Management of radiation-induced severe anophthalmic socket contracture in patients with uveal melanoma.

PURPOSE: High-dose radiotherapy can cause contracture of the anophthalmic socket, but the incidence of this complication in patients with enucleation for uveal melanoma has not been reported previously. The authors reviewed the surgical management and outcomes in terms of successful prosthesis wear in patients with severe contracture of the anophthalmic socket treated with high-dose radiotherapy for high-risk uveal melanoma, and they estimated the relative risk of this complication. METHODS: The medical records of all consecutive patients enrolled in a prospective uveal-melanoma tissue-bankin

Transient loss of prostaglandin synthetic capacity in rabbit iris-ciliary body following anterior chamber paracentesis.

The trauma-induced acute ocular inflammatory response has been characterized by investigating the kinetics of blood-aqueous barrier (BAB) breakdown, prostaglandin (PG) accumulation in the aqueous humor, and cyclooxygenase (PGH synthase) activity of the iris-ciliary body (ICB) following paracentesis in the NZA rabbit. BAB breakdown was assessed by quantifying plasma protein extravasation into the anterior chamber. PGE2 and 6-keto-PGF(1alpha) concentrations in the aqueous humor were quantified by radioimmunoassay. The capacity of ICB tissue homogenates to generate eicosanoids from exogenously su

Exendin-4 improves neurodevelopmental outcome after neonatal germinal matrix hemorrhage.

Germinal matrix hemorrhage (GMH) is a common complication in premature infants and is associated with a high risk of neurodevelopmental impairment and mortality. Currently, there are no specific neuroprotective treatments available. Exendin-4 is a drug used for the treatment of type 2 diabetes mellitus, and it has shown neuroprotective effects in several neurological disorders including Alzheimer's and Parkinson's disease. In this study, we used the preterm postnatal day 5 rat model of GMH to evaluate whether exendin-4 exerts neuroprotective effects in this setting. Our results show that in th

Surgical considerations for adult Graves' disease: a narrative review of indications, perioperative challenges and postoperative outcomes.

INTRODUCTION: Graves' disease (GD) is a systemic autoimmune disorder, ultimately caused by autoantibodies stimulating the TSH receptor (TSHR-Ab) on thyroid follicular cells. Novel targeted therapies are under investigation, but treatment of GD still relies on antithyroid drugs (ATDs), radioactive iodine, or thyroidectomy. AREAS COVERED: Narrative review of original articles, randomized clinical trials, systematic reviews and meta-analyses, guidelines on PubMed from inception to March 2026, using the following terms: GD, management of GD, thyroidectomy, hypoparathyroidism, hypocalcemia, recurr

Transthyretin-Related Familial Amyloidosis Polyneuropathy with Spinal Cord Damage: A Case Report.

INTRODUCTION: Transthyretin protein-related familial amyloidosis polyneuropathy (TTR-FAP) is an autosomal dominant genetic disease caused by mutations in the TTR gene. The disease is characterized primarily by peripheral and autonomic nerve damage. Disease progression is associated with frequent involvement of the heart, lungs, kidneys, eyes, and other organs. The most common TTR mutation is c.148G>A (p.Val50Met), although the FAP resulting from the mutation rarely involves the spinal cord. PATIENT CONCERNS: A 68-year-old man was diagnosed with the TTR c.148G>A (p.Val50Met) mutation by ultras

Clinical rationale for dietary lutein supplementation in long COVID and mRNA vaccine injury syndromes.

Lutein, a plant-derived xanthophyl-carotenoid, is an exceptional antioxidant and anti-inflammatory constituent found in food. High dietary intake of lutein is beneficial against eye disease, improves cardiometabolic health, protects from neurodegenerative diseases, and is beneficial for liver, kidney, and respiratory health. Lutein protects against oxidative and nitrosative stress, both of which play a major role in long COVID and mRNA vaccination injury syndromes. Lutein is an important natural agent for therapeutic use against oxidative and nitrosative stress in chronic illnesses such as car

Cilia-associated wound repair mediated by IFT88 in retinal pigment epithelium.

Primary cilia are conserved organelles that integrate extracellular cues into intracellular signals and are critical for diverse processes, including cellular development and repair responses. Deficits in ciliary function cause multisystemic human diseases known as ciliopathies. In the eye, atrophy of the retinal pigment epithelium (RPE) is a common feature of many ciliopathies. However, the roles of RPE cilia in vivo remain poorly understood. In this study, we first found that mouse RPE cells only transiently form primary cilia. We then examined the RPE in the mouse model of Bardet-Biedl Synd

Carotenoids in the Management of Glaucoma: A Systematic Review of the Evidence.

Primary open-angle glaucoma (POAG) remains a leading cause of irreversible blindness globally. Recent evidence further substantiates sustained oxidative stress, and compromised antioxidant defenses are key drivers in the onset of glaucomatous neurodegeneration. Overwhelming oxidative injury is likely attributed to compounding mitochondrial dysfunction that worsens with age-related processes, causing aberrant formation of free radical species. Thus, a compromised systemic antioxidant capacity exacerbates further oxidative insult in glaucoma, leading to apoptosis, neuroinflammation, and subseque

The Regenerative Potential of Facial Nerve Motoneurons following Chronic Axotomy in Rats.

BACKGROUND: The precise mechanisms of nerve regeneration remain unclear. The potential of facial nerve regeneration and probable mechanisms involved following chronic facial nerve injury should be further studied. METHODS: Adult male Wistar rats were used to model either (i) facial nerve injury (axotomy) or (ii) reinjury (chronic axotomy followed by a second axotomy within 5 months). The rats were housed in the animal facility of the Eye and ENT Hospital of Shanghai Medical School, Fudan University (Shanghai, China). Expression of Shh (sonic hedgehog) and growth-associated protein 43 (GAP43,

Removal of an Impaled Intraocular Hair Comb Following Self-inflicted Trauma.

Ocular trauma is one of the most common and vision-threatening ophthalmic presentations with a wide spectrum of complications, such as bleeding, infection, vision loss, and enucleation. A 64-year-old-male presented to the emergency department (ED) with a self-inflicted orbital penetrating injury with a hair comb. Computed tomography showed the comb traversed the medial orbit inferior to the medial rectus but did not damage the optic nerve; there were no globe or orbital wall fractures. His ocular exam was significant for a right eye afferent pupillary defect and decreased visual acuity 20/800,

Urgent Subconjunctival Needle Decompression for Orbital Compartment Emphysema Caused by Compressed Air Injury.

Orbital compartment syndrome is a critical ophthalmic emergency that needs urgent diagnosis and treatment to prevent permanent vision loss caused by optic nerve compression or retinal ischemia. In this article, we present a child with orbital compartment syndrome caused by orbital emphysema as a result of a rare type of ocular trauma and introduce a simple technique to decompress the pressure. The patient was a 4-year-old boy who experienced a compressed air blast to his left eye. He presented to the emergency department with a frozen globe, tight orbit, and chemosis, without any evidence of g

Lacrimal Gland Repair after Short-term Obstruction of Excretory Duct in Rabbits.

Aqueous tear-deficient dry eye is a multifactorial chronic disorder in which the lacrimal glands fail to produce enough tears to maintain a healthy ocular surface. The existence of lacrimal gland stem/progenitor cells was proposed in several species, yet their origin and characteristics are not very clear. Here, we investigated the presence of resident progenitor cells and their regenerative potential in a rabbit model with lacrimal gland main excretory duct ligation-induced injury. The ligation-injured lacrimal glands temporarily decreased in weight and had impaired tear secretion. Protein ex

Stimulation of axon regeneration in the mature optic nerve by intravitreal application of the toll-like receptor 2 agonist Pam3Cys.

PURPOSE: After injury of the optic nerve, mature retinal ganglion cells (RGCs) are normally unable to regenerate axons and undergo apoptosis. However, inflammatory stimulation in the eye induced by the release of beta/gamma-crystallins from the injured lens or intravitreal zymosan injection transforms RGCs into an active regenerative state, protecting these neurons from cell death and allowing them to regenerate axons back into the optic nerve. METHODS: The authors tested whether intravitreal application of the selective, water-soluble, toll-like receptor 2 agonist Pam(3)Cys can delay axotomi

[Siderosis bulbi. Is it still a problem?].

BACKGROUND: Siderosis bulbi (SB) is caused by un-removed intraocular iron-containing foreign bodies. Our retrospective study was designed to establish the number of patients treated for SB at our ophthalmology department within the past 10 years, and to find out why SB occurred. PATIENTS AND METHODS: Retrospectively, we analyzed the documentation of every patient treated at our department for ocular injury due to an intraocular metal foreign body between 1.1.1992 and 1.1.2002. Particular attention was focussed on the documentation of patients exhibiting symptoms of SB. RESULTS: During this p

[Involutional ophthalmic endotoxicosis as the cause of the involutional vitroretinal syndrome].

The author regards a characteristic complex of ocular changes in elderly subjects, including alterations in the posterior lenticular capsule, vitreous destruction, and retinal glial plaques presenting as coin-like, flap, and other light reflexes of the fundus oculi, as the involution vitreoretinal syndrome (IVS). Examinations of 201 subjects aged 51 to 98 at a nursing home for elderly subjects revealed this syndrome in 17 (34 eyes). Few patients with IVS among elderly subjects and examinations of 124 patients with this condition hospitalized to rule out a chronic inflammatory process gave grou
