Transpupillary argon laser cyclophotocoagulation in the treatment of traumatic glaucoma.

PURPOSE: A patient with traumatic glaucoma who underwent transpupillary argon laser cyclophotocoagulation for management of uncontrolled intraocular pressure (IOP) despite maximally tolerated medical therapy is discussed. METHODS: In this patient, pars plana vitrectomy, lensectomy, and removal of 180 degrees of necrotic iris had been performed after a blunt trauma with a bungee cord. Six weeks after surgery, the patient presented with an IOP of 40 mmHg despite therapy with three aqueous suppressants. The patient refused further surgical intervention and opted for transpupillary argon laser cy

[Binocular problems caused by aniseikonia and anisophoria after cataract operation].

BACKGROUND: Cataract and refractive surgery aiming at emmetropia, runs the risk to induce binocular problems, e.g. asthenopia or diplopia. If the compatibility concerning binocularity is solely estimated by the calculation of the difference of the retinal image sizes, using intraocular lens formulas or so-called "aniseikonia-programs", important physiological facts are not considered. The actual amount of the aniseikonia, this is the difference of the image size which the patient perceives subjectively, depends on 3 parameters: 1. the optically induced difference of the retinal image size, 2.

[Postoperative course of transscleral ciliary sulcus fixation of posterior chamber intraocular lens and ciliary sulcus insertion of posterior chamber intraocular lens as a secondary implantation--postoperative long course of secondary intraocular lens implantation].

We studied the two-year postoperative course of 46 eyes with transscleral ciliary sulcus fixation of a posterior chamber intraocular lens (sutured group) and 30 eyes with ciliary sulcus insertion of a posterior chamber intraocular lens as a secondary implantation (nonsutured group). The following items were evaluated: visual acuity, difference between preoperative predicted refraction (calculated by the SRK II formula) and final postoperative refraction, complications, corneal endothelial cell loss, and anterior chamber depth. In the nonsutured group, visual acuity was favorable in general and

Pharmacodynamics of subconjunctival gentamicin after experimental cataract operations and traumatic lacerations.

We addressed the concern whether retinal toxicity could occur with subconjunctival gentamicin injections after cataract operations or traumatic corneoscleral lacerations. The lacerations were created along the superior limbus of rabbit eyes and the lenses removed. We sutured the right eyes to mimic a cataract operation and the left eyes remained unsutured to mimic a traumatic corneoscleral laceration. Vitreous humor samples were obtained at 1, 2, 4, 8 and 12 hours after the subconjunctival injection, but in no case did vitreal concentration approach retinotoxic levels (> 133 micrograms/ml).

[No-stitch tunnel incision vs. corneoscleral incision. A prospective, randomized study].

An important factor in early visual recovery after cataract surgery is surgically induced astigmatism. In a randomized clinical trial we compared the development of astigmatism and visual acuity after three different types of incision and suture techniques. Sixty-seven patients were randomized into three groups: (a) scleral tunnel incision (5 mm) without suture (no stitch) (n = 23) (b) scleral tunnel incision (5 mm) with suture (single stitch) (n = 23) (c) corneoscleral incision (5 mm) with suture (CS suture) (n = 21). Implants: 5 mm optic size; follow-up period: 6 months. There was no statist

Argon laser photocoagulation of cyclodialysis clefts after cataract surgery.

Three patients with cyclodialysis clefts, hypotony and hypotonic retinopathy subsequent to cataract surgery were treated with argon laser photocoagulation. The hypotony was reversed in each patient and their visual acuity was normalized. Laser photocoagulation is a noninvasive treatment that can be repeated easily and safely. The complications of the treatment are minor. A hypertensive episode commonly occurs in the early postoperative period.

Retrobulbar block fails to prevent an increase in serum cortisol concentration on emergence from anaesthesia after cataract surgery.

We have studied 30 elderly patients undergoing cataract surgery, allocated randomly to receive general anesthesia, local anaesthesia by retrobulbar block or general anaesthesia combined with retrobulbar block given after induction. Retrobulbar block alone prevented the increases in circulating cortisol and glucose values which occurred in those patients receiving general anaesthesia alone. Retrobulbar block given after induction of general anaesthesia, however, suppressed the cortisol and glucose response during surgery, but did not prevent a marked increase in cortisol concentrations during t

[Results of the treatment of acute endophthalmitis after cataract surgery].

Since 1980, the authors have observed 56 cases of endophthalmitis after cataract surgery which they classify into three groups: A. 18 cases of forms confined to the anterior segment, which just received medical treatment; B. 37 cases of more serious forms with vitreous abscess, treated by vitrectomy; C. 1 case of fulminant form which was eviscerated. The results of group B are better than those of group A: organic: 2.7% versus 11.1% of phtisis; functional: 73% of calculable visual acuity versus 66.6%, although the initial forms had been more severe. Vitrectomy, however causes some complication

[Postoperative leakage of silicone oil into the anterior chamber].

BACKGROUND: One reason for complications after silicone oil application is the postoperative entrance of silicone oil into the anterior chamber. In a retrospective study, we tried to define risk factors for oil entrance into the anterior chamber. METHODS: 159 Eyes of 148 patients, vitrectomized and filled with silicone oil between 1988 and 1992 were analysed. In 22 eyes (13.8%) silicone oil entered the anterior chamber postoperatively. Eyes with emulsified oil in the anterior chamber and eyes, where the oil entered the anterior chamber during cataract extraction were not included in this grou

Undesired light images associated with ovoid intraocular lenses.

Ovoid intraocular lenses (IOLs) accounted for approximately 35% of the lenses implanted in 1991. Despite their popularity among surgeons, patient observations and complaints suggest that ovoid lenses are associated with undesired optical images postoperatively. A pilot clinical study was carried out to test that hypothesis. A multicentered investigation of 289 cases with well-centered IOLs determined that 45% of 168 patients with ovoid lenses observed unwanted optical phenomena at some time after surgery, whereas 17% of 121 patients with round IOLs had similar complaints. Based upon the prelim

Visual prognosis following accidental vitreous loss during cataract surgery.

Over the past 7 years there has been a change in the management of accidental vitreous loss during cataract surgery. There has been a reduction in immediate postoperative complications such as hyphaema. No immediate post-operative complications were observed during the last 18 months of the study. Whereas previously many of these patients remained aphakic, there was a change initially to anterior chamber intraocular lenses (IOLs) and now the vast majority receive capsule-supported sulcus-fixated posterior chamber IOLs. All patients had improved vision post-operatively by a mean of 4 Snellen li

Mooren's ulcer and evidence of stromal graft rejection after penetrating keratoplasty.

A Mooren's ulcer developed in a 6-year-old girl after a penetrating keratoplasty for Peters' anomaly. A destructive, circumferential, and centripetal stromal ulceration recurred despite conjunctival resection, corneal gluing, topical and systemic administration of corticosteroids, and repeat graftings. Results of a systemic and a rheumatologic examination were unremarkable. Histopathologic evaluations of donor grafts consistently disclosed a lymphocytic and a plasma cell infiltrate. Indirect immunofluorescent staining using a normal donor cornea substrate was positive at the level of Bowman's

Retinal detachment after cataract surgery. Surgical results.

The authors studied the results obtained by the Retina Associates in 376 eyes of 361 patients operated on for retinal detachment associated with aphakia or pseudophakia with a postoperative follow-up of at least 6 months. All eyes underwent scleral buckling. The series included 103 eyes with aphakia, 17 eyes with iris-fixated intraocular lens, 111 eyes with anterior chamber (AC) IOL, and 145 eyes with posterior chamber (PC) IOL. The overall success rate for retinal detachment was 93%, without significant difference among the different groups. The aphakia and PC IOL groups had significantly hig

The role of preservatives in the conjunctival toxicity of subconjunctival gentamicin injection.

Subconjunctival gentamicin was identified as the cause of conjunctival chemosis and capillary closure in a recent study conducted in this department. The gentamicin preparation used in the study contained preservatives. The current prospective study was set up to investigate the role of preservatives in the conjunctival toxicity of subconjunctival gentamicin. Seventy five patients undergoing cataract surgery were enrolled in the study. They were split into three groups of 25 each. Group A patients were given a subconjunctival injection of a preservative-free aqueous solution of gentamicin at t
