Fabry disease mimicking hypertrophic cardiomyopathy: genetic screening needed for establishing the diagnosis in women.

AIMS: Fabry disease, an X-linked storage disorder caused by defective lysosomal enzyme alpha-galactosidase A activity, may resemble sarcomere-gene-associated hypertrophic cardiomyopathy (HCM). The 'cardiac variant' of Fabry disease which only affects the heart may be missed unless specifically tested for. METHODS AND RESULTS: We evaluated 90 consecutively recruited HCM probands and their relatives. Probands without sarcomere-gene mutations were tested for alpha-galactosidase A gene (GLA) mutations. Of the 90 families, 31 (34%) had sarcomere gene mutations and were therefore excluded. In the r

Determinants of tissue Doppler measures of regional diastolic function during dobutamine stress echocardiography.

BACKGROUND: Diastolic dysfunction induced by ischemia may alter transmitral blood flow, but this reflects global ventricular function, and pseudonormalization may occur with increased preload. Tissue Doppler may assess regional diastolic function and is relatively load-independent, but limited data exist regarding its application to stress testing. We sought to examine the stress response of regional diastolic parameters to dobutamine echocardiography (DbE). METHODS: Sixty-three patients underwent study with DbE: 20 with low probability of coronary artery disease (CAD) and 43 with CAD who und

ROS- and iron ion-responsive gelatin modulates the pathological microenvironment to enhance myocardial infarction treatment.

Myocardial infarction (MI) generates a pathological microenvironment characterized by ischemia and hypoxia, and this increases mortality and morbidity worldwide. We fabricated p-hydroxyphenylpropionic acid-modified gelatin (GTN), a dual-responsive system capable of in situ gelation in the presence of hydrogen peroxide (H2O2) or iron ions. This feature enables GTN to cross-link in the infarcted myocardium, where excessive reactive oxygen species (ROS) and iron ions trigger ferroptosis. The successful modification of gelatin was verified, along with its time- and concentration-dependent responsi

Quantitative imaging analysis in hypertrophic cardiomyopathy: phenotypic differentiation to prognostic stratification.

Hypertrophic cardiomyopathy (HCM) is the most common inherited cardiac disorder and remains associated with sudden cardiac death (SCD), heart failure, and diagnostic uncertainty due to overlap with other causes of left ventricular hypertrophy, including hypertensive heart disease and cardiac amyloidosis. This review aimed to evaluate the evolving applications of radiomics in the context of related quantitative imaging techniques. A review was conducted of studies applying radiomics, texture analysis, machine learning, and deep learning to cardiac imaging in HCM. The evidence base derives predo

Prevalence and Most Commonly Encountered QRS Fragmentation Morphology and Early Repolarization ST-Segment Changes During the Height of the COVID-19 Pandemic: A Single-Center Experience.

BACKGROUND: SARS-CoV-2 infection has been shown to cause instability of myocardial cell membranes through different mechanisms. This instability can manifest as fragmented QRS (fQRS) complexes and early repolarization (ER) on an electrocardiogram (ECG). Since ECG abnormalities during this pandemic have not been previously reported, we investigated the prevalence of fQRS and ER during the height of the COVID-19 pandemic at our institution involving two hospitals. METHODS: A total of 1,476 ECG tracings obtained between December 2020 and March 2021 at University Health-Truman Medical Centers wer

Wire Injury-Induced Moderate Aortic Valve Stenosis in Mice Is Accompanied by a Chronic Systemic Inflammatory Reaction.

BACKGROUND/OBJECTIVES: While the presence of inflammatory processes in stenotic aortic valves is acknowledged, no systematic characterization of the systemic immune reaction upon aortic valve stenosis (AS) has been performed yet. The hypothesis of this study was that AS induces a systemic inflammatory reaction linked with local processes in the heart. METHODS: Murine wire injury (WI) to induce AS, or sham surgery, were performed prior to the 4-week assessment of AS severity, left ventricular (LV) function and hypertrophy with echocardiography (echo). Organ weights, levels of leukocytes, cytok

Cardiovascular Complications in ADPKD.

Autosomal dominant polycystic kidney disease (ADPKD), primarily caused by mutations in the PKD1 or PKD2 gene, is among the most common hereditary kidney diseases worldwide and is associated with significant extrarenal manifestations, including cardiovascular disease. In patients with ADPKD, cardiovascular disease is the major cause of mortality and is associated with a high burden of comorbidities. Cardiovascular manifestations include hypertension, which may lead to left ventricular hypertrophy (LVH) and diastolic dysfunction, as well as valvular abnormalities, aortic aneurysm, and pericardia

Amyloid cardiomyopathy presenting with gastrointestinal symptoms of abdominal pain, back pain, and constipation: A case report.

RATIONALE: Amyloid cardiomyopathy (AC) is a rare and fatal condition, often with a delayed diagnosis due to its nonspecific initial presentations. This case underscores the diagnostic challenge and critical importance of early recognition when patients present with seemingly unrelated gastrointestinal symptoms. PATIENT CONCERNS: A 48-year-old woman initially presented with abdominal pain, back pain, and constipation, which were managed as gastrointestinal disorders. Her condition progressed rapidly to include severe cardiac symptoms, including chest tightness and dyspnea. DIAGNOSES: Cardiac

Etelcalcetide: What we know eight years since its approval.

INTRODUCTION AND OBJECTIVES: The impact of etelcalcetide on patients with chronic kidney disease (CKD) and secondary hyperparathyroidism (SHPT) has been studied since its introduction in 2016/2017. However, only a handful of studies reported clinically relevant outcomes. This narrative review aims to summarize the published data about etelcalcetide, focusing on biochemical, cardiovascular (CV) and bone endpoints, as well as adverse effects and all-cause mortality. MATERIALS AND METHODS: A literature review of the use of etelcalcetide in hemodialysis patients with SHPT was conducted. Several s

Exploring the Interplay Between Kidney Dysfunction and Cardiovascular Disease.

This article reveals the various types of complications that are associated with dialysis and kidney-associated disease, including left ventricular hypertrophy, heart failure, vascular heart disease, arrhythmias, diabetes mellitus, intradialytic hypertension, and coronary heart disease. The molecular mechanisms underlying the development of cardiovascular disease in patients with chronic kidney disease (CKD), including the role of nitric oxide (NO) signaling, have been extensively studied. Patients suffering from CKD need treatment with hemodialysis at the end stages. The kidney is considered

Pathological capillary analysis of ischemia with non-obstructive coronary arteries in a Fabry disease patient receiving enzyme replacement therapy.

Fabry disease is a lysosomal disorder caused by an α-galactosidase A deficiency, which often causes angina in the absence of epicardial coronary artery stenosis. It is conceivable that epicardial and microvascular coronary dysfunction due to globotriaosylceramide and related glycoshingolipid accumulation within the vasculature causes ischemia with non-obstructive coronary arteries (INOCA). However, the precise histology remained unknown. We present the case of a 44-year-old male diagnosed with Fabry disease and treated with enzyme replacement therapy (ERT), who was referred to our cardiology d

Epidemiological Changes in Transthyretin Cardiac Amyloidosis: Evidence from In Vivo Data and Autoptic Series.

Cardiac amyloidosis is an infiltrative disease that causes progressive myocardial impairment secondary to amyloid fibril deposition in the extracellular space of the myocardium. Many amyloid precursors, including transthyretin protein, are known to determine cardiac damage by aggregating and precipitating in cardiac tissue. Transthyretin cardiac amyloidosis may be either caused by rare genetic mutations of the transthyretin gene in the hereditary variant, or may arise as a consequence of age-related mechanisms in the acquired form. Although it has been labeled as a rare disease, in recent year

Cardiac MRI in diagnosis, prognosis, and follow-up of hypertrophic cardiomyopathy in children: current perspectives.

Hypertrophic Cardiomyopathy (HCM) is an inherited myocardial disease characterised by left ventricular hypertrophy, which carries an increased risk of life-threatening arrhythmias and sudden cardiac death. The age of presentation and the underlying aetiology have a significant impact on the prognosis and quality of life of children with HCM, as childhood-onset HCM is associated with high mortality risk and poor long-term outcomes. Accurate cardiac assessment and identification of the HCM phenotype are therefore crucial to determine the diagnosis, prognostic stratification, and follow-up. Cardi

Management of dialysis access in the post-transplantation patient.

Arteriovenous fistula (AVF) is the preferred access for hemodialysis in patients with end-stage renal disease, and arteriovenous grafts are used when AVF creation is not feasible. Post renal transplantation, hemodialysis may be needed due to delayed graft function or transplantation failure. This review aimed to summarize current evidence on the impact of maintaining versus ligating AV access on renal function and cardiovascular outcomes post transplantation. A comprehensive review of literature was conducted, analyzing studies on the effects of AVF and AV graft maintenance or ligation on rena

Molecular detection of a novel mutation in the TPO gene associated with congenital hypothyroidism in a cat: Case report.

OBJECTIVE: The objective of this study was to analyze the sequence of different fragments of the thyroperoxidase (TPO) gene from a cat diagnosed with congenital hypothyroidism (CH). MATERIALS AND METHODS: The feline was diagnosed due to high serum concentrations of thyroid-stimulating hormone and low T4. The analysis of sequences containing mutations in the TPO gene from dogs with CH allowed for the prediction of mutation sites within the gene in an affected cat. In addition, the design of a polymerase chain reaction-based test allowed the amplification and sequencing of these gene segments.

Pathophysiology of the right ventricle in health and disease: an update.

The contribution of the right ventricle (RV) to cardiac output is negligible in normal resting conditions when pressures in the pulmonary circulation are low. However, the RV becomes relevant in healthy subjects during exercise and definitely so in patients with increased pulmonary artery pressures both at rest and during exercise. The adaptation of RV function to loading rests basically on an increased contractility. This is assessed by RV end-systolic elastance (Ees) to match afterload assessed by arterial elastance (Ea). The system has reserve as the Ees/Ea ratio or its imaging surrogate ej

Sudden cardiac death in ischaemic heart disease: coronary thrombosis or myocardial fibrosis?

The mechanisms underlying sudden cardiac death (SCD) in patients with ischaemic heart disease (IHD) caused by coronary atherosclerosis are not yet clarified. For decades, acute coronary causes have been sought as the main triggers of SCD in these patients. In fact, angiographic and pathological studies in cardiac arrest survivors and SCD victims, respectively, consistently show that acute plaque events occur in ∼50% of SCD of patients with IHD. Among the acute events, plaque rupture and erosion triggering acute coronary thrombosis remain the main substrates; however, a significant percentage o
