Expansion Duroplasty For Severe Cervical Spinal Cord Swelling After Traumatic Injury: A Step-by-Step Surgical Protocol.

Acute traumatic cervical spinal cord injury frequently leads to severe cord edema, elevated intraspinal pressure, reduced spinal cord perfusion, and progressive neurological deterioration. Conventional bony decompression via laminectomy may provide limited relief, as the non-elastic dura mater can continue to constrain the swollen spinal cord. Expansion duroplasty is designed to address this limitation by enlarging the intradural space using an artificial dural substitute, allowing controlled expansion of the injured spinal cord. This protocol describes patient positioning, selection of lamine

Catheter-assisted evacuation through limited laminectomy for traumatic long-segment spinal epidural hematoma in ankylosing spondylitis: a technical note and two case reports.

BACKGROUND: Patients with ankylosing spondylitis (AS) are prone to unstable spinal fractures with long-segment spinal epidural hematoma (LSEH). Conventional multilevel laminectomy can achieve complete decompression but may disrupt the posterior tension complex. In this article, we describe a catheter-assisted technique for evacuating LSEH through a limited laminectomy window. METHODS: Two AS patients with traumatic cervicothoracic LSEH underwent emergency posterior stabilization followed by limited laminectomy. A 15 Fr silicone catheter with a distal side-hole was advanced epidurally along th

Extent of facial nerve decompression at the geniculate ganglion and labyrinthine segment via transmastoid supralabyrinthine approach: cadaveric study.

OBJECTIVE: To quantitatively assess the extent of facial nerve decompression achievable at thegeniculate ganglion and labyrinthine segment using the transmastoid supralabyrinthineapproach in cadaveric specimens. METHODS: Eleven formalin-fixed human cadaveric heads (22 temporal bones) underwentpreoperative high-resolution computed tomography (CT) to measure the lengths of thegeniculate ganglion and labyrinthine segment within their bony canals. A standardizedtransmastoid supralabyrinthine approach was then performed to decompress the facialnerve. Postoperative CT was used to determine the leng

Pediatric Arnold-Chiari I Malformation With Syrinx Presenting With Unilateral Hand Weakness: A Case Report.

Diagnostic suspicion of Chiari I malformations (CM-I) can be challenging in the pediatric population due to highly variable neurologic symptoms and cognitive immaturity impairing symptom identification. Especially in an atypical presentation without obvious central neurological abnormalities, the diagnosis can often be missed. We present a case of a left-hand-dominant 14-year-old boy presenting to an orthopedic hand clinic with right-hand weakness, tingling, and impaired grip strength. The medical history was notable for mild scoliosis and acute lymphoblastic leukemia in remission treated with

An unusual case of spinal cord compression secondary to iliopsoas cavernous hemangioma: A case report and literature review.

RATIONALE: We report on a 16-year-old woman who had a history of a vascular malformation involving the left iliopsoas muscle and underwent multiple sclerotherapy sessions over the preceding 5 years without clinical improvement. PATIENT CONCERNS: Ten days before admission, the patient experienced the acute onset of paraparesis. Computed tomography and magnetic resonance imaging revealed a large vascular malformation, measured 90 × 100 × 100 mm, situated at the T12 to L4 vertebral levels within the left iliopsoas muscle, exerted significant compression on the conus medullaris and cauda equina.

Effect of orbital fat decompression on eyelid contour according to midpupil lid distance in graves' ophthalmopathy.

BACKGROUND: This study aimed to describe and compare eyelid contours before and after orbital fat decompression in patients with Graves' ophthalmopathy (GO) by measuring their midpupil lid distances (MPLDs). METHODS: A retrospective comparative study of patients with GO who underwent orbital fat decompression was performed. Standard digital images of primary gaze were analyzed using a self-designed software. Radial MPLDs of the upper and lower eyelids were measured from 0° (nasal) to 180° (temporal) with 15° spacing. Pre- and postoperative MPLDs were compared and the relationship between MPLD

Ultrasound-guided Percutaneous Ulnar Nerve Release at the Elbow: A Stepwise Approach.

Cubital tunnel syndrome is the second most common compressive neuropathy in the upper limb. A wide range of surgical techniques have been described, from simple in situ decompression to anterior transposition, both through open and endoscopic approaches. Recent advances in ultrasonography and the development of dedicated instrumentation have enabled the emergence of ultrasound-guided cubital tunnel release as a feasible, effective, minimally invasive alternative. However, concerns regarding potential complications and inconsistent outcomes have limited widespread adoption. The aim of this stud

Reversal of No Light Perception in Dysthyroid Optic Neuropathy by Emergency Endoscopic Endonasal Orbital Decompression.

INTRODUCTION: Thyroid dysfunction optic neuropathy (DON) is a severe complication of Graves' ophthalmopathy. Orbital decompression surgery should be considered when conservative treatment fails. However, it is often contraindicated in patients who have lost light perception due to poor prognosis. CASE SUMMARY: We present a case of a patient with DON who had lost light perception for several days and showed visual recovery after emergency endoscopic endonasal inferomedial orbital wall decompression. DISCUSSION: Even in cases of total vision loss, endoscopic endonasal decompression of the infe

Upper cervical epidural abscess with emphasis on diabetes as a risk factor: a case report.

BACKGROUND: An upper cervical spine epidural abscess (UCEA) is an epidural abscess that develops in the area between the occiput and the second cervical spine (axis). It is a rare diagnosis that carries the risk of instability of the atlantoaxial joint, and its management is not well-defined. It is known that the skin is the most common source of infection, and that diabetes mellitus (DM) is the most frequently reported risk factor. In this case, we present a patient diagnosed with UCEA, who achieved full neurological recovery postoperatively despite having neurological deficits for over five

The application of wing-shaped window decompression device in the treatment of large odontogenic keratocyst: An observational study.

To enhance the decompression and drainage effects after marsupialization of large odontogenic keratocysts (OKCs) in the jawbone, a novel cyst plug was designed, and its clinical feasibility was investigated. A total of 42 patients with large OKCs requiring decompression were divided into 2 groups: the control group (n = 21), which underwent traditional drainage tube insertion, and the experimental group (n = 21), which received a personalized wing-shaped plug. The clinical efficacy of the wing-shaped plug was assessed, and postoperative recovery times were compared between the 2 groups. The av

Carbon Footprint of Open Carpal Tunnel Release Surgery Performed in the Procedure Room Versus Operating Room Setting.

PURPOSE: Environmental sustainability is an important issue in health care because of large amounts of greenhouse gases attributable to hospitals. The operating room has been highlighted as one of the highest contributors, prompting several initiatives by organizations focused on the care of hand and upper extremity conditions. This study aimed to quantify and compare the carbon footprint of a common hand surgery in two different surgical settings, the procedure room (PR) and operating room. We hypothesized that open carpal tunnel release (oCTR) will generate a greater environmental impact in

Is naso-axial line a better predictor in endoscopic endonasal resection of the odontoid and pannus removal? Technical nuances of maximizing corridor and maintaining occipital cervical joint stability.

BACKGROUND: The transoral transpharyngeal odontoidectomy, followed by occipitocervical fixation, have traditionally been a recognized method for ameliorating ventral compression at the craniovertebral junction (CVJ), despite its associated comorbidities. As an alternative, the endoscopic endonasal odontoid resection is a viable approach for various CVJ abnromalities that preserve the oropharynx and leads to fewer procedure-related complications(1-4). We present our case to detail the technical nuances of the procedure and its advantages over other techniques. CASE DESCRIPTION: In this video,

Orbital Apex Venous Malformation: Case Report on an Unusual Clinical and Radiological Presentation.

The authors present a case of an orbital apex venous malformation that presented with an intermittent oculomotor nerve palsy over a 15-year period. Radiologically, the lesion presented as a well-circumscribed orbital mass with T2 hyperintensity and enhancement that could only be demonstrated on 2-hour delayed phase imaging. The venous malformation was initially steroid-responsive but eventually became refractory to medical treatment alone. This necessitated a medial apical decompression, followed by a deep lateral orbitotomy 18 months later.

A review of arterial thoracic outlet syndrome.

Arterial thoracic outlet syndrome (TOS) is a condition in which anatomic abnormalities in the thoracic outlet cause compression of the subclavian or, less commonly, axillary artery. Patients are usually younger and typically have an anatomic abnormality causing the compression. The condition usually goes undiagnosed until patients present with signs of acute or chronic hand or arm ischemia. Workup of this condition includes a thorough history and physical examination; chest x-ray to identify potential anatomic abnormalities; and arterial imaging, such as computed tomographic angiography or dup

Biportal endoscopic transforaminal thoracic interbody fusion for the treatment of thoracic myelopathy.

BACKGROUND: Biportal endoscopic spine surgery independently controls two hands, similar to microscopic surgery, and utilizes a broader working space that is not disturbed by retractors under clear-magnified endoscopic vision. These advantages facilitate successful neural decompression and safe transforaminal interbody fusion, even in patients with thoracic spondylotic myelopathy. METHODS: A wide laminectomy and precise total facetectomy, in conjunction with partial pediculotomy, establish a secure transforaminal space for cage insertion. Endplate preparation and cage insertion were performed

Effect of Posterior Decompression on the Spinal Cord of Thoracic Ossification of the Posterior Longitudinal Ligament: A Finite Element Analysis.

OBJECTIVE: Thoracic ossification of the posterior longitudinal ligament (T-OPLL) causes myelopathy. Although posterior decompression for T-OPLL has shown positive results, patients with kyphotic curvatures often endure poor outcomes. Posterior decompression with fusion (PDF) has demonstrated better results compared to posterior decompression alone. This study aims to evaluate the effects of the posterior procedures for T-OPLL. METHODS: A 3-dimensional finite element model of the C2-T12 spine, created from medical images, was used to develop the following T3-T4 OPLL compression models: an inta
