A small-molecule inhibitor of the Wnt pathway (SM04690) as a potential disease modifying agent for the treatment of osteoarthritis of the knee.

OBJECTIVES: Osteoarthritis (OA) is a degenerative disease characterized by loss of cartilage and increased subchondral bone within synovial joints. Wnt signaling affects the pathogenesis of OA as this pathway modulates both the differentiation of osteoblasts and chondrocytes, and production of catabolic proteases. A novel small-molecule Wnt pathway inhibitor, SM04690, was evaluated in a series of in vitro and in vivo animal studies to determine its effects on chondrogenesis, cartilage protection and synovial-lined joint pathology. DESIGN: A high-throughput screen was performed using a cell-ba

The cast of clasts: catabolism and vascular invasion during bone growth, repair, and disease by osteoclasts, chondroclasts, and septoclasts.

Three named cell types degrade and remove skeletal tissues during growth, repair, or disease: osteoclasts, chondroclasts, and septoclasts. A fourth type, unnamed and less understood, removes nonmineralized cartilage during development of secondary ossification centers. "Osteoclasts," best known and studied, are polykaryons formed by fusion of monocyte precursors under the influence of colony stimulating factor 1 (CSF)-1 (M-CSF) and RANKL. They resorb bone during growth, remodeling, repair, and disease. "Chondroclasts," originally described as highly similar in cytological detail to osteoclasts

From cells to organs: progress and potential in cartilaginous organoids research.

While cartilage tissue engineering has significantly improved the speed and quality of cartilage regeneration, the underlying metabolic mechanisms are complex, making research in this area lengthy and challenging. In the past decade, organoids have evolved rapidly as valuable research tools. Methods to create these advanced human cell models range from simple tissue culture techniques to complex bioengineering approaches. Cartilaginous organoids in part mimic the microphysiology of human cartilage and fill a gap in high-fidelity cartilage disease models to a certain extent. They hold great pro

Common Pitfalls in Subtalar Joint Preparation for Arthrodesis via Sinus Tarsi Approach.

This cadaveric study assessed factors that could lead to nonunions in subtalar joint arthrodesis. The purpose of this study was to assist surgeons in achieving sufficient joint preparation in hopes to achieve more frequent arthrodesis of the subtalar joint. We evaluated the influence of experience in regard to cartilage preparation of the joint. We also assessed which quadrants of the subtalar joint were more likely to have unprepared cartilage. The subtalar joints in 17 cadaveric limbs were prepared by 17 participants with differing levels of experience. After the cartilage was denuded, the p

Management of Labral and Chondral Disease in Hip Preservation Surgery.

The techniques utilized for the management of articular cartilage and labrum injuries during hip preservation surgery have changed dramatically recently. Conservative treatment may involve image-guided injection of cortisone or viscosupplementation in conjunction with oral NSAIDs and physical therapy. Damage to the labrum runs a broad spectrum, and the treatments are individualized, but span from debridement to repair and reconstruction. The overarching goal of labral treatment is to restore the native functions of the labrum to allow for more normal biomechanical function. Similarly, cartilag

Cleft lip, nose, and palate: the nasal septum as the pacemaker for midfacial growth.

The need to be aware of the dynamics of cartilage development and growth is encountered by surgeons whenever they attempt to correct craniofacial defects such as unilateral or bilateral cleft lip/cleft palate or midfacial injuries after trauma. Within the craniofacial region, the nasal septal cartilage and the sphenoethmoidal and sphenooccipital cranial synchondroses are distinguished from other craniofacial cartilages in possessing intrinsic growth potential. Indeed, growth of the nasal septal cartilage outstrips the growth of other skeletal and soft tissues in the midface to such an extent t

Come together over me: Cells that form the dermatocranium and chondrocranium in mice.

Most bone develops either by intramembranous ossification where bone forms within a soft connective tissue, or by endochondral ossification by way of a cartilage anlagen or model. Bones of the skull can form endochondrally or intramembranously or represent a combination of the two types of ossification. Contrary to the classical definition of intramembranous ossification, we have previously described a tight temporo-spatial relationship between cranial cartilages and dermal bone formation and proposed a mechanistic relationship between chondrocranial cartilage and dermal bone. Here, we further

Influence of the Mechanical Environment on the Regeneration of Osteochondral Defects.

Articular cartilage is a highly specialised connective tissue of diarthrodial joints which provides a smooth, lubricated surface for joint articulation and plays a crucial role in the transmission of loads. In vivo cartilage is subjected to mechanical stimuli that are essential for cartilage development and the maintenance of a chondrocytic phenotype. Cartilage damage caused by traumatic injuries, ageing, or degradative diseases leads to impaired loading resistance and progressive degeneration of both the articular cartilage and the underlying subchondral bone. Since the tissue has limited sel

BMP2 induces chondrogenic differentiation, osteogenic differentiation and endochondral ossification in stem cells.

Bone morphogenetic protein 2 (BMP2), a member of the transforming growth factor-β (TGF-β) super-family, is one of the main chondrogenic growth factors involved in cartilage regeneration. BMP2 is known to induce chondrogenic differentiation in various types of stem cells in vitro. However, BMP2 also induces osteogenic differentiation and endochondral ossification in mesenchymal stem cells (MSCs). Although information regarding BMP2-induced chondrogenic and osteogenic differentiation within the same system might be essential for cartilage tissue engineering, few studies concerning these issues h

Enhancing Biological and Biomechanical Fixation of Osteochondral Scaffold: A Grand Challenge.

Osteoarthritis (OA) is a degenerative joint disease, typified by degradation of cartilage and changes in the subchondral bone, resulting in pain, stiffness and reduced mobility. Current surgical treatments often fail to regenerate hyaline cartilage and result in the formation of fibrocartilage. Tissue engineering approaches have emerged for the repair of cartilage defects and damages to the subchondral bones in the early stage of OA and have shown potential in restoring the joint's function. In this approach, the use of three-dimensional scaffolds (with or without cells) provides support for t

The involvement of subchondral mineralized tissues in osteoarthrosis: quantitative microscopic evidence.

This paper reviews evidence for the role of subchondral bone and calcified cartilage in the initiation and progression of osteoarthrosis (OA). There is consensus that OA is characterized by subchondral sclerosis, but disagreement about whether bone changes are concurrent with, primary to, or secondary to cartilage deterioration. Clinical observation suggests that bone density and cartilage fibrillation are inversely related. Evidence from the rabbit impulsive loading model is consistent with early bone changes, but evidence from other models of subchondral stiffening, such as the sheep metalli

Microfracture and Microfracture Plus.

Articular cartilage damage remains a significant cause for early osteoarthritis in adolescents and young adults. After chondroplasty alone, the mainstay procedure for cartilage injuries is microfracture. Although in small lesions this may be successful long-term, positive results of treating larger lesions this way are less certain. This inconsistency in outcomes has led to augmentation of these defects with scaffolding for autograft regeneration or for allograft cartilage to fill the defect with a hyaline cartilage. This discussion includes current techniques for the addition of scaffolding t

Benign chondrolipomatous tumor of the human female breast.

A benign chondrolipomatous breast tumor occurred in a 66-year-old woman. The preoperative diagnosis, based on mammography and xeroradiography, was fibroadenoma. Grossly, the demarcated lesion resembled a fibroadenoma with islands of cartilage projecting from its cut surface. Histologically, it was composed of benign mature fibrous stroma, fat, breast ducts, and islands of mature cartilage. Most cartilage-containing tumors of the human breast are associated with primary mammary malignant neoplasms. A few of the benign cartilage-containing tumors reported in the literature are discussed. The las

Bacterial arthritis. A staphylococcal proteoglycan-releasing factor.

We report the effect of staphylococci on induction of proteoglycan release from cartilage in organ culture and document the presence of a staphylococcal proteoglycan-releasing factor in the bacterial growth medium. Staphylococci cocultured with cartilage increased release of proteoglycan 3-4-fold within 48 hours. Viable cartilage was essential for maximum loss of proteoglycan. Killed bacteria had no effect on proteoglycan release. Four Staphylococcus aureus strains yielded growth media preparations active in proteoglycan release. Analysis of the proteoglycan subunits released from cartilage sh

Regulation of tumor necrosis factor-alpha and tumor necrosis factor converting enzyme in human osteoarthritis.

A snake venom-like protease isolated by a differential display screen between normal and osteoarthritis (OA)-affected cartilage (designated as cSVP) has a cDNA sequence identical to tumor necrosis factor (TNF)alpha convertase enzyme (TACE) and belongs to the adamalysin group of proteases. It has unique structural properties and when expressed in baculovirus, cleaves preferentially proTNFalpha to TNFalpha. The OA-affected cartilage has upregulated mRNA for TNFalpha and TACE as compared to normal cartilage. TNFalpha and TACE regulate inflammatory mediators in OA-affected cartilage which can be i

Articular shear of the anterior-inferior quadrant of the glenoid: a glenolabral articular disruption lesion variant.

Traumatic cartilage lesions of the shoulder, such as glenolabral articular disruption (GLAD), have previously been recognized in patients with shoulder instability. We describe a new lesion in which the entire anteroinferior quadrant of the glenoid articular cartilage is sheared off in association with an adjacent labral tear. Both patients were teenage athletes who were being treated arthroscopically for shoulder instability. Preoperative imaging showed some evidence of an articular cartilage lesion. One patient was treated with removal of an irreparable fragment and microfracture, while in t

Osteoarthritis: pathobiology-targets and ways for therapeutic intervention.

Osteoarthritis is first and foremost the ongoing destruction of the articular cartilages of joints. Therefore, the extracellular matrix and the cells of the articular cartilages are the primary targets of osteoarthritis therapy. This tries to inhibit enzymatic destruction of the extracellular cartilage matrix as well as the modification of the cellular phenotype of the chondrocytes: cell degeneration and cell death are alongside anabolic activation and stabilization of the cellular phenotype of major interest. However, apart from the cartilage and its cells, other tissues of the joints are als
