Fulminant Eosinophilic Myocarditis in a Young Adult: Successful Recovery After Extracorporeal Membrane Oxygenation (ECMO) Support and Multisystem Complications.

Eosinophilic myocarditis (EM) is a rare inflammatory cardiomyopathy with a highly variable clinical presentation, ranging from mild symptoms to fulminant cardiogenic shock. Its diagnosis is challenging due to non-specific clinical findings and requires a high index of suspicion, with endomyocardial biopsy (EMB) serving as the gold standard. Early initiation of immunosuppressive therapy and, in severe cases, mechanical circulatory support (MCS) are essential to improve outcomes. We report the case of a 21-year-old male who presented with progressive dyspnea, chest pain, and hypoxemia. Echocardi

Association between cardiac magnetic resonance ventricular strain and left ventricular thrombus in patients with ST-segment elevation myocardial infarction.

BACKGROUND: Myocardial strain can analyze early myocardial dysfunction after myocardial infarction (MI). However, the correlation between left ventricular (LV) strain (including regional and global strain) obtained by cardiac magnetic resonance (CMR) imaging and left ventricular thrombus (LVT) after ST-segment elevation myocardial infarction (STEMI) is unclear. METHODS: The retrospective clinical observation study included patients with LVT (n = 20) and non-LVT (n = 195) who underwent CMR within two weeks after STEMI. CMR images were analyzed using CVI 42 (Circle Cardiovascular Imaging, Canad

Surgical Outcomes in Patients Undergoing a Double Switch Operation for Corrected Transposition.

BACKGROUND: Congenitally corrected transposition of the great arteries (CC-TGA) is a rare and complex form of congenital heart disease. Results of physiologic repair proved disappointing due to late right ventricular dysfunction and/or tricuspid regurgitation. The current study was performed to evaluate surgical outcomes in patients undergoing a double switch for CC-TGA. METHODS: This was a retrospective review of 121 patients who underwent a double switch over a 2-decade time frame (2002-2023). Patients were a median age of 32 months. Before the double switch, 49 of 121 patients (40%) had un

Effect of Cardiac Resynchronisation Therapy on Electrical Remodelling.

BACKGROUND: Cardiac resynchronisation therapy (CRT) is an accepted device therapy in patients with low ejection fraction. Beneficial effects of CRT result from mechanical remodelling. Some controversial reports suggest that CRT may also induce electrical remodelling with intrinsic QRS narrowing but still the effect of CRT on electrical remodelling is an issue for debate. The aim of our study was to evaluate the effects of CRT on intrinsic QRS duration. For clarity, our analysis was performed by the signal averaged electrocardiogram (SAECG) which is a high resolution electrocardiographic signal

Successful treatment of cardiac dysfunction due to left main trunk obstruction and severe acute aortic regurgitation secondary to acute type A aortic dissection using Impella: a case report.

BACKGROUND: Acute type A aortic dissection (A-AAD) with severe acute aortic regurgitation (AR) and coronary involvement is a potentially fatal condition that causes left ventricular volume overload and catastrophic acute myocardial infarction. We present the successful management of a patient using Impella 5.5 following cardiopulmonary arrest caused by A-AAD with severe acute AR and left main trunk (LMT) obstruction. CASE PRESENTATION: A 50-year-old man presented with acute anterior chest pain. The patient subsequently experienced a cardiac arrest, and veno-arterial extracorporeal membrane ox

Phase 1 Study of AAV9.LAMP2B Gene Therapy in Danon Disease.

BACKGROUND: Danon disease is a rare, X-linked, monogenic cardiomyopathy caused by mutations in the lysosomal-associated membrane 2 gene (LAMP2), which encodes the LAMP2 protein. In male patients, the predominant phenotype is progressive cardiac hypertrophy, cardiac dysfunction, and early death. There are no directed therapies for the disease. METHODS: In this phase 1 study, we evaluated the safety and efficacy of a single infusion of RP-A501, a recombinant adeno-associated virus serotype 9 containing the transgene LAMP2B, which encodes an isoform of LAMP2. The primary outcomes were the safety

Invasive evaluation of left ventricular diastolic performance.

Diastole can be divided into four phases: 1. isovolumic relaxation; 2. early filling; 3. diastasis; and 4. atrial systole. The amount of left ventricular (LV) filling that occurs during each of these phases depends on: 1. myocardial relaxation; 2. the passive characteristics of the LV; 3. the characteristics of the left atrium, pulmonary veins and mitral valve; and 4. the heart rate. When diastolic function is normal, the net effect of these factors results in LV filling sufficient to produce an adequate cardiac output, while mean pulmonary venous pressure is maintained below 12 mm Hg. Diastol

Fetal diagnosis of left-ventricular noncompaction cardiomyopathy in identical twins with discordant congenital heart disease.

Left-ventricular noncompaction (LVNC) is characterized by prominent myocardial trabeculae with deep intratrabecular recesses. Associated with ventricular dysfunction, LVNC can present in isolation or in conjunction with structural heart defects (i.e., "nonisolated" LVNC). Although it was once considered to be difficult and unreliable, successful detection of this condition by way of fetal echocardiography has been well documented. We present what is to our knowledge the first prenatal diagnosis of LVNC in monochorionic-monozygotic twins. Nonisolated LVNC was identified in twins A and B at 23 w

Symptoms and signs of heart failure in patients with myocardial infarction: reproducibility and relationship to chest X-ray, radionuclide ventriculography and right heart catheterization.

102 patients with myocardial infarction (MI) were examined by three clinicians, who independently recorded the following symptoms and signs: dyspnoea, a displaced apex beat, S3-gallop, rales, neck vein distension, hepatomegaly, and dependent oedema. Chest X-ray, radionuclide ventriculography, and (in 40 patients) right heart catheterization were carried out immediately after the physical examination. The clinicians frequently disagreed as to the presence of physical signs of heart failure in individuals. Moreover, these signs were of limited value in identifying patients with pulmonary vascula

Takotsubo cardiomyopathy triggered by wasabi consumption: can sushi break your heart?

Takotsubo cardiomyopathy is a left ventricular dysfunction that typically occurs after sudden intense emotional or physical stress and mimics myocardial infarction. We describe a case of a 60-year-old woman that presented to the emergency department with chest pain after she attended a wedding and ate a large amount of wasabi, assuming it to be an avocado. To the best of our knowledge, this is the first report of takotsubo cardiomyopathy triggered by wasabi consumption.

Refining risk stratification for primary prevention ICD therapy: Insights from cardiac MRI in the DO-IT registry.

BACKGROUND: Implantable cardioverter-defibrillators (ICDs) are recommended for the primary prevention of sudden cardiac death in patients with reduced left ventricular ejection fraction (LVEF). However, reliance on LVEF alone may inadequately identify patients at the highest risk. OBJECTIVE: This study aimed to evaluate the prognostic value of cardiac magnetic resonance imaging (CMR), particularly late gadolinium enhancement (LGE), in patients with ischemic cardiomyopathy (ICM) and nonischemic dilated cardiomyopathy (NIDCM). METHODS: This multicenter substudy of the Dutch Outcome in ICD Ther

[The role of aldosterone-antagonists in the treatment of congestive heart failure].

The role of aldosterone-antagonists in the treatment of congestive heart failure. Despite the advances of the treatment of congestive heart failure, nearly half of the patients diagnosed with this disease five years ago are alive today. Experimental and human studies have demonstrated, that under special pathologic condition, the heart extracts aldosterone, and aldosterone extraction in the heart stimulates increased collagen turnover culminating in ventricular remodeling. Aldosterone blockade has been shown to be effective in reducing total mortality and hospitalization for heart failure in p

Electromechanical window as an integrative marker of remodeling and arrhythmic risk in biventricular vs. left bundle branch area pacing.

BACKGROUND: The electromechanical window (EMW), defined as the difference between mechanical and electrical systole, is an emerging marker of arrhythmic risk and ventricular dysfunction. Although cardiac resynchronization therapy (CRT) improves mechanical synchrony, the influence of different pacing strategies on EMW is not well established. This study compared the effects of conventional biventricular (BiV) pacing and left bundle branch area pacing (LBBAP) on EMW, reverse remodeling, CRT response, and arrhythmic burden. METHODS: In this prospective, observational study, 80 patients with left

Cardiac afferent signaling partially underlies premature ventricular contraction-induced cardiomyopathy.

BACKGROUND: The mechanisms underlying premature ventricular contraction (PVC)-induced cardiomyopathy (PIC) remain unknown. Transient receptor potential vanilloid-1 (TRPV1) afferent fibers are implicated in the reflex processing of cardiac stress. OBJECTIVE: The purpose of this study was to determine whether cardiac TRPV1 afferent signaling promote PIC. METHODS: A PIC swine model (50% PVC burden) was created via an implanted pacemaker. We selectively depleted cardiac TRPV1 afferent fibers using percutaneous epicardial application of resiniferatoxin (RTX). Animals were randomized to PVC only (

Takotsubo Syndrome Following Pulmonary Vein Isolation With Pulse Field Ablation.

BACKGROUND: Takotsubo syndrome (TTS) is triggered by emotional or physical stress and characterized by transient left ventricular wall motion abnormalities in the absence of coronary culprit lesion. CASE SUMMARY: A 76-year-old woman underwent pulse field ablation (PFA) for paroxysmal atrial fibrillation. While ST-segment elevation and cardiac dysfunction were observed during the procedure, emergent coronary angiography showed no significant stenosis. The following day, left ventricular wall motion was almost normalized with T-wave inversion and prolonged QT interval. She was diagnosed with TT
