Addressing Structural Barriers to HIV Care among Triply Diagnosed Adults: Project Bridge Oakland.

People who are "triply diagnosed" with HIV, mental health issues, and substance-related disorders face tremendous barriers connecting to and remaining in HIV care. Authors of this article implemented Project Bridge Oakland (PBO), an intervention based on harm reduction and trauma-informed care, to help maintain continuity of care for triply diagnosed adults through cycles of criminal justice involvement. From August 2011 to December 2014, a clinical social worker and an HIV physician provided intensive case management for 19 clients living in Oakland, California. By working with clients across

Values that Fathers Communicate to Sons about Sex, Sexuality, Relationships, and Marriage.

African American males between ages 13 and 24 are disproportionately affected by unintended pregnancy and sexually transmitted infections (STIs) such as chlamydia, gonorrhea, syphilis, and HIV/AIDS; indeed, they account for 50% of HIV infections among all youth. Clear communication between parents and their youth about sex is associated with higher rates of sexual abstinence, condom use, and intent to delay initiation of sexual intercourse, which can prevent STIs and unintended pregnancy. However, barriers exist for parents to educate their youth about sexual health. The purpose of this articl

Organ transplantation in Greece: the need for mediation.

Organ transplants are not regarded as an exclusively medical process, because they involve financial, religious, philosophic, and bioethical parameters. It becomes clear that if they are to achieve their purpose, which we believe extends well beyond the medical dimension, the creation of a comprehensive framework of communication between the involved parties is of paramount importance. The aim of this paper is to present an outline and a number of considerations regarding the communicational, bioethical, and legal issues that arise from a rather dramatic state of affairs in Greece today: In 20

The unmet medical needs of correctional populations in the United States.

The goal of this study was to document the health needs of members in the criminal justice system and the capacity of the system to meet those needs. Using data from the National Criminal Justice Treatment Practices survey, we estimated the number of adults under correctional control who need medical care and the capacity of the correctional system to provide needed care. A mailed survey of 431 adult correctional agencies and administrators was used to define the capacity and techniques of the correctional system to meet the medical needs of the offender population. Most offenders in jail and

Adoption and the communication of genetic risk: experiences in Huntington disease.

Adoption agencies can use genetic information to determine the eligibility of prospective adoptive parents and to establish a child's suitability for adoption. We describe experiences and implications of communicating genetic risk for Huntington disease (HD) in the context of adoption. A secondary analysis was employed using data collected from a cross-sectional survey (n = 233) and two qualitative studies on the psychosocial effects of predictive testing for HD. We demonstrate several ethical and practical challenges in the search for and communication of genetic information for adoptees and

Family caregiver engagement in a coping and communication support intervention tailored to advanced cancer patients and families.

We describe family caregiver (FCG) participation in a tailored coping and communication support intervention for advanced cancer patients and families. Preliminary descriptive data on characteristics and patterns of early engagement in the intervention for 132 FCGs are presented. Engagement assessed at an initial in-person meeting and the following 6 weeks includes problems raised, strategies used, and initiation, frequency, and mode of contact. A sizable proportion of FCGs were male, African American, and nonspouse. The FCGs had modest scores on a standard survey measure of burden yet volunte

Community Health Workers for Primary Healthcare Access (COMPASS), integrating a comprehensive CHW intervention in primary healthcare in Belgium: a cluster-randomized controlled trial protocol.

BACKGROUND: Inequality in access to Primary Health Care (PHC) is increasing in Belgium, in contrast to an overall decreasing trend throughout the European Union. A (cost-)effective solution for access-to-care challenges for people who experience challenges accessing primary healthcare (PECAP) in Belgium is needed; to address this knowledge gap, the research team drew on lessons from the CHW model in the Family Health Strategy (Brazil) and Re-engineering PHC (South Africa), in line with reciprocal innovation, to develop the Community Health Workers for Primary Healthcare Access (COMPASS) interv

Barriers and effective interventions associated with diabetes management among the East Asian immigrant population: A scoping review.

BACKGROUND: The prevalence of diabetes has rapidly increased for East Asian immigrant populations, exceeding rates in East Asian populations in their home countries and the general population of host countries. The increased risk highlights the complex interplay between genetic predisposition and socio-cultural environmental factors associated with migration. Managing diabetes and navigating unfamiliar healthcare systems are challenging for immigrant populations underscoring the need for research on barriers and effective targeted strategies. OBJECTIVES: Identify barriers to best diabetes man

Consent in organ transplantation: putting legal obligations and guidelines into practice.

Consent in medical practice is a process riddled with layers of complexities. To some extent, this is inevitable given that different medical conditions raise different sets of issues for doctors and patients. Informed consent and risk assessment are highly significant public health issues that have become even more prominent during the course of the Covid-19 pandemic. In this article we identity relevant factors for clinicians to consider when ensuring consent for solid organ transplantation. Consent to undergo solid organ transplantation is more complex than most surgical and other clinical

On Defense of the Hazard Ratio.

In this short communication, we describe the recent debate on whether the hazard function should be used for causal inference in time-to-event studies and consider three different potential outcomes frameworks (by Rubin, Robins, and Pearl, respectively), as well as use the single-world intervention graph to show mathematically that the hazard function has causal interpretations under all three frameworks. In addition, we argue that the hazard ratio over time can provide a useful interpretation in practical settings.

The Flavonoids Hesperidin and Diosmin: Adjuvant Therapy for Obesity.

The flavonoids are polyphenolic plant products, characterized by a 15-carbon skeleton which includes two benzene rings. Two active flavonoids, diosmin and hesperidin, are used as venotonic and vasculoprotective agents in clinical practice. They are indicated for the management of varicose veins, chronic venous insufficiency and haemorrhoids. They help improve circulatory tone, reduce inflammation, strengthen venous walls, and alleviate symptoms such as discomfort, pain and swelling. In this communication, we explore their potential use in obesity, especially obesity related oedema (obestasis).

Listening to the Voice of Laryngeal Dystonia: Advocating from the Inside Out.

Some 29 million people in the United States have the neurological voice disorder spasmodic dysphonia (SD), a type of what is now more accurately called laryngeal dystonia (LD), characterized by an altered voice, the result of involuntary and intermittent contractions of the laryngeal muscles. This personal account details key facts about LD, as its etiology and treatment are still in research. It attests to how LD compromises communication and can threaten people's self-esteem, even their lives, and provides implications for nursing practice.

Access to 2-Oxabicyclo[2.1.1]hexanes and their use in Scaffold Hopping.

Saturated isosteres of the ortho-substituted benzene ring remain rare due to the paucity of methods to access complex bridged systems. Using blue-light-mediated [2 + 2] photocycloaddition chemistry, we have developed a quick and practical route to provide novel 2-oxabicyclo[2.1.1]hexanes from simple feedstock materials in only three steps. Matched pair analysis confirmed that this motif could prove useful in the future to the drug discovery community as a scaffold endowed with remarkable properties.

Diagnosis and Management of Laryngotracheoesophageal Cleft.

Laryngotracheoesophageal cleft (LTEC) is a rare anatomic cause of aspiration in the newborn and infant. The clinical presentation of infants with LTEC is variable and will usually vary with the degree of anatomic communication between the airway and esophagus. The diagnosis of LTEC is often challenging, particularly in cases where the presentation and pathology are more subtle. Management of LTEC depends upon the clinical presentation, degree of aspiration, and anatomic depth of the cleft. The goal of this review is to present the clinician with a practical approach to diagnosis and management

Diagnostic Approach to the Acute Abdomen.

The acute abdomen is a term used to characterize an animal presented as an emergency, in a more or less severe critical state, and for which medical and possibly surgical treatment will be necessary. To succeed, the clinician should use a systematic approach, have an excellent knowledge of the bovine abdominal anatomy, and have a good understanding of the pathophysiology of abdominal pain. Good clinical judgment, critical analysis, and good client communication skills are also essential. This article presents and discusses those necessary skills in the context of field practice.

Endoscopic third ventriculostomy: anesthetic implications.

Endoscopic third ventriculostomy (ETV) is one of the recent neurosurgical advances for the treatment of obstructive hydrocephalus. There has been number of publications, which have established the role of ETV in neurosurgical practice, particularly in hydrocephalus. ETV has developed into a therapeutic alternative to shunting for the management of patients with non-communicating hydrocephalus. This procedure requires a general anesthetic and necessitates violation of the brain parenchyma and manipulation via neural structures to access the floor of the third ventricle. This discussion will foc
