Comorbid status in patients with osteomyelitis is associated with long-term incidence of extremity amputation.

INTRODUCTION: Osteomyelitis is associated with significant morbidity, including amputation. There are limited data on long-term amputation rates following an osteomyelitis diagnosis. We sought to determine the incidence of amputation in patients with osteomyelitis over 2 years. RESEARCH DESIGN AND METHODS: Observational cohort study of 1186 inpatients with osteomyelitis between 2004 and 2015 and stratified by osteomyelitis location status to evaluate the impact on amputation, mortality rates, readmission data, and inpatient days. RESULTS: Persons with diabetes had 3.65 times greater probabil

Lower Extremity Orthopedic Augmentation Utilizing a Synthetic Hybrid-Scale Fiber Matrix.

Introduction Lower extremity injuries represent about 15% of all emergency room visits in the United States, with ankle injuries accounting for 20% of these. Surgical site infections resulting from ankle reconstructive surgery can result in severe complications, including amputation. Depending on the procedure performed, surgical site infections of ankle reconstructive surgery can occur at a rate as high as 14%. A synthetic hybrid-scale fiber matrix with an engineered design resembling a native human extracellular matrix could be utilized to augment these difficult surgical procedures. Physiotherapy Rehabilitation Following Lower Extremity Split Skin Grafting in Necrotizing Fasciitis.

Necrotizing fasciitis is defined as a highly progressive microbial infection. The diagnosis of necrotizing fasciitis is mostly based on clinical signs, and early diagnosis is key. There are several difficulties that patients confront following surgery. Physical therapy helps to regain the functional range of motion, enhance muscle strength, reduce stiffness and make the patient self-independent. We presented a case of a 54-year-old male with presenting complaints of pain over right lower limb while doing movements and difficulty in performing daily activities. The patient was diagnosed with ne

Do You Really Need a Hand Surgeon? Common Tendinopathies of the Upper Extremity.

Lateral epicondylitis, de Quervain tenosynovitis, and trigger finger are among the most common tendinopathies of the upper extremity. Lateral epicondylitis is a common condition with a prolonged course that can be frustrating for patients. Nonetheless, most patients improve with a simple wait-and-see approach. Therapy has been shown to be helpful and surgical management is usually successful in recalcitrant cases. Cortisone, although commonly used in the past, has been shown to have worse results than placebo in the medium and long term. de Quervain tenosynovitis responds well to nonsurgical m

An Extremely Rare Case of Prostate and Bladder Wall Involvement of Chronic Lymphocytic Leukemia.

Chronic lymphocytic leukemia (CLL) is the most common leukemia in adults and is characterized by monoclonal proliferation of B-cell lymphocytes which are morphologically mature, but immunologically dysfunctional. The primary sites of disease involvement include peripheral blood, lymph nodes, spleen, and bone marrow. CLL can also present locally and aggressively at extranodal sites. We describe the case of a 74-year-old gentleman with multiple medical comorbidities who was Foley catheter-dependent at baseline for bladder outlet obstruction. He was detected to have Rai stage I CLL following an i

Carmustine wafer implantation for supratentorial glioblastomas, IDH-wildtype in "extreme" neurosurgical conditions.

We assessed the feasibility of Carmustine wafer implantation in "extreme" conditions (i.e. patients > 80 years and Karnofsky Performance Status score < 50) and of implantation ≥ 12 Carmustine wafers in adult patients harbouring a newly diagnosed supratentorial glioblastoma, IDH-wildtype. We performed an observational, retrospective single-centre cohort study at a tertiary surgical neuro-oncological centre between January 2006 and December 2021. Four hundred eighty patients who benefited from a surgical resection at first-line treatment were included. We showed that Carmustine wafer implantatio

Noninvasive and invasive imaging of lower-extremity acute and chronic venous thrombotic disease.

The spectrum of venous thromboembolic (VTE) disease encompasses both acute deep venous thrombosis (DVT) and chronic postthrombotic changes (CPC). A large percentage of acute DVT patients experience recurrent VTE despite adequate anticoagulation, and may progress to CPC. Further, the role of iliocaval venous obstruction (ICVO) in lower-extremity VTE has been increasingly recognized in recent years. Imaging continues to play an important role in both acute and chronic venous disease. Venous duplex ultrasound remains the gold standard for diagnosing acute VTE. However, imaging of CPC is more comp

Acute Abdomen in an Extremely Low-Birth-Weight Preterm Neonate: A Case of Appendicitis.

INTRODUCTION: Neonatal appendicitis is a very rare surgical entity. Non-specific symptoms such as feeding intolerance, abdominal distension, vomiting, increased gastric residue, lethargy, and fever may be present. The majority of reported cases could not be identified early. In this report, we present an extremely low-birth-weight preterm neonate who has been diagnosed with appendicitis. CASE PRESENTATION: A 980-gram preterm baby girl was born at 31 1/7 weeks of gestation. The physical examination was normal at birth. Her initial clinical course was uneventful. On the 7th day of life, she dev

Thyrolipoma presentation as a huge multinodular goiter; A case report of an extremely rare entity.

IMPORTANCE AND IMPORTANCE: Thyroid lipomatosis is a rare entity of thyroid gland lesions. The exact etiopathogenesis of this condition is unknown. Most patients presented with compression symptoms. Radiological investigations such as neck ultrasonography (U/S) and computed tomography (CT) are crucial to evaluate and diagnose fat-containing thyroid tumors, while the definitive diagnosis is achieved by the histopathological study. CASE PRESENTATION: A 78-year-old female patient, with a background medical history of diabetes mellitus type II and chronic kidney disease, presented to our hospital

A Case of JAK2V617F-Negative Myeloproliferative Neoplasm in a Young Female Presenting With Extreme Thrombocytosis.

Thrombocytosis is a commonly observed condition in clinical practice and typically results from various pathophysiological factors, such as iron deficiency, blood loss, infection, medications, rheumatologic conditions, malignancy, asplenia, post-splenectomy, or familial factors. However, extreme thrombocytosis, defined as a platelet count > 10,000 K/UL (equal or greater than a million), is a rare occurrence. In this report, we present a compelling case of severe thrombocytosis attributed to underlying chronic myelogenous leukemia (CML), further complicated by coexisting iron deficiency. It is

Joint analysis of multiple phenotypes for extremely unbalanced case-control association studies.

In genome-wide association studies (GWAS) for thousands of phenotypes in biobanks, most binary phenotypes have substantially fewer cases than controls. Many widely used approaches for joint analysis of multiple phenotypes produce inflated type I error rates for such extremely unbalanced case-control phenotypes. In this research, we develop a method to jointly analyze multiple unbalanced case-control phenotypes to circumvent this issue. We first group multiple phenotypes into different clusters based on a hierarchical clustering method, then we merge phenotypes in each cluster into a single phe

Extreme climatic events and human biology and health: A primer and opportunities for future research.

Extreme climatic events are increasing in frequency, leading to hotter temperatures, flooding, droughts, severe storms, and rising oceans. This special issue brings together a collection of seven articles that describe the impacts of extreme climatic events on a diverse set of human biology and health outcomes. The first two articles cover extreme temperatures extending from extreme heat to cold and changes in winter weather and the respective implications for adverse health events, human environmental limits, well-being, and human adaptability. Next, two articles cover the effects of exposure

Self-expanding transcatheter aortic valve replacement in patients with extremely horizontal aortas.

OBJECTIVES: To evaluate the feasibility of self-expanding transcatheter aortic valve replacement (TAVR) in patients with aortic stenosis and extremely horizontal aortas (aortic angulation ≥70°). BACKGROUND: As TAVR using a self-expanding prosthesis is an off-label treatment for patients with extremely horizontal aortas, these patients are often excluded from randomized controlled trials involving self-expanding TAVR. METHODS: This study enrolled 27 consecutive patients with extremely horizontal aortas who underwent self-expanding TAVR for severe aortic stenosis. RESULTS: The patients' avera

Amputee, clinician, and regulator perspectives on current and prospective upper extremity prosthetic technologies.

Existing prosthetic technologies for people with upper limb amputation are being adopted at moderate rates. Once fitted for these devices, many upper limb amputees report not using them regularly or at all. The primary aim of this study was to solicit feedback about prosthetic technology and important device design criteria from amputees, clinicians, and device regulators. We compare these perspectives to identify common or divergent priorities. Twenty-one adults with upper limb loss, 35 clinicians, and 3 regulators completed a survey on existing prosthetic technologies and a conceptual sensor

Comparison of Work- and Nonwork-Related Lower Extremity Burn Injuries: A Retrospective Analysis.

Lower extremity burn injuries are among the most affected anatomical regions in hospitalized burn patients. Our objective was to compare burn patients with work-related isolated lower extremity burn injuries (w-ILEBI) and patients with nonwork-related isolated lower extremity burn injuries (nw-ILEBI). Sixty-four (15.7%) of 407 patients (16-92 years) were in the w-ILEBI cohort. The most extensive burn wounds were among patients in the fire-flame group with a median total body surface area percent (%TBSA) of 27.0 (interquartile range = 11.0%-45.0%). While 50 (76.9%) patients in the electrical gr
