A 78-year-old woman underwent magnetic resonance (MR) imaging to assess the deterioration of her right hemiparesis initially caused by putaminal hemorrhage that occurred 5 years prior to referral. MR imaging detected an anterior communicating artery aneurysm but no new lesion was seen that might have caused the deterioration was observed. Hemiparesis improved within several days and she was referred for endovascular treatment of the aneurysm. Digital subtraction angiography demonstrated a 4.4 mm long aneurysm at the A1-A2 junction of her left anterior cerebral artery. Information on the risk a

We have improved the scan protocol of three-dimensional computed tomographic angiography (3D-CTA) to maximize the accuracy of examining unruptured cerebral aneurysms. Sixty-seven cases of suspected internal carotid-posterior communicating (Pcom) and internal carotid-anterior choroidal (Ant Cho) artery aneurysms were evaluated as to the opacification of arteries. In the 46 cases of suspected fetal-type Pcom and Ant Cho aneurysms, the arteries were opacified from its origin to the periphery in all cases. A final diagnosis of the presence of an aneurysm was made possible by complete opacification

Cerebral haemorrhage without hypertension, arteriosclerosis or clotting defect has not been reported in patients with Turner's syndrome before. In a 51 year old female patient with non-mosaic Turner's syndrome, acute aphasia and right-sided hemiplegia occurred, due to left-sided basal ganglia haemorrhage. The history for hypertension was negative, blood pressure was normal throughout hospitalisation as well as during 24 h monitoring, and all tests for secondary hypertension were negative. There was no indication of arteriosclerosis or a clotting defect. Since there were hypermobile joints, hyp

Two patients who underwent gamma knife radiosurgery for ruptured cerebral arteriovenous malformations (AVM) developed cystic lesions at 78 and 111 months after undergoing treatment. Both patients presented initially with intracerebral hemorrhage. In one patient, the cystic lesion was discovered during routine follow-up imaging and clinical examination revealed homonymous hemianopsia; the second patient presented with seizure and the lesion was identified more than 9 years after radiosurgery. One patient underwent resection of the nidus and histologic analysis of the resected specimen showed ve

A 75-year-old female fell down in the road and hit her head. Two days later her right eye abduction was slightly limited on right lateral gaze. Cerebral angiography revealed an ipsilateral internal carotid-posterior communicating artery aneurysm. Craniotomy found the aneurysm had compressed the oculomotor nerve and was successfully clipped. The ocular motor paresis completely resolved after the operation. Therefore, the ocular motor paresis was possibly due to the aneurysmal compression to the oculomotor nerve. The oculomotor nerve stretched over the aneurysm was probably vulnerable to sudden

We studied the clinical significance of atrial septal aneurysm on brain embolism. Of two-hundred twenty-eight consecutive ischemic stroke patients, 128 cases were performed transesophageal echocardiography to find cardiac lesions that could be embolic sources. Three cases were diagnosed as having atrial septal aneurysm with right-to-left shunt demonstrated by contrast echocardiography under Valsalva maneuver. These cases were all diagnosed as brain embolism from clinical courses and findings from brain CT scan, and cerebral angiography. Neurosonologic study, 24 hours Holter ECG, and transthora

We report the diagnostic evaluation of an elderly woman admitted with recurrent syncopes induced by head rotation. A cardiac origin was exclude by Holter ECG, echocardiography and carotid sinus compression. Angiography showed unilateral vertebral artery hypoplasia, suggesting the possibility of hemodynamic vertebrobasilar insufficiency. This origin, however, could be ruled out by monitoring of blood flow velocities in both posterior cerebral arteries with transcranial Doppler, which showed no alteration during syncope induced by head rotation. Detailed analysis of the course of the syncope rec

We studied 8 symptomatic patients aged 32-56 years with isolated stenosis of the middle cerebral artery who had TIAs or minor strokes. Angiography showed MCA stenosis in the M1 segment in 7 patients, and proximal M1 occlusion that later recanalized with persistent stenosis in 1 patient, without evidence of more widespread atherosclerotic disease. A potential cardiac source of embolism was detected in only 2 patients with mitral valve prolapse. During follow-up for 39-82 months, no further ischemic events occurred, and transcranial Doppler showed unchanged or decreased stenosis in 6 patients. W

The authors investigated the possibility of a thyrotropin-releasing hormone-related mechanism in a 43-year-old Japanese woman with Hashimoto's encephalopathy who experienced three relapses closely associated with the menstrual cycle. Her symptoms began at ovulation, worsened during the luteal phase, and improved during the menstruation phase. No abnormalities were found by brain magnetic resonance imaging and cerebral angiography. Intravenous administration of thyrotropin-releasing hormone induced symptoms of myoclonus and tremor similar to those observed during an exacerbation. The intensity

The authors report a thirty-seven-year-old woman with systemic lupus erythematosus (SLE), a coronary aneurysm, and myocardial infarction. SLE was diagnosed at twenty-three years of age and treated with prednisolone. Seven years later, she developed inferior myocardial infarction, and coronary angiography showed an aneurysm in the proximal right coronary artery without associated stenosis. At the age of thirty-seven years, she died from cerebral infarction and sepsis. Autopsy revealed an aneurysm (6 mm in diameter) in the proximal right coronary artery and an old inferior myocardial infarction.

Carotid-cavernous sinus fistulae (CCF) represent pathological communications between the intracavernous internal carotid artery and the cavernous venous sinus. Although trauma is the most common cause of high flow carotid cavernous fistulae, they are relatively uncommon in patients sustaining craniofacial trauma. Nevertheless, CCF require early diagnosis and rapid and effective treatment in order to prevent severe and significant morbidity. Clinical presentation may include pulsatile proptosis, ocular and orbital erythema, chemosis, diplopia, headaches, and visual loss. Cerebral angiography is

Nine patients with dissections of the cervical arteries are presented. Dissections cause approximately three per cent of non-haemorrhagic stroke and are usually observed in young and middle-aged patients. Dissections very often give rise to head or neck pain. Carotid artery dissection may lead to lower cranial nerve dysfunction and an incomplete Horner's syndrome in case of subadventitial dissection, and to cerebral ischaemia in case of subintimal spread. Vertebral artery dissection may cause brain stem ischaemia (subintimal dissection) or in rare cases a subarachnoid haemorrhage (subadventiti

With the introduction of computed tomography (CT), the diagnosis of cysticercosis of the central nervous system (CNS) has been considerably improved. This is very important, especially for the countries in which the disease is still endemic. Plain skull radiographs or invasive procedures such as pneumography, ventriculography, and cerebral angiography can be used, but now CT has become the examination of choice because it is a safe, noninvasive, and accurate method thus making the other procedures unnecessary. In the present paper, 8,676 CT scans are reviewed from which 710 are selected patien

A 35 year-old caucasian man suffered from paroxysmal nocturnal haemoglobinuria (PNH) or Marchiafava-Micheli's disease diagnosed in 1976 and complicated by several thrombotic episodes. He developed a benign intracranial hypertension. A digitalized intravenous angiography showed occlusion of both lateral sinuses. Partial improvement followed lombo-peritoneal shunting and steroid therapy. Cerebral venous thrombosis is a well-known complication of PNH but only a few cases have been radiologically and/or pathologically proven. It usually involves the superior longitudinal sinus and/or cortical vein

A 66-year-old man was admitted with 2-year history of progressing gait difficulty and speech disturbance. His neurological examination on admission demonstrated dysarthria, truncal ataxia, mild foot weakness and emotional incontinence. Computerized tomography revealed enlarged ventricle with marked brain stem atrophy. Cerebral angiography showed 90% stenosis of left upper vertebral artery, 90% stenosis of mid basilar artery, occlusion of right upper vertebral artery and 50% stenosis with ulcer of right cervical carotid artery. Right cervical carotid endarterectomy was carried out with continuo

BACKGROUND: Moyamoya disease (MMD) is a progressive, occlusive disease of the internal carotid arteries and their proximal branches, with the subsequent development of an abnormal vascular network that is rupture-prone. Steno-occlusive changes in the posterior cerebral arteries (PCAs) may contribute to worsened outcomes in patients with MMD; however, there is little information on the incidence and natural history of posterior circulation MMD (PCMMD). We describe clinical PCMMD characteristics in a large cohort of patients with MMD.
METHODS: We retrospectively reviewed patients with MMD treat

OBJECTIVE: To report our initial experience with the Medina Embolic Device (MED) in unruptured intracranial aneurysms either as sole treatment or in conjunction with additional devices.
METHODS: 15 consecutive patients (6 women, 9 men) with unruptured aneurysms were treated between September 2015 and April 2016. The aneurysm fundus measured at least 5 mm. We evaluated the angiographic appearances of treated aneurysms at the end of the procedure and at follow-up, the clinical status, complications, and requirement for adjunctive devices.
RESULTS: The MED was successfully deployed in all but o

OBJECTIVE: Cutting the posterior communicating artery (PComA) can improve the neurosurgeon's view and maneuverability while performing surgery within the interpeduncular fossa. Although mentioned in some reports, its technique and safety have not been studied in detail thus far.
METHODS: Patients undergoing surgery for vascular and neoplastic lesions in the interpeduncular fossa in which the PComA was cut were retrospectively analyzed regarding the location where the PComA was divided, the size of the PComA in relation to the posterior cerebral artery, and the vascular risk factors. Clinical

The authors present a patient with a large acoustic neuroma that exhibited an unusual vascular architecture. Magnetic resonance imaging demonstrated multiple flow voids in and around the mass. At surgery, intra- and extratumoral vascularity was arterialized due to luxurious shunting. Two attempts at removal produced severe engorgement and pulsatility of the surrounding brain, dramatically narrowing the operative exposure. Piecemeal resection of the tumor seemed to redistribute blood flow resulting in engorgement of the surrounding brain, analogous to perfusion breakthrough following AVM resect
