Impact of vertebral exclusion on precision error and least significant change in Trabecular Bone Score measurement.

UNLABELLED: Trabecular Bone Score (TBS) provides indirect information on trabecular bone microarchitecture. TBS follow-up may be useful in patients receiving anabolic therapies and in those treated with denosumab for more than five years. However, TBS precision error and least significant change (LSC) thresholds should be specifically determined for each vertebral combination analyzed. AIM: To evaluate the impact of different vertebral combinations on TBS measurement precision and LSC. MATERIALS AND METHODS: A prospective study was carried out as part of the precision assessment program of a

Vertebral infection after kyphoplasty: distinguishing misdiagnosis from true postoperative infection and implications for management.

BACKGROUND: Spinal infection following percutaneous kyphoplasty (PKP) is rare but clinically devastating. However, its true incidence, diagnostic challenges, and optimal revision strategies remain poorly defined, particularly in referral-based cohorts. METHODS: We conducted a retrospective study comprising two components: (1) an institutional incidence analysis of 6,328 PKP procedures performed between 2015 and 2025, and (2) a pooled clinical cohort of 39 patients treated for post-PKP spinal infection, including 3 hospital-acquired cases and 36 referred cases. Patients were managed with eithe

Natural Language Processing to Improve Vertebral Fracture Identification in Older Adults Receiving Osteoporosis Therapy.

INTRODUCTION: Vertebral fracture identification using administrative codes is challenging because diagnoses often occur outside hospital and orthopedic care settings. Outpatient diagnoses improve capture but increase misclassification. Accurate methods are important for population-level research and care. OBJECTIVE: To develop an accurate natural language processing (NLP) algorithm to apply to radiology reports for vertebral fractures. METHODS: Adult members of an integrated health care delivery system who were aged 50-89 years at bisphosphonate initiation (1998-2019) and members 3 years lat

Disc collapse aggravates vertebral recollapse following percutaneous kyphoplasty: a clinical review and biomechanical study.

BACKGROUND: Promoting uniform load distribution is an essential biomechanical function of intervertebral discs (IVDs), but disc degeneration (DD) may compromise this and lead to localized stress concentration. However, whether DD can trigger stress concentration on the cement-augmented vertebral body and result in more severe augmented vertebral recollapse has yet to be determined. The aim of this study was to investigate clarify this issue and provide theoretical guidance regarding the mechanism underlying this complication. METHODS: Augmented vertebrae height was measured immediately after

Patient specific optimization and planning of vertebral augmentation in osteoporosis: Finite element and Artificial Intelligence approaches.

BACKGROUND: Osteoporotic Vertebral Compression Fractures (OVCFs) are responsible for significant disability, especially among individuals with low Bone Mineral Density (BMD). Although vertebral augmentation methods such as percutaneous vertebroplasty and kyphoplasty offer rapid pain relief and stability, the optimal combination of bone cement properties like volume, elasticity, and distribution remains uncertain. OBJECTIVE: This study aimed to optimize these parameters using a hybrid Finite element (FE) analysis and Artificial Intelligence (AI) framework integrating Artificial Neural Networks

Hemodynamic disturbances in vertebral artery dominance: a potential pathogenic mechanism in acute isolated pontine infarction.

BACKGROUND: Isolated pontine infarction (IPI) is closely linked to the hemodynamic properties of the vertebral artery. However, the underlying pathogenic mechanisms remain insufficiently defined. This study was designed to explore the contribution of vertebral artery hemodynamic disturbances to the development of IPI and identify the optimal hemodynamic cut-off values for predicting IPI risk in patients with vertebral artery dominance (VAD). MATERIALS AND METHODS: A total of 381 individuals were enrolled. These included 205 patients with acute IPI and 176 control individuals without cerebrovas

A cross-vertebrate brain protein interaction map identifies conserved neural and non-neural complexes.

Protein-protein interactions underlie core brain functions, including neurotransmitter release, receptor activation, and intracellular signaling. Here, we present a compendium of protein-protein interactions conserved across vertebrate brains, spanning over 300 million years of evolution. From 2,197 biochemical fractions across five vertebrate species, we identify over 81,000 high-confidence interactions among 6,108 conserved proteins. This interaction map (VerteBrain) reveals both regulatory and structural complexes, including extensive synaptonemal protein associations likely involved in int

A Metallic Screw Gun-Traumatic Vertebral Artery Dissection Injury: A Case Report and Literature Review.

Traumatic vertebral artery dissection (VAD) is a tear of the intimal layer, which may extend into the tunica media, resulting in blood tracking within the arterial wall and consequent luminal stenosis. It presents complex challenges for surgical and neurovascular management. The most commonly affected vertebral artery segments are V2 and V3, due to their anatomical vulnerability to mechanical stress during neck motion. Potential complications include ischemic stroke, thromboembolism, subarachnoid hemorrhage, and pseudoaneurysm, which necessitate early diagnosis and timely intervention using en

Contralateral middle cerebral artery embolization following transradial vertebral artery stenting: a case report.

BACKGROUND: Transradial vertebral artery (VA) stenting is an effective, and relatively safe treatment for symptomatic extracranial VA stenosis-the main cause of posterior circulation ischemia. Still, rare non-target anterior circulation cerebral embolisms can occur intraoperatively. CASE PRESENTATION: A 63-year-old male with prior cerebral infarction (NIHSS score 0, mRS score 1) presented with recurrent dizziness. Pre-admission CTA imaging confirmed severe (> 70%) bilateral VA ostial stenosis. The patient underwent right transradial VA stenting. Ten minutes post-procedure, he developed altere

Recurrent embolic cerebellar infarction due to bow hunter's syndrome of the non-dominant vertebral artery.

We describe a rare presentation of bow hunter's syndrome (BHS) involving the non-dominant vertebral artery (VA), leading to recurrent embolic cerebellar infarction. A male in his mid-70s presented with sudden-onset immobilising vertigo. MRI revealed acute cerebellar ischaemia with occlusion of the left superior cerebellar artery. Dynamic duplex ultrasonography and angiography revealed an occlusion of the left VA in the V2 segment during head rotation to the left due to an osteophyte protruding from the C3/C4 vertebrae. The patient was diagnosed with embolic strokes due to compression of the le

Endovascular Treatment of Bow Hunter's Syndrome Presenting With Acute Vertebral Artery Occlusion: A Case Report.

BackgroundBow Hunter's Syndrome (BHS) is a rare condition in which dynamic vertebral artery (VA) compression during neck rotation induces transient symptoms, such as vertigo, whereas major arterial occlusion is extremely uncommon. There is no standardized treatment for BHS, and management includes conservative, surgical, and rarely endovascular approaches. We report a unique case of BHS with acute VA occlusion successfully treated by mechanical thrombectomy followed by elective stent placement.Case ReportAn 87-year-old woman presented with right gaze deviation, dysarthria, right hemiparesis, a

Skeletons of swiftly swimming sharks: Three-dimensional analysis of lamniform vertebral morphology and mineral architecture.

Order Lamniformes consists of 15 extant shark species that are ecologically diverse and utilize different swimming modes and speeds. Family Lamnidae includes the shortfin mako, porbeagle, and white shark which are fast, athletic sharks that swim using oscillations confined to the caudal body and fin. Other lamniforms, like the common thresher shark (Alopiidae), sand tiger (Carchariidae), and basking shark (Cetorhinidae), swim via oscillations that begin anteriorly, impacting a greater proportion of the axial body. Swimming oscillations subject the body to repeated bending cycles, including the

Total en bloc resection of a giant cell tumour of the cervical spine involving the vertebral artery.

A woman in her 20s presented with neck pain and was diagnosed with a primary giant cell tumour (GCT) involving the C5 vertebral body and encasing the left vertebral artery (VA). Denosumab therapy was initiated to reduce tumour vascularity and facilitate resection. After preoperative embolisation of the left VA, total en bloc resection of the tumour was performed. Postoperatively, the patient experienced transient left upper extremity weakness, which improved significantly over time. Follow-up imaging showed successful bony fusion and no recurrence. This case highlights the importance of multid
