Intraoperatively accurate limb alignment after opening wedge high tibial osteotomy can be lost by large knee joint line convergence angle during surgery.

INTRODUCTION: The purpose of this study was to assess early postoperative loss of achieved correction and associated factors after opening wedge high tibial osteotomy (OWHTO). MATERIALS AND METHODS: OWHTO was performed in 121 patients with osteoarthritis of the knee (mean age 66 years, 154 knees). Anteroposterior radiographs of the knee and full-length leg, and varus and valgus stress radiographs of the knee were taken, and the femorotibial angle (FTA), joint line convergence angle (JLCA), and medial proximal tibial angle (MPTA) were measured. The changes in the FTA, JLCA, and MPTA were defin

Mapping Stresses on the Tibial Plateau Cartilage in an Ovine Model Using In-Vivo Gait Kinematics.

Understanding stresses within the knee joint is central to understanding knee function, and the etiology and progression of degenerative joint diseases such as post-traumatic osteoarthritis. In this study, in vivo gait kinematics of four ovine subjects were recorded using a highly accurate Instrumented Spatial Linkage (ISL) as each subject walked on a standard treadmill. The subjects were then sacrificed, and the right hind limbs removed. Ten purpose-built Fibre Bragg Grating (FBG) sensors were positioned within each stifle joint and used to measured contact stresses on the articulating surfac

Correction of pes varus deformity in a Miniature Dachshund by true spherical osteotomy with a dome saw blade.

CASE DESCRIPTION: A 6-month-old male Miniature Dachshund was referred for examination and correction of a unilateral pes varus deformity. CLINICAL FINDINGS: Clinical examination and diagnostic imaging revealed varus, procurvatum, and internal torsion of the distal aspect of the left tibia causing intermittent lameness. TREATMENT AND OUTCOME: True spherical osteotomy performed with a dome-shaped saw allowed correction of the deformity in 3-D, and orthogonal internal plate fixation provided stabilization. No intraoperative or immediate postoperative complications developed. Outcome as judged b

Large medial proximal tibial angles cause excessively medial tibiofemoral contact forces and abnormal knee kinematics following open-wedge high tibial osteotomy.

BACKGROUND: Recurrent varus deformity and poor outcome sometimes occur following open-wedge high tibial osteotomy, but the mechanism remains unclear. The hypothesis of this study was that an excessively large medial proximal tibial angle with lateral joint surface inclination can worsen postoperative knee biomechanics. METHODS: A computer-simulated knee model was validated based on a volunteer knee. Osteotomy models with medial proximal tibial angles ranging from 90° to 97° in 1° increments were developed. Varus alignment correction of the distal femur was performed in each model to maintain

Central Implantation of the Femoral Component Relative to the Tibial Insert Improves Clinical Outcomes in Fixed-Bearing Unicompartmental Knee Arthroplasty.

BACKGROUND: The direct relationship between clinical outcomes and femoral component positioning relative to a tibial insert remains unknown. We determined whether the femoral component position relative to the tibial insert could affect clinical outcomes after fixed-bearing unicompartmental knee arthroplasty (UKA). METHODS: The femoral component position relative to the tibial insert of 66 patients with anteromedial osteoarthritis and osteonecrosis of the knee who underwent fixed-bearing UKA was assessed at 2 weeks postoperatively. We classified patients according to the contact point of the

Patellar Instability and Dislocation: Optimizing Surgical Treatment and How to Avoid Complications.

Patellar instability is a common problem seen by the orthopedic surgeon. Surgery is indicated in recurrent dislocation to improve patellar tracking and ligamentous restraint in order to decrease risk of recurrence, osteochondral injury, and eventual progression to arthritis. Preoperative imaging studies identify anatomic risk factors that increase risk of patellar dislocation to inform surgical decision making. Surgical management starts with medial patellofemoral ligament reconstruction, which is effective in many cases. Tibial tubercle osteotomy realigns the extensor mechanism and is useful

Lower Extremity Joint Contributions to Trunk Control During Walking in Persons with Transtibial Amputation.

Controlled trunk motion is crucial for balance and stability during walking. Persons with lower extremity amputation often exhibit abnormal trunk motion, yet underlying mechanisms are not well understood nor have optimal clinical interventions been established. The aim of this work was to characterize associations between altered lower extremity joint moments and altered trunk dynamics in persons with unilateral, transtibial amputation (TTA). Full-body gait data were collected from 10 persons with TTA and 10 uninjured persons walking overground (~1.4 m/s). Experimentally-measured trunk angular

The role of blood vessels in broiler chickens with tibial dyschondroplasia.

Tibial dyschondroplasia (TD) is an intractable tibiotarsal bone disorder of rapid growing avian species, which leads to huge economic losses and compromised poultry welfare. However, the exact pathogenesis and treatment of TD remain largely unknown. Based on continuous research findings, we propose the TD pathogenesis hypothesis: during skeletal development of TD chickens, due to the absence of vasculature of proximal tibial growth plates (TGP), hypertrophic chondrocytes of the TGP are unable to complete calcification in normal bone development and less dead chondrocytes in the corresponding a

Staged Reconstruction for Type IV Tibial Deficiency (Distal Tibiofibular Diastasis): A Report of 2 Cases.

CASE: Congenital tibiofibular diastasis is a relatively rare form of limb deficiency, characterized by distal tibial tapering, absent ankle mortise, equinovarus foot deformity, and variable lower leg shortening. Treatment described has ranged from various forms of foot centralization with or without leg lengthening to amputation. We describe 2 cases treated in childhood by staged foot centralization by soft-tissue distraction, distal tibiotalar fusion, tibial lengthening, and subsequent limb length discrepancy equalization. At skeletal maturity, both patients ambulated independently without ai

Identification and expression analysis of microRNAs in tibial growth plate of chicken through thiram toxicity.

Thiram is a widely known tibial dyschondroplasia (TD) inducer. TD, a common metabolic cartilage disease, presents in rapidly growing poultry birds. There are evidences that miRNAs are involved in diverse aspects of normal skeletal development, but very less is known about the role of miRNAs in TD. Therefore, this study aimed to determine which genes and pathways show differential expression between TD suffered chickens and normal chickens. We collected growth plates from ten-days-old TD chickens and control chickens and performed high-throughput RNA sequencing (RNA-Seq). Afterwards, target pre

Finite element models of the tibiofemoral joint: A review of validation approaches and modelling challenges.

The knee joint is a complex mechanical system, and computational modelling can provide vital information for the prediction of disease progression and of the potential for therapeutic interventions. This review provides an overview of the challenges involved in developing finite element models of the tibiofemoral joint, including the representation of appropriate geometry and material properties, loads and motions, and establishing pertinent outputs. The importance of validation for computational models in biomechanics has been highlighted by a number of papers, and finite element models of th

Clinical efficacy of preoperative 3D planning for reducing surgical errors during open-wedge high tibial osteotomy.

Increases in posterior tibial slope (PTS) with open-wedge high tibial osteotomy (OWHTO) are often related to two surgical errors: Symmetric opening of the osteotomy gap and a tendency to open the gap from the anteromedial direction. The study objective was to define trends in these two errors using computer simulation and clinical effects of their countermeasures. First, 60 knees with varus deformity were assessed with three-dimensional (3D) planning using computed tomography to allow for the mechanical axis to pass through a point at 62.5% of the width of the tibial plateau, defined as the we

High Tibial Osteotomy and Medial Meniscus Transplant.

Concomitant high tibial osteotomy and meniscal allograft transplant is indicated in patients with medial meniscus deficiency and varus deformity. A synergistic relationship likely exists between knee realignment and restoration of meniscal and chondral tissue. Therefore, it is important to address all sources of pathology, such as meniscal or chondral defects, when addressing axis malalignment. In this article, we review the indications, preoperative considerations, technique, postoperative rehabilitative protocol, and outcomes in patients undergoing high tibial osteotomy with medial meniscal

A complete posterior tibial stress fracture that occurred during a middle-distance running race: a case report.

Posterior tibial stress fractures are more frequent than anterior tibial stress fractures, and they are considered to have a good prognosis for returning to sports; cases leading to a complete fracture are rare. A 17-year-old male involved in high school athletics middle-distance running had a 3-week history of pain with training. He was running up to 300 km/week on streets and cross-country in an even distribution. He had posterior tibial stress fractures, but despite the lower leg pain, he continued running. One year later, he was brought to the emergency department after having sustained an
