Inflammatory bowel disease in children: Indian perspective.

The magnitude of inflammatory bowel disease in childhood in India is largely unknown. At the Pediatric Gastroenterology section of PGIMER, Chandigarh, 15 out of 294 children (5%) admitted for colonic disorders were diagnosed to have ulcerative colitis. Diagnosis is suspected on the basis of clinical presentation and established by sigmoidoscopy, rectal biopsy and double contrast barium enema. Widely prevalent bacillary dysentery, acute amoebic colitis and antibiotic induced colitis need to be kept in differential diagnosis of acute presentation of ulcerative colitis. Chronic or recurrent colit

Role of the serotoninergic system in the development of diseases of the intestinal and biliferous tracts.

Primary sclerosing cholangitis and nonspecific ulcerative colitis were modeled before and after serotonin and spiperone injections. Activation of the serotoninergic system prevented the development of primary sclerosing cholangitis, but promoted more severe course of nonspecific ulcerative colitis.

THE ONSET OF CLINICAL MANIFESTATIONS IN INFLAMMATORY BOWEL DISEASE PATIENTS.

BACKGROUND: The diagnosis of inflammatory bowel disease is often delayed because of the lack of an ability to recognize its major clinical manifestations. OBJECTIVE: Our study aimed to describe the onset of clinical manifestations in inflammatory bowel disease patients. METHODS: A cross-sectional study. Investigators obtained data from interviews and the medical records of inflammatory bowel disease patients from a reference centre located in Brazil. RESULTS: A total of 306 patients were included. The mean time between onset of symptoms and diagnosis was 28 months for Crohn's disease and 19

Evaluation of plasma Short chain fatty acid levels as markers for Inflammatory bowel disease.

BACKGROUND: Specific variations of short chain fatty acids in fecal samples have been shown for patients with inflammatory bowel disease. The aim of this study was to assess if Crohn's disease and ulcerative colitis are associated with altered concentrations of short chain fatty acids also in blood plasma. METHOD: Between 2016-2019, Swedish adults referred to a tertiary center for colonoscopy were asked to participate in a cross-sectional study. Individuals with Crohn's disease or ulcerative colitis as well as individuals with no findings on the colonoscopy (defined as clean colon) were inclu

[Prognosis and treatment of chronic inflammatory bowel disease].

BACKGROUND, MATERIAL AND METHODS: Most studies of the prognosis of inflammatory bowel disease have not been population-based; they are retrospective reviews. Moreover, they lack uniform methods for assessment of outcome. The clinical course is difficult to predict and the prognosis has changed over the last decades as a result of progress in medical therapeutics and treatment principles and surgical methods. RESULTS: Patients suffering from Crohn's disease or ulcerative colitis will probably alternate between remission and relapse, with 10% having a relapse-free course after ten years, and on

Increased risk of inflammatory bowel disease associated with oral contraceptive use.

Research on inflammatory bowel disease risk among oral contraceptive users has reached conflicting conclusions. This population-based case-control study evaluated the effects of oral contraceptive use on ulcerative colitis and Crohn's disease risk. Cases were women enrollees, aged 15-68 years, of Group Health Cooperative of Puget Sound (a prepaid health plan based in western Washington State) who had ulcerative colitis (n = 211) or Crohn's disease (n = 91). Age-matched controls were randomly selected from the health plan enrollment file. An in-person interview obtained information about lifeti

[Anatomopathological criteria of progression in chronic idiopathic colitis].

Idiopathic inflammatory colitis are diseases of remissions and exacerbations of various severity. The criteria of activity are based on a conjunction of clinical, radiologic, endoscopic and anatomopathological data. From the macroscopical point of view, they are different in ulcerative colitis and Crohn's disease. Ulcerative colitis is characterized by a diffuse and uniform mucosal inflammation; any biopsy outside an ulcerative area can be used to evaluate the inflammatory components which are in the acute phase (chronic active phase): oedema, vascular congestion and an inflammatory infiltrate

The epidemiology of inflammatory bowel disease in Cape Town 1980-1984.

Patients with inflammatory bowel disease diagnosed in the greater Cape Town area between the years 1980 and 1984 inclusive were studied to establish the incidence and presenting features of these diseases. Case details were obtained from the records of the Groote Schuur Hospital group and Tygerberg Hospital as well as from questionnaires completed by 97% of 731 medical practitioners in the area. There were 134 patients with Crohn's disease, 197 with ulcerative colitis and 16 with indeterminate colitis. The incidences of Crohn's disease in the coloured, white and black population groups were ca

Azathioprine-induced liver injury in a patient with multiple autoimmune syndrome.

Extraintestinal manifestations are frequent in patients with ulcerative colitis. They are most commonly associated with active bowel disease but can also occur prior to bowel disease or during a period of remission. The disease often requires long-term therapy. The tolerability of drugs is essential for the comprehensive management of patients. Azathioprine is considered an indispensable drug for the treatment of severe ulcerative colitis patients. It is used to prevent relapse or to treat patients who are refractory to corticosteroids. We report a patient who had alopecia areata, vitiligo, re

Chronic inflammatory bowel disease in patients with coeliac disease.

Six patients with coeliac disease and inflammatory bowel disease are described. Of special interest were two patients with coeliac disease and dermatitis herpetiformis and ulcerative colitis, one of whom also had sclerosing cholangitis. Three patients had both coeliac disease and ulcerative colitis, and one of them also had sclerosing cholangitis. In one patient with coeliac disease Crohn's disease of the small bowel was diagnosed. There seems to be association between coeliac disease without dermatitis herpetiformis, and ulcerative colitis. The possible combination of coeliac disease and infl

[Undetermined colitis].

Indeterminate colitis combines features of chronic ulcerative colitis and Crohn's colitis and contemplates a dilemma for the patient and surgeon contemplating ileal pouch anal anastomosis. Some series have reported a high incidence of failure of pouch procedures in patients with indeterminate colitis. A series reported by the authors in 1989 reported a failure rate on approximately 9% with indeterminate colitis. In updating this series with additional patients operated on since that time, the incidence in our series approaches 20%. We feel that an 80% success rate, however, justifies performin

Indeterminate colitis predisposes to perineal complications after ileal pouch-anal anastomosis.

This study retrospectively evaluated 288 patients who had undergone ileal pouch-anal anastomosis to determine the incidence of perineal complications and to relate these findings to the pathologic diagnosis, with the goal of specifically clarifying the appropriate surgical management of patients with indeterminate colitis. Of these 288 patients, 235 patients (82 percent) had a diagnosis of chronic ulcerative colitis, 18 patients (6 percent) had indeterminate colitis, 6 patients (2 percent) had Crohn's disease, and 29 patients (10 percent) had familial polyposis. All complications occurred at l

Human alpha defensin 5 is a candidate biomarker to delineate inflammatory bowel disease.

Inability to distinguish Crohn's colitis from ulcerative colitis leads to the diagnosis of indeterminate colitis. This greatly effects medical and surgical care of the patient because treatments for the two diseases vary. Approximately 30 percent of inflammatory bowel disease patients cannot be accurately diagnosed, increasing their risk of inappropriate treatment. We sought to determine whether transcriptomic patterns could be used to develop diagnostic biomarker(s) to delineate inflammatory bowel disease more accurately. Four patients groups were assessed via whole-transcriptome microarray,

Impact of Stopping Thiopurines Under Anti-Tumor Necrosis Factor Therapy in Inflammatory Bowel Disease: A Population-Based Study.

BACKGROUND & AIMS: The impact of stopping thiopurines after escalating to treatment with an anti-tumor necrosis factor agent in patients with inflammatory bowel disease is currently unclear. We aimed to assess the impact of stopping vs continuing thiopurines after initiating anti-tumor necrosis factor therapy. METHODS: In this retrospective cohort study, we analyzed data from the Korean National Health Insurance claims database of 6235 patients with ulcerative colitis or Crohn's disease on thiopurines, who started anti-tumor necrosis factor treatment between 2007 and 2020. Patients were class

Refractory Inflammatory Bowel Disease.

Therapeutic options for refractory colonic inflammation in patients with ulcerative colitis or Crohn's disease have recently been expanded with the introduction of biologic therapies. Intravenous corticosteroids and cyclosporine A remain the standard therapies for severe ulcerative colitis. Monoclonal antibodies directed at tumor necrosis factor-a have proven to be exceptionally efficacious in patients with severe or refractory Crohn's disease. Immunomodulatory therapy with azathioprine, 6-mercaptopurine, or methotrexate has demonstrated efficacy for maintenance of remission in patients with r

Axonal degeneration/necrosis: a possible ultrastructural marker for Crohn's disease.

Axonal degeneration/necrosis has previously been demonstrated in the small bowel autonomic nerve plexus of patients with Crohn's disease. It was suggested that this feature might be helpful in the differential diagnosis of Crohn's disease from other conditions, specifically ulcerative colitis. We performed a blind prospective study and examined tissue from 34 colonic specimens, in 15 patients, by electron microscopy. Cases of Crohn's disease (3 cases), ulcerative colitis (5 cases), and other controls [adenocarcinoma (4 cases), familial polyposis (1 case), peritonitis (1 case), and radiation co

Influence of urban/rural and coastal/inland environment on the prevalence, phenotype, and clinical course of inflammatory bowel disease patients from northwest of Spain: a cross-sectional study.

BACKGROUND: Data on the influence of rural/urban and coastal/inland environment on inflammatory bowel disease (IBD) are either conflicting or lacking. Our aim was to analyze whether the environment has any influence on the prevalence, phenotype, and course of IBD. MATERIALS AND METHODS: We carried out a multicenter retrospective study in 1194 IBD patients from Galicia, Spain. Urban areas were defined as those with over 25,000 inhabitants. Sex, age, family history, smoking, Montreal classification, extraintestinal manifestations, steroid dependence/refractoriness, and treatment were assessed.

Loss of the PDLIM2 protein during chronic colitis promotes inflammation, impaired epithelium recovery, alterations to the microbiome and oxidative stress.

INTRODUCTION: Ulcerative colitis (UC) involves impaired wound healing processes contributing to sustained immune and microbial interactions that aggravate intestinal injury and may progress to colitis-associated cancer (CAC). Here we investigated whether PDLIM2, a known regulator of both epithelial and immune cell fate, contributes to colitis progression. METHODS: PDLIM2 knockout mice (-/-) and wildtype littermates (+/+) were assessed for responses to dextran sodium sulphate (DSS)-induced colitis, and to aoxymethane +DSS. Microbiota were assessed using 16s rRNA amplicon sequencing. Mechanisti
