Chronic nonprogressive headaches in children and adolescents.

Chronic nonprogressive headaches (CNPHA) are common in children and increase in frequency in adolescents. Features are usually, but not always, distinct from those of migraine. CNPHA have also been called chronic daily headaches, tension-type headaches, muscle contraction headaches, and psychogenic headaches. These headaches represent a diagnostic and therapeutic challenge to family physicians, pediatricians, and pediatric neurologists. The evaluation is time-consuming and the treatment frustrating and often unsuccessful. They are a significant cause of school absences. This review addresses t

Biological studies in depressed children and adolescents.

The objective was to review the literature on the biological correlates of major depressive disorder (MDD) in children and adolescents. A computerized search for articles published during the last 20 years was done and selected studies presented. To date, examination of growth hormone (GH), prolactin, and cortisol levels after pharmacological stimulation have shown abnormalities in the secretion of these hormones (e.g. blunted GH secretion after the administration of growth hormone releasing hormone). Identical results have been found in never-depressed children at high risk to develop MDD due

An analysis of burst fractures of the spine in adolescents.

Burst fractures are less common in children than in adults because of the greater mobility and elasticity of the pediatric spine. Because of these spine characteristics, these fractures may behave differently in childhood than in adulthood. To try to address these differences, we reviewed our experience with 11 children (5 boys, 6 girls) treated for burst fractures. Average age at time of injury was 14.4 years. Follow-up averaged 9 years. All fractures were categorized using the Denis classification system for burst fractures. Three children had associated spinal cord injury. Five children wer

Chlorhexidine varnishes prevent gingivitis in adolescents.

BACKGROUND: Chlorhexidine, a cationic anti-microbial agent, prevents gingivitis by inhibiting plaque accumulation. Gingivitis is the most predominant form of periodontal disease in children and adolescents. The purpose of this study was to determine the effects of a chlorhexidine varnish on the gingival status of adolescents. METHODS: The subjects of this study were 10-15 years old adolescents randomly divided into a control (C) and a treatment (T) group, n = 53 and n = 57, respectively. After restoration of all caries lesions, a prophylaxis was given to both C and T groups. A 10 percent chlo

Pemphigus vulgaris in adolescence: case report.

Pemphigus vulgaris is an uncommon mucocutaneous disease caused by autoantibodies against desmosomal antigens. It affects mainly middle-aged adults, and juvenile cases are rare. The authors present a case of pemphigus vulgaris in adolescence and review the literature. A 16-year-old girl showed oral and cutaneous lesions suggestive of pemphigus vulgaris. Histopathology and direct immunofluorescence of the oral mucosa confirmed the diagnosis and systemic steroid therapy was efficient in controlling the disease. The recognition of the oral lesions of pemphigus by the clinician, its early diagnosis

HIV postexposure prophylaxis for children and adolescents.

HIV postexposure prophylaxis (PEP) is now a well-established part of the management of health care workers after occupational exposures to HIV. Use of PEP for adults exposed to HIV after sexual contact or injection drug use in nonoccupational settings remains controversial with limited data available. There is even less information available concerning HIV PEP for children and adolescents after accidental needlestick injuries or sexual assault. The objective was to describe the current practice of and associated problems with HIV PEP for children and adolescents at an urban academic pediatric

Screening adolescents with mental retardation for depression.

This study investigated the reliability and validity of the Children's Depression Inventory (CDI) using 27 adolescents with mild mental retardation attending schools in a large metropolitan area. Cronbach's alpha was 0.79 and the test/retest correlation was 0.90 at 10 days to 2 weeks, indicating adequate reliability. Scores on the self-report CDI were compared with scores on an observer-completed inventory of psychopathology filled out by teachers and other staff, the Reiss Scales for Children's Dual Diagnosis. For female students, statistically significant negative associations were found bet

Personality in adolescents with chronic fatigue syndrome.

Our aim was to study the presence of personality traits and disorder in adolescents with Chronic Fatigue Syndrome (CFS). Personality was then compared to other measures of functioning such as presence of psychiatric disorder and rating on the Child Behavior Checklist 4-18 (CBCL) and in relation to CFS outcome. Twenty-five adolescents with CFS followed-up after contacts with tertiary paediatric/psychiatric clinics were compared with 15 matched healthy controls. Interviews and questionnaires from parents and youngsters included Personality Assessment Schedule (PAS), Kiddie-SADS Psychiatric Inter

Human immunodeficiency virus infections in adolescents.

Human immunodeficiency virus (HIV) infection, once largely confined to adolescents with hemophilia, has come to involve the general adolescent population. Individuals younger than 22 years comprise up to 25% of the people newly infected with HIV. Unsafe sexual practices, intravenous drug abuse, homelessness, psychiatric disorders, and inadequate psychosocial support are only some of the factors that have contributed to the epidemic. Present data indicate that interventions have had a positive impact on adolescent behavior in preventing the continuing spread of the disease, but more needs to be
