Delayed cord clamping in South African neonates with expected low birthweight: a randomised controlled trial.

OBJECTIVE: To evaluate safety and haematological effects of delayed cord clamping (DCC) in infants with expected low birthweight born in a resource-poor setting. METHODS: Randomised controlled trial involving pregnant women in early labour ≥18 years with intrapartum symphysal-fundal height ≤32 cm. Mothers were randomised for either early cord clamping (ECC, <30 s) or DCC (2-3 min after birth). RESULTS: We included 104 vigorous infants born by vaginal delivery, of whom 39% had a birthweight <2500 g. Infant haemoglobin (Hb) levels 24 h after birth were significantly higher in the DCC group (18

Optimising growth in very preterm infants: reviewing the evidence.

Infants born before 32 weeks' postmenstrual age are at a high risk of growth failure. International guidelines have long recommended that they match the growth of an equivalent fetus, despite the challenges posed by ex utero life and comorbidities of prematurity. Several groups have recently questioned the necessity or desirability of this target, shifting attention to aiming for growth which optimises important long-term outcomes. Specifically, recent research has identified the neurodevelopmental benefits of enhanced growth during the neonatal period, but work in term infant suggests that ra

Chorioamnionitis: a risk factor for fetal and neonatal morbidity.

Despite widespread use of drugs to arrest preterm labor, there has been no decrease in the numbers of low-birth-weight or preterm infants in the last 20 years. Evidence from many sources links preterm birth to symptomatic and subclinical infections. Recently, an increasing body of evidence has suggested that not only is subclinical infection responsible for preterm birth but also for many serious neonatal sequelae, including periventricular leukomalacia, cerebral palsy, respiratory distress and even bronchopulmonary dysplasia and necrotizing enterocolitis. Proxies of intrauterine infection inc

Narcotic addiction, pregnancy, and the newborn.

Between 1954 and 1973, 101 heroin-addicted mothers gave birth to 149 babies at Vancouver General Hospital. Thirty-seven percent of the infants had low birth weights and two thirds were born preterm. Average birth weight was 2,710 gm as compared with an overall average of 3,420 gm for this hospital. Tobacco and alcohol abuse, and poor maternal nutrition probably contributed to the growth retardation. Withdrawal symptoms were observed in 68% of the babies, and this may have been aggravated by multiple drug use, which was prevalent, including alcohol, barbiturates, and "soft drugs." Neonatal mort

Prenatal depression comorbid with diabetes or hypertension and the risk of adverse neonatal outcomes: A systematic review and meta-analysis.

PURPOSE: This systematic review and meta-analysis aim to synthesise evidence on the association between prenatal depression comorbid with diabetes and/or hypertension and adverse birth outcomes. METHODS: A comprehensive literature search was conducted across PubMed, Medline, Scopus, Web of Science, Embase, PsycINFO, and Google Scholar. Eligible studies assessed the effect of comorbid prenatal depression and diabetes or hypertension on obstetric or neonatal outcomes. Methodological quality was appraised using the Joanna Briggs Institute tool. Random-effects meta-analyses were conducted using i

Association of air purifier usage during pregnancy with adverse birth outcomes: the Japan Environment and Children's Study.

OBJECTIVE: Previous studies have reported that ambient air pollutants such as PM2.5 can increase the risk of adverse birth outcomes. The objective of this study was to ascertain whether air purifier usage during pregnancy is associated with a lower risk of adverse birth outcomes in a large Japanese birth cohort. METHODS: We conducted a prospective cohort analysis using data from the Japan Environment and Children's Study. Use of air purifiers during pregnancy was assessed using a self-administered questionnaire. Primary outcomes were the prevalence of preterm birth (PTB), small for gestationa

Parity and the risks of adverse birth outcomes: a retrospective study among Chinese.

BACKGROUND: Nulliparity is considered to be a risk factor of preterm birth (PTB), low birth weight (LBW) and small for gestational age (SGA). With the new two-child policy launched in 2016, more Chinese women have delivered their 2nd baby. Yet few studies have assessed the impact of parity on adverse birth outcomes in China. This study aimed to examine the association between parity and risks of PTB, LBW and SGA in a Chinese population. The combined effects of maternal age and parity on adverse birth outcomes were also assessed. METHODS: This retrospective study included all non-malformed liv

Zinc supplementation for improving pregnancy and infant outcome.

BACKGROUND: It has been suggested that low serum zinc levels may be associated with suboptimal outcomes of pregnancy, such as prolonged labour, atonic postpartum haemorrhage, pregnancy-induced hypertension, preterm labour and post-term pregnancies, although these associations have not yet been established. This is an update of a review first published in 1997 and subsequently updated in 2007, 2012 and 2015. OBJECTIVES: 1. To compare the effects on maternal, fetal, neonatal and infant outcomes in healthy pregnant women receiving zinc supplementation versus no zinc supplementation, or placebo.

Assisted Reproductive Technology Surveillance - United States, 2017.

PROBLEM/CONDITION: Since the first U.S. infant conceived with assisted reproductive technology (ART) was born in 1981, both the use of ART and the number of fertility clinics providing ART services have increased steadily in the United States. ART includes fertility treatments in which eggs or embryos are handled in the laboratory (i.e., in vitro fertilization [IVF] and related procedures). Although the majority of infants conceived through ART are singletons, women who undergo ART procedures are more likely than women who conceive naturally to have multiple-birth infants because multiple embr

Analysis of Randomised Trials Including Multiple Births When Birth Size Is Informative.

BACKGROUND: Informative birth size occurs when the average outcome depends on the number of infants per birth. Although analysis methods have been proposed for handling informative birth size, their performance is not well understood. Our aim was to evaluate the performance of these methods and to provide recommendations for their application in randomised trials including infants from single and multiple births. METHODS: Three generalised estimating equation (GEE) approaches were considered for estimating the effect of treatment on a continuous or binary outcome: cluster weighted GEEs, which

Prevention of periviable birth: bed rest, tocolysis, progesterone, and cerclage.

The periviable period is associated with a rapid improvement in both mortality and morbidity with increasing gestational age. Therapies that can prolong gestation have the potential for markedly improving outcome compared to later in pregnancy. The therapies commonly prescribed in order to prolong gestation include bed rest, tocolysis, progesterone, and cerclage. Although these therapies are used to prevent preterm birth throughout gestation, their impact on perinatal outcome during the periviable period remains largely unknown. These therapies are discussed relative to the periviable period e

Low birthweight breech infant: short-term and long-term outcome by method of delivery.

The effect of the delivery method on the short-term and long-term morbidity and mortality of the very low birthweight breech-presenting infants was evaluated. Although some previous studies question the benefit of cesarean section for the premature breech infants, the mortality rate and the incidence of birth injuries were significantly lower in the abdominally delivered group than in those delivered vaginally. The long-term follow-up clearly demonstrates that the vaginally delivered infants had a substantially higher incidence of cerebral palsy, visual damage, deafness, and severe development

Ultrasound-guided neuronavigation improves safety of ventricular catheter insertion in preterm infants.

BACKGROUND: Intra-ventricular hemorrhage (IVH) is a frequent cause of shunt-dependent hydrocephalus. The management of IVH in preterm babies remains a challenge both for neonatologists and pediatric neurosurgeons, compounded by the lack of low-risk, validated therapy techniques. OBJECTIVE: The aim of this study was to evaluate the feasibility and safety of a novel technique involving the ultrasound-guided placement of a central catheter connected with a Rickham-Capsule in a cohort of preterm, low-birth-weight babies with post-hemorrhagic hydrocephalus (PHH). METHODS: Eight preterm infants wi

A clinical follow-up study of 260 women with hypertension in pregnancy.

In 1969-73, 260 women had pre-eclampsia or hypertension in pregnancy. 237 were examined 7-12 years later. 172 were found to be normotensive, 24 were classified as having borderline hypertension while 62 had hypertension (antihypertensive treatment or BP greater than or equal to 160/100 mm Hg). Late hypertension was more often found after severe pre-eclampsia and gestational hypertension than after mild pre-eclampsia. Hypertension preceding pregnancy was more common with severe pre-eclampsia. A family history of hypertension and repeated hypertensive pregnancies seem to predispose for later hyp

Total parenteral nutrition in low-birth-weight neonates with complicated surgical disorders; effects and difficulties.

Twenty low-birth-weight, surgical neonates were given total parenteral nutrition (TPN). They were divided into two groups according to birth-weights. Group A consisted of 8 patients weighing less than 2,000 gm, and group B consisted of 12 patients weight between 2,000 and 2,500 gm. The infusate of TPN consisted of 15% glucose and 3% amino acids solution, and 10% fat emulsion. The dosage administered was usually 120 ml/kg/day in the amino acid glucose solution and 15 ml/kg/day in the fat emulsion, but varied depending on the clinical condition of the patients. The duration of parenteral nutriti

The association of maternal sedentary behaviour before and during pregnancy and offspring health outcomes: A systematic review and meta-analysis.

BACKGROUND: Sedentary behaviour is prevalent and associated with adverse maternal outcomes. However, conflicting results exist regarding the relationship between maternal sedentary behaviour before or during pregnancy and the consequences for offspring. AIM: The present systematic review and meta-analysis aimed to quantitatively and qualitatively synthesise the existing evidence on the associations of maternal sedentary behaviour before or during pregnancy with offspring health outcomes. METHODS: Studies were obtained by searching relevant literature using seven databases from inception to F
