We describe three families to highlight the variability of expression and penetrance that can occur in the craniosynostoses. In two of the families, gene carriers were only identified in retrospect by looking at photographs of other family members. In the third family, identical twins were initially thought to be discordant for sagittal craniosynostosis until early skull x rays were examined and both were found to be affected. The dilemmas faced when counselling these families are discussed.

About a series of 33 patients operated in the ENT Depart. Prof. Soudant-La Pitié-Salpêtrière Hospital-Paris. Any emphasis was put on 11 cases of aspergillosis sinusitis. Most of the time, clinical signs are non specific. But X rays studies are of great help for the diagnosis. Treatment of chronic sinusitis of dental origin, particularly aspergillosis, remains surgical: caldwell-luc operation or endonasal approach. As a conclusion, the authors suggest systematic removal of all intrasinusal foreign body.

The induction of malignant transformation was examined in a standard promotion protocol in which BALB/3T3 cells were incubated continuously with tritiated water (3HOH) following acute treatment with various doses of either X-rays or benzo(a)pyrene (BP). In no case was there any evidence that protracted exposure to ionizing radiation from 3HOH enhanced the yield of transformants induced by the primary carcinogen over that predicted if the effects of the two agents were additive.

One hundred eighty-eight patients, 24% of those originally injected, were followed for 10-20 years after chemonucleolysis and discography. An attempt was made to correlate their clinical results with the degenerative changes seen on their follow-up x-rays. No relationship was found. Narrowing of the treated discs were routinely seen early, with little additional narrowing over the subsequent years. No long-term deleterious effects were seen. Discography without chemonucleolysis did not cause degenerative changes in a disc.

The harlequin-staining technique has been used to study, after PHA-stimulation, the cell proliferation of cow lymphocytes in culture and to assess the radiosensitivity in first mitosis cells. At the 48 h fixation time, only 34% of the cells are in first mitosis whereas 55% are already in second and 11% in third mitosis. The exposure of cow lymphocytes to 200 rad X-rays result in the production of 16% dicentric chromosomes in first mitosis cells.

Avulsion and posterior dislocation of the posterior dislocation of the posterior part of the vertebral apophysis is a rare condition, occurring mainly in adolescence. If the avulsed part is ossified it may be diagnosed with conventional X-rays including myelography. The present case shows that CT scan with sagittal reconstruction gives exact information of the nature of the lesion as well as its extension into the spinal canal, and that CT alone is sufficient for preoperative diagnosis.

Calcified caval thrombus should be considered in any infant or child where calcifications are noted in the high right retroperitoneal area on plain x-rays of the abdomen. Although typically bullet-shaped in configuration, the calcium distribution in the neonate may be atypical or incompletely developed, suggesting neuroblastoma. Definitive diagnosis can be made by inferior vena cavagram. As no deaths or complications have been attributed to the lesion in the cases thus far reported, no specific treatment is recommended.

Seventeen cases of neonatal intestinal obstruction due to inspissated milk curds were seen at this hospital in the 9 years 1964-1972. The obstruction started on the 2nd to 10th day of life. Half the babies passed blood from the rectum and in most the x-rays were diagnostic. All survived, 14 after operation and 3 after medical treatment with a Gastrografin enema. Incomplete absorption of solids, particularly the fat from cows' milk feeds, is suggested as the cause.

A 63-year-old woman with long-standing AIDS and previous Kaposi sarcomas of the lower limb presented to our consultation complaining of a painful left ring finger with pulp enlargement. X-rays revealed an osteolytic lesion of the distal phalanx. We suspected an isolated osseous Kaposi sarcoma and at surgery we found a hemorrhagic lesion with bone extension into the phalanx. Bone involvement is rare in Kaposi sarcoma and even rarer in patients without a cutaneous location.

We report the case of dicephalic conjoined twins discovered incidentally on a routine ultrasound at 24 weeks of gestation. There were 2 heads and a neck that fused with 1 thorax, but the spines continued all the way to the coccyx. The spines were connected medially by a fused rib, and laterally, there were ribs that went around the thorax in a more normal fashion. Antenatal ultrasound images are supplemented by postnatal photographs and x-rays.

Multiple fractures of the long bones can be seen in the intrauterine period as a result of disorders such as skeletal dysplasias or maternal trauma, but isolated intrauterine femoral fracture is an extremely rare condition. Congenital short femur should be kept in mind, especially during the intrauterine period in the differential diagnosis. We report a case diagnosed as isolated femoral fracture in the 34th gestational week with post-partum confirmation of the diagnosis by X-rays and physical examination.

Thymomas are tumours that rarely occur in children, are almost invariably benign, and are usually discovered incidentally in the anterior mediastinum on chest X-rays. Whereas in adults these tumours are often associated with myasthenia gravis and other autoimmune diseases, this occurrence is very rare in the paediatric population. Multiple localisation and/or extra-thoracic recurrence of thymomas in children also appears to be exceptional with no reported cases in the English literature. We report one rare paediatric case.

Wrist braces with, and without, thumb extensions (so-called "Futura" splints) were introduced into the Emergency Departments of Cork City Hospitals in July 2001. Constructed of rigid aluminium with a soft synthetic covering and Velcro strapping, they are designed for use in less severe upper limb injuries (such as wrist or thumb sprains or clinically suspected scaphoid fractures with normal X-rays). Their introduction coincided with a relaxation of the guidelines for immobilizing suspected scaphoid fractures.

Atlanto-axial fixed subluxation (Grisel's syndrome) is an uncommon complication of upper neck inflammatory processes and head and neck surgery. We present the case of a 6 year old patient who developed a Grisel's syndrome after an upper respiratory infection. X-rays and CT scans demonstrated a fixed C1 and C2 subluxation which was treated with reposition and application of a Halofixateur. We give a review of the pathogenesis, diagnosis and treatment of this rare syndrome.

Renal osteodystrophy may present with a wide spectrum of bone lesions, ranging from high bone turnover to low bone turnover. The authors present a case of multiple bone fracture in 12 year old boy with chronic renal failure. This boy was hospitalized because of retention of urea. The limping was observed by admision. In the X-rays of the long bones the multiple fracture were detected. The potential causes of these fractures and diagnostic problems were discussed.

Fifty-five percent of children with HIV infection, aged two months to ten years, were admitted at our hospitals because of respiratory conditions. Pulmonary complications found at admission in these children were lymphoid interstitial pneumonitis, Pneumocystis carinii pneumonia, fungal over-infection, tuberculosis, and bacterial complications. Also, non-specific infectious bronchial conditions, probably of viral origin. The most representative chest-X rays of these pulmonary conditions were analyzed; together with data from clinical records a clinico-radiological diagnosis was obtained.

Acute lateral ankle ligament sprains represent a very frequent examinated and treated injury in the daily traumatological routine. A cursory review of recent articles published on this subject gives a confusing picture regarding diagnosis and treatment of such injuries. An air of controversy surrounds the interpretation of diagnostic x-rays and the management of these common ankle problems. This review article is to summarize current thoughts of different aspects concerning the treatment of acute lateral ankle ligament sprains.

An 11-year-old girl developed cerebellar infarction presenting as a posterior fossa mass lesion after stretching and flexing her neck. Cerebral angiography demonstrated irregular narrowing of the right vertebral artery at the C2 level and x-rays of the upper cervical spine showed atlantoaxial subluxation with os odontoideum. She underwent surgical decompression with removal of infarcted tissue. The cerebellar infarction probably resulted from occlusion of the vertebral artery, followed by brain swelling due to recanalization.

Albers-Schönberg disease (osteopetrosis) is a rare condition, and it very seldom occurs that a patient requiring a hip endoprosthesis is also suffering from this disease. The report describes the difficulties which may confront a surgeon unprepared for such a case and the author illustrates the problem taking the case history of a 46-year-old patient with X-rays and histological tests as an example. The reasons for implantation of either a cemented or cementless endoprosthesis are discussed.

Isolated trans-carpal injuries in children are rare. This paper presents three cases of trans-carpal injury associated with an ipsilateral distal radial fracture. Two cases involved simultaneous fractures of the scaphoid and the capitate. The third case involved the scaphoid and the triquetrum. The full extent of these injuries was not recognized on the initial X-rays. If a child presents with a displaced fracture of one carpal bone it may not be an isolated injury.
