Elective lymph node irradiation with intensity-modulated radiotherapy: is conventional dose fractionation necessary?

PURPOSE: Intensity-modulated radiation therapy (IMRT) is the standard of care for head-and-neck cancer (HNC). We treated patients with HNC by delivering either a moderate hypofractionation (MHF) schedule (66 Gy at 2.2 Gy per fraction to the gross tumor [primary and nodal]) with standard dose fractionation (54-60 Gy at 1.8-2.0 Gy per fraction) to the elective neck lymphatics or a conventional dose and fractionation (CDF) schedule (70 Gy at 2.0 Gy per fraction) to the gross tumor (primary and nodal) with reduced dose to the elective neck lymphatics. We analyzed these two cohorts for treatment ou

Platelet-endothelial interactions: sepsis, HIT, and antiphospholipid syndrome.

Acquired abnormalities in platelets, endothelium, and their interaction occur in sepsis, immune heparin-induced thrombocytopenia (HIT), and the antiphospholipid syndrome. Although of distinct pathogeneses, these three disorders have several clinical features in common, including thrombocytopenia and the potential for life- and limb-threatening thrombotic events, ranging from microvascular (sepsis > antiphospholipid > HIT) to macrovascular (HIT > antiphospholipid > sepsis) thrombosis, both venous and arterial. In Section I, Dr. William Aird reviews basic aspects of endothelial-platelet interact

Concurrent primary carcinoma of the gallbladder and acute cholecystitis.

BACKGROUND/AIMS: Primary carcinoma of the gallbladder is rare and associated with a late diagnosis and poor prognosis. Concurrent acute cholecystitis frequently obscures the presence of carcinoma. The information regarding gallbladder carcinoma with acute cholecystitis is limited. In order to better understand the presentation of gallbladder carcinoma with acute cholecystitis, we retrospectively reviewed the data of patients with primary carcinoma of the gallbladder. METHODOLOGY: The data of 86 patients with primary carcinoma of the gallbladder treated between 1979 and 1994 were compiled and

Treatment of stable burst fracture of the atlas (Jefferson fracture) with rigid cervical collar.

STUDY DESIGN: A retrospective review of a clinical series. OBJECTIVE: To evaluate the use of a rigid cervical collar alone as the treatment for stable Jefferson fracture, and to devise an algorithm for treatment of Jefferson fracture with or without an associated cervical injury. SUMMARY OF BACKGROUND DATA: The traditional treatment for Jefferson fracture, if there is no indication for surgery, is immobilization by halo vest. Because halo vest placement is associated with intracranial infection and a significant degree of patient discomfort, slightly less rigid forms of external immobilizati

Indacaterol, a once-daily beta2-agonist, versus twice-daily beta₂-agonists or placebo for chronic obstructive pulmonary disease.

BACKGROUND: Indacaterol is an inhaled long-acting beta2-agonist that is administered once daily and has been investigated as a treatment for chronic obstructive pulmonary disease (COPD). Four different doses have been investigated (75 mcg, 150 mcg, 300 mcg and 600 mcg). The relative effects of different doses of once-daily indacaterol in the management of patients with COPD are uncertain. OBJECTIVES: To compare the efficacy and safety of indacaterol versus placebo and alternative twice-daily long-acting beta2-agonists for the treatment of patients with stable COPD. Irisin as a Neuroprotective Agent in Parkinson's Disease: The Role of Physical Exercise in Modulating Dopaminergic Neurons.

Exercise-induced myokines have emerged as crucial mediators of the beneficial effects of physical activity on neurodegenerative diseases through complex molecular mechanisms involving oxidative stress reduction, neuroinflammation suppression, and synaptic plasticity enhancement. Among these myokines, irisin, encoded by the FNDC5 gene, has gained significant attention as a potential therapeutic target in neurodegenerative conditions due to its ability to cross the blood-brain barrier and exert pleiotropic neuroprotective effects. This review synthesizes current evidence from both preclinical an

Pharmacological management of spondyloarthritis associated with inflammatory bowel disease: a systematic review of efficacy, safety, and emerging therapies.

BACKGROUND: Spondyloarthritis (SpA) is a prevalent extraintestinal symptom of inflammatory bowel disease (IBD), impacting up to 20% of patients and considerably contributing to the disease burden. The coexistence of inflammatory bowel disease and spondyloarthritis poses therapeutic problems due to the necessity for simultaneous management of intestinal and musculoskeletal inflammation. OBJECTIVE: To conduct a comprehensive review and synthesis of the current evidence regarding pharmacological treatments for IBD-associated SpA. This review will assess the efficacy of these treatments on both gu

Acute Kidney Injury in Hospitalized Patients With Exertional Rhabdomyolysis.

IMPORTANCE: An association between serum creatine kinase (CK) levels and the risk of kidney failure in patients with exertional rhabdomyolysis (ERM) has been suggested. However, the actual incidence of AKI in hospitalized patients with ERM along with the contributing cofactors that may increase the risk of AKI have rarely been investigated. OBJECTIVES: To examine the incidence of kidney injury in hospitalized patients with ERM and to identify additional cofactors that might contribute to the development of kidney injury in patients with ERM. DESIGN, SETTING, AND PARTICIPANTS: This retrospect

Experimental Chemotherapy-Induced Mucositis: A Scoping Review Guiding the Design of Suitable Preclinical Models.

Mucositis is a common and most debilitating complication associated with the cytotoxicity of chemotherapy. The condition affects the entire alimentary canal from the mouth to the anus and has a significant clinical and economic impact. Although oral and intestinal mucositis can occur concurrently in the same individual, these conditions are often studied independently using organ-specific models that do not mimic human disease. Hence, the purpose of this scoping review was to provide a comprehensive yet systematic overview of the animal models that are utilised in the study of chemotherapy-ind

Cost analysis of removing pediatric ureteral stents with and without a retrieval string.

BACKGROUND: Indwelling ureteral stents are commonly placed in urologic surgeries where optimal urinary drainage is necessary. In the pediatric population, removing a stent without retrieval string (SWOS) requires a secondary operation and additional anesthetic exposure. Although these burdens can be mitigated through the placement of a stent with retrieval string (SWS), fears of complications may prevent widespread adoption of this practice by pediatric urologists. OBJECTIVE: The authors sought to assess the differential cost of removing SWS and SWOS. It was hypothesized that costs associated

Safety evaluation of the concomitant use of clozapine and benzodiazepines: a retrospective, cross-sectional chart review.

OBJECTIVE: Previously published case reports have noted severe adverse reactions such as cardiac arrest, respiratory arrest, and sudden death when clozapine (CLZ) and benzodiazepines (BZDs) are used concomitantly. As CLZ and BZD are both used regularly to treat psychiatric illness, it is important to have additional information concerning this potential interaction. The objective of this study was to contribute to the evolving literature by evaluating the occurrence of sudden deaths and cardiac or respiratory events leading to death in patients treated concurrently with CLZ and BZD. METHOD: A

The effect of music on breastfeeding: a systematic review and meta-analysis of breast milk production, composition, and maternal stress.

BACKGROUND: Breastfeeding is the gold standard for infant nutrition, yet maternal stress and anxiety, especially in the Neonatal Intensive Care Unit (NICU) or due to postnatal pain, can inhibit the oxytocin-mediated milk ejection reflex, leading to reduced milk production and early cessation. Music intervention has been proposed as a non-pharmacological strategy to mitigate these stress responses. OBJECTIVE: This systematic review and meta-analysis aims to evaluate the effect of music interventions on objective breastfeeding outcomes (milk volume and composition), salivary cortisol stress mar

Transthoracic Ultrasound to Predict Exudative Pleural Effusion Etiology.

OBJECTIVES: Pleural effusions often require invasive sampling to establish underlying etiologies. Transthoracic ultrasound (TUS) has shown promise in the diagnostics of pleural effusions; however, there lacks consensus regarding its clinical application. We evaluated the diagnostic utility of specific TUS findings for exudative effusions, specifically complex parapneumonic effusion and empyema. METHODS: Ultrasound-guided pleural effusion drainage cases were retrospectively reviewed at a single university-based medical center from July 2015 to May 2023. Procedure-related images were reviewed f

Segmental Zoster Paresis: A Literature Review.

BACKGROUND: Herpes zoster is an acute infectious skin disease that is induced by the re-activation of the virus incubated in nerve ganglions following initial infection with varicella-zoster virus in childhood. Herpes zoster mainly affects sensory nerves, resulting in severe acute pain, which is also the most common reason for medical intervention in this patient group. The concurrent involvement of motor nerves could induce the symptoms of segmental zoster paresis, which is manifested by localized asymmetric myasthenia, whose range generally follows the distribution of myomere with skin rashe

Theragenerative Nanomaterials: Integrating Therapy, Regeneration, and Diagnosis-Navigating the Shared Pathways Between Tissue Repair and Malignancy.

BACKGROUND: Nanotechnology convergence with regenerative medicine and oncology creates opportunities and challenges. Nanomaterials can be programmed to modulate biological responses, yet tissue repair and cancer share fundamental signaling pathways-including Wnt/β-catenin, Mitogen-Activated Protein Kinase (MAPK)/ Extracellular Signal-Regulated Kinase (ERK), Notch, and Hedgehog-raising a critical concern: pro-regenerative stimuli may inadvertently activate malignant programs. This review introduces the "theragenerative" paradigm, defined as nanomaterial platforms that concurrently integrate the
