Peroneal Tendoscopy Technique.

BACKGROUND: Peroneal tendoscopy is indicated for patients with persistent posterolateral ankle pain in which other intraarticular or extraarticular etiologies have been excluded. Physical examination findings and magnetic resonance imaging (MRI) evaluation are often equivocal, making direct visualization critical in characterizing the injury. Peroneal tendoscopy is a minimally invasive technique that is used to achieve this with little disruption to surrounding anatomy and minimal complications. INDICATIONS: We present a 57-year-old woman with 3 months of left posterolateral ankle pain and su

Double-Bundle PCL Reconstruction in the Multiple-Ligament Injured Knee.

BACKGROUND: Multi-ligament knee injuries and knee dislocations are potentially devastating injuries that typically result in injury to both cruciate ligaments and variable injury to collateral ligament complexes. INDICATIONS: Surgical intervention typically leads to better patient outcomes than nonoperative management. We describe our preferred technique for bicruciate ligament reconstruction with a focus on double-bundle posterior cruciate ligament (PCL) reconstruction. TECHNIQUE DESCRIPTION: A transtibial approach is used to drill the PCL tibial tunnel. A safety incision on the proximal me

Treatment of an Iatrogenic Lateral Meniscal Root Tear After ACL Reconstruction.

BACKGROUND: The meniscal roots are essential for preserving the structural and biomechanical properties of the tibiofemoral joint. Posterior meniscal root avulsions can cause meniscal extrusion, joint space narrowing, and progressive knee arthritis. Iatrogenic avulsions after malpositioning of the transtibial tunnels during anterior cruciate ligament (ACL) reconstruction have previously been reported in the literature to account for poor long-term outcomes seen in some patients following ACL reconstruction. Therefore, correct transtibial tunnel placement during ACL reconstruction is essential

Use of quality-of-life instruments for people living with HIV: a global systematic review and meta-analysis.

INTRODUCTION: Due to the effectiveness of combined antiretroviral therapy and its growing availability worldwide, most people living with HIV (PLHIV) have a near-normal life expectancy. However, PLHIV continue to face various health and social challenges that severely impact their health-related quality-of-life (HRQoL). The UNAIDS Global AIDS Strategy discusses the need to optimize quality-of-life, but no guidance was given regarding which instruments were appropriate measures of HRQoL. This study aimed to review and assess the use of HRQoL instruments for PLHIV. METHODS: We conducted a globa

A qualitative study of attitudes towards, typologies, and drivers of concurrent partnerships among people of black Caribbean ethnicity in England and their implications for STI prevention.

BACKGROUND: Partner concurrency, (having sexual partnerships overlapping in time), especially when condoms are not used, can facilitate sexually transmitted infections (STI) transmission. In Britain, STI diagnoses rates and the reporting of concurrency are higher among black Caribbeans than other ethnic groups. We explored attitudes towards, drivers, characteristics, and contexts of concurrent partnerships, and their implications for STI risk among black Caribbeans in England. METHODS: Purposive sampling, by sex and age-groups, was used to recruit participants (overall n = 59) from five sexua

Cervical disc arthroplasty: do conflicts of interest influence the outcome of clinical studies?

BACKGROUND CONTEXT: Cervical disc arthroplasty (CDA) is an emerging technique for the treatment of cervical degenerative disease. Multiple studies have investigated the outcomes of CDA, particularly in comparison with cervical arthrodesis techniques such as anterior cervical discectomy and fusion (ACDF). As many entities have financial interests in CDA implants, it is imperative to consider the influence of conflicts of interest on the results of studies investigating the efficacy of CDA. PURPOSE: This study aimed to determine if there is an association between the presence of conflicts of in

Cervical arthroplasty: a critical review of the literature.

BACKGROUND CONTEXT: Cervical disc arthroplasty (CDA) is a motion-preserving procedure that is an alternative to fusion. Proponents of arthroplasty assert that it will maintain cervical motion and prevent or reduce adjacent segment degeneration. Accordingly, CDA, compared with fusion, would have the potential to improve clinical outcomes. Published studies have varying conclusions on whether CDA reduces complications and/or improves outcomes. As many of these previous studies have been funded by CDA manufacturers, we wanted to ascertain whether there was a greater likelihood for these studies t

Consulting communities on feedback of genetic findings in international health research: sharing sickle cell disease and carrier information in coastal Kenya.

BACKGROUND: International health research in malaria-endemic settings may include screening for sickle cell disease, given the relationship between this important genetic condition and resistance to malaria, generating questions about whether and how findings should be disclosed. The literature on disclosing genetic findings in the context of research highlights the role of community consultation in understanding and balancing ethically important issues from participants' perspectives, including social forms of benefit and harm, and the influence of access to care. To inform research practice
