Regional left ventricular wall motion in pacing induced angina.

Left ventricular cineangiograms (30 degrees right anterior oblique projection) and high fidelity left ventricular pressure were recorded at rest in 10 individuals with normal hearts and at rest and during an episode of angina provoked by rapid atrial pacing in 14 patients with obstructive coronary artery disease. Angiograms were digitised frame by frame. The ventricular perimeter was divided into 40 segments and regional ventricular function was examined by means of isometric and contour plots and by the construction of segmental pressure-wall displacement loops. In 10 patients 12 abnormal res

How Does the Dose and Type of Exercise Impact Acute Cardiovascular Function in Healthy Individuals?

During aerobic exercise, the cardiovascular system is tasked with delivering oxygen to active skeletal muscle via muscle blood flow while regulating mean arterial blood pressure. The impact of aerobic exercise on acute cardiovascular function may be modulated by the dose and type of exercise. Acutely, dose is the product of exercise intensity and time, whereas exercise type may refer to common aerobic modalities like cycling, running, swimming, or rowing. Each modality is unique for its medium of completion as well as the implications on blood flow arising from the position of active muscle ma

Case report: A novel surgical technique for rapid valve-in-ring implantation into the native aortic annulus during left ventricular assist device implantation.

The implantation of a left ventricular assist device (LVAD) has become an essential requirement for managing patients with end-stage heart failure. However, aortic valve insufficiency is a contraindication for LVAD implantation in patients with end-stage heart failure, partly because of the decreasing efficiency of mechanical circulatory support and the eventual development of right ventricular failure. Herein, we present the first case of performing transcatheter aortic valve replacement in valve-in-ring along with LVAD implantation for the treatment of a 60-year-old male suffering from refra

Guidelines for genetic testing of inherited cardiac disorders.

Inherited gene variants have been implicated increasingly in cardiac disorders but the clinical impact of these discoveries has been variable. For some disorders, such as familial hypertrophic cardiomyopathy, long QT syndrome, and familial hypercholesterolaemia, genetic testing has a high yield and has become an integral part of family management. For other disorders, including dilated cardiomyopathy, arrhythmogenic right ventricular cardiomyopathy, Brugada syndrome, catecholaminergic polymorphic ventricular tachycardia, and atrial fibrillation, relatively less is known about the genes involve

Effect of delayed cord clamping and cord milking on cerebral oxygenation and cardiovascular function: a secondary analysis of the PCI trial.

Umbilical cord clamping management may affect cerebral oxygenation (rSO2C), but previous studies have only investigated effects in the first minutes of life. Our objective was to determine whether delayed cord clamping (DCC) and umbilical cord milking (UCM) differently affect cerebral oxygenation and cardiovascular function in the first 24 h of life in preterm infants. A post-hoc secondary outcome analysis of a multicentre prospective randomised clinical trial (PCI), conducted between April 2016 and February 2023 at 8 Italian neonatal intensive care units. The present ancillary study included

Evaluation of Long-Term Key Outcomes and Safety in Pulmonary Embolism: The EKOS-PE Study.

BACKGROUND: Pulmonary embolism (PE) is a major cause of morbidity and mortality, particularly in massive and submassive cases that lead to right ventricular (RV) strain and long-term complications. The EkoSonic Endovascular System (EKOS) offers a catheter-directed thrombolytic treatment option for patients with contraindications to systemic thrombolysis, but data on long-term outcomes remain limited. We aimed to evaluate long-term mortality, RV function, and quality of life (QoL) in patients with massive or submassive PE who were treated with EKOS therapy. METHODS: The EKOS-PE is a retrospect

Effect of ECMO Flow Variations on Pulmonary Capillary Wedge Pressure in Patients With Cardiogenic Shock.

BACKGROUND: The hemodynamic effects of femoro-femoral venoarterial (VA) extracorporeal membrane oxygenation (ECMO) on pulmonary capillary wedge pressure (PCWP) remain poorly defined. High ECMO flow is believed to increase PCWP and the risk of pulmonary edema; yet, supporting in vivo physiological data are lacking. OBJECTIVES: The purpose of this study was to evaluate the impact of incremental femoro-femoral VA-ECMO flow variations on PCWP, hemodynamic, and echocardiographic parameters in patients with cardiogenic shock during the early phase of VA-ECMO support, after stabilization. METHODS:

Prognostic significance of right ventricular dysfunction in patients presenting with acute left-sided heart failure.

BACKGROUND: The prognostic value of right ventricular (RV) function in chronic heart failure (HF) has lately been well established. However, research on its role in acute heart failure (AHF) is sparse. RESULTS: This study comprised 195 patients, aged between 18 and 80 years, with acute left-sided heart failure (HF) and a left ventricular ejection fraction (LVEF) < 50%. Patients with LVEF ≥ 50%, mechanical ventilatory or circulatory support, poor echocardiographic windows, prosthetic valves, congenital heart diseases, infective endocarditis, and/or life expectancy < 1 year due to non-cardiac c

Echocardiographic assessment for cardiopulmonary function in patients with congenital heart disease-related pulmonary arterial hypertension.

BACKGROUND: For patients with congenital heart disease-related pulmonary arterial hypertension (CHD-PAH), cardiopulmonary exercise testing (CPET) can reflect cardiopulmonary reserve function. However, CPET may not be readily accessible for patients with high-risk conditions or limited mobility due to disability. Echocardiography, on the other hand, serves as a widely available diagnostic tool for all CHD-PAH patients. This study was aimed to identify the parameters of echocardiography that could serve as indicators of cardiopulmonary function and exercise capacity. METHODS: A cohort of 70 pat

Simultaneous biventricular pressure-volume analysis in rats.

The pressure-volume (PV) analysis is used for an accurate assessment of load-independent cardiac function and is important for the study of cardiovascular diseases and various therapeutic modalities. PV analysis is often performed on one of the ventricles, or on both ventricles but sequentially. Since both ventricles interact with each other and their functions are mutually interdependent, especially in various disease conditions such as pulmonary hypertension or heart failure, it is important to quantify the function of both ventricles at the same time. Therefore, our aim was to describe a st

The effect of placental transfusion on hemodynamics in premature newborns: a randomized controlled trial.

UNLABELLED: Despite of growing evidence of the beneficial effects of placental transfusion techniques, there is no available sufficient data about their effects on vulnerable hemodynamics and myocardium of premature infants. The purpose of this work is to study ventricular functions and hemodynamics after applying different placental transfusion techniques, delayed cord clamping (DCC), cut cord milking (C-UCM), and intact cord milking (I-UCM). Sixty-four infants delivered whether by C-section or vaginal delivery were randomly assigned to undergo C-UCM (20-30 cm), I-UCM (3-4 strippings), and DC

Impact of intravenous ferric carboxymaltose on heart failure with preserved and reduced ejection fraction.

AIMS: Heart failure (HF) is a proinflammatory disease often associated with the onset of iron deficiency (ID). ID alters mitochondrial function, reducing the generation of cellular energy in skeletal muscle and cardiomyocytes. This study aimed to analyse the response of patients with HF to intravenous iron administration according to the type of HF: preserved ejection fraction (HFpEF) or reduced ejection fraction (HFrEF). METHODS AND RESULTS: We conducted a retrospective, single-centre study of 565 consecutive outpatients diagnosed with HF, recruited over 5 years, who were given intravenous f

Impact of Right Ventricular Geometry and Left Ventricular Hypertrophy on Right Ventricular Mechanics and Clinical Outcomes in Hypoplastic Left Heart Syndrome.

BACKGROUND: Right ventricular (RV) function is a major determinant of survival in hypoplastic left heart syndrome (HLHS). However, the relation of RV geometry to myocardial mechanics and their relation to transplant-free survival are incompletely characterized. METHODS: We retrospectively studied 48 HLHS patients from the Hospital for Sick Children, Toronto, (median age, 2.2; interquartile range, 3.62 years) at different surgical stages. Patients were grouped by the presence (n = 23) or absence (n = 25) of RV "apical bulging" defined as a sigmoid-shaped septum with the RV leftward apical segm
