Comparison of del Nido and St Thomas Cardioplegia Solutions in Pediatric Patients: A Prospective Randomized Clinical Trial.

We conducted a prospective randomized trial to compare del Nido (DN) cardioplegia with conventional cold blood cardioplegia (St Thomas [STH]) in pediatric patients. We randomized 100 pediatric patients aged ≤12 years undergoing elective repair of ventricular septal defects and tetralogy of Fallot to the DN and the STH groups. In the DN group, a 20 mL/kg single dose was administered. In the STH group, a 30 mL/kg dose was administered, followed by repeated doses at 25- to 30-minute intervals. The primary outcome was cardiac index that was measured 4 times intra- and postoperatively. Troponin-I,

Right ventricular remodeling after pulmonary valve replacement: early gains, late losses.

BACKGROUND: Although early results of pulmonary valve replacement (PVR) after tetralogy of Fallot repair have been described, information about late postoperative ventricular size and function is lacking. This study was designed to characterize right ventricular (RV) remodeling up to 10 years after PVR. METHODS: Retrospective analysis was conducted of cardiovascular magnetic resonance (CMR) data from 2002 to 2011 in 101 patients (244 studies) who underwent PVR and had one or more post-PVR CMR studies at five post-PVR time intervals up to 10 years. RESULTS: Compared with pre-PVR values, in th

Heart transplantation to a physiologic single lung in patients with congenital heart disease.

BACKGROUND: Heart-lung transplantation has been recommended for patients with end-stage congenital heart disease (CHD) and single-lung physiology due to either discontinuous pulmonary arteries (PAs) and unilateral PA hypertension (HTN) or absence of 1 PA. METHODS: Eleven patients with CHD and single-lung physiology underwent heart transplantation (HT). Diagnoses included: tetralogy of Fallot, absent left PA (n = 4); single-ventricle s/p classic Glenn (n = 7), with absent left PA (n = 1); and severe left PA HTN (n = 6). RESULTS: Mean time from last surgery was 13 +/- 8 years; mean number of o

The Feasibility and Reproducibility of Measuring Myocardial Performance Index Using Two-Dimensional Phase Contrast Imaging in Patients with Congenital Heart Disease.

The myocardial performance index (MPI) integrates systolic and diastolic function to provide a geometry-independent measure of global ventricular performance. Despite its clinical utility, MPI remains underutilized in cardiovascular magnetic resonance (CMR), especially in patients with congenital heart disease (CHD). This study aimed to evaluate the feasibility and reproducibility of MPI measurement using mitral inflow phase-contrast CMR (PCCMR) in children and young adults with both normal cardiac anatomy and CHD, and to explore its relationship with ventricular function. A total of 124 parti

Interpregnancy interval and prevalence of selected birth defects: A multistate study.

BACKGROUND: Both short and long interpregnancy intervals (IPIs) have been associated with adverse birth outcomes. We undertook a multistate study to describe the prevalence of selected birth defects by IPI. METHODS: We obtained data from nine population-based state birth defects registries for singleton live births in 2000-2009 among mothers with a previous live birth identified through birth certificates. IPI was calculated as the difference between prior birthdate and start of the current pregnancy (conception date). We estimated prevalence of selected defects per 10,000 live births and pre

Extracorporeal membrane oxygenation as a novel management strategy for interventricular septal hematoma following ventricular septal defect repair.

OBJECTIVES: Interventricular septal hematoma (IVSH) is a rare complication, which may result from ventricular septal defect (VSD) repair. IVSH can result in conduction and/or hemodynamic abnormalities related to impaired ventricular filling or outflow tract obstruction. We report the novel use of extracorporeal membrane oxygenation (ECMO) for management. METHODS: Echocardiography reports (January 1980 to December 2016) were searched for the term "hematoma" in our institutional database and reviewed to determine appropriate cases. Charts and imaging (reports) data were abstracted. All intraope

Sustainability of Infant Cardiac Surgery Early Extubation Practices After Implementation and Study.

BACKGROUND: The Pediatric Heart Network Collaborative Learning Study (PHN CLS) successfully changed practice at four hospitals to increase the rate of early extubation within 6 hours after infant heart surgery. It is unknown whether this practice continued after study completion. METHODS: We linked the PHN CLS dataset to the Pediatric Cardiac Critical Care Consortium registry to compare outcomes at four active hospitals between the study period (post-clinical practice guideline [CPG]) and the first year after study completion (follow-up) after a 3-month washout. Ventricular tachycardia in nonpostoperative pediatric patients: role of radiofrequency catheter ablation.

Experience concerning radiofrequency catheter ablation (RFCA) of ventricular tachycardia (VT) in pediatric patients is limited. In adults, success rates vary widely based on the etiology of the VT. Highest success rates have been achieved in patients without structural heart disease. Between March 1998 and December 1999, five young patients (age, 5 months to 15 years; body weight, 5.5-61.6 kg) underwent RFCA for VT at our institution [structurally normal heart (n = 4), preoperative tetralogy of Fallot (n = 1)]. Monomorphic VT was present in four children, and an infant with MIDAS syndrome had

Valve-in-Valve Repair in a Critically Ill Obstetric Patient with Severe Pulmonary Stenosis: A Rare Case.

Background: Among patients with congenital heart disease, particularly those with a history of undergoing the Fontan operation, pregnancy presents a significant maternal-fetal risk, especially when complicated by severe valvular dysfunction. Lung reperfusion syndrome (LRS) is a rare but life-threatening complication occurring following valve intervention. Multidisciplinary management, including by Cardio-Obstetrics teams, is essential for optimizing outcomes in such high-risk cases. Methods: We present the case of a 37-year-old pregnant patient with previously repaired tetralogy of Fallot (via

Right atrial reverse remodeling and risk of atrial arrhythmias after surgical pulmonary valve replacement.

BACKGROUND: Right atrial (RA) dysfunction and atrial arrhythmias are relatively common in adults with repaired tetralogy of Fallot. The purpose of this study was to determine whether RA function improved after surgical pulmonary valve replacement (PVR), and the association between postoperative RA reverse remodeling and late postoperative atrial arrhythmias. METHOD: RA reverse remodeling (ΔRA reservoir strain based speckle tracking echocardiography) was calculated as: ([postoperative RA reservoir strain - preoperative RA reservoir strain]/preoperative RA reservoir strain)x100. Optimal RA reve

Heart Rate Recovery Following Exercise Testing in Pediatric Patients with Acyanotic Repaired Congenital Heart Disease.

Attenuated heart rate recovery (HRR) following peak exercise has been shown to be a predictor of mortality in populations of adults with Fontan palliation, coronary artery disease, heart failure, and heart transplantation. However, few have studied HRR in children and adolescents with congenital heart disease (CHD). This case-control study compared HRR patterns from exercise stress testing in children and adolescents with and without repaired acyanotic CHD (raCHD). Retrospective analysis included patients aged 10-18 years who had exercise testing between 2007 and 2017. The raCHD cohort include

Clinical Experience With the Subcutaneous Implantable Cardioverter-Defibrillator in Adults With Congenital Heart Disease.

BACKGROUND: Sudden cardiac death is a major contributor to mortality for adults with congenital heart disease. The subcutaneous implantable cardioverter-defibrillator (ICD) has emerged as a novel tool for prevention of sudden cardiac death, but clinical performance data for adults with congenital heart disease are limited. METHODS AND RESULTS: A retrospective study involving 7 centers over a 5-year period beginning in 2011 was performed. Twenty-one patients (median 33.9 years) were identified. The most common diagnosis was single ventricle physiology (52%), 9 palliated by Fontan operation and

Facile minimally invasive cardiac surgery via ministernotomy.

BACKGROUND: The public's and surgeons' perception of minimally invasive operations are frequently at odds. Nevertheless, real or perceived benefits may result from limiting skin and skeletal trauma. METHODS: Beginning in January 1996, we began approaching most infant and pediatric open heart procedures through an upper sternal split incision using a 1- to 3-inch skin opening and then extended this technique using a 2.5- to 3.5-inch incision for adult aortic and mitral valve replacement. RESULTS: A total of 82 patients, 57 infants and children and 25 adults, have been operated on using this a

Temporal change in cardiac function and clinical indices in adults with valvular pulmonic stenosis.

BACKGROUND: Patients with palliated pulmonary valve stenosis (PVS) have less cardiac remodeling and symptoms as compared to patients with repaired tetralogy of Fallot (TOF) presenting with similar severity of right ventricular outflow tract (RVOT) disease. What is not known is whether patients with PVS versus TOF presenting with similar severity of RVOT disease at baseline, would have similar (or different) pace of cardiac remodeling and disease progression over time. The study objective was to compare temporal changes in clinical and cardiac function indices between adults with palliated PVS
