Profile of patients with chemical injury and sensitivity.

Patients reporting sensitivity to multiple chemicals at levels usually tolerated by the healthy population were administered standardized questionnaires to evaluate their symptoms and the exposures that aggravated these symptoms. Many patients were referred for medical tests. It is thought that patients with chemical sensitivity have organ abnormalities involving the liver, nervous system (brain, including limbic, peripheral, autonomic), immune system, and porphyrin metabolism, probably reflecting chemical injury to these systems. Laboratory results are not consistent with a psychologic origin

Off-Label Use of Gabapentinoids in Non-diabetic Patients Referred to a Rheumatology Clinic: A Cross-Sectional Study.

Background Gabapentin is FDA-approved for postherpetic neuralgia and partial-onset seizures, while pregabalin is approved for several neuropathic pain conditions, fibromyalgia, and as adjunctive therapy for partial-onset seizures. Despite limited evidence for many off-label uses, gabapentinoids are frequently prescribed, raising concerns about adverse effects, misuse, and dependence. This study examines gabapentinoid prescribing patterns among patients referred to our rheumatology clinic. Patients and methods A cross-sectional study of patients without diabetes (N=466) was conducted among newl

Longitudinal association of mental-health problems with subsequent diagnosis of persistent somatic symptom disorders: A large-scale registry-based observational study in primary care.

BACKGROUND: Somatic symptoms of common persistent somatic symptom (PSS) syndromes like irritable bowel syndrome (IBS), chronic fatigue syndrome (CFS), and fibromyalgia (FM) are not fully attributed to well-established biomedical pathological processes. These syndromes are often under-recognized and diagnosis is often delayed. This study assessed the extent to which mental health problems precede the onset of IBS, CFS, and FM diagnoses using large-scale registry-based data from primary care settings. METHOD: Data from 11,409 patients were anonymously extracted from primary care data-bases in t

Centrally mediated pain features predict persistent pain in early inflammatory arthritis: a 2-year prospective study.

Patients with inflammatory arthritis (IA) often report pain not necessarily coupled to active inflammation. We previously demonstrated that a significant subset of patients with newly diagnosed IA report features of centrally mediated pain. In this prospective study following the same IA cohort, we tracked the relative contributions of inflammatory/noninflammatory mechanisms to pain over 2 years. We used patient-reported outcome measures (PROMs), clinical data (including ultrasound), and standardised quantitative sensory testing. Mixed-effects regression models examined longitudinal associatio

Real-world effectiveness of b/tsDMARD switching in low-inflammatory difficult-to-treat rheumatoid arthritis: insights from the FIRST registry.

OBJECTIVES: To assess the effectiveness of switching biological or targeted synthetic disease-modifying antirheumatic drugs (b/tsDMARDs) in patients with low-inflammatory difficult-to-treat rheumatoid arthritis (D2T RA). METHODS: Using the multicentre FIRST registry, we identified D2T RA between August 2013 and March 2024. Low-inflammatory D2T RA was defined as a swollen 28-joint count ≤1 and C-reactive protein <10 mg/L. In the low-inflammatory D2T RA group, we compared those who underwent b/tsDMARD switching (the switch group) with the non-switch group. The primary outcome was the 6-month ch

Attention-deficit/hyperactivity disorder and chronic pain: a scoping review of epidemiology, clinical phenotypes, mechanisms, and treatment.

INTRODUCTION: Emerging evidence suggests a potential association between attention-deficit/hyperactivity disorder (ADHD) and chronic pain; however, extant findings are dispersed across disciplines and have not yet been comprehensively synthesized. This scoping review aimed to systematically map the epidemiological evidence, clinical phenotypes, proposed neurobiological mechanisms, and reported therapeutic interventions related to comorbid ADHD and chronic pain. METHODS: A comprehensive literature search was conducted in PubMed, PsycINFO, and the Cochrane Library from database inception to Dec

Research progress on glycolytic reprogramming in ophthalmic diseases.

OBJECTIVES: More than 20% of rheumatoid arthritis (RA) patients have comorbid fibromyalgia (FM+), which may elevate DAS28-ESR (disease activity score 28-erythrocyte sedimentation rate) and other indices, resulting in challenges to assess inflammatory disease activity. Although several reports indicate that elevated patient global assessment (PATGL) may elevate DAS28 in the absence of inflammatory activity, less information is available concerning the other three components, tender joint count (TJC), swollen joint count (SJC), and erythrocyte sedimentation rate (ESR), to possibly elevate DAS28

The prescribing of opioids for chronic non-cancer pain in the menopausal and postmenopausal population: a drug utilisation study in the UK.

BACKGROUND: Opioid use for chronic non-cancer pain (CNCP) is consistently higher in menopausal and postmenopausal women than in younger women or men, elevating their risk of opioid-related adverse health outcomes. Since pain severity increases with hormonal changes accompanying menopause, these women should be a focus of opioid stewardship efforts. AIM: To examine opioid prescribing trends for CNCP in menopausal and postmenopausal women diagnosed with a musculoskeletal condition. DESIGN & SETTING: Population-based drug utilisation study, which was undertaken with data from IQVIA Medical Rese

Tender joints in early autoantibody-negative rheumatoid arthritis: Should they be included in the scoring of joint involvement of the 2010 ACR/EULAR classification criteria?

OBJECTIVES: The interpretation of joint tenderness as a sign of inflammation in patients with autoantibody-negative rheumatoid arthritis (RA) is uncertain. This may hinder disease classification and create selection bias for patient enrollment in clinical trials. Here we tested whether reclassifying the pattern of joint involvement based on swollen joints increases specificity for persistent arthritis in autoantibody-negative early RA. METHODS: From a prospective early arthritis cohort in the years 2005-2018, all autoantibody-negative patients fulfilling the 2010 ACR/EULAR RA criteria at enro
