Exercise during pregnancy: how much active are pregnant women at risk of gestational diabetes despite few contraindications?

INTRODUCTION: Diet and physical activity are cornerstones in prevention and treatment of Gestational Diabetes (GDM) though some caution may be required under specific circumstances. The aims of this study were to evaluate activity habits during pregnancy and contraindications to physical activity in women at risk for GDM. METHODS: 536 pregnant women (age 35 ± 5 years; gestation week 25 ± 4; pre-pregnancy BMI 24.6 ± 12.9 kg/m2), selective screened for GDM, filled out a standardized questionnaire recording physical activity during pregnancy. RESULTS: Of 536 women, 73.4% reported regular exerci

Forced activity and environmental enrichment mildly improve manifestation of rapid cerebellar degeneration in mice.

Exercise therapy represents an important tool for the treatment of many neurological diseases, including cerebellar degenerations. In mouse models, exercise may decelerate the progression of gradual cerebellar degeneration via potent activation of neuroprotective pathways. However, whether exercise could also improve the condition in mice with already heavily damaged cerebella remains an open question. Here we aimed to explore this possibility, employing a mouse model with dramatic early-onset cerebellar degeneration, the Lurcher mice. The potential of forced physical activity and environmenta

Lever-actuated resonance assistance (LARA): a wheelchair-based method for upper extremity therapy and overground ambulation for people with severe arm impairment.

People with severe arm impairment have limited technologies available for retraining their arms, and, if they also have difficulty walking, they often cannot effectively use a manual wheelchair because they cannot grasp and push the pushrim. We are using Lever-Actuated Resonance Assistance (LARA) to solve these problems. A LARA-based device can attach to a manual wheelchair and allow it to be used by people with severe arm weakness in a stationary exercise mode, or for self-powered overground ambulation. LARA uses a lever drive and arm support to appropriately position the arm and to reduce th

Screening and evaluation of the cardiovascular and pulmonary systems in patients presenting with upper extremity impairments.

UNLABELLED: NARRATIVE REVIEW: Given the prevalence of cardiovascular and pulmonary (CV-P) disease, it is likely that a substantial portion of patients seeking services from hand therapists have diagnosed or yet to be diagnosed disease in one or both of these systems. Pain originating from these systems is more common in the chest, shoulder, and scapular regions, but both systems can refer pain into the medial aspect of the forearm and hand. Pancoast's tumors of the lung, myocardial ischemia, and myocardial infarction are examples of specific pathologies capable of referring pain into these upp

Resistance to aspirin in vitro at rest and during exercise in patients with angiographically proven coronary artery disease.

BACKGROUND: Acetylsalicylic acid, or aspirin, is widely used in secondary prevention of coronary artery diseases, but the inhibition of platelet aggregation is not uniform in all individuals. OBJECTIVE: To investigate the prevalence of aspirin resistance at rest and during exercise in coronary artery disease patients. MATERIALS AND METHODS: Fifty patients with stable coronary artery disease were prospectively studied. All patients received aspirin (75-300 mg/day for >1 month) and no other antiplatelet therapy. Aspirin resistance was studied, at rest and immediately after a stress test, using

Response to bronchial provocation and exercise in children with cystic fibrosis.

Ten of fifteen cystic fibrosis children with positive skin prick tests to common antigens gave an immediate bronchial reaction to the antigen inhaled, five of them also gave a late reaction; however only one gave a history of asthma. The antigen most commonly eliciting a positive skin reaction in cystic fibrosis patients is Aspergillus fumigatus. In six children tested to this antigen the bronchial response varied, two were negative, one gave an immediate reaction and three gave a dual (immediate and late) reaction. None of the children showed the characteristic pattern of response to exercise

Physiotherapist-supervised exercises versus unsupervised home-based exercises after nonsurgically treated proximal humerus fracture: a multicenter randomized controlled trial.

BACKGROUND: Proximal humerus fracture (PHF) is a common fragility fracture in older adults and can have a substantial impact on upper limb function. Although most patients with PHF can be treated nonsurgically, it is unknown whether older adults benefit from supervised exercise therapy after PHF. Therefore, the objective of this trial was to investigate whether 10 weeks of physiotherapist-supervised exercises once a week were superior to 10 weeks of unsupervised home-based exercises in older adults with a nonsurgically treated displaced 2-part PHF. METHODS: This was designed as an assessor-bl

Effectiveness of three vestibular rehabilitation exercises for treating acute unilateral peripheral vestibular dysfunction: a multicenter randomized study.

BACKGROUND: Vestibular rehabilitation therapy (VRT) is an established intervention for patients with acute unilateral peripheral vestibular dysfunction (aUPVD). However, the relative effectiveness of different VRT strategies remains uncertain. This multicenter randomized pilot trial aimed to compare the effects of customized vestibular rehabilitation therapy (CVRT), generic Cawthorne-Cooksey exercises (GVRT), and vestibulo-ocular reflex (VOR) adaptation exercises on vestibular symptom recovery and functional improvement. METHODS: A total of 34 patients with aUPVD were enrolled across three ce

The response to individualized treatment after a standardized treatment protocol among neck pain sufferers: a secondary analysis of a randomized controlled trial.

BACKGROUND: Manual therapy and exercise are recommended for patients with neck pain. In a recent randomized controlled trial, home stretching exercises with or without manual therapy were offered to subjects with persistent or recurrent neck pain. No difference in pain or disability between the treatment groups were found after the two-week intervention period. We aimed to investigate whether these patients had a better outcome after individual tailoring of the treatment content two months after the initial structured intervention period. METHODS: This manuscript is a secondary analysis of a

Protocol for a phase III RCT and economic analysis of two exercise delivery methods in men with PC on ADT.

BACKGROUND: Androgen deprivation therapy (ADT) is commonly used to treat prostate cancer. However, side effects of ADT often lead to reduced quality of life and physical function. Existing evidence demonstrates that exercise can ameliorate multiple treatment-related side effects for men on ADT, yet adherence rates are often low. The method of exercise delivery (e.g., supervised group in-centre vs. individual home-based) may be important from clinical and economic perspectives; however, few studies have compared different delivery models. Additionally, long-term exercise adherence and an unders

Use of manipulative therapy for mechanical pain of spinal origin.

Manipulation of the spinal joints for the relief of pain is an ancient remedy that is in widespread use today. Clinical trials investigating the effectiveness of manipulative therapy are briefly reviewed, and some experimental evidence suggesting possible causes of spine pain is presented. The rationale for the use of spinal manipulative therapy within the context of R. A. McKenzie's diagnostic and therapeutic system is detailed and explicitly stated. Spinal mobilization and manipulation are presented as a logical treatment progression from the use of postural instruction and exercise therapy.

Effectiveness of remote graded motor imagery therapy for patients with central sensitisation low back pain: study protocol for a randomised controlled trial.

INTRODUCTION: Chronic low back pain (CLBP) is a prevalent global condition that significantly impairs quality of life, reduces work productivity and imposes substantial healthcare burdens. Approximately 70% of adults worldwide experience at least one episode of low back pain during their lifetime. Central sensitisation, a common complexity of CLBP, involves heightened responsiveness of the central nervous system, resulting in amplified neural signalling and increased pain sensitivity. This phenomenon suggests that pain in CLBP extends beyond purely biomedical origins, involving multiple factor

Lessons Learned in the Early Adoption of FaME Across Ireland: A Convergent Mixed-Methods Implementation Study (FaME Ireland).

BACKGROUND: Falls are the most common cause of hospitalisation amongst older adults in Ireland. Structured exercise programmes are recommended to manage falls, and the Falls Management Exercise (FaME) programme has been shown to reduce falls and improve physical function. However, FaME delivery within an Irish context needs to be explored to ensure it is implemented effectively and equitably. OBJECTIVE: To explore key stakeholders' experiences and examine key outcomes during the early adoption of the FaME programme across three sites in Ireland. DESIGN: A convergent mixed-methods design was

Comparative efficacy of different exercise interventions for cancer-related fatigue, health-related quality of life, anxiety and depression symptoms in cancer survivors: a systematic review and network meta-analysis.

BACKGROUND: Cancer survivors commonly experience persistent treatment-related symptoms including fatigue, reduced health-related quality of life (HRQoL), anxiety, depression, and physical function decline. Whilst exercise broadly benefits survivors, the comparative efficacy of specific exercise modalities remains insufficiently characterised to inform modality-specific clinical prescription. This network meta-analysis evaluated the comparative effectiveness of aerobic exercise (AE), resistance training (RT), combined aerobic-resistance training (CART), and mind–body exercises (MBE) across five

The dose-response relationship of aquatic exercise for musculoskeletal pain relief: a systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Musculoskeletal pain constitutes a pervasive global health burden and represents a primary driver of disability worldwide. While aquatic exercise (AQE) is a promising intervention, its clinical application remains limited by the lack of quantitative dose guidance and standardized exercise prescription parameters. Therefore, this systematic review and meta-analysis aimed to evaluate the independent analgesic effect of AQE compared with passive controls and to explore the dose-response relationship between exercise dose and pain relief. METHODS: Four major databases were searched fr
