Unilateral megalencephaly: hamartoma or neoplasm?

The clinical and pathologic findings in three cases of unilateral megalencephaly, showing a spectrum of mild to severe involvement, are presented. The first case demonstrated an increase in neuronal size and mild gliosis and is contrasted with the second and third cases, which showed a progressive enlargement and bizarre appearance of neurons associated with an increase in number and size of astrocytes. The relationship of these cases to developmental and neoplastic disease in the brain is discussed.

Elevated brain natriuretic peptide (BNP) is an early marker for patients at risk for complications after cytoreductive surgery and hyperthermic intraperitoneal chemotherapy (CRS + HIPEC).

BACKGROUND: Elevated BNP is associated with adverse cardiac outcomes after noncardiac surgery. We assessed BNP values as markers of perioperative fluid status and their correlation with major/cardiopulmonary (CP) complications following CRS + HIPEC. METHODS: Fluid balance, BNP levels, and morbidity data were collected for all patients undergoing CRS + HIPEC between 6/2014 and 2/2016. RESULTS: One hundred and twenty-nine patients underwent CRS + HIPEC for appendiceal adenocarcinoma (n = 99), mesothelioma (n = 16), and colon cancer (n = 14). Less than 10% had CP comorbidities. The median PCI w

Type17 T-cells in central nervous system autoimmunity and tumors.

Interleukin-17 (IL-17) producing Type17 T-cells, specifically T-helper (Th)17 cells reactive to central nervous system (CNS) autoantigens, manifest a higher migratory capability to the CNS parenchyma compared with other T-cell subpopulations due to their ability to penetrate the blood brain barrier (BBB). In the field of cancer immunotherapy, there are now a number of cell therapy approaches including early studies using T-cells transduced with chimeric antigen receptors in hematologic malignancy, suggesting that the use of T-cells or genetically modified T-cells could have a significant role

Intraoperative neurophysiology for surgery in and around the brainstem: role of brainstem mapping and corticobulbar tract motor-evoked potential monitoring.

INTRODUCTION: New advancements of intraoperative neurophysiology for surgery in and around the brainstem have been described. NEUROPHYSIOLOGICAL TECHNIQUES: Brainstem mapping (BSM) is applied to locate cranial nerves and their motor nuclei (CMN) on the floor of the fourth ventricle. Corticobulbar tract (CBT) motor-evoked potential (MEP) monitoring is used to achieve on-line monitoring of the cranial motor nerves' functional integrity. DISCUSSION: Each of these procedures bears a specific role: BSM can help avoid direct damage to CMNs on the fourth ventricular floor; CBT-MEP can provide simult

Musical hallucinations following insular glioma resection.

Hallucinations can be auditory, visual, tactile, gustatory, or olfactory, and can be caused by psychiatric (such as schizophrenia and depression), neurological (such as cerebrovascular accidents, neoplasia, and infection), or endocrine and metabolic disorders. Musical hallucinations related to neurological disorders are rare. The authors present a case of a patient with a right insular glioma who developed transient musical hallucinations after microsurgical resection of the tumor.

Cladophialophora bantiana and Candida albicans mixed infection in cerebral abscess of an HIV-negative patient.

Cladophialophora bantiana is a black fungus recorded rarely as a cause of brain abscess. We describe the first case seen in Vietnam. A 35-year-old, previously healthy male, HIV negative, came to the hospital with a clinical diagnosis of brain tumor. He underwent surgery to remove the well-delineated circumscribed lesion. Pathology, directed physical examination and mycological examination revealed an abscess due to Cladophialophora bantiana, and Candida albicans. After two weeks of treatment with Fluconazole 400 mg per day intravenously, the clinical response remained unsatisfactory. Therapy w

The pyramidal tract has a predictable course through the centrum semiovale: a diffusion-tensor based tractography study.

PURPOSE: To identify reproducible anatomical landmarks that would allow predicting the course of the pyramidal tract (PT) through centrum semiovale. MATERIALS AND METHODS: A total of 20 normal volunteers (12 males, eight females) with a mean age of 34 years (range, 20-59 years) were scanned using a 1.5-Tesla clinical MR unit to assess the trajectory of the PT. Neuroanatomical characteristics of the PT passing through the centrum semiovale were assessed by two independent observers. Tractography data of 10 consecutive patients with brain tumor were used to test the precision of anatomy-based p

HHV-8/KSHV is not associated with AIDS-related primary central nervous system lymphoma.

Primary central nervous system lymphoma (PCNSL) is a major complication of the late stages of human immunodeficiency virus (HIV) disease. Epstein Barr virus (EBV) infection is the only genetic lesion consistently associated with this neoplasia. Recently, it has been proposed that the pathogenesis of AIDS-related PCNSL (AIDS-PCNSL) may be associated with infection by human herpesvirus-8/Kaposi's sarcoma associated herpesvirus (HHV-8/KSHV), although at present such association remains controversial. In order to conclusively assess the link between HHV-8/KSHV infection and AIDS-PCNSL, we performe

MR imaging in cerebral gliomas. Tissue component analysis in correlation with histopathology of whole-brain specimens.

A comparative analysis between MR examinations and histopathologic whole-brain sections regarding tumour components was performed in 5 brain specimens from patients with malignant glial brain tumours. All cases were examined with MR imaging in vitro and in 2 cases a close comparison with the MR examinations in vivo was also possible. The most homogeneous hypercellular area in malignant gliomas, giving the highest tumour grade, was not visualised on MR imaging as an isolated entity, either in vitro or in vivo. The most conspicuous tumour component, reflecting the heterogeneity of malignant glio

[Neuropeptide factors of pathogenesis of brain-related motor disorders].

Overall 136 patients with cerebral tumors, sequels of craniocerebral injury and brain strokes with motor disorders of varying intensity were examined. Factors of the peptide nature, provoking postural asymmetry of the homolateral hind limb in an experimental animal were detected in the blood and CSF of the overwhelming majority of the patients. Provided the compensatory process runs a favorable course (recovery of motor functions), the neuropeptide factors are inactivated with inactivating substances. The data obtained attest to the advisability of further studies into the activity and nature

CT-scan or two-dimensional echoencephalography in neonatal intracranial pathology?

The authors compare advantages and disadvantages of CT-scan and two-dimensional echoencephalography in examination of intracranial space of small babies. CT is more precise and complex, but it requires anesthesia and transport (CT-scanners are only in regional centers), moreover the child is endangered by irradiation. Echoencephalography is not so descriptive, but is noninvasive, simple, mobile, cheaper, enables to follow the dynamics of the process, examination in three planes and can be very useful during the operation.

Assessment of brain perfusion with MR imaging.

A technique for assessing brain perfusion with magnetic resonance (MR) imaging is described. This technique uses two spin echo sequences that are identical except that the second is sensitised to blood flow by use of a pair of unipolar gradients on either side of the 180 degree pulse. Differences in phase between the two sequences are displayed with a sensitivity to flow rates of +/- 0.5, +/- 1, and +/- 2 mm/s per full scale (+/- pi radians) deflection. The technique was validated for measurement of flow at these rates with a water phantom. Ten patients with cerebrovascular disease, multiple s

Epigenetic personalized medicine in glioblastoma.

Glioblastoma is a rare tumor that is responsible for 4% of all cancer deaths and is thus considered one of the deadliest human tumors. The new biologic drugs that have met success in many solid and hematologic malignancies constantly failed in glioblastoma, even though it harbors the same molecular alterations successfully hit in other tumors. The last advancement in glioblastoma therapy was in 2005, when it was shown that a subset of patients has a survival benefit from a multimodal combined therapy that includes irradiation and chemotherapy with the alkylating agent temozolomide. The patient

Robotic systems in cranial surgery: a systematic review of accuracy, clinical outcomes, and adverse events.

Robotic-assisted surgery is becoming more prevalent in neurosurgery. Many studies have classified the advantages and disadvantages, particularly in spinal robotic surgeries. However, the use of robotics in cranial procedures has been limited to individual studies without large-scale review of the different modalities. This study is a comprehensive systematic review of the major robotics utilized in cranial neurosurgical procedures and their respective outcomes. Following Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, a systematic review was conducted ov

Exploring Novel Compounds and Nanocomplexes to Target Cellular and Molecular Interactions in the Glioblastoma Microenvironment.

Glioblastoma is the most lethal primary brain tumor, characterized by a profoundly immunosuppressive microenvironment. This stroma includes various immunosuppressive and tumor-killing cells. This review critically evaluates emerging strategies for reprogramming the glioblastoma microenvironment to restore antitumor immunity. We focus on novel small molecules, natural products, adjuvants, and nanomedicine platforms that can remodel immunosuppressive microenvironment of glioblastoma. Preclinical studies demonstrate that therapy modalities can induce various forms of cell death and the release of

Clinical practice guidelines for the diagnosis and treatment of diffuse glioma-related epilepsy: 2025 update.

Diffuse glioma-related epilepsy (dGRE) frequently presents with epilepsy as the initial symptom and is closely associated with tumor progression or recurrence, imposing significant social and psychological burdens on patients. The pathogenesis of dGRE is highly complex, involving both peritumoral microenvironmental mechanisms and tumor-intrinsic factors. Diagnosis requires a comprehensive approach integrating neuroimaging, EEG, molecular biomarkers, and spatial correlation between the tumor and the epileptogenic zone. Management aims to control seizures and improve prognosis. Non-enzyme-induci

Gut microbiome-mediated modulation of the glioblastoma tumor microenvironment for enhanced immunotherapy response: Mechanistic insights and future perspectives.

Glioblastoma (GBM) is known to be one of the most aggressive and deadly brain tumors in adults, with a very poor prognosis. An immunosuppressive tumor microenvironment, the blood-brain barrier's (BBB's) protective nature, and genetic heterogeneity mediate resistance to conventional treatments, such as immune checkpoint inhibitors. Recent studies have shed light on the important role of the gut-brain axis in regulating GBM pathogenesis. Studies have demonstrated that patients with GBM frequently exhibit gut dysbiosis, with limited beneficial microbial populations, thereby enhancing immunosuppre
