A customized large single-piece bifrontal implant for post-craniectomy defect reconstruction: A case study.

BACKGROUND: Large bifrontal defects pose unique reconstruction challenges due to their complex curvature and mechanical requirements. This case demonstrated how computer-aided design/manufacturing (CAD/CAM) enabled precise single-piece polymethyl methacrylate (PMMA) implant fabrication, thereby overcoming traditional limitations. CASE PRESENTATION: A 25-year-old male who had undergone bifrontal decompressive craniectomy suffered a severe traumatic brain injury. The autologous bone flap had been temporarily stored in a subcutaneous fat area of the abdomen for 3 months to preserve its viability

Deciding whether to do elective neck dissection in patients with salivary gland tumors with no evidence of neck lymph node metastasis.

AIM: The treatment of salivary gland cancers with clinically negative neck is controversial, with divergent guidelines about elective neck dissection (END). Even though these guidelines are widely used, they mostly rely on retrospective research that are subject to methodological flaws and selection bias. METHODS: A critical narrative review. Problem description, identification of likely clinical scenarios, appraisal of previous recommendations, critical examination of available data, and presentation of clinical decision-making options comprised the article creation. RESULTS: One of the mos

Is it Time to Ditch the DICH (Delayed Intracranial Hemorrhage) Yet?

Background and objective Delayed intracranial hemorrhage (DICH) is a well-known injury that is rare among patients who are on antiplatelet and/or anticoagulants with head trauma. In this study, we aimed to test the hypothesis that DICH is unlikely to occur without a history of head impact and signs of head and/or face trauma. Materials and methods We conducted a two-year (2020-2021) retrospective study regarding the incidence of DICH at our institution. During the study period, our institution had created a protocol specifically for this patient population who possibly suffered head trauma whi

Another Rare Cause of Hypertrophic Olivary Degeneration Following Cavernous Malformation Hemorrhage: A Case Report.

Introduction: Hypertrophic olivary degeneration (HOD) is a rare form of trans-synaptic degeneration involving the Guillain-Mollaret triangle, characterized by enlargement of the inferior olivary nucleus-unlike the atrophy typical of most neurodegenerative processes. It is usually associated with stroke, surgical injury, or demyelination, but rarely follows hemorrhage from a cavernous malformation (CM). This report presents a case of HOD secondary to a mesencephalic CM hemorrhage, with emphasis on imaging findings and diagnostic considerations. Case Description: A 55-year-old woman presented wi

In-depth summary of adverse events associated with Flurbiprofen: A real-world pharmacovigilance study from 2004 to 2024 using the FAERS database.

BACKGROUND: Flurbiprofen, as a widely used nonsteroidal anti-inflammatory drug (NSAID), is commonly employed to relieve mild to moderate pain and inflammation. Understanding its adverse reactions in real-world usage is of significant importance. METHODS: Reports of all adverse drug events (ADEs) related to flurbiprofen were extracted from the FAERS database, covering the period from Q1 2004 to Q3 2024. These reports were standardized and analyzed using various signal quantification techniques, including Reporting Odds Ratios (ROR), Proportional Reporting Ratios (PRR), Bayesian Confidence Prop

Assessment of infraorbital foramen location based on skeletal malocclusion, maxillary sinus pneumatization and infraorbital canal morphology using 3D CBCT scans.

INTRODUCTION: Understanding the infraorbital foramen (IOF) location is crucial in terms of regional anaesthesia as well as facial filler injection and radiofrequency neurotomy. We aimed to define a safe infraorbital foramen (IOF) location zone using three dimensional (3D) cone beam computed tomography (CBCT) scans and to evaluate the influence of skeletal malocclusion (SM), maxillary sinus pneumatization (MSP), and infraorbital canal (IOC) morphology on IOF position. MATERIALS AND METHODS: 3D models of 300 (170 female, 130 male, range 18-50 years) CBCT scans were used to identify the SM, MSP,

VTA dopaminergic neurons involved in chronic spared nerve injury pain-induced depressive-like behavior.

Affective disorders, such as depression, are commonly associated with the development of chronic pain, but the underlying mechanisms still remain unclear. The dopaminergic system, located in the midbrain, is considered one of the regions where algesia and emotional processing overlap. This suggests a structural basis hypothesis for the comorbidity of chronic pain and depression, highlighting the interplay between nociceptive and affective processing. But there are more and more evidences show that somatic and head/facial pain involve different neuronal overlap. In previous study, the research

Case Series: Therapeutic Combination of VoluDerm Radiofrequency Microneedling and Glycolic Acid Peel in Scaled-Up Concentrations.

BACKGROUND: Glycolic acid (GA) is an α-hydroxy peeling agent that causes controlled removal of the epidermis, with or without the dermis. Studies have shown the ability of GA to stimulate fibroblast proliferation, induce collagen synthesis, and decrease collagen degradation. The VoluDerm radiofrequency microneedling (RFMN; Pollogen, Tel Aviv, Israel) utilizes an array of microelectrodes to penetrate the epidermis and deliver energy to the skin. The controlled fractional thermal injury promotes neocollagenesis in the correction of skin laxity and wrinkle reduction. It was theorized that GA and

The Effect of Lag Time Between Injury and Treatment of Zygomatic Complex Fractures on the Neurosensory Disturbances of the Infraorbital Nerve.

INTRODUCTION: Zygomatic complex fractures are the second most common facial fractures. Neurosensory disturbances of the infraorbital nerve are common following zygomatic complex fractures. Fracture complexity and displacement of fracture segments can cause neurosensory disturbances of the infraorbital nerve following zygomatic complex fractures. Therefore, fractures of the zygomatic complex are characterized by sensory neuropathy in the area of innervations of the infraorbital nerve as a presenting symptom. AIM AND OBJECTIVES: To evaluate the neurosensory disturbances of infraorbital nerve in

Low-fluence treatment with a novel fractionated 2,910-nm fiber laser improves photodamage.

BACKGROUND: Facial rejuvenation by lasers that target water has been a mainstay of esthetic laser treatments for decades. Modern lasers more commonly treat a fraction of the skin surface using ablative, semi-ablative, or nonablative pulses. METHODS: Twenty subjects with visible evidence of chronic photoaging on the face were enrolled in this study. All subjects received two full-face, single-pass treatments spaced 2 months apart with the superficial mode of a 2910 nm fiber laser with an estimated penetration depth of 10 μm, 25% coverage, delivered in a 15 mm × 15 mm square microbeam pattern.

Trigeminal neuropathic pain reduces 50-kHz ultrasonic vocalizations in rats, which are restored by analgesic drugs.

Trigeminal neuralgia (TN) is a severe form of neuropathic pain frequently associated with anxiety. The chronic constriction injury of the infraorbital nerve (CCI-ION) of rodents is a well-established model to study sensory alterations related to TN. However, few studies have addressed the emotional component of pain, which is fundamental to increase its translational capability. Emission of ultrasonic vocalization (USV) is considered a reliable measure of the emotional state of rats. Rats emit 50-kHz USVs in social and appetitive situations, whereas 22-kHz USVs may index a negative state. Stud

Ancient Schwannoma: Case Report of an Unusual Entity in an Unusual Oral Location.

BACKGROUND Schwannoma is an uncommon benign tumor that develops from Schwann cells in the peripheral nervous system; 25-50% of such tumors occur in the head and neck. A rare variety of schwannoma, known as an ancient schwannoma, exhibits calcification, cystic degeneration, hemorrhage, myxoid stroma, pleomorphism, and nuclear hyperchromatism. Such characteristics may contribute to this tumor being incorrectly diagnosed as malignant. Almost all reported intra-oral ancient schwannomas behave innocuously. CASE REPORT A 20-year-old man presented to the oral surgery clinic with a right facial swelli

A systematic review and meta-analysis of different mask ventilation schemes on management of general anesthesia in patients with respiratory failure.

BACKGROUND: Difficulty in mask ventilation is one of the more dangerous factors in general anesthesia. The traditional mask has some problems, such as air leakage and facial skin compression injury. The head cover is a new interactive non-invasive ventilation (NIV) model. NIV studies comparing hoods and masks have all been single-center and small-sample randomized trials, and extensive clinical studies are lacking. METHODS: We conducted a computerized search in the databases of PubMed, Embase, Medline, Chinese Biomedical Literature (CBM), and others for randomized controlled trials (RCTs) on

Endonasal Free Flap Reconstruction Combined With Draf Frontal Sinusotomy for Complex Cerebrospinal Fluid Leak: A Technical Report & Case Series.

BACKGROUND: Frontal sinus cranialization with closure via bifrontal pericranial flaps is the gold standard for separating the nasofrontal recess from the intracranial cavity for posterior table defects. Despite the high success rate, cerebrospinal fluid (CSF) leak may persist and is particularly challenging when vascularized reconstructive options from the bicoronal incision are exhausted. OBJECTIVE: To assess a novel endonasal technique using an adipofascial radial forearm free flap delivered to the frontal recess through a Draf sinusotomy to repair complex CSF leaks from the frontal sinus.
