Optimal bus bridging service for urban rail transit disruptions with stochastic passenger demand.

Disruptions in urban rail transit (URT) systems can significantly impact operational efficiency, while well-designed bus bridging service (BBS) can effectively mitigate such effects. To address the surge in travel demand caused by disruptions, this study comprehensively considers alternative transportation modes that affected passengers may adopt (including taxis, shared bicycles, bridging buses, and walking), aiming to minimize both the operational costs of bridging buses and the total travel time of passengers. A travel choice model based on the random regret minimization (RRM) theory is dev

The impact of a patient decision aid for patients with advanced laryngeal carcinoma - a multicenter study.

PURPOSE: Patients with advanced larynx cancer face challenging treatment decisions. To address this, we developed and tested a patient decision aid (PDA), aiming to reduce decisional conflict (DC), and enhance knowledge and perceived shared decision-making (SDM). METHODS: In this multicenter study (ClinicalTrials.gov ID: NCT03292341, 2016-2023), a pre/post study design was used. Participants, meeting the inclusion criteria of advanced larynx cancer without distant metastasis, completed questionnaires on knowledge, DC and SDM immediately after counseling (T1) and 6 months post-treatment (T2).

Development and evaluation of a patient decision aid for patients considering ongoing medical or surgical treatment options for ulcerative colitis using a mixed-methods approach: protocol for DISCUSS study.

INTRODUCTION: Approximately 20%-30% of patients with ulcerative colitis (UC) require surgery, the majority of these being elective due to chronic symptoms refractory to medical treatment. The decision for surgery is difficult and dependent on patient preferences. Current resources for patients considering surgery have been found not to meet minimum international standards. The overall aim of the 'DISCUSS' study is to develop and evaluate a new patient decision aid (PtDA) for patients considering surgery for UC created in line with international minimum standards. METHODS AND ANALYSIS: This is

An advance care plan decision support video before major surgery: a patient- and family-centred approach.

OBJECTIVE: Video-based advanc care planning (ACP) tools have been studied in varied medical contexts; however, none have been developed for patients undergoing major surgery. Using a patient- and family-centredness approach, our objective was to implement human-centred design (HCD) to develop an ACP decision support video for patients and their family members when preparing for major surgery. DESIGN AND METHODS: The study investigators partnered with surgical patients and their family members, surgeons and other health professionals to design an ACP decision support video using key HCD princi

Systematic review of paediatric track and trigger systems for hospitalised children.

CONTEXT: Early and accurate recognition of the deteriorating hospitalised child is complex. Paediatric track and trigger systems (PTTS) support clinical decision-making by 'tracking' the child's condition through monitoring of clinical signs and 'triggering' a request for an appropriate review when pre-determined criteria are breeched. OBJECTIVE: To describe the number and nature of published PTTS and appraise the evidence on their validity, calibration, and effect on important patient outcomes (death, cardiac and/or respiratory arrest, unplanned transfer to intensive/high dependency care, im

Use of a decision aid including information on overdetection to support informed choice about breast cancer screening: a randomised controlled trial.

BACKGROUND: Mammography screening can reduce breast cancer mortality. However, most women are unaware that inconsequential disease can also be detected by screening, leading to overdiagnosis and overtreatment. We aimed to investigate whether including information about overdetection of breast cancer in a decision aid would help women aged around 50 years to make an informed choice about breast screening. METHODS: We did a community-based, parallel-group, randomised controlled trial in New South Wales, Australia, using a random cohort of women aged 48-50 years. Recruitment to the study was don

Some Health States Are Better Than Others: Using Health State Rank Order to Improve Probabilistic Analyses.

BACKGROUND: Probabilistic sensitivity analyses (PSA) may lead policy makers to take nonoptimal actions due to misestimates of decision uncertainty caused by ignoring correlations. We developed a method to establish joint uncertainty distributions of quality-of-life (QoL) weights exploiting ordinal preferences over health states. METHODS: Our method takes as inputs independent, univariate marginal distributions for each QoL weight and a preference ordering. It establishes a correlation matrix between QoL weights intended to preserve the ordering. It samples QoL weight values from their distrib

Supporting medical planning by mitigating cognitive load.

OBJECTIVES: Developing a care plan for a patient is a complex task, requiring an understanding of interactions and dependencies between procedures and of their possible outcomes for an individual patient. Decision support for planning has broader requirements than are typically considered in medical informatics applications. We consider the appropriate design of software to assist medical planning. METHODS: The likely cognitive loads imposed by planning tasks were assessed with a view to directly supporting these via software. RESULTS: Five types of cognitive load are likely to be important.

Quantitative Medicine to Support Drug Development and Regulatory Decisions: A UK Regulatory Approach Informed by ICH M15.

Quantitative medicine supports modern drug development and regulatory decision-making by integrating non-clinical and clinical data to inform dose selection, trial optimization, and extrapolation across populations and benefit-risk assessment. The Medicines and Healthcare products Regulatory Agency (MHRA) has adopted quantitative approaches across the product lifecycle within a risk-proportionate regulatory framework. Internationally, the ICH M15 guideline provides a harmonized framework for planning, evaluation and documentation of model-informed drug development (MIDD) evidence which is alig

Coping Self-Efficacy and Its Relationship with Psychological Morbidity after Genetic Test Result Disclosure: Results from Cancer-Unaffected BRCA1/2 Mutation Carriers.

Women who are found to carry a BRCA1/2 pathogenic variant experience psychological distress due to an increased risk of breast and ovarian cancer. They may decide between different preventive options. In this secondary analysis of data collected alongside a larger randomized controlled trial, we are looking at 130 newly found BRCA1/2 pathogenic variant carriers and how their coping self-efficacy immediately after genetic test result disclosure is related to their psychological burden and status of preventive decision making. Participants received the Coping Self-Efficacy Scale, the Hospital An

[A quasi-experimental study on the decisions, behavioral regulation, and metacognition of Ecuadorian men].

INTRODUCTION: Decision-making is one of the most critical components of people's lives. It involves cognitive factors, which are responsible for processing the stimuli present in the task, remembering past experiences, and assessing the possible consequences that emotional decisions can have. So far, there are no studies of this nature in Ecuador, so this research seeks to establish the characteristics of the Ecuadorian population. With the results obtained, we expect to gain insight into the way this population handles decisions in daily life. OBJECTIVE: To relate decision-making to behavior

Machine learning to predict the decision to perform surgery in hepatic echinococcosis.

BACKGROUND: Cystic echinococcosis (CE) is a significant public health issue, primarily affecting the liver. While several management strategies exist, there is a lack of predictive tools to guide surgical decisions for hepatic CE. This study aimed to develop predictive models to support surgical decision-making in hepatic CE, enhancing the precision of patient allocation to surgical or non-surgical management pathways. METHODS: This retrospective analysis included 406 hepatic CE patients treated at our center (2009-2021). Clinical, imaging, and treatment data were used to develop a Cox regres

Postpartum Sterilization after a Preterm Delivery Is Not Associated with Decision Regret.

BackgroundAlthough sterilization is one of the most effective methods of birth control, some physicians may hesitate to perform postpartum sterilizations on patients after preterm birth, as preterm labor and delivery may preclude adequate counseling.MethodsThis is a cross-sectional study conducted at a single, tertiary care, academic institution of adult pregnant patients who experienced a spontaneous or iatrogenic preterm delivery between March 15, 2011, and May 10, 2014 and underwent postpartum female surgical sterilization within 12 wk of delivery. A validated Decision Regret Scale was admi

Evaluation of "nursing ethics-professional teaching" integrated ward-round in practicing nursing students: a practical study.

BACKGROUND: Competence in ethical decision making and moral sensitivity are becoming increasingly important in nursing education. While teaching reforms have been conducted in nursing ethics, these reforms often fail to drive the adequate development of students’ critical thinking and ethical decision-making skills. To address this gap, in this study, a “nursing ethics–professional teaching” program is constructed on the basis of Rest’s ethical decision-making model, and its effects on enhancing the ethical decision-making ability and sensitivity of nursing interns are explored. METHODS: Limit

Decisions About Suppressive Antibiotics Among Clinicians at Veterans Affairs Hospitals After Prosthetic Joint Infection.

IMPORTANCE: Indefinite suppressive antibiotic therapy (SAT) is sometimes prescribed after initial antibiotic treatment for prosthetic joint infection (PJI). Limited evidence on outcomes after SAT exists, and using SAT for patients at low risk who may not need it could be associated with antibiotic resistance and adverse events. OBJECTIVES: To characterize clinical decision-making about SAT after PJI and identify stewardship intervention opportunities to stop or reduce SAT for patients who may not benefit. INTRODUCTION: Pathological myopia (PM) is a serious visual impairment that may lead to irreversible visual damage or even blindness. Timely diagnosis and effective management of PM are of great significance. Given the increasing number of myopia cases worldwide, there is an urgent need to develop an automated, accurate, and highly interpretable PM diagnostic technology. METHODS: We proposed a computational model called PMPred-AE based on EfficientNetV2-L with attention mechanism optimization. In addition, Gradient-weighted class activation mapping (Grad-CAM) technology was used to provide an
