Examining the light and dark sides of emerging adults' identity: a study of identity status differences in positive and negative psychosocial functioning.

Identity is a critical developmental task during the transition to adulthood in Western societies. The purpose of the present study was to evaluate an empirically based, cluster-analytic identity status model, to examine whether all four of Marcia's identity statuses (diffusion, foreclosure, moratorium, and achievement) would emerge empirically, and to identify different patterns of identity formation among American college-attending emerging adults. An ethnically diverse sample of 9,034 emerging-adult students (73% female; mean age 19.73 years) from 30 U.S. universities completed measures of

[A case of metastatic colorectal cancer suffering from hyperammonemic encephalopathy induced by 5-FU, continuously treated with FOLFOX therapy].

We report a rare case of metastatic colorectal cancer who suffered from hyperammonemic encephalopathy induced by 5- FU and was continuously treated with FOLFOX therapy. A 50-year-old man with ileus was diagnosed with ascending colon cancer Stage IV, and a right hemicolectomy was performed. Postoperative chemotherapy with modified FOLFOX6 was performed. Complications of nausea and vomiting were seen on day 2 , with confusion and cognitive disturbances on day 3 . None of the other radiographic examinations provided an explanation for his symptoms. Laboratory examination revealed hyperammonemia,

How do we measure dental fear and what are we measuring anyway?

Dental anxiety shares similar characteristics with many clinical anxiety disorders, and this is especially the case with other specific fears and phobias. These often debilitating conditions comprise several different dimensions, including cognitive, emotional, behavioural and physiological components. In addition, dental anxiety and fear are associated with a range of aversive health consequences. A number of indices have been developed to measure dental anxiety and fear, but their sheer number is indicative of a continuing problem with delineating the concept of dental fear and anxiety and h

A case of 5-fluorouracil induced encephalopathy.

Patients with reduced dihydropyrimidine dehydrogenase (DPD) activity are at risk for experiencing serious adverse effects following 5-fluorouracil (5-FU) based chemotherapy. Neurotoxicity is considered an extremely rare side effect of 5-FU. We report here on an unusual case of 5-FU induced encephalopathy. A 38-year-old woman with advanced gastric carcinoma was treated with adjuvant chemotherapy that consisted of infused 5-FU (1,000 mg/m²) for 5 days and cisplatin (60 mg/m²) on day 1 following total gastrectomy. Nineteen days after starting chemotherapy, the patient displayed a sudden onset of

Gabapentin reduces neurovisceral pain of porphyria.

PURPOSE: Gabapentin is well known for pain control. Here, we report that gabapentin is a good adjunct for visceral pain in a porphyria patient. CASE REPORT: A young female was admitted due to acute abdomen. On admission, she was noted to have hyponatremia, tachycardia, and hypertension. Then, she had episodes of seizure and confusion. Gabapentin was prescribed for the control of seizure and pain before the diagnosis of acute intermittent porphyria was confirmed. Seizure did not occur after gabapentin. Pain severity also significantly reduced with visual analogue scale from 10 to 4. The severi

Multiple antenatal bookings among pregnant women in Enugu, Nigeria.

Some pregnant women do book concurrently with multiple antenatal care providers. Structured questionnaires were administered to women attending antenatal clinics in 12 facilities in Enugu, Nigeria. A total of 535 women responded: 372 (69.5%) were booked into more than one facility and 163 (30.5%) were booked into a single facility; 280 (52.3%) booked into the two Teaching Hospitals and 91 (12.5%) booked with a traditional birth attendant. Reasons for multiple bookings included selecting a facility with affordable prices (43.9%); selecting a facility promising vaginal delivery (35.3%); avoiding

Compressive myelopathy mimicking transverse myelitis.

OBJECTIVES: Spinal cord compression may be associated with a fusiform cord lesion on T2-weighted magnetic resonance imaging (MRI) images, leading to confusion with transverse myelitis and delaying effective surgical treatment. RESULTS: We describe 5 patients referred for evaluation of suspected neuromyelitis optica in whom the final diagnosis was symptomatic cervical spinal stenosis. The patients had gradually progressive myelopathy, with symptoms progressing over an average of 34.4 weeks. Although cervical spinal cord MRI identified long T2 hyperintense lesions, gadolinium enhancement was lo

A variety of atypical manifestations in giant cell arteritis.

Giant cell arteritis (GCA) is a chronic granulomatous vasculitis of large and medium size vessels in the elderly. A new-onset headache is the most frequent symptom. An anterior ischemic optic neuropathy (AION) is one of the most common causes of permanent visual loss. There are four cases with unusual presentation of giant cell arteritis, scalp abscess, prolonged transient monocular visual loss (TMVL), bilateral central retinal artery occlusion (CRAO), and chronic ear pain. All patients had pathologically proven giant cell arteritis, and three of them progressed to blindness in the end. Scalp

The structure of emotional and cognitive anxiety symptoms.

A sample of 327 patients with primary panic disorder or social phobia completed a questionnaire comprising 77 emotional and cognitive anxiety symptoms from which 12 index scales were constructed. Explorative factor analysis yielded two factors, but confirmatory factor analysis indicated that the factor solution was not invariant across diagnoses. Nevertheless, the two-factor structures fitting data from patients with panic disorder and social phobia, respectively, had similarities in content. The first factor, emotions and cognitive-social concerns, comprised emotional expressions (sadness, fe

New generation vs. first generation antipsychotics debate: pragmatic clinical trials and practice-based evidence.

There has been significant confusion about the relative advantage of new generation antipsychotics over first generation antipsychotics as well as of clinical trials performed to evaluate their efficacy vs. those designed to evaluate their effectiveness. Pragmatic or effectiveness clinical trials like CATIE and CUtLASS sponsored by governments have challenged the current worldview of the greater advantages of new generation over first generation antipsychotics and suggested more clinical applicability of older antipsychotics. Public policy regarding the role and place of modern antipsychotics

Complete neurological recovery from fulminant hepatic failure with subarachnoid hemorrhage by living donor liver transplantation: a case report.

A 29-year-old man was referred to our hospital with fulminant hepatic failure (FHF) and stage III hepatic coma (somnolence and confusion). Living donor liver transplantation (LDLT) was planned for 2 days after admission to our hospital. However, on the day after admission, he lapsed into stage IV hepatic coma: no right reflexes and no response to pain stimuli. Emergency cranial computed tomography revealed a subarachnoid hemorrhage (SAH), but no aneurysm was seen on magnetic resonance angiography. We speculated that the cause of the SAH may have been bleeding of intracranial veins secondary to

Systemic lupus erythematosus presenting as hypoglycaemia with insulin receptor antibodies and insulin autoantibodies.

A 37-year-old man presented with sweating, confusion, palpitations, hunger and tremor of 3 months duration. The symptoms disappeared after ingestion of food. After 3 months, he suffered from irregular fever, arthritis, rash, photosensitivity, and was admitted to the hospital. His antinuclear antibody, anti-double stranded DNA antibody, anti-smith antibody and lupus erythematosus cell phenomenon were all positive. Urine analysis showed albuminuria; his 24-h urine protein was 4.7 g. During hospitalisation, the patient presented with loss of consciousness three times because of hypoglycaemia. His

[New approach to apraxia].

Our collective understanding of our approach to apraxia has not dramatically changed over the last century. The classical approach to apraxia has occasionally caused confusion and disagreements because it lacks established definitions for apraxia and related terms such as "praxicon" or "ideation". Rather, the current system allows each researcher to define these terms differently, making it extremely difficult to compare cases reported by different researchers. New approaches, however, are providing more concrete definitions, enabling us to describe apraxic disorders more operationally, this m

Unexplained high thyroid stimulating hormone: a "BIG" problem.

Macro-hormones and macro-enzymes are high molecular weight conjugates of hormones or enzymes, respectively, often with immunoglobulins. These are referred to as macromolecular complexes, and may cause artefactually elevated biochemical tests results. Macro enzymes of the most commonly measured serum enzymes have been identified and are recognised as a source of elevated measurements that may cause diagnostic confusion; macro-creatine kinase and macro-amylase are the two better known macro-enzymes in clinical practice. Literature on macro-hormones is largely restricted to macro-prolactin. We pr

Reversible posterior leukoencephalopathy syndrome in childhood: report of nine cases and review of the literature.

Reversible posterior leukoencephalopathy syndrome (RPLS) is recently described disorder with typical radiological findings in the posterior regions of the cerebral hemisphere and cerebellum. Its clinical symptoms include headache, decreased alertness, mental abnormalities, such as confusion, diminished spontaneity of speech, and changed behavior ranging from drowsiness to stupor, seizures, vomiting and abnormalities of visual perception like cortical blindness. RPLS is caused by various heterogeneous factors, the commonest being hypertension, followed by non-hypertensive causes such as eclamps

EEG abnormalities in poikilothermia suggesting Creutzfeldt-Jakob disease.

A 87-year-old woman was admitted with a rapidly progressive confusion, disorientation and myoclonus, all suggestive of sporadic Creutzfeldt-Jakob disease (sCJD). This diagnosis was initially strongly supported by the EEG, which showed slow background activity and triphasic waves, combined with the finding of an increased level of 14-3-3 protein in the cerebrospinal fluid. Remarkably, this patient had also developed hypothermia, which, after warming-up, resulted in alleviation of the mental disturbances and disappearance of myoclonus. Over time, the EEG abnormalities disappeared. She recovered

Cardio-embolic cerebellar stroke secondary to mitral valve chordae rupture as a delayed complication of a high-voltage electrical injury.

A 41-year-old farmer sustained a high-voltage electrical injury resulting in confusion, electrical burns and paroxysmal atrial fibrillation which spontaneously reverted to sinus rhythm after a few hours. Two weeks later he presented with a sudden onset of headache, unsteadiness, horizontal oscillopsia and paraesthesia in the right side of his face. Examination revealed nystagmus to the right, right-sided limb ataxia and a tendency to veer toward the right when walking. An MRI of the brain demonstrated an acute infarct of the right cerebellar hemisphere in the territory of the right posterior i

Asymmetric mammographic findings based on the fourth edition of BI-RADS: types, evaluation, and management.

Asymmetric breast findings are frequently encountered at screening and diagnostic mammography. However, there has been some confusion about the applicable terminology for describing asymmetric breast findings. The fourth edition of the American College of Radiology Breast Imaging Reporting and Data System (BI-RADS) has incorporated changes in the lexicon for asymmetric breast findings to address such problems and improve the clinical utility of reporting. The fourth edition has replaced "asymmetric breast tissue" with "global asymmetry," "density seen in only a single projection" with "asymmet

The multiple ultrasound patterns of ectopic pregnancy.

Ectopic pregnancy (EP) has a variable and misleading clinical presentation contributing to the confusion with medical or other gynecological disorders. The rapid recourse to diagnostic methods, human chorionic gonadotropin (beta-hCG) titers and transvaginal ultrasonography, represents the best approach not only in the early diagnosis but also in the management and monitoring of patients with diagnosed EP. The purpose of this article is to provide a pictorial essay about EP and its multiple ultrasound (US) patterns. We present a large spectrum of EP aspects diagnosed on US and confirmed by path
