Computerized approach to X-linked mental retardation syndromes.

Photographs of normal and abnormal relatives with X-linked mental retardation (XLMR) and clinical data are combined in a Macintosh II computer system to provide an objective and versatile means of evaluating developmental abnormalities. The system includes a flatbed scanner for the entry of photographs and text, additional memory to enhance resolution of photographs, and several programs which facilitate searches for keywords. The primary advantages of the system include an easy interaction between descriptive words and photographs, and rapid comparison of whole faces and specific traits from

[Trans-catheter closure of patent ductus arteriosus in childhood--an alternative to surgical ligature].

Transcatheter closure of a persistently patent ductus arteriosus (PDA) was successfully performed in four children using a Rashkind double disk umbrella device. Mean age was 5.1 +/- 1.5 (2.8 to 6.3 years) and weight 18.8 +/- 3.8 kg (14.1 to 22.3 kg). In all children the PDA was the only cardiac lesion and was diagnosed clinically and by means of continuous wave and color-coded doppler echocardiography. After angiographic measurement of PDA size the correlating device was implanted. The ductus was closed immediately in two children, whereas there was a residual angiographic shunt in the others.

[Follow-up of surgically treated congenital heart disease using Doppler echocardiography].

Surgery can restore normal cardiac anatomy in some forms of congenital heart disease but in others this is not possible even after a "complete correction". In yet other forms, surgery is only palliative. Echocardiography coupled with pulsed, continuous wave and color coded Doppler should be carried out after clinical, electrocardiographic and radiological examination to assess the surgical results and detect possible complications in the short or long-term. It is therefore important to be familiar with the "normal" appearances of operated congenital heart disease in order to be able to detect

A covered benefit with an untrusted on-ramp: Why medicare principal illness navigation still is not reaching patients.

Principal Illness Navigation (PIN) represents an important Medicare policy advance, but coverage has not translated reliably into access. PIN addresses a real need: patients with cancer and other serious chronic illnesses often struggle with fragmented communication, scheduling complexity, insurance confusion, caregiver burden, and uncertainty about next steps. Medicare recognizes PIN under Healthcare Common Procedure Coding System (HCPCS) Level II codes G0023 and G0024, with payment established through the Medicare Physician Fee Schedule. Yet the main barriers to PIN implementation are admini

A modular protein-nucleic acid nanodevice for non-genetic engineering of mesenchymal stem cells to target pathological collagen in liver fibrosis.

Mesenchymal stem cell (MSC) therapies hold immense promise for regenerative medicine, yet their clinical translation is hindered by inefficient delivery and poor homing at injury sites. Here, we exploited a modular protein-nucleic acid nanodevice for non-genetic engineering of MSCs, which enables the cells to dock onto pathological collagen-rich tissues. The protein-nucleic acid nanodevice integrates two key modules: a multi-functional fusion protein for collagen binding and clickable covalent conjugation between protein and DNA, and a MSC-specific binding DNA nanostructure. Benefiting from th

AI-driven Test-Free Prediction of ICU Admission, Insulin Dependence, and Exocrine Dysfunction after Acute Pancreatitis.

OBJECTIVES: Acute pancreatitis (AP) has heterogeneous trajectories: some patients deteriorate rapidly and require ICU care, whereas others develop delayed sequelae such as exocrine pancreatic dysfunction (EPD) and pancreatogenic diabetes. Existing scoring systems are burdensome, not available at first presentation, and poorly suited for forecasting longer-term outcomes. METHODS: Our AI platform operating exclusively on routinely collected EHR predict three outcomes after a first recorded AP diagnosis: (i) ICU admission (same-day, within 1 wk, within 2 wk), (ii) incident EPD, and (iii) inciden

Local Referral of Peripheral Arterial Disease Patients to Improve Surgical Outcomes.

OBJECTIVE: The BEST-CLI trial demonstrated that in select patients with chronic limb threatening ischemia , open surgical bypass yields superior outcomes compared to an endovascular-first approach. Challenges in access to vascular surgeons who perform lower extremity bypass may hinder the adoption of BEST-CLI informed treatment strategies into routine clinical practice. We evaluated the contemporary procedural landscape of providers performing lower extremity peripheral vascular interventions (PVIs) and assessed the potential travel burden associated with transitioning care from non-surgical i

Racial and Ethnic Differences in Ostomy Creation in Pediatric Patients With Crohn's Disease.

INTRODUCTION: Crohn's disease (CD) has historically been more prevalent among non-Hispanic White populations; however, recent studies demonstrate rising incidence in non-White populations. Despite this shift, limited data exist regarding differences in emergent surgical intervention and ostomy creation in non-White versus White populations. This study evaluated whether non-White pediatric patients with CD experience higher rates of emergent surgical intervention and ostomy creation. METHODS: The National Surgical Quality Improvement Program Pediatric Database was queried from 2015 to 2021. Pa

RNA-based mechanisms underlying oncogenicity and drug resistance in NRG1-rearranged non-small cell lung cancer.

Non-small cell lung cancer (NSCLC), accounting for nearly 85 % of lung tumors, remains one of the most fatal cancers worldwide. Among the various oncogenic drivers identified in NSCLC, neuregulin-1 (NRG1) gene fusions, though rare, occurring in approximately 0.1-0.3 % of cases, are clinically significant, particularly in invasive mucinous adenocarcinomas. These genomic events trigger abnormal heterodimerization of ERBB2 and ERBB3, stimulating downstream PI3K/AKT and MAPK pathways, which in turn foster uncontrolled growth, metastatic potential, and therapeutic resistance. These genomic events c

The Power of ECHO Autism: Improving Confidence and Holistic Care for Autistic Adults.

BACKGROUND: Autistic adults describe difficulties accessing high-quality primary care services. Primary care practitioners (PCPs) report a lack of confidence and knowledge in working with autistic adults. The Extension for Community Healthcare Outcomes (ECHO) model is a longitudinal clinician education program that utilises a 'hub and spoke' model, community-building strategies and peer case-based learning to improve the care of patients with medical complexity. METHODS: The present study utilised the ECHO model ('ECHO Autism Adult Healthcare') to train PCPs in how to best care for autistic a
