto-iliacocclusive disease, aortic aneurysm and two hybrid procedures performed using the da Vinci system. MATERIALS/METHOD: Between November 2005 and December 2008, we performed 130 robot-assisted laparoscopic aortoiliac procedures. One hundred sixteen patients were prospectively evaluated for occlusive disease, ten patients for abdominal aortic aneurysm, two for a common iliac a

Laparoscopic transuterine fetal vesicostomy: a feasibility study.

PURPOSE: We evaluate the feasibility of applying minimally invasive techniques for fetal vesicostomy. We also evaluate whether transuterine fetal vesicostomy can be performed laparoscopically. MATERIALS AND METHODS: A total of 25 pregnant ewes were time dated at approximately 90 days of gestation. With the animals under general anesthesia a low open abdominal incision was made and the uterus was brought out through the incision. With a 14 gauge needle the amniotic sac was filled with 1 to 2 l warm glycine. Three to 4, 5/12 blunt tip balloon trocars were placed in the uterus. Using laparoscopi

Robotic vaginal natural orifice transluminal endoscopic myomectomy.

OBJECTIVE: To describe a novel, minimally invasive technique for performing myomectomy, a fertility-sparing procedure. DESIGN: This technique was developed based on similar techniques for other surgeries that showed a benefit. Liu et al. (1) described vaginal natural orifice transluminal endoscopic surgery (vNOTES) for myomectomy, in which a 6-cm myoma was resected transvaginally. An anterior colpotomy was made, and single-site surgical skills were used to perform the entire myomectomy without an abdominal incision and with minimal blood loss (1). Another study showed that this technique was

Laparoscopic Gastric Resection for Gastric Cancer: Is Intracorporeal Anastomosis Necessary?

BACKGROUND AND OBJECTIVE: In surgical dissection, laparoscopic approach and open techniques do not differ significantly, but there is still no consensus on how anastomosis should be performed in both cardia and distal gastric tumors. Anastomosis can be performed by laparoscopy-assisted mini-laparotomy or by intracorporeal suture techniques. In this study, we aim to present our four years of clinical experience and short-term surgical results from 133 cases in order to evaluate the necessity of laparoscopic anastomosis. METHODS: This study was approved by Ethics Committee (No: 1-8-19, date: 14

Effect of the suture augmentation on the stability and strength of ulnar collateral ligament reconstruction: The anchor and bone tunnel methods.

BACKGROUND: The initial fixation strength of the ulnar collateral ligament (UCL) reconstruction is increased by combining a suture augmentation (SA). However, no reports have investigated the stability of UCL reconstruction with SA across multiple elbow flexion angles and the influence of SA on the stability and failure strength of the anchor and bone tunnel methods. This study aimed to compare the displacement against a valgus load at multiple elbow flexion angles of the anchor and bone tunnel methods between with and without SA, and to compare the failure strength between these methods with
