Extracorporeal Cytokine Adsorption in Acute Cardiovascular Care: Pathophysiological Insights and Clinical Perspectives.

Background: Cardiogenic shock (CS) and post-cardiac arrest syndrome (PCAS) are frequently associated with a systemic inflammatory response resulting from ischemia-reperfusion injury, endothelial dysfunction, and microcirculatory impairment. This inflammatory biology may be further amplified by temporary mechanical circulatory support (tMCS) through blood-surface interactions and shear-related hemolysis. Extracorporeal cytokine adsorption has therefore been proposed as an adjunctive strategy to attenuate hyperinflammation and facilitate shock reversal in selected patients. Methods: We conducted

Polysaccharide-based microneedles loaded with botanical formulation for dual-pathway neuroimmune modulation in chemotherapy-induced nausea and vomiting.

Chemotherapy-induced nausea and vomiting (CINV) remain a major clinical challenge arising from neuroimmune dysregulation and limited efficacy of conventional antiemetics subjected to hepatic metabolism. Herein, we designed a carrier-free microneedle system (DZS@BMNs) by integrating Bletilla striata polysaccharide (BSP) supramolecular matrices with the botanical formulation Dai-Zhe-Shi-San (DZS) to achieve dual-pathway neuroimmune modulation. The triple-helical structure of BSP forms stable hydrogen-bonded complexes with DZS, serving simultaneously as a structural scaffold and a functional biop

Autoimmune Hemolytic Anemia Complicated by Portal Vein Thrombosis in a Pediatric Liver Transplant Recipient: A Case Report.

Post-transplant autoimmune hemolytic anemia (AIHA) occurs in approximately 1% to 3% of pediatric liver transplant recipients. Its co-occurrence with portal vein thrombosis (PVT) is exceedingly rare and potentially life-threatening. We report a 10-month-old female who underwent living-donor liver transplantation for biliary atresia and presented with AIHA 5 months later. Laboratory evaluation revealed severe hemolytic anemia (hemoglobin 7.5 g/dL), markedly elevated lactate dehydrogenase (1751 IU/L), hyperbilirubinemia (total bilirubin 4.92 mg/dL), thrombocytopenia (75 × 103/μL), prolonged PT/IN

Accidental Administration of Anti-D Immunoglobulin to an Rh(D)-Positive Neonate: A Case Report.

Anti-D immunoglobulin (RhIg) is administered to Rh(D)-negative mothers to prevent alloimmunization in Rh(D)-incompatible pregnancies, and is contraindicated in Rh(D)-positive individuals. Herein, we report a case of accidental intramuscular administration of RhIg to an Rh(D)-positive neonate. A female infant was born at 39 weeks of gestation with a birth weight of 2,432 g. Shortly after birth, one vial of RhIg (250 μg), which should have been administered to the mother, was mistakenly injected into the neonate. The infant was transferred to our neonatal intensive care unit at one day of age fo

Engineering lipopeptide-gramicidin S conjugates to combat multidrug-resistant Acinetobacter baumannii and other ESKAPE pathogens.

Gramicidin S (GS) is a cyclic decapeptide that primarily targets Gram-positive bacteria and due to its hemolytic toxicity, is mainly used for the topical treatment of eye, ear, and throat infections caused by S. aureus. Due to its unique mechanism of action, no clinically significant resistance to GS has been reported to date. With the aim of expanding the therapeutic scope of GS to Gram-negative bacteria, we developed novel GS analogues by conjugating with cationic amino acid residues and lipopeptides on the ornithine (Orn) side-chain. A total of 14 peptides were synthesized and assessed for

Vasoplegia in Cardiac Surgery and Mechanical Circulatory Support: From Cardiopulmonary Bypass to Advanced Circulatory Support Devices.

Vasoplegia remains one of the most challenging and incompletely understood complications across the spectrum of mechanical circulatory support (MCS). Initially described following cardiopulmonary bypass, it is increasingly recognized in patients supported with veno-arterial extracorporeal membrane oxygenation (V-A ECMO) and combined unloading strategies such as ECPELLA (ECMO and Impella support) as well as in patients bridged to heart transplantation with temporary or durable mechanical circulatory support (MCS). Despite occurring in different clinical settings, these syndromes share common pa

A Dual-Functioning Embolic Microsphere Platform for Combined Antiangiogenic and Anti-Inflammatory Therapy for Rheumatoid Arthritis.

Rheumatoid arthritis (RA) is a chronic autoimmune disorder characterized by progressive synovial inflammation and joint destruction, with knee involvement prevalent in approximately 90% of patients. Current local therapies, such as intra-articular injections, often yield transient benefits owing to rapid drug clearance and failure to target pathological neovascularization, highlighting the need for innovative approaches. In this study, we developed a dual-functioning embolic microsphere platform, ETN-GMA/MS, which combines genicular artery embolization with the sustained local delivery of etan

Acute liver failure and secondary TMA-like phenomenon following SARS-CoV-2 and Mycoplasma pneumoniae co-detection in a child: a case report.

BACKGROUND: In the post-pandemic era, the co-detection of SARS-CoV-2 and Mycoplasma pneumoniae (M. pneumoniae) is increasingly recognized in children, but its full clinical spectrum remains poorly characterized. CASE REPORT: We describe a previously healthy 20-month-old girl who presented with sepsis, acute liver failure (ALT, 17,134 U/L), and extreme hyperinflammation (ferritin, 10,929.6 ng/mL; interleukin-6 [IL-6], 74 pg/ml) associated with SARS-CoV-2 and M. pneumoniae co-detection. Despite initial improvement in the liver function with glucocorticoid therapy, she subsequently developed a s

Systemic lupus erythematosus complicated by thrombotic microangiopathy with atypical HUS features: A case report.

RATIONALE: Systemic lupus erythematosus (SLE)-associated thrombotic microangiopathy with features of atypical hemolytic uremic syndrome (aHUS) is a rare but life-threatening complication. Early recognition is challenging because of its overlapping clinical manifestations with other thrombotic microangiopathies. PATIENT CONCERNS: A 49-year-old man with SLE presented with anemia, thrombocytopenia, and multi-system involvement. Despite initial immunosuppressive therapy, his hematologic parameters progressively deteriorated, with hemoglobin decreasing to 89 g/L and the platelet count to 54 × 109/

A multifunctional hydrogel dressing for wound healing and in vitro cytotoxicity against breast cancer cells: association with GRP78 expression.

Investigating molecular pathways associated with breast cancer progression is important for improving post-surgical therapeutic strategies. Since certain chemotherapeutic agents can activate the unfolded protein response (UPR), with glucose-regulated protein 78 (GRP78) acting as a key regulator of this pathway, this study aimed to develop a post-surgical hydrogel nanocomposite and evaluate its anticancer activity together with its potential effects on GRP78 localization. Chitosan nanoparticles (CsNPs) were synthesized and incorporated into a hydrogel matrix, and the resulting system was charac

Blood-Based α-Synuclein Biomarkers in Parkinson's Disease: Molecular Diversity, Analytical Advances and Clinical Translation.

Early and accurate diagnosis of Parkinson's disease (PD) remains challenging in routine practice, as cardinal motor symptoms emerge only after substantial neurodegeneration has occurred. Blood-based biomarkers are therefore of considerable interest for identifying prodromal disease, enabling biologically stratified clinical trials, and supporting longitudinal monitoring. Among candidate markers, α-synuclein (α-Syn) is centrally relevant due to its fundamental role in PD pathogenesis. However, its peripheral measurement is complicated by marked molecular heterogeneity, uneven compartmental dist

Construction of hepatic stellate cells-targeted nanosystem of verteporfin for the effective treatment of liver fibrosis through mechanical signal modulation.

The mechanical traction caused by high cross-linking extracellular matrix (ECM) can promote the continuous activation of hepatic stellate cells (HSCs) by yes-associated protein (YAP)-mediated mechanical signal. Therefore, targeted inhibiting the activation of HSCs induced by mechanical traction is the main measure to improve liver fibrosis. The activated HSCs (aHSCs)-targeted drug delivery nanosystem of verteporfin@polyethyleneimine-vitamin A (VP@PSA), which loaded VP a small molecule inhibitor of YAP in the Hippo signaling pathway to regulate mechanical signal transduction was constructed. Multifunctional natural polysaccharide peach gum-encapsulated selenium quantum dot nanocrystals monitor and scavenge reactive oxygen species in living cells.

Reactive oxygen species act as essential mediators in physiological processes, yet excess reactive oxygen species cause cellular damage and inflammation. Current strategies for reactive oxygen species monitoring and intervention remain limited in clinical practice. Here, we report selenium quantum dots (SeQDs) coated with peach gum (PG@SeQDs), a fluorescent nanocomposite for simultaneous reactive oxygen species detection and scavenging. SeQDs serve as fluorescent emitters and reactive oxygen species-active centers encapsulated by peach gum as a stabilizing matrix. Transmission electron microsc

More than malaria: Exploring Babesia, Leishmania and Trypanosoma in sickle cell disease.

Vector-borne parasites, such as species of Babesia, Trypanosoma, and Leishmania, are emerging and re-emerging causes of infectious diseases that contribute substantially to global morbidity and mortality. Epidemiological shifts, such as expansion of arthropod vector ranges, increased international travel, and transfusion-associated transmission, have amplified their public health impact. A key distinction among these parasites is their localization within the host. Babesia spp. infect erythrocytes, Leishmania spp. reside within macrophages, and Trypanosoma cruzi invades muscle cells, whereas T

A lethal combination: fulminant liver failure due to Babesia microti and Borrelia burgdorferi co-infection.

Babesiosis and Lyme disease are tick-borne infections endemic to the northeastern and upper Midwestern United States. Although both diseases can involve multiple organ systems, acute liver failure (ALF) is exceedingly uncommon. Co-infection with Babesia microti and Borrelia burgdorferi may exacerbate disease severity, create diagnostic uncertainty, and delay appropriate therapy. We describe a 77-year-old man with type II diabetes mellitus, stage IV chronic kidney disease, and heart failure with preserved ejection fraction who presented with dyspnea, cough, and peripheral edema. A biomimetic urchin-like photothermal nanoplatform for the efficient eradication of Streptococcus mutans and biofilm dispersion.

BACKGROUND: Dental caries, primarily driven by Streptococcus mutans (S. mutans) biofilms, remains a formidable clinical challenge due to the protective extracellular polymeric substance (EPS) matrix and the limited efficacy of conventional antibiotics. METHODS: To address this, we report a biomimetic photothermal nanoplatform (FWA NPs) featuring a hierarchical, urchin-like multi-spiked architecture, synthesized via the co-assembly of ferrocene-tryptophan conjugates and in situ biomineralized gold nanoparticles. RESULTS: Benefiting from this unique topological structure, FWA NPs maximize inte

Virulence Phenotypes Differentiate Persistent vs. Resolving Isolates of Human Staphylococcus aureus Bacteremia.

BACKGROUND: Staphylococcus aureus bacteremia (SAB) is a common and life-threatening bloodstream infection often caused by methicillin-resistant SA (MRSA) isolates. Up to 35% of SAB patients fail to clear infection with gold-standard anti-MRSA antibiotics, even if the isolate meets susceptibility breakpoints in conventional assays in vitro. Such outcomes are termed persistent and may involve small colony variant (SCV) adaptation of SA in vivo. METHODS: In this study, we assessed virulence phenotypes and mechanisms in persistent (PB) vs. resolving (RB) MRSA isolates from SAB. RESULTS: Overall,
