Combining Whole-Body Vibration, Serial Casting, and Therapy to Treat Hemiplegic Cerebral Palsy: A Case Report.

PURPOSE: This case report describes the combined use of whole-body vibration (WBV), serial casting (SC), and physical therapy (PT) for a child with hemiplegic cerebral palsy (HCP). SUMMARY OF KEY POINTS: A 3-year 10-month-old male with HCP was followed for 10 months. Treatment included 9 weeks of SC due to ankle plantarflexion contracture, hypertonicity, poor orthotic tolerance, and gait limitations. WBV was introduced during week 4 to address hypertonicity and poor tolerance of manual therapy. PT sessions included stretching, gait training, and strengthening exercises, integrating SC and WBV

Muscle-controlled physics simulations of bird locomotion resolve the grounded running paradox.

Humans and birds use very different running styles. Unlike humans, birds adopt "grounded running" at intermediate speeds-a running gait where at least one foot always maintains ground contact. Avian grounded running is a paradox: Animals usually minimize locomotor energy expenditure, but birds prefer grounded running despite incurring higher energy costs. Using predictive gait simulations of the emu (Dromaius novaehollandiae), we resolve this paradox by demonstrating that grounded running represents an optimal gait for birds, from both energetics and muscle excitations perspectives. Our virtua

Physiotherapy Approach to a Stage V Parkinson's Disease Patient: A Case Report.

Parkinson's disease (PD) is a neurodegenerative disorder caused due to decreased dopamine, a neurotransmitter, advancing to a range of motor and non-motor attributes. There is a death of dopamine-producing neurons (dopaminergic neurons) in the Substantia Nigra. Bradykinesia, postural instability, resting tremor, and rigidity are four main symptoms in this patient. A variety of other symptoms, like hypomimia, micrographia, freezing gait, decreased movement amplitude, constipation, cognitive impairments, etc., can be seen in this patient. In this paper, we report a 62-year-old female with stage

Bilateral asymmetric hip stiffness applied by a robotic hip exoskeleton elicits kinematic and kinetic adaptation.

Wearable robotic exoskeletons hold great promise for gait rehabilitation as portable, accessible tools. However, a better understanding of the potential for exoskeletons to elicit neural adaptation-a critical component of neurological gait rehabilitation-is needed. In this study, we investigated whether humans adapt to bilateral asymmetric stiffness perturbations applied by a hip exoskeleton, taking inspiration from asymmetry augmentation strategies used in split-belt treadmill training. During walking, we applied torques about the hip joints to repel the thigh away from a neutral position on

Association of vascular parkinsonism with impaired neuronal integrity in the striatum.

The purpose of this study is to identify the underlying differences between patients with white matter lesions (WMLs) who manifested gait disturbance suggestive of vascular parkinsonism (VaP) and those who did not, using the PET scan. Fourteen patients with extensive WMLs, as determined by MRI, were divided into two groups - 7 with gait disturbance and 7 without it. Neuronal integrity was evaluated with a PET scan using [(11)C]flumazenil (FMZ) by calculating the distribution volume of FMZ (FMZ-V(d)) in various regions of interest by non-linear curve fitting. Additionally, tracer kinetic analys

[Worsened orthostatic hypotension due to levodopa administration in a case of Parkinson's disease].

A 70-year-old woman began to experience gait disturbance in 1995, followed by the appearance of action tremor of the left hand in 1996. This tremor was subsequently noted in the right hand. Her doctor initially diagnosed Parkinson's disease and administered amantadine and levodopa. The gait disturbance and tremor improved. However, she stopped taking the medication and her symptoms worsened in April, 1998. She was admitted to our hospital, and showed rigidity and action tremor predominantly on the left side, frozen gait, postural reflex disturbance and orthostatic hypotension. Following levodo

The Heidelberg Functional Foot Model-Application to Cavovarus and Equinovarus Feet.

Multisegment foot models have become increasingly important in biomechanical research and clinical gait analysis but often face limitations in defining joint positions. Often, they rely on simplified methods, such as using the midpoint between two markers to represent a joint, which lacks functional verification. In contrast, phenomenological angles, such as the medial arch angle, bypass joint center calculations, and offer sensitive, radiologically aligned indicators of foot mechanics. The Heidelberg functional foot model (HFFM) integrates functionally verified joint positions in combination

Step width modification to change rearfoot eversion and medial longitudinal arch angle during walking and running in individuals with pronated feet.

BACKGROUND: Individuals with pronated feet often experience altered foot biomechanics, leading to increased risk of lower limb injuries. Step width modification has been proposed as a potential intervention to improve foot alignment during gait. RESEARCH QUESTION: Does modifying step width influence rearfoot eversion and medial longitudinal arch angle (MLAA) in individuals with pronated feet during walking and running? METHODS: Twenty individuals with pronated feet underwent analysis during walking and running on treadmill, maintaining increased or decreased step width using real-time visual

[Pregnancy-associated femoral nerve affection].

BACKGROUND: Common neuropathies during pregnancy are carpal tunnel syndrome or peripheral facial paralysis. However, there is little information about femoral nerve affection during pregnancy. CASE REPORT: We report on a female at 30 weeks of gestation, complaining of pain in hips and thighs and gait difficulty. Pregnancy was normally developed. Neurological examination showed a bilateral weakness in both quadriceps and iliopsoas muscles combined with soreness in hips and thighs, without deficits of sensibility. Laboratory findings and results of a lumbar punction were normal. Pelvic ultrasou

Levodopa influence on turning dynamics in people with Parkinson's disease.

Turning behaviors are affected in people with Parkinson's disease (PwPD) leading to falls and Freezing of Gait (FOG). Levodopa therapy is commonly recommended for PwPD to help alleviate motor symptoms. Knowledge of turning dynamics with levodopa therapy is limited and could improve clinical care. We enrolled 44 PwPD already on levodopa therapy and analyzed their turns on an instrumented gait mat. Participants made 180-degree turns in their OFF-state (> 8 h off levodopa) then ON-state (1 h after levodopa). Thirteen turn measures were assessed for changes between the OFF- to ON-state. Compared t

Concurrent validity of Protokinetics movement analysis software for estimated centre of mass displacement and velocity during walking.

BACKGROUND: The possibilities of out-of-lab gait analysis are ever increasing, with a recent development from Protokinetics movement analysis software (PKMAS) which, among other parameters, proposes an estimation of centre of mass (COMe) displacement. This measure can be useful in both clinical and sports performance environments. RESEARCH QUESTION: Does PKMAS provide an accurate estimation of the COM displacement and velocity? METHODS: Fifteen participants completed 10 walking trials over the Zeno™ Walkway while simultaneously being equipped with a full-body marker set for VICON analysis. T

Ability of a multi-segment foot model to measure kinematic differences in cavus, neutrally aligned, asymptomatic planus, and symptomatic planus foot types.

BACKGROUND: Multi-segment foot models (MFMs) provide a better understanding of the intricate biomechanics of the foot, yet it is unclear if they accurately differentiate foot type function during locomotion. RESEARCH QUESTION: We employed an MFM to detect subtle kinematic differences between foot types, including: pes cavus, neutrally aligned, and asymptomatic and symptomatic pes planus. The study investigates how variable the results of this MFM are and if it can detect kinematic differences between pathologic and non-pathologic foot types during the stance phase of gait. METHODS: Independe

A comprehensive deep learning model for motor phenotypes of Parkinson's disease using three-dimensional kinect V2 detectors.

BACKGROUND: Gait impairments are common for Parkinson's disease (PD). With the development of artificial intelligence (AI) technology and three-dimensional Kinect V2 Detectors, it is possible to enable more accurate characterization of gait impairment. We develop a comprehensive prediction model by combining skeleton gait energy image with relative distance and angle for PD motor phenotypes. RESEARCH QUESTION: Does the hybrid convolutional neural network-long short-term memory (CNN-LSTM) deep learning model improve diagnostic accuracy and outperform CNN or LSTM models in diagnosing different

Effect of stiffness-optimized ankle foot orthoses on joint work in adults with neuromuscular diseases is related to severity of push-off deficits.

BACKGROUND: People with plantar flexor weakness generate less ankle push-off work during walking, resulting in inefficient proximal joint compensations. To increase push-off work, spring-like ankle foot orthoses (AFOs) can be provided. However, whether and in which patients AFOs increase push-off work and reduce compensatory hip and knee work is unknown. METHODS: In 18 people with bilateral plantar flexor weakness, we performed a 3D gait analysis at comfortable walking speed with shoes-only and with AFOs of which the stiffness was optimized. To account for walking speed differences between co

The assessment of walking skills: Italian version.

Apraxia is a neuropsychological disorder that impairs voluntary, purposeful movements, with "Gait apraxia" specifically affecting walking. Because of the lack of standardized diagnostic tools, in Italian, we translated and adapted the "Assessment of Walking Skills" (AWS) scale, originally developed for identifying gait apraxia in Alzheimer's patients. The AWS includes 42 items that evaluate trunk and leg movements, useful for comparing the performance between patients and healthy controls. This translation and adaptation represent a critical step toward standardizing the AWS scale for the Ital

Loeys-Dietz syndrome with bilateral radial head dislocations: a case report.

Loeys-Dietz syndrome is characterised by vascular aneurysms, hypertelorism, and a bifid uvula. We report on an 11-year-old boy with Loeys-Dietz syndrome who presented with bilateral radial head dislocations and severe osteopaenia with changes of avascular necrosis in both hips causing an out-toeing, wide gait. Considering the poor prognosis for elbow movement and possible radial head dysplasia, surgical reduction of the radial heads was deferred. A subtrochanteric de-rotation osteotomy of the left hip was performed to improve the gait.

Review of preorthotic biomechanical considerations.

The application of an ankle-foot orthosis in poststroke hemiplegia can improve the mechanics of ambulation and functional gait. However, prior to the fitting of an orthotic, the clinician should perform a thorough assessment of the biomechanical elements contributing to mobility problems within the patient. Many of these problems such as joint contracture or weakness should be addressed through physical therapy interventions prior to orthotic prescription. This article reviews the common biomechanical elements that contribute to mobility and gait disability in patients with stroke with special

[Reconstruction of large foot sole areas with microsurgical tissue transfers].

In 14 patients with microsurgically reconstructed areas of the sole videografic gait analysis was performed. The best results are reached after reconstruction of the forefoot. Loss of more than two thirds of the sole leads to functional impairment. For all reconstructed areas and all types of flaps gait is improved significantly by shoe adaptation: elevation of the shoe under the reconstructed part of the sole directs the main load to the opposite side of the foot and by this relieves the reconstructed area. Even bone corrections can reduce load on a circumscribed area.
