A Cochlear Implant Performance Prognostic Test Based on Electrical Field Interactions Evaluated by eABR (Electrical Auditory Brainstem Responses).

BACKGROUND: Cochlear implants (CIs) are neural prostheses that have been used routinely in the clinic over the past 25 years. They allow children who were born profoundly deaf, as well as adults affected by hearing loss for whom conventional hearing aids are insufficient, to attain a functional level of hearing. The "modern" CI (i.e., a multi-electrode implant using sequential coding strategies) has yielded good speech comprehension outcomes (recognition level for monosyllabic words about 50% to 60%, and sentence comprehension close to 90%). These good average results however hide a very impor

Palisade cartilage tympanoplasty for management of subtotal perforations: a comparison with the temporalis fascia technique.

UNLABELLED: The use of cartilage as a grafting material has been advocated in cases at high risk for failure, such as subtotal perforations, adhesive processes, tympanosclerosis and residual defects after primary tympanoplasties. The cartilage palisade technique, in which the TM is fully reconstructed with palisade-shaped cartilage pieces, was first described by Heermann. Cartilage material has been criticized because of concerns regarding hearing results, however, large cartilage plates with thicknesses <or=0.5 mm have been suggested as an acceptable comprise and the graft take of this techni

Analysis of the glycerin test for Menière's disease.

The glycerin test has low sensitivity for Menière's disease using present published criteria. The purpose of this study is to compare the efficacy of standard empirical criteria with our theoretically based criteria. Subjects were 47 patients with Menière's disease and 45 patients with other causes of hearing loss and dizziness. Pure-tone thresholds and word-identification scores were determined before and 3 hours after administration of glycerin. The results were submitted to decision and multivariate discriminate analyses. The theoretically based criterion applied to pure-tone threshold test

Hearing outcomes after cochlear implantation in two patients with ATP6V1B2-related deafness and onychodystrophy.

The gold standard recommendation for congenital sensorineural hearing loss (SNHL) care is cochlear implantation (CI). Adjusting for confounding factors such as developmental comorbidities is crucial when assessing expected outcomes of the procedure for the patients, their families, and their medical teams. We describe two clinical cases of the deafness and onychodystrophy (DOD) spectrum and the benefit of molecular diagnosis to underline the importance of genetic testing when evaluating potential CI outcomes in syndromic congenital SNHL.

Newborn hearing thresholds measured by both insert and earphone methods.

Auditory brainstem response (ABR) absolute thresholds were obtained from 31 ears of 28 newborns using both the insert and the earphone methods to deliver the stimuli. The two estimates on each ear were acquired in a single test session, and they differed by 10 dB or less in all cases. The results suggest that when the earphone is used, it rarely if ever collapses the ear canal to cause an artificial conductive hearing loss.

Pitfalls in neurotologic diagnosis.

Several cases illustrating potential pitfalls in neurotologic diagnosis from the viewpoint of the otologist are presented. The role of auditory brain stem response testing is specifically emphasized in cases with discordant audiologic and radiographic findings. Included are one case of a "false negative" ABR in a patient with an intracanalicular acoustic neuroma, a case of a "false positive" CT scan in a patient with Meniere's disease, and a case of a patient with normal hearing in whom an acoustic neuroma was discovered serendipitously.

Loudness and the acoustic reflex.

Fifteen subjects with unilateral sensorineural hypacusis were tested to determine the sound intensity eliciting the acoustic reflex in normal and impaired ears and the sound intensity in the normal ear which is equal in loudness to the intensity eliciting the acoustic reflex in the abnormal ear. Results suggest that loudness is not the mediating factor in the acoustic reflex, justifying the suggestion that low sensation level reflexes observed in hearing-impaired patients should not be considered an unequivocal indicant of recruitment.

Assessing the Role of Nystagmus Slow Phase Velocity as a Prognostic Indicator in Idiopathic Sudden Sensorineural Hearing Loss: A Prospective Study.

INTRODUCTION: Idiopathic sudden sensorineural hearing loss (ISSNHL) exhibits a diverse range of clinical presentations and treatment responses. Currently, there are no established variables that clinicians can utilize for the purposes of patient counseling when treating ISSNHL. MATERIALS AND METHODS: This prospective study involved 35 patients diagnosed with ISSNHL who were treated with oral and intratympanic steroid injections. We measured the peak and average slow phase velocity (SPV) (°/sec) of nystagmus after caloric stimulation during the first session of intratympanic steroid injection

Routine Hearing Screening for Older Adults in Primary Care: Insights of Patients and Clinic Personnel.

BACKGROUND AND OBJECTIVES: Hearing loss frequently goes undiagnosed and untreated, with serious sequelae. Hearing screening facilitates diagnosis and treatment but is not routinely conducted in primary care. This study addresses the attitudes and insights of patients and primary care clinic personnel relative to the routinization of hearing screening in primary care for older adults. RESEARCH DESIGN AND METHODS: Data presented are from the qualitative portion of a larger study. The main study compared screening completion for 3 approaches to coordinating telephone-based hearing screening with

Hearing loss in Norwegian adults with achondroplasia.

BACKGROUND: Achondroplasia is the most common form of disproportionate skeletal dysplasia. The condition is caused by a mutation in the FGFR3 gene, affecting endochondral bone growth, including the craniofacial anatomy. Recurrent otitis media infections, chronic middle ear effusion, and hearing loss are common in children with achondroplasia, but few studies have investigated hearing loss in adults with this condition. OBJECTIVES: This population-based study investigated the prevalence, severity, and type of hearing loss in Norwegian adults with achondroplasia. METHODS: We collected data on
