In this article, one-stage repair of complicated cases of penile hypospadias when there is no penile skin available for safe reconstruction is described. An island of the most median hairless part of the scrotal skin around the median raphe was used as a skin flap based on its scrotal septal pedicle with an extended and free arc of rotation for repair of distant urethral defects. A novel repair of the distal urethra with glanular and preputial reconstruction is described for distal penile hypospadias; 52 consecutive patients underwent this procedure without catheter splintage or urinary diversion. Pad test, cystometry and analysis of micturition were performed in 36 women with stress incontinence before and a year after operation with either retropubic urethrocystopexy (n = 22) or pubococcygeal repair (n = 14). There was no difference in the subjective cure rate between the two groups of women (77% and 79%, respectively). We have reviewed the complications and follow-up in 6 patients who underwent kidney transplantation with cutaneous ureterostomy. Four patients had tuberculosis, 1 myelomeningocele and another congenital stenosis of the posterior urethra. Cutaneous ureterostomy was performed by suturing the edges of the distal end of the ureter to the skin. The mean follow-up was 40.6 months (range 2-105). A 47-year-old architect and a 30-year-old designer of audio goods presented with thrombosis of left and right posterior scrotal vein, respectively. The surgical management of sphincteric incompetence in patients with myelodysplasia is a formidable problem. In 10 female patients and 4 male patients with myelodysplasia a free graft of rectus fascia was used to suspend the urethra, termed the periurethral and puboprostatic sling procedures, respectively. Sixty-nine premenopausal and 53 postmenopausal women had a colposuspension operation for urinary stress incontinence. A significant postoperative reduction (p less than 0.001) of symptoms of frequency, nocturia, urgency and urge incontinence was obtained in both groups. Urodynamic measurements were performed before and after acupuncture at 2 different points in 52 women who presented with frequency, urgency and dysuria. An association has been noted between trisomy 18 and genitourinary abnormalities, with six previous reports of the prune-belly syndrome occurring in patients with trisomy 18. We have observed a 120-g fetus of 18 weeks' gestational age at autopsy in whom there was severe prune-belly syndrome and trisomy 18.
