[Giant serpentine aneurysm followed up for more than 10 years: report of a case and review of the literature].

We report a case of a giant serpentine aneurysm (GSA) located at the left internal carotid artery (ICA) and middle cerebral artery (MCA) treated by ligation of the left ICA with superficial temporal artery-middle cerebral artery (STA-MCA) anastomosis. The aneurysm form changed variously during the follow-up period. A 35-year-old man was admitted with severe headache and convulsion. CT scan demonstrated subarachnoid hemorrhage. Left carotid angiogram demonstrated a giant serpentine aneurysm. Ligation of the ICA with STA-MCA anastomosis was performed because of the difficulty involved in clippin

[Cerebral malaria in children in Yaounde, Cameroon. Clinical, paraclinical and developmental aspects].

Cerebral malaria is one of the major and deadly complications of malaria. In Cameroon, recent reports indicate that severe cases of malaria are increasingly more prevalent, particularly in children. The present study aims at describing the clinical presentation and laboratory findings of cerebral malaria in children in Yaounde. All patients admitted in the paediatric ward of Yaounde Central Hospital with malaria, who presented neurological signs and were tested positive for Plasmodium in their peripheral blood were recruited into the study. 36 cases were enrolled in all, making up 2.7% of all

[Early infantile epileptic encephalopathy (Ohtahara syndrome) with poly-microgyria].

A boy, born after normal pregnancy and delivery, began to have fits at 3 days. The seizures were composed of tonic or tonic-clonic convulsions at the upper extremities but myoclonus was absent. These attacks were not easy to control. There was gross developmental delay. Laboratory investigations were almost normal except for cerebrospinal fluid: pleocytosis and high protein content. EEG showed "suppression-burst" and MRI revealed high signal intensity in the left temporo-occipital region on T2 weighted image. At three and a half months of age, EEG changed into hypsarrhythmia. The child died at

An autopsy case of intracranial hemorrhage during tissue plasminogen activator infusion.

We describe an autopsy case of severe intracranial hemorrhage which occurred during the infusion of tissue plasminogen activator (t-PA) for acute myocardial infarction. A 75-year-old man was admitted with substernal chest pain of 3-h duration and electrocardiographic changes consistent with an acute inferior myocardial infarction. Physical examination was unremarkable, except for an initial blood pressure reading of 160/96 mmHg. The patient received 3,000 IU intravenous heparin followed by a 2.4 x 10(6) IU bolus dose of tissue plasminogen activator (t-PA) (Alteplase). This was followed by a dr

Involvement of the central nervous system in renal hypertension.

Involvement of the central nervous system (CNS) is a major complication of renal hypertension and is usually due to hypertensive encephalopathy. During a 17-year period we observed 25 children with renal hypertension associated with CNS manifestations in the absence of (group A) and 33 in the presence of advanced renal failure (group B). Convulsions were the most important symptom (65% in both groups combined). Other manifestations were reduced consciousness (69%), visual disturbances (28%), hemiplegia (14%), and cranial nerve palsy (10%). In 56% of children in group A, hypertension was recogn

[A case of apnea attacks due to an unilateral infarction in the right medulla oblongata].

We described a patient presenting with apnea attacks due to an unilateral small infarction in the right medulla oblongata. The patient was a 55-year-old man and was referred to our hospital because of severe headache and progressive left hemiparesis. Neurological examinations revealed left hemiparesis and the impairment of pain, touch, temperature and vibration sense in the left side of his body. Position sense was normal. Cranial nerves were intact including the trigeminal nerve. Ataxia was not noted. After an admission, the patient suffered from three episodes of apnea attack of sudden onset

Neonatal cerebral infarction: symptoms, CT findings and prognosis.

In a retrospective multi-center study, we investigated eighteen infants with unilateral cerebral infarctions confirmed by computed tomography (CT) scans. The initial symptoms were observed in all the patients between 0 and 3 days of age. Convulsions or apneic attacks were the initial symptoms in all but one. Only 4 patients had complicated obstetric histories and none showed polycythemia or electrolyte abnormalities. All of the initial CT scans revealed unilaterally localized hypodense areas. In 10, the initial CT scans were performed within 24 hours after the clinical onset. In 16, the lesion

Serum prolactin response to metoclopramide during status epilepticus.

Transient elevation of serum prolactin frequently follows generalised tonic-clonic and complex partial seizures. However, the levels of prolactin during status epilepticus are not increased above the normal range. Exhaustion of central prolactin supplies has been proposed as a possible mechanism for the absence of prolactin increase during status epilepticus. To test this hypothesis we injected intravenous metoclopramide (10 mg) in eight consecutive patients with status epilepticus. One patient had generalised tonic-clonic status epilepticus. Seven patients had EEG-verified non-convulsive stat

[Late postpartum eclampsia, myth or reality?].

It has never been formally established whether eclampsia can come on more than 48 hours after delivery. We report a case of a patient who had convulsions together with transitory raised blood pressure coming on 14 days after her delivery. MRI was carried out 24 hours after the attack and showed pathological sub-cortical images and there was a hyper signal at T2. This is now a well known feature and has been described in the course of typical eclampsia fits. On the other hand MRI is able to eliminate a certain number of differential diagnoses such as cerebral thrombophlebitis, cerebral vascular

A case of 25-year-old woman with mitochondrial encephalomyopathy, lactic acidosis and stroke-like episodes (MELAS) was reported. She had short stature, episodic vomiting with headache, several episodes with homonymous hemianopsia, progressive intellectual decline, generalized convulsion, muscular atrophy, sensory disturbance on the left side of the body, and primary amenorrhea. Lactate, pyruvate and the lactate to pyruvate ratio were elevated in the serum and cerebrospinal fluid. Muscle biopsy revealed ragged-red fibers. [Exanthema subitum, encephalopathy and hepatitis caused by human herpesvirus type 6 (HHV-6) in a 10-month-old infant].

We describe the unusual clinical course in a case of exanthema subitum with affection of the liver and central nervous system in a 10-months-old girl. HHV-6 infection was confirmed serologically (positive HHV-6 IgM from 10th to 29th day, increasing IgG-titres). At the beginning of the illness convulsions with preference to the right side were noticed, which were consistent with an encephalitis (on top to a suspected pre/perinatal lesion) and resulting in spastic triplegia. Nuclear magnetic resonance imaging and cranial computertomographic results showed severe, predominantly left-sided cerebra

Scorpion sting in children in the Jerusalem area: a review of 54 cases.

Fifty-four children from the Jerusalem area were studied prospectively following scorpion envenoming. Their ages ranged from 11 months to 10 years. Severe symptoms (convulsions, brain oedema, shock, respiratory distress and myocarditis) were encountered in 19. Respiratory distress was the main feature in 17 of the children, in two cases owing to pulmonary oedema and in a third because of adult respiratory distress syndrome and myocarditis; mechanical ventilation was required in three cases. The severity of the symptoms and signs was not related to sex, age, weight, interval between scorpion st

A prefecture-wide survey of mumps meningitis associated with measles, mumps and rubella vaccine.

A survey of untoward reactions, especially central nervous system reactions, after the administration of a newly introduced measles, mumps and rubella (MMR) vaccine in Gunma Prefecture, Japan, was initiated soon after 4 patients were hospitalized for aseptic meningitis. Thirty-five, 6 and 2 children developed meningitis, convulsive disorders and parotitis, respectively, within 2 months after MMR vaccination during the 8-month period extending from April to November, 1989. The time lag between MMR vaccination and meningitis ranged from 14 to 28 days in the 35 cases of meningitis. Mumps virus, i

Interaction of high pressure and a narcotic dose of ethanol on spontaneous behavior in rats.

This study analyses the spontaneous motor activity of rats that had received a narcotic dose of ethanol (3.5 g/kg) and were then exposed to 1 atmosphere absolute pressure (ATA) air or to 1 or 72 ATA of helium-oxygen (heliox). The ambient temperature was adjusted to offset ethanol-and helium-induced hypothermia. Ethanol administration prevented the occurrence of convulsions but did not alter the total number of myoclonic jerks at stable pressure. The ethanol-intoxicated animals exposed to high pressure did not exhibit normal locomotion but showed a trend towards increased activity during the la

Prilocaine for Bier's block: how safe is safe?

Prilocaine has become the agent of choice for Bier's block (or intravenous regional anaesthesia--IVRA), since 1983 when the product licence of bupivacaine was withdrawn for this purpose owing to fatal or serious complications. No serious complications have been documented in the literature relating to prilocaine in IVRA and we have conducted a survey within the U.K. which indicates that about 45,000 Bier's blocks have been carried out with prilocaine without convulsion, arrhythmia or fatality. This includes cases of accidental cuff deflation or even failure to inflate the cuff, resulting in bo

Eclampsia in Kelantan.

A review of eclampsia in Kelantan was undertaken from 1983-1988. There were 146 documented cases in the state (66 per 100,000 deliveries). Eight maternal deaths occurred. Sixty seven (45.9%) were primigravida. Six of the 79 multiparous women developed eclampsia for the first time following remarriages to new partners. The multisystem dysfunction resulting from eclampsia resulted in varied maternal complications. Fatal cerebral haemorrhage (3 cases), acute pulmonary oedema (8 cases), acute renal failure (6 cases), HELLP Syndrome (8 cases) and acute abruptio placentae were the commoner complicat

[Comparative dynamics of the epileptic syndrome in patients with open and closed war-time head injuries in the late period].

The dynamics of the epileptic syndrome was studied in 55 patients with penetrating cranial wound and in 65 patients with closed cranial injury. Account was taken of the anamnesis, all-round clinical examination data, EEG and computer-aided tomography readings. The patients had been followed up over 40 years. Among the patients with fighting cranial injury, the incidence of the epileptic syndrome amounted to 14.01%. The epileptic syndrome was established to occur most frequently during the first year after injury, with that time (in patients with penetrating cranial wound) being the less the gr

Seizure prognosis and EEG evolution in complex partial seizures of childhood onset.

We studied the clinical course and seizure prognosis of 126 children with complex partial seizures regularly followed up for more than 4 years in our clinic. Clinical and EEG features of 63 seizure-free patients were compared with those of 63 patients with persistent seizures. The features contributing to poor prognosis were 1) mental retardation, 2) a history of status epilepticus and 3) abnormal basic rhythm in EEG. CT abnormality, a history of febrile convulsions (FC), the clustering of seizures and association with other types of seizures did not influence prognosis. We divided the patient

Survey of seizure disorders in the French southwest. I. Incidence of epileptic syndromes.

An epidemiologic survey began on March 1, 1984, and ended on February 28, 1985. During this period, all neurologists and electroencephalographers of the department of Gironde, an administrative district of the French Southwest (1,128,164 residents in 1982) obtained information by questionnaire from all persons who had experienced an epileptic seizure for the first time in their lives. Recurrent, isolated, and situation-related seizures were included. Febrile convulsions and neonatal seizures were excluded. The global incidence rate of diagnosed epileptic seizures was 71.3/100,000.
