Nosocomial Cholera Outbreak in a Mental Hospital: Challenges and Lessons Learnt from Butabika National Referral Mental Hospital, Uganda.

During the last four decades, Uganda has experienced repeated cholera outbreaks in communities; no cholera outbreaks have been reported in Ugandan health facilities. In October 2008, a unique cholera outbreak was confirmed in Butabika National Mental Referral Hospital (BNMRH), Uganda. This article describes actions taken to control the outbreak, challenges, and lessons learnt. We reviewed patient and hospital reports for clinical symptoms and signs, treatment and outcome, patient mental diagnosis, and challenges noted during management of patients and contacts. Out of 114 BNMRH patients on two

Comparative susceptibility of Channel Catfish, Blue Catfish, and their hybrid cross to experimental challenge with Bolbophorus damnificus (Digenea: Bolbophoridae) cercariae.

The digenetic trematode Bolbophorus damnificus has been implicated in significant losses in catfish aquaculture since the late 1990s. The complex life cycle sequentially involves the American white pelican Pelecanus erythrorhynchos, the marsh rams horn snail Planorbella trivolvis, and Channel Catfish Ictalurus punctatus. Research supports anecdotal reports from the industry, suggesting that the hybrid of Channel Catfish×Blue Catfish I. furcatus is less susceptible to disease agents that have been historically prohibitive to Channel Catfish production, namely the gram-negative bacteria Edwardsi

A five-year survey of dematiaceous fungi in a tropical hospital reveals potential opportunistic species.

Dematiaceous fungi (black fungi) are a heterogeneous group of fungi present in diverse environments worldwide. Many species in this group are known to cause allergic reactions and potentially fatal diseases in humans and animals, especially in tropical and subtropical climates. This study represents the first survey of dematiaceous fungi in Malaysia and provides observations on their diversity as well as in vitro response to antifungal drugs. Seventy-five strains isolated from various clinical specimens were identified by morphology as well as an internal transcribed spacer (ITS)-based phyloge

Foodborne listeriosis acquired in hospitals.

Listeriosis is characterized by bacteremia or meningitis. We searched for listeriosis case series and outbreak investigations published in English by 2013, and assessed the strength of evidence for foodborne acquisition among patients who ate hospital food. We identified 30 reports from 13 countries. Among the case series, the median proportion of cases considered to be hospital-acquired was 25% (range, 9%-67%). The median number of outbreak-related illnesses considered to be hospital-acquired was 4.0 (range, 2-16). All patients were immunosuppressed in 18 of 24 (75%) reports with available da

Prevalence and etiology of onychomycosis in the HIV-infected Mexican population.

Onychomycosis is a fungal infection of the nail caused by dermatophytes, yeasts, and non-dermatophyte molds. Human immunodeficiency virus (HIV)-infected patients are predisposed to this infection. In a cross-sectional study, we estimated the prevalence of onychomycosis and the frequency of fungal agents among HIV-infected patients in Mexico. We enrolled HIV-infected patients diagnosed clinically with onychomycosis from 2008 to 2010. Samples were collected from 300 (84% men) HIV-positive patients by scraping of subungual hyperkeratotic debris and nail plate clipping. All specimens were subjecte

Nosocomial myiasis in a patient with diabetes.

This report describes a case of nosocomial myiasis caused by larvae of Sarcophaga (Bercaea) africa. The parasitosis developed in an ulcer on the heel of a patient with type 2 diabetes. The ulcer was dressed when the deposition occurred. An experiment was performed in order to demonstrate the ability of Sarcophaga larvae to move through bandages and reach purulent wounds. This report highlights the need for particular attention, education and specific protocols in hospitals in order to avoid myiasis, which can compromise the organization's reputation for hygienic standards with possible legal c

Assessing risk of health care-acquired Legionnaires' disease from environmental sampling: the limits of using a strict percent positivity approach.

BACKGROUND: Elevated percent positivity (≥30%) of Legionella in hospital domestic water systems has been suggested as a metric for assessing the risk of health care-acquired Legionnaires' disease (LD). METHODS: We examined the validity of this metric by analyzing data from peer-reviewed studies containing reports of Legionella prevalence in hospital water (ie, percent positivity) and temporally matched reports of patients with health care-acquired LD. RESULTS: Our literature review identified 31 peer-reviewed publications reporting matched data. We abstracted a total of 206 data points, repr

[Tuberculosis infection control - recommendations of the DZK].

The epidemiological situation of tuberculosis (TB) in Germany has improved considerably during the past few years. However, those in unprotected contact with infectious tuberculosis patients frequently and/or over longer periods of time and/or intensively continue to have a higher risk for TB infection. Rapid diagnosis, prompt initiation of effective treatment, and adequate infection control measures are of particular importance to prevent infection. The present recommendations depict the essentials of infection control as well as specific measures in the hospital (isolation, criteria for its

[Surveillance of nalidixic acid-resistant and extended-spectrum beta-lactamase-producing Salmonella spp. isolated from human feces].

Nalidixic acid (NA)-resistant and extended-spectrum beta-lactamase (ESBL)-producing Salmonella sp. isolates from human specimens are associated with clinical failure or delayed response in subjects treated with fluoroquinolone or third-generation cephalosporins. We studied drug susceptibility in 604 Salmonella enterica isolates from human feces in 2007. Of these, 39 (6.5%) were resistat to NA. Of these, 46% were resistant to two or more drugs and 2% susceptible to NA were resistant to multiple drugs (p < 0.001). Three ESBL-producing Salmonella sp. isolated were of the CTX-M family gene type. O

[Gut translocation septicemia in infant].

Intestinal microbiotype necessary for life is a source of complications in childhood. Bacterial translocation is responsible of endogenous septicaemia and invasive complications. We report five cases of severe invasive infections associated with diarrhoea, digestive bleeding or sepsis. Biological parameters for inflammation are highly positive, and blood cultures reveal bacterial identification: salmonella enteridis, enterobacter cloacae, campylobacter jejuni, escherichia coli or clostridium difficile. We describe the predisposing factors and susceptibility status to develop translocation: inv

Nosocomial invasive infection caused by Cunninghamella bertholletiae: case report.

Human infection by Cunninghamella bertholletiae occurs almost exclusively in immunocompromised patients. Infections due to this microorganism have been most frequently diagnosed in patients with hematological malignancies, with neutropenia and in diabetes mellitus patients. This work reports a case of fungal infection by Cunninghamella bertholletiae isolated from blood in a man with a complex clinical picture, involving diabetes and pharmacological immunosupression. Blood culture at room temperature and at 37 degrees C on Sabouraud agar grew a single mold with characteristic properties of Cunn

Nosocomial nasal myiasis owing to Cochliomyia hominivorax: a case in French Guiana.

Abstract The larval stages of the fly Cochliomyia hominivorax are responsible for myiasis, which primarily affects wounds. We report the case of a bed-ridden patient with dementia who developed right nasal myiasis during his stay at Cayenne Hospital. Progression was favorable, but the nasal pyramid was partially destroyed. In zones where this fly is endemic, particular attention should be given to hospitalized patients with wounds and consciousness problems.

[Legionella prevention in intensive care units].

From time to time hospitals are in the focus of the local and national media because of cases of nosocomial Legionnaires' disease. In particular intensive care patients are a risk group because of their high amount of immunocompromised patients with an increased risk of developing a Legionellosis. Therefore it is an important aim to maintain an effective prevention system. Nevertheless in some hospitals legionella can be identified. Particularly in these hospitals a high level of attention is required to recognize cases of legionellosis and to take adequately steps to avoid outbreaks of legion

Invasive neonatal candidiasis: an overview.

Until the 1980s, recovery of Candida species from normally sterile body sites in high-risk infants was often dismissed as contamination. Such delay in diagnosis often resulted in death, multifocal disease, or significant morbidity, outcomes that still occur today. Problems establishing the diagnosis of invasive candidiasis persist, while the frequency of this infection in increasingly fragile and smaller premature infants appears to be increasing. Standard culture techniques have limited sensitivity and often take several days for recovery of Candida when positive. Heightened suspicion and imp

Managing the risk of nosocomial transmission of prion diseases.

PURPOSE OF REVIEW: Prion diseases such as Creutzfeldt-Jakob disease represent a unique infection control problem because prions exhibit an unusual resistance to conventional chemical and physical decontamination methods. This paper reviews the recent literature and provides recommendations for the prevention of nosocomial transmission of prion agents. RECENT FINDINGS: Recommendations to prevent the cross-transmission of infection from medical devices potentially contaminated with prions have been based primarily on prion inactivation studies. Newer recommendations consider inactivation data,
