Ventricular fibrillation following blunt chest trauma from a baseball.

OBJECTIVE: Baseball injuries account for a significant number of sports-related fatalities in children. We present a case of a 12-year-old male who died after being struck in the chest by a high velocity baseball propelled from a pitching machine. We examine the pathophysiology of blunt chest trauma, discuss possible explanations for the development of arrhythmias, review baseball-related fatalities, and suggest injury prevention strategies. DESIGN: A case report and review of the literature. SETTING: A pediatric emergency department (ED) in an urban, tertiary care, general hospital. PATIEN

Buffer therapy during out-of-hospital cardiopulmonary resuscitation.

The effects of infusing a buffer solution on resuscitability and outcome was tested in patients during out-of-hospital cardiac arrest. A number (502) of adults with asystole or ventricular fibrillation with failure of first defibrillation attempt were entered into a prospective, randomized, double-blind, controlled trial. Of these, 245 patients received 250 ml of sodium bicarbonate-trometamol- phosphate mixture with buffering capacity 500 mmol/l and 257 patients received 250 ml 0.9% saline. Except for the investigational infusion, all patients were resuscitated according to international guide

Reduced blood-brain barrier permeability after cardiac arrest by conjugated superoxide dismutase and catalase in piglets.

BACKGROUND AND PURPOSE: Cardiac arrest and resuscitation in immature piglets result in a delayed increase in blood-brain barrier permeability. We tested the hypothesis that pretreatment with oxygen radical scavengers reduces postischemic permeability. METHODS: Permeability was assessed by measuring the plasma-to-brain transfer coefficient of the small amino acid, alpha-aminoisobutyric acid, in 2- to 3-week-old anesthetized piglets. Three groups were studied: (1) a nonischemic time control group (n = 5), (2) an ischemia group (n = 8) pretreated with 5 mL of polyethylene glycol vehicle, and (3)

Comparison of a vectorcardiographically derived 12-lead electrocardiogram with the conventional electrocardiogram during wide QRS complex tachycardia, and its potential application for continuous bedside monitoring.

Previous investigators published conflicting reports comparing a vectorcardiographically derived electrocardiogram (ECGD) with the conventional 12-lead one (ECG). Prior comparisons were obtained in adults during sinus rhythm, but never in patients with wide QRS complex tachycardia. The ECGD was evaluated during baseline rhythms in patients with varying cardiac diagnoses, and the diagnostic accuracy of the 2 methods was compared during 64 episodes of wide QRS complex tachycardia in 49 patients during cardiac electrophysiologic study. All leads of the 12-lead ECGD closely resembled the conventio

[Hereditary prolonged QT interval (Romano-Ward syndrome) in a female patient with non-elective cesarean section].

In the Romano-Ward syndrome, an inherited form of abnormally prolonged QT duration is the matter of concern. The frequency-corrected QTc value in the ECG amounts to more than 0.44 s (upper mean value: 0.44 s). Persons suffering from Romano-Ward syndrome are vitally endangered by attacks of arrhythmia, syncope, and cardiac arrest that are triggered or aggravated by stress or certain drugs. In obstetrical anesthesia, attention should be paid to the fetus, which, due to the genetic dominance, will have a 50% probability of having the disorder. Heart-transplant and mono-amine oxidase inhibitors.

A case of anesthesia for a heart-transplant operation on a patient on mono-amine oxidase inhibitors (M.A.O.I.) is reported. This 63-year-old farmer was in end-stage cardiac failure due to familial cardiomyopathy. For 24 hours before surgery, he was on a dobutamine infusion (3 mcg/kg/min). He had been taking nialamide (100 mg/day) for 8 years for reactional depression and had not stopped it, despite advice. Anesthesia was induced with etomidate and succinylcholine, and maintained with fentanyl (25 mcg/kg/min) and pancuronium. Cardio-vascular stability was maintained during induction and first s

Cardiac arrhythmias--update 1987.

Arrhythmias may result from abnormalities of impulse initiation (automaticity), conduction (slow conduction, block, reentry), or a combination. The central and peripheral nervous systems have an important influence on the genesis of cardiac arrhythmias. Sympathetic and parasympathetic fibers innervate both atria and ventricle. The study of clinical cardiac arrhythmias includes the use of invasive and noninvasive testing procedures. The ECG, ambulatory monitoring, esophageal recording, exercise testing, and signal averaging techniques are the currently used noninvasive tests. Intracardiac elect

Clinical significance of ventricular fibrillation-flutter induced by ventricular programmed stimulation.

Two hundred twenty-four patients underwent ventricular programmed stimulation (VPS) without prior documentation of the clinical occurrence of sustained ventricular tachycardia (VT) or ventricular fibrillation-flutter (VF). Indications for VPS were: palpitations or nonsustained VT during ambulatory monitoring (85 patients), syncope or presyncope (137 patients), and a family history of sudden death (two patients). Sustained VF requiring transthoracic defibrillation was initiated by VPS in 18 patients (8.0%). Four patients were treated for inducible VF with antiarrhythmic agents directed by elect

Cardiac pacing in retrospect.

As precursors of permanent pacemakers, Lidwill (1929) and Hyman (1932) introduced temporary pacemakers for resuscitation. Callaghan (1950) intravenously paced the sinus nodal region for bradycardia in hypothermic dogs. Zoll (1952) used external electrodes to treat Adams-Stokes attacks, and Lillehei (1957) fixed stainless steel electrodes to the myocardium, successfully treating iatrogenic total atrioventricular block with a percutaneous pacemaker. Since 1951, by experimental and clinical use of ventricular fibrillation to obtain a functional cardiac standstill during open heart surgery, we use

Predictive Value of the CALLY Index for Pocket Hematoma After Cardiac Implantable Electronic Device Procedures.

BACKGROUND: Pocket hematoma is one of the most common early complications after cardiac implantable electronic device (CIED) procedures and is associated with increased morbidity. Reliable risk stratification tools are needed to identify patients at higher risk. METHODS: In this retrospective single-center study, 158 patients who underwent CIED implantation between January 2024 and June 2025 were analyzed. Patients who developed a pocket hematoma (n = 35) were compared with those without a hematoma (n = 123). Demographic, clinical, laboratory, and procedural parameters were assessed. The Hype

Novel Percutaneous Management of Giant Vegetations With Pulmonary Emboli.

BACKGROUND: Large vegetations in cardiac implantable electronic devices (CIEDs) are associated with high mortality due to risks of major pulmonary artery obstruction. The rising incidence of CIEDs infections has complicated management, particularly in patients with delayed diagnoses. Current guidelines recommend open-heart surgery for vegetations > 2.5 cm, but this approach poses significant risks for high-morbidity patients. This study evaluates the feasibility and outcomes of percutaneous vacuum-assisted aspiration using the AngioVac system in managing giant vegetations (≥ 4.0 cm) in patient

Intraoperative pectoral nerve blocks during cardiac implantable electronic device procedures.

BACKGROUND: Cardiac implantable electronic device (CIED) procedures can cause significant postoperative pain. Opioid use for postoperative pain is associated with risk of persistent use. The benefits of pectoral nerve (PECs) blocks have been established for other chest wall surgeries, but adoption in electrophysiology has been limited. OBJECTIVES: The purpose of this study was to evaluate the efficacy of intraoperative ultrasound-guided PECs blocks performed at the time of CIED procedures by the implanting physician from within the device pocket. METHODS: Patients undergoing a pectoral CIED

Relationship between predictive factors and atrial high-rate episodes in heart failure with reduced ejection fraction patients with cardiac implantable electronic devices.

OBJECTIVE: Atrial high-rate episodes (AHRE) are atrial tachyarrhythmia episodes detected by implanted cardiac devices, characterised by an atrial rate exceeding 180-190 beats per minute. Recent studies have linked AHRE to the development of atrial fibrillation (AF) and increased stroke risk, especially when episodes last longer than 5-6 min. This study aimed to evaluate the relationship between predictive factors and the occurrence of AHRE in heart failure with reduced ejection fraction (HFrEF) patients with cardiac implantable electronic devices (CIEDs). METHODS: This single-centre, retrospe

Impact of Atrial Fibrillation Burden on Health Care Costs and Utilization.

BACKGROUND: Integrating patient-specific cardiac implantable electronic device (CIED)-detected atrial fibrillation (AF) burden with measures of health care cost and utilization allows for an accurate assessment of the AF-related impact on health care use. OBJECTIVES: The goal of this study was to assess the incremental cost of device-recognized AF vs no AF; compare relative costs of paroxysmal atrial fibrillation (pAF), persistent atrial fibrillation (PeAF), and permanent atrial fibrillation (PermAF) AF; and evaluate rates and sources of health care utilization between cohorts. METHODS: Usin

Excellent Outcomes After First-Line Ablation in Post-MI Patients With Tolerated VT and LVEF >35.

BACKGROUND: Post-myocardial infarction (MI) patients with ventricular tachycardia (VT) are considered at risk for VT recurrence and sudden cardiac death (SCD). Recent guidelines indicate that in selected patients catheter ablation should be considered instead of an implantable cardioverter-defibrillator (ICD). OBJECTIVES: This study aimed to analyze outcomes of patients referred for VT ablation according to left ventricular ejection fraction (LVEF), tolerance of VT, and acute ablation outcome. METHODS: Post-MI patients without prior ICD undergoing VT ablation at a single center between 2009

Tyrosine kinase inhibitor-associated ventricular arrhythmias: a case series and review of literature.

BACKGROUND: Tyrosine kinase inhibitors (TKIs) have been increasingly used as first-line therapy in hematologic and solid-organ malignancies. Multiple TKIs have been linked with the development of cardiovascular complications, especially atrial arrhythmias, but data on ventricular arrhythmias (VAs) is scarce. METHODS: Herein we describe five detailed cases of VAs related to TKI use in patients with varied baseline cardiovascular risk factors between 2019 and 2022 at three centers. Individual chart review was conducted retrospectively. RESULTS: Patient ages ranged from 43 to 83 years. Three pa
