Implementation of a standardized transition to practice program for nurse practitioners and physician associates.

BACKGROUND: High turnover among nurse practitioners (NPs) and physician associates (PAs) disrupts care continuity, increases costs, and strains teams, particularly in high-acuity academic settings. Structured transition to practice (TTP) programs aim to improve retention, readiness, and workforce stability. LOCAL PROBLEM: In a tertiary neuroscience department, NP and PA turnover reached 16.7% in 2023. Onboarding varied by division and relied on brief shadowing without standardized competencies, mentorship, or structured development. Early attrition within the first year reflected gaps in role

Development and National Implementation of a Standardized Physiotherapy Assessment Package for Pediatric Asthma and Dysfunctional Breathing.

INTRODUCTION: Variation in pediatric respiratory physiotherapy assessment for asthma and dysfunctional breathing (DB) across NHS services creates inconsistency in clinical reasoning, workforce development, and data harmonization. We aimed to design and implement a standardized, evidence-informed assessment package to support physiotherapists working with children and young people (CYP) with asthma and DB. METHODS: Evidence from a previously conducted structured scoping review (PROSPERO ID: 112227) informed key assessment domains. A national collaborative of specialist pediatric respiratory ph

A Joint Model for (Un)Bounded Longitudinal Markers, Competing Risks, and Recurrent Events Using Patient Registry Data.

Joint models for longitudinal and survival data have become a popular framework for studying the association between repeatedly measured biomarkers and clinical events. Nevertheless, addressing complex survival data structures, especially handling both recurrent and competing event times within a single model, remains a challenge. This causes important information to be disregarded. Moreover, existing frameworks rely on a Gaussian distribution for continuous markers, which may be unsuitable for bounded biomarkers, resulting in biased estimates of associations. To address these limitations, we

Incidence and predictors of stroke in patients with rheumatic heart disease.

OBJECTIVE: Ischaemic stroke is a severe complication of rheumatic heart disease (RHD), which may result in permanent disability and death. This study aimed to assess the incidence and predictors of stroke in patients with RHD in the current era of evidence-based recommendations for prevention. METHODS: Consecutive patients with RHD diagnosed by clinical and echocardiographic criteria were selected. A structured clinical and neurological assessment was performed to determine the aetiology and classification of stroke at enrolment. The primary endpoint was an ischaemic cerebrovascular event, wh

Cytotoxicity of replication-competent adenoviruses powered by an exogenous regulatory region is not linearly correlated with the viral infectivity/gene expression or with the E1A-activating ability but is associated with the p53 genotypes.

BACKGROUND: Replication-competent adenoviruses (Ad) produced cytotoxic effects on infected tumors and have been examined for the clinical applicability. A biomarkers to predict the cytotoxicity is valuable in a clinical setting. METHODS: We constructed type 5 Ad (Ad5) of which the expression of E1A gene was activated by a 5' regulatory sequences of survivin, midkine or cyclooxygenase-2, which were highly expressed in human tumors. We also produced the same replication-competent Ad of which the fiber-knob region was replaced by that of Ad35 (AdF35). The cytotoxicity was examined by a colorimet

An evaluation of patient self-testing competency of prothrombin time for managing anticoagulation: pre-randomization results of VA Cooperative Study #481--The Home INR Study (THINRS).

Prior studies suggest patient self-testing (PST) of prothrombin time (PT) can improve the quality of anticoagulation (AC) and reduce complications (e.g., bleeding and thromboembolic events). "The Home INR Study" (THINRS) compared AC management with frequent PST using a home monitoring device to high-quality AC management (HQACM) with clinic-based monitoring on major health outcomes. A key clinical and policy question is whether and which patients can successfully use such devices. We report the results of Part 1 of THINRS in which patients and caregivers were evaluated for their ability to per

Matching Artificial Intelligence and digital simulation to educational purposes in a competency-first pharmacy education framework.

OBJECTIVES: To synthesize pharmacy-education evidence across artificial intelligence (AI) and digital simulation technologies, compare modalities by educational function, evidence maturity, implementation requirements, and risk, and derive a competency-first framework for curricular selection, implementation, assessment, and governance. FINDINGS: A narrative search of PubMed, Scopus, and Web of Science prioritized pharmacy-education studies and reviews published from 2018 through August 2026. Educational function, rather than AI status alone, provided the primary organizing principle. Simulat

Prevalent and incident heart failure hospitalization in patients with significant tricuspid regurgitation: insights from a competing risk analysis.

INTRODUCTION AND OBJECTIVES: Tricuspid regurgitation (TR) is closely linked to heart failure (HF). However, the burden and predictors of HF hospitalization at diagnosis and during follow-up have not been fully characterized, particularly after accounting for the competing risk of death. METHODS: We conducted a retrospective multicenter cohort study including 757 patients with significant TR. Prevalent HF was defined as HF hospitalization at the time of diagnosis. Among patients without prevalent HF, the cumulative incidence of first HF hospitalization was assessed using competing risk analysi

A qPCR-Based Screening Platform for Exploratory Assessment of Phage Training Outcomes in Enterobacter cloacae and Stenotrophomonas maltophilia.

Bacteriophages (phages) represent promising therapeutic agents. Their clinical use is challenged by the rapid rise of resistant bacterial clones. To overcome this problem, phages can be trained in vitro to improve their ability to cope with the possible resistance that may arise. Here, we co-evolved phages with their hosts under different conditions and assessed their ability to infect an adapted bacterial panel using qPCR. The co-evolution experiment yielded a panel of bacterial clones adapted either to a phage, a competing phage, or a cocktail of both. Phages were adapted either in the conti

Exposure-adjusted event rates for evaluating bioprosthetic valve failure after transcatheter aortic valve implantation in the presence of competing mortality.

BACKGROUND: Bioprosthetic valve failure (BVF) is a key clinical endpoint for evaluating valve durability after transcatheter aortic valve implantation (TAVI). However, in populations with substantial competing mortality or heterogeneous follow-up durations, the estimated incidence of BVF may vary according to evaluation metric. We evaluated how different analytic approaches influence estimates of the long-term BVF incidence. METHODS: We analyzed data from a multicenter registry of patients undergoing TAVI with balloon-expandable valves (SAPIEN XT or SAPIEN 3). Outcomes of interest were all-ca

Improved method to stratify lymphoma patients with risk of secondary central nervous system involvement: A multicenter retrospective analysis.

Secondary central nervous system (SCNS) involvement is an infrequent but universally fatal event in diffused large B-cell lymphoma. The occurrence rate of SCNS involvement is approximately 5% but comes with a poor prognosis ever after. However, existing risk models to predict the incidence and prognosis of these patients with SCNS involvement lack both efficiency and accuracy. Controversy has also been reported regarding which risk factor may best identify the population with a high CNS relapse rate. In this study, we retrospectively analyzed 831 patients with diffused large B-cell lymphoma, d

Assessing and Providing Culturally Competent Care in Radiation Oncology for Deaf Cancer Patients.

PURPOSE: Recognition of disparities for vulnerable populations in the field of oncology is increasing, but little attention has been paid to deaf patients. At least a million Americans are culturally deaf and use American Sign Language. Poor linguistic and cultural competency among physicians is a barrier to care delivery for these patients, placing them at risk for treatment disparities. To better educate oncology practitioners, including radiation oncologists, regarding the unique needs of this cohort, we performed an evidence-based literature review of culturally competent care for deaf pat

Effect of community-based tabletop simulation on disaster nursing competency, preparedness, transfer motivation, and self-efficacy: a quasi-experimental study.

BACKGROUND: Nurses play a critical role in community-based disaster response; however, gaps remain in disaster preparedness and in translating disaster-related competencies into clinical practice. Despite the widespread use of simulation-based education, evidence regarding the effectiveness of low-cost, scalable approaches, such as tabletop simulation, in undergraduate nursing education remains limited. This study aimed to evaluate the effectiveness of a community-based tabletop simulation program on disaster nursing competency, preparedness, transfer motivation, and learning self-efficacy. M

Integrated treatment of the severely injured patient with coexistent comorbidities. A case report and literature review.

Major trauma represents a paradigmatic clinical condition, needing multidisciplinary and structured approach. When a patient is affected by significant comorbidities such as diabetes mellitus and other conditions that compromise the immune competence the risk of infections in the presence of exposed fractures is remarkably high. Although usually managed by multidisciplinary teams, these patients often have unfavorable functional outcomes. We describe here the case of a young diabetic and immunosuppressed man, treated for a destroying injury of the left lower limb, with more than 60 centimeters

Why is homocysteine toxic for the nervous and immune systems?

Hyperhomocysteinemia is a risk factor for a number of neurodegenerative and cardiovascular diseases. We have shown that homocysteine induces excitotoxic effects in cells expressing glutamate receptors of the NMDA class. These receptors were found not only in neurons but also in immune-competent cells, neutrophils, red blood cells, cardiomyocytes, and osteoblasts. Activation of these cells by homocysteine results in an increase in cytoplasmic calcium ions, accumulation of reactive oxygen species, and activation of MAP kinase. An overload of immune-competent cells activates both necrotic and apo
