Emergence of methicillin resistance predates the clinical use of antibiotics.

The discovery of antibiotics more than 80 years ago has led to considerable improvements in human and animal health. Although antibiotic resistance in environmental bacteria is ancient, resistance in human pathogens is thought to be a modern phenomenon that is driven by the clinical use of antibiotics1. Here we show that particular lineages of methicillin-resistant Staphylococcus aureus-a notorious human pathogen-appeared in European hedgehogs in the pre-antibiotic era. Subsequently, these lineages spread within the local hedgehog populations and between hedgehogs and secondary hosts, includin

Polypharmacological drug actions of recently FDA approved antibiotics.

The current epidemic of antibiotic resistant bacterial infections has fueled the demand for novel antibiotics exhibiting both antibacterial efficacy and anti-drug resistance. This need has not been fully satisfied by the conventional "one target-one molecule" approach. Consequently, there has been rising interest in the development of multi-target antibiotics. Over the past two decades, 52% (14 out of 27) of the FDA approved antibiotics have demonstrated synergistic, multi-target mechanisms of action. Among these are three second-generation lipoglycopeptides, five new generation quinolones and

Short- and long-term predictors of spontaneous bacterial peritonitis in Singapore.

INTRODUCTION: Spontaneous bacterial peritonitis (SBP) is the commonest complication of liver cirrhosis. Timely and appropriate treatment of SBP is crucial, particularly with the rising worldwide prevalence of multidrug-resistant organisms (MDROs). We aimed to investigate the clinical outcomes of SBP in Singapore. METHODS: All cirrhotic patients with SBP diagnosed between January 2014 and December 2017 were included. Nosocomial SBP (N-SBP) was defined as SBP diagnosed more than 48 hours after hospitalisation. Clinical outcomes were analysed as categorical outcomes using univariate and multivar

Oral antibiotics for the treatment of Gram-negative bloodstream infections: A retrospective comparison of three antibiotic classes.

OBJECTIVES: Treatment of bacteraemia with oral antibiotics has the potential to reduce hospital length of stay, treatment costs and line-related complications. To date, small trials have supported the use of specific classes of antibiotics, primarily fluoroquinolones (FQs), in the treatment of Gram-negative bloodstream infections (GNBSIs). Currently, limited data exist evaluating treatment with β-lactams (BLs) or trimethoprim/sulfamethoxazole (SXT). The purpose of this study was to compare treatment of GNBSIs across three different oral antibiotic classes. METHODS: A retrospective cohort of h

Fight Against Antimicrobial Resistance: We Always Need New Antibacterials but for Right Bacteria.

Antimicrobial resistance in bacteria is frightening, especially resistance in Gram-negative Bacteria (GNB). In 2017, the World Health Organization (WHO) published a list of 12 bacteria that represent a threat to human health, and among these, a majority of GNB. Antibiotic resistance is a complex and relatively old phenomenon that is the consequence of several factors. The first factor is the vertiginous drop in research and development of new antibacterials. In fact, many companies simply stop this R&D activity. The finding is simple: there are enough antibiotics to treat the different types o

Characterization of antibiotic resistance genes in the species of the rumen microbiota.

Infections caused by multidrug resistant bacteria represent a therapeutic challenge both in clinical settings and in livestock production, but the prevalence of antibiotic resistance genes among the species of bacteria that colonize the gastrointestinal tract of ruminants is not well characterized. Here, we investigate the resistome of 435 ruminal microbial genomes in silico and confirm representative phenotypes in vitro. We find a high abundance of genes encoding tetracycline resistance and evidence that the tet(W) gene is under positive selective pressure. Our findings reveal that tet(W) is

Bacteria isolated from companion animals in Japan (2014-2016) by blood culture.

We aimed to identify microorganisms isolated by blood culture (BC) from companion animals and to determine antimicrobial resistance of these isolates during 2014-2016 at veterinary laboratory, in comparison with those during 2010-2013, in Japan. Clinical data (animal species, visiting animals/hospitalized animals, and others except for disease type and clinical course including history of antimicrobial agent use) on ill animals at veterinary clinics or hospitals were obtained. We retrospectively analyzed animal-origin BC results extracted from the database in 2014-2016 and those obtained in 20

Predictors of ertapenem therapeutic efficacy in the treatment of urinary tract infections (UTIs) in hospitalized adults: the importance of renal insufficiency and urinary pH.

In hospitalized adults acute uncomplicated cystitis (AUC) and catheter associated bacteriuria (CAB) may be treated with oral antibiotics. With AUC or CAB due to extended spectrum ß-lactamase (ESBL) + Gram negative bacilli (GNB) physicians often use intravenous therapy, e.g., ertapenem. We reviewed our recent experience in hospitalized adults with AUC and CAB treated with ertapenem. Therapeutic efficacy of ertapenem was assessed by decreased pyuria/bacteriuria, and elimination of the uropathogen. The effectiveness of ertapenem in the presence of renal insufficiency (CrCl < 50 ml/min) and acid a

Prevalence and antimicrobial resistance profile of Staphylococcus species in chicken and beef raw meat in Egypt.

Coagulase-positive (CPS) and coagulase-negative (CNS) staphylococci cause staphylococcal food poisoning. Recently, CPS and CNS have received increasing attention due to their potential role in the dissemination of antibiotic resistance markers. The present study aimed to evaluate CPS and CNS species distribution and their antibiotic resistance profile isolated from chicken and beef meat. Fifty fresh, uncooked chicken parts and 50 beef meat cuts (local n=27; imported n=23) were used. One hundred staphylococcal isolates belonging to 11 species were isolated and identified from chicken (n=50) and

Carbapenem-resistant Pseudomonas aeruginosa in Taiwan: Prevalence, risk factors, and impact on outcome of infections.

BACKGROUND: The prevalence and clinical impact on mortality of carbapenem-resistant Pseudomonas aeruginosa (CRPA) is unclear in Taiwan. We aim to clarify these clinical issues by using data from the Taiwan Surveillance of Antimicrobial Resistance (TSAR) program. METHODS: Patients from five hospitals with their P. aeruginosa isolates collected by TSAR II-VII (2000-2010) program were considered as the potential study population. All patients with CRPA were enrolled as case patients. Patients with carbapenem-susceptible P. aeruginosa were randomly selected in a 1:1 ratio to case patients as cont

Porin involvement in cephalosporin and carbapenem resistance of Burkholderia pseudomallei.

BACKGROUND: Burkholderia pseudomallei (Bps) is a Gram-negative bacterium that causes frequently lethal melioidosis, with a particularly high prevalence in the north and northeast of Thailand. Bps is highly resistant to many antimicrobial agents and this resistance may result from the low drug permeability of outer membrane proteins, known as porins. PRINCIPAL FINDINGS: Microbiological assays showed that the clinical Bps strain was resistant to most antimicrobial agents and sensitive only to ceftazidime and meropenem. An E. coli strain defective in most porins, but expressing BpsOmp38, exhibit

Antimicrobial resistance in human and animal pathogens in Zambia, Democratic Republic of Congo, Mozambique and Tanzania: an urgent need of a sustainable surveillance system.

A review of the published and unpublished literature on bacterial resistance in human and animals was performed. Sixty-eight articles/reports from the Democratic Republic of Congo (DRC), Mozambique, Tanzania and Zambia were reviewed. The majority of these articles were from Tanzania. There is an increasing trend in the incidence of antibiotic resistance; of major concern is the increase in multidrug- resistant Escherichia coli, Klebsiella pneumoniae, Staphylococcus aureus, Vibrio cholera, non-typhoid Salmonella and other pathogens responsible for nosocomial infections. The increase in methicil

Results of the antimicrobial agent susceptibility study raised in a representative, cross-sectional monitoring study on a national basis.

The use of antimicrobial substances in human and veterinary medicine inevitably results in a selection pressure for drug resistance in exposed bacteria. Preventive measures, apt to avoid the consequent development of new resistances and selection for existing ones, respectively, have to be elaborated. Moreover, it has to be ensured that neither resistant bacteria nor resistance genes are spread to and consequently via the food chain. Respiratory diseases as well as mastitis in dairy cattle belong to the most frequently occurring diseases in food-producing animals. For the first time in Germany

[Antimicrobial resistance of Streptococcus pneumoniae strains isolated in the Reunion Island during 2004].

OBJECTIVE AND METHOD: Available data from the Southern Reunion Island Medical Group was processed to assess the evolution of Streptococcus pneumoniae resistance to antibiotics since 1994 when the first penicillin-non-susceptible S. pneumoniae (PNSSP) was identified. In addition, 249 strains, isolated between 1998 and 2004, were tested against telithromycin and moxifloxacin. RESULTS: Between 1994 and 2004, the percentage of PNSSP increased from 0 to 59.2%. Among PNSSP, 13.9% were resistant strains in 2004 with MICs<4 microg/ml. Before 2001 the rate of resistance to penicillin was superior to 5

Antibiotic-Induced Manic Episodes: Clinical Recognition, Mechanistic Pathways, and Management.

Antibiotic-induced manic episodes describe the emergence of manic or hypomanic symptoms during or shortly after antimicrobial therapy. Although uncommon, these reactions are clinically important because they may be misattributed to primary bipolar disorder, delirium, or neuropsychiatric effects of infection. Hence, the objective of this study was to synthesize the evidence on antibiotic-induced manic episodes, with emphasis on clinical recognition, plausible mechanisms, management, and verified sequential citations. A targeted narrative review and reference-verification process was conducted u

Genomic signatures of host adaptation in Pasteurella multocida from poultry, cattle, swine, and cats: insights from interpretable machine learning.

Pasteurella multocida is a major zoonotic pathogen causing fowl cholera and other animal-derived infectious diseases, posing significant threats to the poultry industry and public health. Rapidly identifying its host preference is critical for disease control and understanding cross-species transmission. This study collected genomic data of P. multocida from four key hosts (chicken, cat, pig, cattle) in the NCBI Pathogen Database, yielding 814 qualified strains. Phylogenetic analysis showed host-correlated branching: cat strains formed a distinct clade, while cattle, pig, and chicken strains c

Prophylactic antibiotics to prevent postcesarean infection: which antimicrobial, when, how, and why?

Before the introduction of routine antibiotic administration, the rate of postcesarean infection exceeded 30% to 50%. Since the 1970s, postpartum infection occurs in approximately 1% to 2% of patients after vaginal birth and in 5% to 20% following cesarean delivery. Evidence from randomized clinical trials and systematic reviews has demonstrated that the administration of a single dose of antibiotics within 60 minutes before skin incision significantly reduces maternal infection-related morbidity without adverse neonatal outcomes. These results have informed clinical guidelines of professional
