Comparison of atracurium and "mini-dose" succinylcholine for preventing succinylcholine-induced muscle fasciculations: a randomized, double-blind, placebo-controlled study.

UNLABELLED: Several drugs have been used to prevent or attenuate succinylcholine- induced muscle fasciculations. There are possible advantages to succinylcholine pretreatment. We designed the present study to compare the effectiveness of atracurium versus "mini-dose" (5 mg) succinylcholine pretreatment for the prevention of muscle fasciculations. METHODS: Under standard monitoring, 79 patients were randomly assigned to three groups after premedication: Group 1 (n = 26) received normal saline as a placebo, Group 2 (n = 27) received 0.03 mg/kg atracurium, and Group 3 (n = 26) received 5 mg succ

Repeated vertebrobasilar thromboembolism in a patient with severe upper cervical instability because of rheumatoid arthritis.

BACKGROUND CONTEXT: Although many reports have examined upper cervical rheumatoid arthritis (RA) and spinal cord disorders resulting from RA lesions, few cases of thromboembolic events in the vertebrobasilar system associated with RA lesions of the upper cervical spine have been reported. PURPOSE: We encountered a rare case of repeated vertebrobasilar thromboembolism with severe upper cervical instability resulting from RA. Furthermore, we obtained clinical images of the vertebrobasilar system just before and after the first thromboembolic event. We thus present the case of a patient with RA

Is primary anastomosis safe in the surgical management of complications of acute diverticulitis?

The purpose of this article is to determine whether primary anastomosis is a safe option in the surgical management of complications of acute diverticulitis in low-risk patients. Over the past century, the management of diverticulitis has evolved from a three-stage procedure to resection and primary anastomosis. In the beginning of the century, Mayo described drainage and proximal colostomy, a three-stage procedure. This was done by performing a diverting colostomy but leaving the diseased segment of colon, hoping that the inflammation would subside. Later, the patient went back for resection

[Iatrogenic spinal epidermoid tumors. A late complication of spinal puncture].

INTRODUCTION. Epidermoid tumors in the spinal canal are rare. Whether congenitally or iatrogenically caused, they form as the result of epidermal cells implanted within the spinal channel. Such implantation can occur during a variety of procedures and events such as bullet wounds, surgery, myelography or punctures for diagnosis, anesthesia or treatment. Although this complication is not discussed in books or journals on anesthesiology, we have found it mentioned in over 100 published cases reporting iatrogenically caused spinal epidermoid tumors. ETIOPATHOGENESIS. Iatrogenic epidermoid tumors

[Current issues in the endoscopic treatment of tracheobronchial foreign bodies in pediatric age: personal observations].

The present paper reports some organizational hypotheses and presents personal experience in the endoscopic management of tracheobronchial foreign bodies in children. Many changes have taken place in endoscopy and anesthetic techniques over the last twenty years and these are reviewed. The present experience is based on data for 42 pediatric patients who had undergone tracheobronchial endoscopy from 1974 to 1994. Within this time lapse 3 separate periods have been identified-1974-1978, 1979-1990 and 1991-1994-for which there was a progressive improvement in endoscopic techniques. During the fi

Management of perioperative dysrhythmias.

This paper reviews cardiac dysrhythmias occurring in the perioperative period. Electrocardiography was the first application of electronic monitoring to anesthesia care. The detection of dysrhythmias remains the most important use of this technology today. While the description of dysrhythmias dates back to the early 1900's, the first large series was reported in 1936. Early descriptions of the kinds seen and the predisposing factors have changed little in the past 50 years. Several factors tend to emerge when one evaluates perioperative dysrhythmias. These are the anesthetic given, the site o

Development and Validation of the Shoulder Arthroplasty Risk of Non-Home Discharge (SHORD) Score: A Risk Stratification Model for Non-Home Discharge Following Primary Total Shoulder Arthroplasty.

BACKGROUND: Non-home discharge (NHD) after total shoulder arthroplasty (TSA) increases cost, complications, and readmission, yet most existing risk tools depend on postoperative data or are not built to directly estimate the risk of this outcome. We developed and validated the Shoulder Arthroplasty Risk of Non-home Discharge (SHORD) Score, a purely preoperative bedside tool. METHODS: We carried out a retrospective prognostic model development and validation study of 29,382 patients undergoing primary TSA in the American College of Surgeons National Surgical Quality Improvement Program (ACS-NS

Early to mid-term outcomes of the custom-made 3D-printed triflange acetabular component (aMace) : the largest UK dual-centre study.

AIMS: Various techniques have been described to manage massive acetabular bone loss in revision hip arthroplasty. The aMace system has gained popularity due to its custom design, which aims to restore the hip centre of rotation and provide initial mechanical stability in cases of large, non-contained defects. This study aimed to assess the short- and mid-term outcomes of patients who underwent revision surgery using the aMace system. METHODS: A retrospective review was conducted of all patients who underwent aMace implantation at two revision centres between June 2013 and October 2022, with a

Long-term disease-free survival in young-onset rectal cancer patients surgically treated with curative intent: a retrospective cohort study.

PURPOSE: Although colorectal cancer incidence is declining among individuals aged over 50 years, possibly due to screening, young-onset rectal cancer (YORC) in those under 50 years of age is increasing. YORC is often diagnosed at advanced stages, leading to worse outcomes and higher mortality. This study compared short- and long-term clinical, surgical, and pathological outcomes, including survival, between patients with YORC (< 50 years) and older age groups. METHOD: A retrospective single-center study of patients undergoing curative-intent resection for rectal cancer at a Norwegian universi
