Which Patients Are Candidates for Minimally Invasive Mitral Valve Surgery? - Establishment of Risk Calculators Using National Clinical Database.

BACKGROUND: Although minimally invasive mitral valve surgery via a right minithoracotomy (MICS-mitral) is widely performed, no tool to evaluate its risk has been reported. We sought to establish MICS-mitral risk calculators using a national clinical database for selection of appropriate patients. METHODS AND RESULTS: Between 2008 and 2015, 3,240 patients (mean age 59±14 years, males 1,950) underwent a MICS-mitral procedure in Japan and were registered in a national clinical database. We examined mortality and composite outcome (operative mortality, stroke, reoperation for bleeding) using mult

Evidence-based guideline for clinical practice in the diagnosis, treatment, management, and prevention of recurrent wheezing in infants and toddlers in China.

Wheezing is a common respiratory symptom in infants and toddlers, and recurrent wheezing is a significant respiratory disorder affecting this age group. Given the multifactorial etiology of recurrent wheezing, clinical practice lacks standardized diagnostic and therapeutic protocols. Recent years have witnessed substantial progress in clinical research and therapeutic advancements, yielding robust evidence to inform clinical decision-making for recurrent wheezing in infants and toddlers. The guideline is developed by integrating the latest domestic and international clinical consensus statemen

[Translational therapeutic strategies and timing of surgery for peritoneal metastasis in colorectal cancer].

Peritoneal metastasis from colorectal cancer is characterized by aggressive biological behavior, rapid disease progression, and poor overall prognosis, making it one of the most challenging conditions in clinical practice. With advances in surgical techniques and multimodal treatment strategies, cytoreductive surgery combined with hyperthermic intraperitoneal chemotherapy can achieve satisfactory cytoreduction in carefully selected patients, thereby conferring meaningful survival benefits. However, marked heterogeneity exists among patients with peritoneal metastases, and complete cytoreductio

[Specific respiratory tract infections in acute cough-when is targeted diagnosis and treatment required?].

In most cases, acute cough is caused by a self-limiting respiratory infection. However, symptoms, pre-existing conditions, or environmental factors may indicate a cause requiring treatment; red flags point to dangerous courses, the NEWS2 score can assess the risk of septic deterioration, and the CRB-65 score supports decision-making regarding hospital admission in pneumonia. Pneumonia cannot be reliably diagnosed clinically but is unlikely in the absence of clinical indicators. Laboratory tests can support the decision for antibiotic treatment, while the diagnosis is confirmed by chest X‑ray.

An Opportunity for Cognitive Task Analysis in Neonatal Resuscitation.

Approximately 10% of newborn infants require resuscitative intervention at birth. Ideally, this care is provided by a team of expert healthcare professionals who possess exceptional cognitive, psychomotor, and communication skills. Human errors and deviations from resuscitation protocol are common and may be attributable to excessive cognitive demand experienced by the resuscitation team. Cognitive Task Analysis (CTA) is a group of methods used to assess knowledge, judgments, goals, and decision-making of expert healthcare professionals. These methods may be used during neonatal resuscitation

Efficient Edge AI for Neonatal Care: Implementing HIE Grading on Snapdragon Edge Device.

Hypoxic-Ischemic Encephalopathy (HIE) requires prompt diagnosis within the first six hours post-encephalopathy injury to maximize treatment effectiveness. Efficient and accurate grading of HIE severity is therefore essential. Current clinical methods for HIE diagnosis are complex and time-intensive, demanding specialised skills and often require a multi-model diagnostic approach. In response to this challenge, we developed and implemented an automated HIE grading system to aid with clinical decision-making. The EEG signal is converted to the audio domain, with the spectrogram representation of

A machine learning-based model for predicting recurrence in intermediate- and high-risk differentiated thyroid cancer: insights from a retrospective single-center study of 2388 patients.

PURPOSE: Current guidelines provide a recognized yet broad framework for stratifying recurrence risk in differentiated thyroid cancer (DTC) patients. More precise tools are needed for intermediate- and high-risk groups. This study aims to identify recurrence-associated risk factors and develop a machine learning-based predictive model. METHODS: In this retrospective analysis, 2,388 DTC patients were randomly assigned to a training group (1,910 cases) and a validation group (478 cases). Predictive factors were identified using univariate and multivariate analyses. Six machine learning models w

The role of resting myocardial blood flow and myocardial blood flow reserve as a predictor of major adverse cardiovascular outcomes.

Cardiac perfusion PET is increasingly used to assess ischemia and cardiovascular risk and can also provide quantitative myocardial blood flow (MBF) and flow reserve (MBFR) values. These have been shown to be prognostic biomarkers of adverse outcomes, yet MBF and MBFR quantification remains underutilized in clinical settings. We compare MBFR to traditional cardiovascular risk factors in a large and diverse clinical population (60% African-American, 35.3% Caucasian) to rank its relative contribution to cardiovascular outcomes. Major adverse cardiovascular events (MACE), including unstable angina

Dynamic hematologic recovery patterns to guide clinical differentiation along the dengue-hemophagocytic lymphohistiocytosis hyperinflammatory spectrum: A case report.

Dengue infection may trigger a hyperinflammatory response that closely resembles secondary hemophagocytic lymphohistiocytosis (HLH), making therapeutic decision-making particularly challenging in patients who fulfill multiple HLH-associated diagnostic criteria. Distinguishing self-limited dengue-associated immune activation from progressive HLH requiring immunomodulatory therapy remains an important clinical dilemma. We report a 19-year-old woman presenting with prolonged fever, pancytopenia, hepatosplenomegaly, marked hyperferritinemia, hypofibrinogenemia, hypertriglyceridemia, elevated solub

Explainable artificial intelligence for gait analysis: advances, pitfalls, and challenges - a systematic review.

Machine learning (ML) has emerged as a powerful tool to analyze gait data, yet the "black-box" nature of many ML models hinders their clinical application. Explainable artificial intelligence (XAI) promises to enhance the interpretability and transparency of ML models, making them more suitable for clinical decision-making. This systematic review, registered on PROSPERO (CRD42024622752), assessed the application of XAI in gait analysis by examining its methods, performance, and potential for clinical utility. A comprehensive search across four electronic databases yielded 3676 unique records,

Neurochemical drivers of effort: The roles of dopamine and beyond in physical and cognitive exertion.

Motivation depends on the ability to evaluate whether potential rewards are worth the effort required to obtain them. Disruptions in this process give rise to debilitating motivational syndromes such as apathy and anhedonia, which are prominent across neurological and psychiatric disorders, including depression and Parkinson's disease. Although mesolimbic dopamine has long been central to theories of effort-based decision-making, converging evidence indicates that motivational drive emerges from the interaction of dopamine with a broader set of neuromodulatory systems. Here, we review animal a
