Preserving Cervical Mobility: A Novel Robot-Assisted Approach for Atlas Fracture Fixation.

BACKGROUND The management of unstable atlas fractures remains a subject of ongoing debate and controversy. The conservative surgical treatment commonly involves fusion, resulting in severe loss of cervical spine mobility, and a large incisions and extensive tissue dissection are required. We aim to introduce a novel concept and surgical approach for treating atlas fracture, one that involves minimizing trauma while maintaining mobility of the upper cervical spine without resorting to fusion. CASE REPORT We present the case of a 48-year-old woman with an atlas fracture who underwent robot-assis

Spinal Leclercia adecarboxylata infection in an immunocompetent patient: case report.

BACKGROUND: Leclercia adecarboxylata (Family: Enterobacteriaceae) is a gram-negative bacillus that is found in diverse environments but has rarely been isolated from the human microbiota. Notably, no patient with a spinal disc infection caused by L. adecarboxylata has been documented to date. CASE SUMMARY: A 60-year-old male presented with chronic lower back pain and decreased extensor hallucis longus muscle power upon examination. Magnetic resonance imaging of the spine revealed canal stenosis at L3-4 and L4-5, along with migrating L4-5 disc material compressing the left lateral recess and f

Proximal interphalangeal joint arthroplasty using self-locking type surface replacement implant for symptomatic osteoarthritis: Extended dorsal central splitting approach and intermediate-term outcome.

BACKGROUND: This retrospective study introduced wide exposure and radical osteophytectomy by employing the extended dorsal central splitting approach (EDCSA) to mitigate complications associated with surface implant arthroplasty for osteoarthritis of the proximal interphalangeal (PIP) joint using a self-locking type of implant. METHODS: In total, 171 fingers of 113 patients underwent cementless surface non-constrained PIP implantation using the EDCSA. The surgical approach involved larger implant placement, optional procedures, and complex collateral ligament repair following wide exposure an

Assessment of Union Rates in Ballistic Hand Fractures.

BACKGROUND: Ballistic hand fractures (BHFs) are complex, multistructure injuries, unique from other hand fractures. We sought to determine BHF union rates and assess factors contributing to nonunion. METHODS: Metacarpal or phalangeal BHFs sustained from 2016 to 2023 were identified from an institutional database. Charts and radiographs were reviewed for fracture characteristics and union by 90 days. Fractures without union by 90 days or without minimum 90-day follow-up were excluded. Comminuted fractures were categorized by severity: (1) complete bone loss (NC); (2) partial bony contact (PC);

Older age is a predictor for hardware failure in open lower extremity fractures requiring free flap coverage.

BACKGROUND: The primary objective of this study was to investigate incidence and factors associated with hardware failure in a cohort of patients with open tibial and ankle fractures who underwent free flap coverage in conjunction with open reduction and internal fixation (ORIF) and hardware placement. Secondary objectives were to determine incidence of fracture union and limb salvage. Finally, we sought to identify flap-related factors that might impact rates of infection, flap failure, and nonunion. METHODS: A consecutive single-surgeon series was studied. Baseline patient characteristics,

Molecular Basis of Fracture Pseudarthrosis Associated with Neurofibromatosis Type 1.

UNLABELLED: Neurofibromatosis Type 1 (NF1) is a tumor-predisposition syndrome caused by heterozygous mutations in the NF1 gene. In addition to oncologic manifestations, individuals with NF1 face a heightened risk of developing secondary skeletal complications associated with somatic loss-of-heterozygosity. Long bone dysplasia and post-fracture pseudarthrosis are among the earliest skeletal manifestations observed in children with NF1. Most commonly occurring in the tibia, pseudarthroses are often treated with a combination of resection, fixation, bone grafting, and rhBMP2. Although rates of in

Outcomes of Tibiotalocalcaneal Fusion After Partial Talectomy in the Charcot Ankle.

BACKGROUND: Whether a sequela of neuropathy or a result of infectious processes to dysvascular changes, talar degeneration in the Charcot ankle proves difficult to treat. Total resection followed by bone void filler or grafts is costly, with varying levels of success. Although tibiocalcaneal fusion allows stabilization, this approach remains a procedure of last resort before amputation. This series presents outcomes of partial talectomy with tibiotalocalcaneal arthrodesis. METHODS: Nineteen patients with Charcot's neuroarthropathy necessitating a tibiotalocalcaneal fusion were included in thi

Efficacy of airway stenting and nasogastric tube insertion in airway-esophageal fistula patients with airways compromised by advanced malignancy.

INTRODUCTION: Whether airway-compromised airway-esophageal fistula (AEF) patients should undergo combined airway and esophageal stenting is controversial. This study was designed to evaluate the survival prognosis and poststent interventions in AEF patients with airways compromised by advanced malignancy with or without airway stents. METHODS: A retrospective analysis of the medical records, survival times, and poststent interventions of 17 patients with or without airway stents was performed. RESULTS: The causes of AEF were esophageal cancer (11/17, 64.7%), lung cancer (6/17, 29.4%), and th

Impact of Prolonged Ischemia on the Immunohistochemical Expression of Programmed Death Ligand 1 (PD-L1).

Antibodies targeting programmed death receptor 1 or programmed death ligand 1 (PD-L1) have become a standard of care to treat different cancers; for some of these tumors, there is a correlation between tissue expression of PD-L1 and response rates in patients. Although most of the analytical challenges in the evaluation of PD-L1 expression have been standardized, preanalytical issues have been less explored. The objective of this study was to evaluate the impact of time of ischemia on the performance of 2 commonly used antibodies against PD-L1. Sixteen tonsillectomy samples were kept in ischem

Feasibility of Microwave Scissors-Based Off-Clamp Laparoscopic Partial Nephrectomy in a Porcine Model.

OBJECTIVES: To assess the feasibility of off-clamp laparoscopic partial nephrectomy using microwave scissors. METHODS: We performed transperitoneal laparoscopic partial nephrectomy, without hilar clamping or renorrhaphy, using only microwave scissors for renal resection in a porcine model. For each kidney, 2 types of procedures were performed: a middle pole resection excising an area of 2-cm diameter and approximately 1-cm depth and a lower pole resection at the level of the lower polar line. The renal calyces exposed during renal resection were sealed and transected using microwave scissors.

Balloon kyphoplasty combined with percutaneous pedicle screw (PPS) for the treatment of osteoporotic thoracolumbar fractures has minimum correction loss 2 years after surgery: compared to vertebroplasty using hydroxyapatite block combined with PPS.

BACKGROUND: Balloon kyphoplasty (BKP) is a useful treatment performed in patients with painful osteoporotic vertebral body fracture (OVF). However, in cases with large intra-vertebral clefts and cases with posterior spinal tissue damage, adjacent vertebral body fractures (AVFs), and cement migration may occur early after BKP, which may be a factor for poor results. In such cases, percutaneous vertebroplasty (PVP) combined with percutaneous pedicle screw (PPS) is useful treatment. This study compared BKP combined with PPS (BKP + PPS) compared to PVP using hydroxyapatite (HA) block combined with

Patient perspective on the use of carbon fibre plates for extremity fracture fixation.

INTRODUCTION: Carbon fibre-reinforced polyetheretherketone (CFR-PEEK) plates represent an exciting development within trauma and orthopaedic surgery, offering advantages including radiolucency, material properties similar to bone, and lack of localised tissue reaction. As more call for trials examining their use, there is no data available as to the acceptability of these implants to patients. This study aimed to therefore examine the acceptability of CFR-PEEK plates to patients undergoing fracture surgery. METHODS: This was a prospective cross-sectional survey of patients undergoing surgery

Umbilical cord mesenchymal stem cells combined with secretome for treating congenital pseudarthrosis of the Tibia: a case series.

INTRODUCTION: Most patients with congenital pseudoarthrosis of tibia (CPT) do not have promising outcomes despite multiple attempts using conventional surgeries. The combination of umbilical cord-derived mesenchymal stem cells and conditioned medium (secretome) contains major components pivotal for the enhancement of fracture healing. The purpose of this study was to address fracture healing in CPT cases that were treated using the combined implantation of umbilical-cord mesenchymal stem cells (UC-MSCs) and secretome. MATERIALS AND METHODS: From 2016 to 2017, six patients with CPT who were tr
