Koch's stulates and experimental ocular histoplasmosis.

The present study shows clearly that focal choroiditis is produced in rabbits by infecting the animals with a mycelial form of H. capsulatum. Identification of this organism as the pathogenic agent was made by histopathological and mycological observations. This fungus was recovered from infected ocular lesions in those eyes enucleated within four weeks following the appearance of uveitus-a time period consistent with the clinical and pathological appearance of multiple granulomas in the choroid. The absence of organisms in the contralateral eyes of these same animals at eight weeks suggests p

[Nocardia farcinica: life-threatening chorioiditis under systemic immunosuppression].

BACKGROUND: Immunosuppressed patients occasionally suffer from a multifocal infection with Nocardia. It is important to distinguish Nocardia farcinica from Nocardia asteroides, because of different sensitivity against antibiotics. PATIENT AND METHODS: A 40-year-old patient with polycystic renal disease successfully underwent a kidney transplantation without complications. Immunosuppression consisted of: corticosteroids, azathioprin and ciclosporin A. Ten weeks later he developed acute choroiditis with consecutive retinal detachment and neovascular glaucoma in one eye. In addition, 14 weeks af

Subretinal fibrous membrane following argon laser photocoagulation of presumed histoplasmic maculopathy.

Following an initial improvement after argon laser slit-lamp photocoagulation of histoplasmic choroiditis, progressive visual loss occurred. Histopathological examination showed a dense fibrous membrane between Bruch's membrane and the sensory retina. There was no recurrence of hemorrhage or subretinal fluid. This case report provides an explanation for visual loss in the absence of recurrent inflammation or vascular leakage.

Atypical sarcoidosis-associated uveitis: diagnostic challenges.

Sarcoidosis is a multisystem disease of unknown aetiology with pulmonary involvement in most patients. Uveitis is common and often characteristic. We report a case of ocular sarcoidosis with grossly atypical contiguous optic neuropathy and choroiditis and describe the diagnostic challenges in this highly unusual presentation. High-dose systemic corticosteroid and immunosuppressive treatment was required for sustained control of intraocular inflammation.

Vogt-Koyanagi-Harada Disease: A Case Report Through Poliosis and Inflammatory Relapses.

Vogt-Koyanagi-Harada (VKH) disease is a rare autoimmune stromal choroiditis affecting melanocyte-rich tissues and typically presenting with bilateral ocular inflammation associated with neurological, auditory, or integumentary manifestations. Early diagnosis is essential because prompt initiation of intensive immunosuppressive therapy is associated with improved long-term outcomes. An 18-year-old Hispanic man presented in September 2022 with a one-week history of bilateral visual loss and ocular redness. He had experienced a first episode of anterior uveitis in June 2022 and subsequently devel

Schlaegel lines: History, etiology, multimodal imaging, and differential diagnosis of curvilinear streaks in chorioretinal disorders.

Schlaegel lines (SL) represent distinctive curvilinear patterns of chorioretinal atrophy that have emerged as a distinctive, yet non-pathognomonic, funduscopic sign across diverse ocular disorders. First described as related to presumed ocular histoplasmosis syndrome in the early 1980s in Histoplasma-endemic regions, these peripheral fundus lesions have since been documented across numerous other etiologies. The majority of SL arise in the context of inflammatory chorioretinal disorders, with idiopathic multifocal choroiditis and punctate inner choroiditis constituting the most frequent underl

Placoid lesions of the retina: progress in multimodal imaging and clinical perspective.

Placoid lesions of the retina may be secondary to a wide spectrum of acquired inflammatory conditions that have been reported as single entities with different presentation and clinical course. These conditions include acute posterior multifocal placoid pigment epitheliopathy, persistent placoid maculopathy, serpiginous choroiditis, serpiginous-like choroiditis, relentless placoid chorioretinitis and acute syphilitic posterior placoid chorioretinitis. In this article, we will group these conditions under the name of 'placoids'. The recognition of the specific condition may be challenging in cl

Bilateral atypical optic neuritis associated with tuberculosis in an immunocompromised patient.

A 27 year-old lady, presented with sudden loss of vision in the right eye for a week. It was followed by poor vision in the left eye after 3 days. It involved the whole entire visual field and was associated with pain on eye movement. She was diagnosed to have miliary tuberculosis and retroviral disease 4 months ago. She was started on anti-TB since then but defaulted highly active anti-retroviral therapy (HAART). On examination, her visual acuity was no perception of light in the right eye and 6/120 (pinhole 3/60) in the left eye. Anterior segment in both eyes was unremarkable. Funduscopy sho

Tuberculosis.

PURPOSE OF REVIEW: The purpose of this report is to present an update on the manifestations and management of ocular tuberculosis. RECENT FINDINGS: Tuberculosis affects one-third of the world's population. The incidence of tuberculosis has increased with the increase in the HIV infected population. Following a resurgence of the disease in the US, the incidence has recently declined. Patients may develop scleritis that can be focal, nodular or diffuse with or without keratitis. Anterior granulomatous uveitis may occur. The posterior segment reveals vitritis, choroiditis, and can mimic serpigin

Elevation of von Willebrand factor is independent of erythrocyte sedimentation rate and persists after glucocorticoid treatment in giant cell arteritis.

Von Willebrand factor (vWF) antigen levels were measured in 39 patients with biopsy-proven giant cell arteritis (GCA), 16 patients with polymyalgia rheumatica (PMR), 12 patients with Sjögren's syndrome, and 7 with choroiditis, as well as in 20 age- and sex-matched controls. Among GCA patients, 31 (group A) had clinically inactive disease and were taking low-dose glucocorticoids for at least 1 year; their vWF levels were high even in the presence of a normalized erythrocyte sedimentation rate (ESR). When the ESR and vWF were analyzed in another 8 GCA patients (group B) before and after treatmen

Combined Therapy Versus Fortified Anti-VEGF Monotherapy in Type C Polypoidal Choroidal Vasculopathy: Long-Term Outcomes and Exploratory Biomarker Insights.

While standard anti- vascular endothelial growth factor (VEGF) therapy, with or without photodynamic therapy (PDT), is effective for patients with polypoidal choroidal vasculopathy (PCV), not all achieve optimal visual outcomes. This study aimed to compare fortified (double the dose and the volume of the standard one) anti-VEGF combined with PDT versus fortified anti-VEGF monotherapy and to investigate biomolecular profiles and disease relationships among PCV, neovascular age-related macular degeneration (nvAMD), and central serous chorioretinopathy (CSCR). The goal was to identify novel pathw

Case Report: Synchronous germline BAP1-negative choroidal melanoma and clear cell renal cell carcinoma characterized by pan-cancer genomic profiling.

BACKGROUND: Synchronous occurrence of choroidal melanoma and clear cell renal cell carcinoma (ccRCC) is exceptionally rare and traditionally suspicious for BAP1 tumor predisposition syndrome. Distinguishing synchronous primaries from metastasis poses diagnostic challenges. CASE PRESENTATION: A 64-year-old Chinese male with a 2-year history of progressive vision loss in his right eye was found to have an intraocular melanocytic tumor by multimodal imaging. Staging PET/CT incidentally detected a hypermetabolic mass in the right kidney. Guided by imaging and multidisciplinary team (MDT) discussi

Quantitative evaluation of pigment epithelial encapsulation using polarization-sensitive optical coherence tomography after anti-VEGF therapy for myopic choroidal neovascularization.

PURPOSE: We aimed to investigate the association between post-treatment changes in polarimetric entropy (PE), which is a noninvasive surrogate marker of structural heterogeneity associated with pigment epithelial remodeling and measured by polarization-sensitive optical coherence tomography (PS-OCT), and the need for multiple anti-vascular endothelial growth factor (VEGF)-injections in myopic choroidal neovascularization (mCNV). METHODS: We retrospectively analyzed 11 eyes that received intra-vitreal anti-VEGF injections for treatment-naïve mCNV. Based on treatment response during the first 3

Real-world outcomes after switching to faricimab in treatment-resistant diabetic macular edema: A 1-year observational study with choroidal thickness assessment.

Approximately 40% of diabetic macular edema (DME) cases are resistant to conventional anti-vascular endothelial growth factor (VEGF) therapy. Although faricimab (an antibody against VEGF-A and angiopoietin-2 [Ang-2]) has shown efficacy in treatment-naïve DME, real-world evidence on switching strategies in resistant cases remains limited. We evaluated the 1‑year real‑world effectiveness of switching to faricimab in resistant DME and explored central choroidal thickness (CCT) changes and their relationship to retinal and choroidal responses. This retrospective study included 28 eyes from 19 pati
