Opportunistic infections caused by protozoan parasites.

Parasitic protozoa have played a major role in the discovery of the HIV epidemic. It was the occurrence of clustered cases of Pneumocystis pneumonia which caught the attention of the epidemiologists indicating that something unusual was going on. Very recently remarkable progress was made in understanding the epidemiology. Knowledge is evolving that Pneumocystis is host-specific to an extent which seems to justify species rank for the cause of death in AIDS patients for many years. Toxoplasma gondii infections were also recognized very early in AIDS patients. Cerebral toxoplasmosis has now rep

Cryptosporidiosis in patients with AIDS: correlates of disease and survival.

Although 10%-15% of patients with AIDS in the United States may acquire cryptosporidium infection, little data exist on clinical or histological characteristics that differentiate clinical outcomes. A case-control study of 83 HIV-positive adult patients with cryptosporidiosis was conducted, as was a histopathologic review of data on gastrointestinal biopsy specimens from 30 patients. Four clinical syndromes were identified: chronic diarrhea (36% of patients), choleralike disease (33%), transient diarrhea (15%), and relapsing illness (15%). A multivariate analysis of data for cases and controls

A Trichuris specific diagnostic antigen from culture fluids of Trichuris suis adult worms.

Trichuris suis is a nematode parasite of swine which can cause serious pathology in infected pigs, such as anorexia, diarrhea, anemia, and death in heavy infections, especially in young animals. Infection with Trichuris suis is difficult to diagnose because much of the pathology occurs when only larval stages are present, and because of sporadic egg production by adult female worms. We have isolated a 20 kDa excretory/secretory (E/S) glycoprotein from culture fluids of adult worms by column chromatography and preparative gel electrophoresis which is diagnostic for Trichuris suis infection in p

Possible involvement of the total amount of morphine infused in the development of acute morphine dependence in rats.

The severity of naloxone-precipitated withdrawal in rats infused intravenously with morphine at the rates of 2.5, 5 and 10 mg/kg/hr over various time periods was investigated. Plasma morphine concentration reached a constant and rate-dependent level at 1 hr after the start of morphine infusion, and this level was maintained until the termination of infusion. Naloxone (2.0 mg/kg, s.c.) was challenged 18 hr after infusion was stopped, and the withdrawal was evaluated by plasma corticosterone (PCS) increase, diarrhea and body weight loss. The incidence of naloxone-precipitated withdrawal signs wa

[The traveler returning from the tropics in clinical practice].

Travellers returning from the tropics frequently consult a physician even if they have no actual symptoms. Physical check-ups in asymptomatic returnees rarely detect dangerous conditions. The most common laboratory finding is intestinal parasites. Blood eosinophilia may indicate helminthic infections, such as strongyloidosis, filariasis, schistosomiasis and others. If there are no diagnostically suggestive symptoms a systematic, step-by-step workup is recommended (stool parasitology, serology, and special methods to demonstrate parasites in blood or tissues). The most common symptom of returne

Modification to the RITARD surgical model.

The Removable Intestinal Tie-Adult Rabbit Diarrhea (RITARD) procedure is the surgical model of choice for studying pathogenesis of diarrheas of Campylobacter, Escherichia coli, and Aeromonas origin. Other than nonhuman primates, the RITARD rabbit is the only model that closely resembles human infection. Classically, the RITARD model is created by isolating the terminal ileum using umbilical tape placed in a slipknot fashion, and injecting the occluded segment with the organism of choice. The slipknot is removed at 4 h postinoculation and patency of the gut reestablished. We found removal of th

Use of somatostatin analog in management of carcinoid syndrome.

Carcinoid tumors are the most frequent gut neuroendocrine tumors accounting for more than 50% of all tumors of the gastroenteropancreatic (GEP) axis. These tumors appear to derive from a stem cell line capable of differentiating into a variety of malignant cells that secrete many different peptides and amines. The symptoms of carcinoid tumors are often non-specific, vague abdominal pain that may precede the diagnosis by a median of 9 years. Carcinoid syndrome occurs in less than 10% of patients. We evaluated the effects of SMS 201-995 in 14 such patients, 12 with diarrhea, 8 with flushing, 3 w

Cryptosporidiosis in a day-care center.

Cryptosporidiosis has typically been considered a disease of animals, and the occasional human case has been considered a rare zoonosis or evidence of immunoincompetence. In this report, person-to-person transmission is postulated for illness documented in two of five rooms of a day-care center. Of 46 persons (34 children and 12 staff members), 29 (63%) became ill, and 27 (58.7%) had Cryptosporidium in their stools. Symptoms in children included diarrhea (55%), weight loss (25%), flatulence (15%), and fever (10%). Symptoms of cryptosporidiosis ceased without treatment, and three months later a

Clinical studies of intestinal folate conjugases.

Clinical differences between the two human intestinal mucosal folate conjugases were assessed by measurement of their activities in normal individuals and in patients with chronic diarrhea of differing causes. Intracellular folate conjugase (ICFC) was 15-fold more active than brush border folate conjugase (BBFC) in jejunal mucosa from seven obese patients undergoing elective gastric bypass surgery. The activity of ICFC was similar among normal volunteers and patients with diarrhea of unknown origin (DUO), gluten-sensitive enteropathy (GSE), inflammatory bowel disease (IBD), and the short bowel

The histopathologic spectrum of acute self-limited colitis (acute infectious-type colitis).

Acute self-limited colitis (ASLC) is a self-limiting diarrheal illness which is often caused by known infectious agents (Campylobacter, Salmonella, and Shigella), but many cases are of unknown etiology. This report describes the histopathologic features of acute self-limited colitis as related to its natural history. The extent of inflammation and regeneration varies with the duration of the disease. In the peak activity stage (within 0-4 days of onset of bloody diarrhea) there is mucosal edema, cryptitis, crypt ulcers, and abscesses. At the time of resolution (within 6-9 days of onset of bloo

Arterial blood gas and acid-base values in dogs with various diseases and signs of disease.

Results of arterial blood gas and acid-base analysis on initial samples prior to therapy were reviewed for 220 dogs admitted to the University of Georgia Veterinary Teaching Hospital. Acidemia or alkalemia was detected in 61 of 220 dogs (28%). The most common acid-base abnormality was metabolic acidosis (79 of 220 dogs--36%). Primary metabolic acidosis was the acid-base category associated most frequently with the combination of vomiting and diarrhea, dehydration, and the combination of polydipsia and polyuria, whereas normal mean arterial PCO2 and [HCO3-] values and primary metabolic acidosis

Copper metabolism and requirements in total parenteral nutrition.

Copper metabolism and requirements in patients receiving total parenteral nutrition were studied in 28 patients with gastrointestinal diseases. During each of the 3 wk of the study period, each of 24 patients received in their total parenteral nutrition solutions, a daily dose of copper amounting to 0.25 mg, 1.05 mg, or 1.85 mg, in a random order. The other 4 patients received a fixed daily dose of 1 mg throughout the 3 wk. Increased losses of copper through the gastrointestinal tract occurred in patients with diarrhea or high-output stomas or fistulas. Patients with abnormalities of liver exc

First Evidence of Porcine Epidemic Diarrhoea Virus and African Swine Fever Virus Natural Co-Infection in Pigs from Karnataka, India: An Epidemiological Investigation.

This study reports a diarrhoeal outbreak in pigs primarily caused by Porcine Epidemic Diarrhoea Virus (PEDV), with co-infection of African Swine Fever Virus (ASFV)-a first evidence of natural PEDV-ASFV co-infection in India with outbreak implications. Distinct clinical signs were observed: majority of pigs exhibited profuse yellow watery diarrhoea and vomiting typical of PED, while a few showed high fever, systemic haemorrhages, and coffee-coloured diarrhoea indicative of ASF. The outbreak lasted 25 days and followed a propagated epidemic curve, suggesting pig-to-pig transmission. Epidemiologi

Vaccines for preventing rotavirus diarrhoea: vaccines in use.

BACKGROUND: Rotavirus results in more diarrhoea-related deaths in children under five years than any other single agent in countries with high childhood mortality. It is also a common cause of diarrhoea-related hospital admissions in countries with low childhood mortality. Rotavirus vaccines that have been prequalified by the World Health Organization (WHO) include a monovalent vaccine (RV1; Rotarix, GlaxoSmithKline), a pentavalent vaccine (RV5; RotaTeq, Merck), and, more recently, another monovalent vaccine (Rotavac, Bharat Biotech). OBJECTIVES: To evaluate rotavirus vaccines prequalified by

Elimination of bovine viral diarrhoea virus in New Zealand: a review of research progress and future directions.

The major impacts of bovine viral diarrhoea (BVD) on cattle health and production have prompted many countries to embark on national elimination programmes. These programmes typically involve identifying and removing persistently infected (PI) cattle in infected herds and implementing biosecurity measures, such as pre- or post-movement testing. In order to design a systematic national control programme to eliminate BVD in New Zealand, which achieves the greatest benefits to the industries at the lowest cost to individual farmers, an accurate understanding is necessary of the epidemiology, econ

Transmission of bovine viral diarrhoea virus through the semen of acutely infected bulls under field conditions.

Epidemiological investigations implicated the semen of artificial insemination (ai) bulls as the only plausible source of infection with bovine viral diarrhoea virus (bvdv) in 10 Finnish dairy herds. The infection was traced back to two northern Finncattle bulls that had been transiently infected when their semen had been collected while they were in a gene bank herd containing persistently infected (pi) animals. The isolates of bvdv from the animals in the gene bank herd, from the semen of the two bulls and from a pi calf born in one of the herds using the semen belonged to a rare genetic typ

Brain malformations in ovine fetuses associated with the cytopathogenic biotype of bovine viral-diarrhoea virus.

A total of six ewes were intravenously inoculated at between 65 and 68 days of gestation with the Indiana strain of bovine viral diarrhoea virus (BVDV), containing both non-cytopathogenic (ncp) and cytopathogenic (cp) biotypes. Eight transplacentally infected fetuses were sequentially removed from the infected ewes and were found to have inflammatory lesions and malformations of the brain. BVDV RNA was isolated from formalin-fixed, paraffin-embedded brain tissue sections and detected by nested polymerase chain reaction after reverse transcription. The two biotypes of BVDV were distinguished by

HIV seropositivity in children admitted with diarrhoea at Eldoret District Hospital, Kenya.

Fifty seven children aged 2 years and below admitted to Paediatric ward of Eldoret District Hospital with a diagnosis of diarrhoea were studied for HIV infection. A total of 29 (50.9%) cases were found to be positive with no gender difference. 17 (70.8%) of 24 cases with diarrhoea of over 14 days duration were positive for HIV compared to 12 (36.7%) cases out of 33 cases with diarrhoea of less than 14 days' duration. There was a high (80%) prevalence of HIV positivity rate in patients who presented with pneumonia and malnutrition in addition to diarrhoea when compared to children who presented

Etiology and epidemiology of diarrheal diseases in the United States.

Accurate data on the frequency of acute diarrheal illness and the distribution of pathogens are not available for several reasons, including the facts that only a small fraction of cases come to the attention of physicians and that available diagnostic tests establish an etiology in only about half of these. In a survey of three groups of patients in a community (upper-middle class and lower class outpatients and hospitalized infants), a possible cause was found in fewer than 20 percent of outpatients, and rotavirus accounted for the majority. However, asymptomatic colonization with rotavirus

Bacteria, parasitic agents and rotaviruses associated with acute diarrhoea in hospital in-patient Indonesian children.

Faeces from children (aged from one month to 12 years) with acute diarrhoea admitted to hospital in Yogyakarta, Indonesia, from June 1978 to June 1979, were examined for the presence of enteric pathogens. One or more recognized enteropathogens were identified in 56% of children. Rotaviruses were identified in 38% of all children. Toxigenic coliforms (predominantly Escherichia coli) were isolated from 12% of children. Salmonella sp. (6%), Shigella sp.
