Comparison and identification of serum metabolomic profiles in Stage B and Stage C ejection fraction preserved heart failure.

BACKGROUND: Disturbed metabolism correlates with the progression of heart failure with preserved ejection fraction (HFpEF). However, the discrepancy in metabolism between asymptomatic (Stage B) and symptomatic (Stage C) HFpEF patients remains unclear. This study aimed to explore the metabolic differences between Stages B and C HFpEF patients and to screen metabolites to distinguish between the two groups of patients. METHODS: A total of 97 Stage B and 31 Stage C HFpEF patients were included from a previous cohort, named Frailty and Comprehensive Geriatric Assessment in Hospitalized Elderly Pa

Experiences of healthcare interactions before and after suicidal behaviour among older adults attending geropsychiatric services: an interpretative phenomenological analysis.

OBJECTIVES: Physical illness and functional disability are common in older adult populations and strongly linked to suicidal behaviour. The aim was to explore how older adults who engaged in a suicidal act experienced their interactions with healthcare providers. DESIGN: This study re-examined transcripts from a broader study involving experiences of older adults who took part in two separate semi-structured interviews focusing on their experiences before and after a suicidal act. Interpretative phenomenological analysis was applied. SETTING: A geriatric psychiatric outpatient clinic in a la

Association of long-term glycemic variability with longitudinal motor and nonmotor progression in patients with Parkinson's disease: an 8-year follow-up.

BACKGROUND: Although recent evidence suggests that glycemic variability (GV) has a negative impact on neurodegeneration, its role in Parkinson's disease (PD) remains unclear. OBJECTIVE: To explore the association between long-term GV and longitudinal motor and nonmotor progression in patients with PD and to uncover the disease-specific and nonspecific mechanisms underlying this association. METHODS: We used data obtained from the Parkinson's Progression Markers Initiative cohort. Three hundred seventy-seven patients with early untreated PD underwent annual motor and nonmotor assessments cove

Sex and the Multidimensional Prognostic Index in 3.5-year post-COVID-19 mortality among older adults: evidence of a time-varying effect.

PURPOSE: The long-term prognostic impact of frailty in older adults recovering from COVID-19 remains underexplored. The Multidimensional Prognostic Index (MPI) has shown utility in predicting short-term outcomes, but its role over extended follow-up requires further investigation. The objective of this study is to evaluate the ability of an MPI-based model to predict 3.5-year mortality in older adults hospitalized for COVID-19. METHODS: This single-center cohort study with prospective follow-up included 183 patients aged ≥ 65 years hospitalized with confirmed SARS-CoV-2 infection. MPI was cal

[The role of telemedicine in urological care for the elderly - benefits, challenges, and future perspectives].

The aging population has led to an increasing number of patients with urological conditions, such as urinary tract infections, prostate and bladder cancer, and nephrolithiasis. These diseases often require regular medical visits, posing challenges for elderly patients with usually limited mobility, as well as for the burdened healthcare system. Telemedicine, through remote consultations and telemonitoring tools, can improve the care of urological patients, which require further analysis. A literature review in Polish and English was conducted using the PubMed, Scopus, and Web of Science databa

Determining the Median Effective Dose of Remimazolam for Anesthesia Induction in Patients Undergoing Endoscopic Retrograde Cholangiopancreatography: An Age-Stratified Study.

BACKGROUND: Endoscopic Retrograde Cholangiopancreatography (ERCP) requires precise anesthesia control. Remimazolam is ideal for induction due to its rapid onset and minimal cardiorespiratory impact, but its optimal dose across ages is unclear. This study determined the median effective dose (ED5 0) of remimazolam for ERCP induction and assessed the influence of age. METHODS: In this prospective study, 110 patients aged 50-89 years (ASA I-III, BMI 18-30 kg/m2) scheduled for ERCP were stratified into four age groups: R1 (50-59), R2 (60-69), R3 (70-79), and R4 (80-89). Cholecystokinin inhibits myotube fusion and may contribute to sarcopenia: Evidence from clinical and C2C12 myoblast studies.

BACKGROUND: Sarcopenia, characterized by a progressive loss of muscle mass and strength, is increasingly recognized in aging populations. Although gut hormones may play a role in sarcopenia pathogenesis, their clinical relevance remains unclear. In this study, we investigated whether circulating cholecystokinin (CCK) is associated with sarcopenia and its effects on myogenesis. METHODS: From 2018 to 2022, 179 adults (aged ≥65 years) at the Taipei Veterans General Hospital were enrolled and classified as having sarcopenia or not based on the 2019 Asian Working Group for Sarcopenia criteria. [Expert consensus on the treatment of aging related to growth hormone deficiency].

The secretion of growth hormone progressively decreases with advancing age, and some of the elderly develop age-related growth hormone deficiency. Physiological concentration of growth hormone is crucial for the regulation of material metabolism and the maintenance of organ and systemic functions in adults. Clinically, adult growth hormone deficiency manifests as a complex constellation of symptoms including obesity, fatty liver, sarcopenia, osteoporosis, diminished sexual function, reduced physical performance, and depression, which are partially analogous to the clinical phenotypes of aging.

Case report: Depression in an older patient with dysgeusia as the initial symptom.

INTRODUCTION: Depression is the most common mental illness among older adults, with substantial and persistent mood depression as the main clinical feature, which is unfavorable for improvement. The clinical manifestations can range from melancholy to grief or even numbness. Approximately one-third of older adult patients exhibit physical discomfort as the first symptom; dysgeusia as the first symptom, is very rare in clinical practice. Dysgeusia is a clinical symptom with no specific objective indicator; thus, the likelihood of misdiagnosis and missed diagnosis is high. CASE PRESENTATION: In
