Neonatal skin care.

The relative importance of neonatal health and neonatal skin care has been highlighted in recent years as infant mortality rates have decreased while death rates during the neonatal period remain unacceptably high in many areas of the world. During the neonatal period, many newborns develop preventable, clinically apparent skin problems, and many more, especially preterm neonates, experience morbidity caused by compromised skin barrier integrity. Several strategies are available for protecting the integrity and promoting the hygiene of the skin and augmenting its function as a barrier to TEWL

Survival and Short-Term Morbidities Among Extremely Preterm Infants: A Prospective Cohort Study.

BACKGROUND: Outcomes of extremely preterm (EPT) infants in low- and middle-income countries (LMIC) remain heterogeneous, with limited gestational age (GA)-defined data from India. OBJECTIVE: To evaluate survival, short-term morbidities, and predictors of mortality among infants born ≤28 weeks' gestation in a North Indian tertiary neonatal intensive care unit (NICU). METHODS: This prospective observational cohort included all inborn infants ≤28 weeks admitted between November 2020 and October 2021, excluding those with major anomalies. Maternal, perinatal, and neonatal data were collected pro

Expert review of CLABSI prevention in the NICU: supporting the transition of investigational drugs into clinical practice.

UNLABELLED: Central venous catheters (CVCs) are essential for administering life-saving medications, parenteral nutrition, and fluids in extremely premature (EP; i.e., 28 weeks' gestational age) infants. Indeed, CVCs have enabled increased survival and improved outcomes in EP infants over the last several decades. However, CVCs remain a major risk factor for central line-associated bloodstream infection (CLABSI), which can lead to serious complications in this vulnerable population. While many neonatal intensive care units (NICUs) have adopted CVC bundles to reduce CLABSI risk, implementation

Effectiveness of Non-Nutritive Sucking on Sucking Performance in Preterm Infants: A Systematic Review.

INTRODUCTION: Non-nutritive sucking (NNS) is commonly employed to assist in the development of preterm infants facing feeding challenges. The effectiveness of NNS interventions on sucking performance (suction and expression/compression component of sucking) in this population has not yet to be systematically explored. AIMS: To review the literature on the effects of NNS on sucking performance in preterm infants. METHODS: A comprehensive search was conducted across five databases, encompassing publications from the inception of these databases until July 2024. The studies included only random

Early Skin-to-Skin Contact Does Not Affect Cerebral Tissue Oxygenation in Preterm Infants <32 Weeks of Gestation.

AIM: It was the aim of our study to determine the regional cerebral tissue oxygenation saturation (rcSO2) as an additional monitoring parameter during early skin-to-skin contact (SSC) in preterm infants with a gestational age of <32 gestational weeks. METHODS: We conducted two observational convenience sample studies using additional monitoring with near-infrared spectroscopy (NIRS) in the first 120 h of life: (a) NIRS 1 (gestational age of 26 0/7 to 31 6/7 weeks) and (b) NIRS 2 (gestational age of 24 0/7 to 28 6/7 weeks). The rcSO2 values were compared between resting time in the incubator (

Development of a universal psycho-educational intervention to prevent common postpartum mental disorders in primiparous women: a multiple method approach.

BACKGROUND: Prevention of postnatal mental disorders in women is an important component of comprehensive health service delivery because of the substantial potential benefits for population health. However, diverse approaches to prevention of postnatal depression have had limited success, possibly because anxiety and adjustment disorders are also problematic, mental health problems are multifactorially determined, and because relationships amongst psychosocial risk factors are complex and difficult to modify. The aim of this paper is to describe the development of a novel psycho-educational in

Epidemiologic impact and cost-effectiveness of universal infant vaccination with a 7-valent conjugated pneumococcal vaccine in the Netherlands.

BACKGROUND: Streptococcus pneumoniae is one of the main causes of bacterial meningitis, bacteremia, pneumonia, and otitis media in the Netherlands. These diseases lead to substantial mortality, morbidity, and costs. The societal impact is especially severe because most cases occur in very young infants. OBJECTIVE: The aim of this study was to estimate the epidemiological impact and cost-effectiveness of universal infant vaccination with a 7-valent conjugated pneumococcal vaccine in the Netherlands. METHODS: Decision analysis was performed using epidemiological data and data on health care re

Brief Report: Improving Perinatal HIV Care During the COVID-19 Pandemic: Implementing a Mobile Integrated Health Program to Close the Gap.

BACKGROUND: Pregnant people with HIV (PWH) often experience loss to follow-up and viral nonsuppression after delivery, contributing to morbidity and HIV transmission. The COVID-19 pandemic disrupted health systems and exacerbated health disparities, including for PWH and their infants. To improve perinatal HIV outcomes, we implemented a perinatal care program in September 2020 that offered multidisciplinary home visits through a mobile integrated health (MIH) unit within a large, safety-net health care system in Atlanta, GA. METHODS: We analyzed data collected from PWH who delivered 1 year be

Harnessing a neonatal encephalopathy registry for regional quality improvement efforts.

OBJECTIVE: Improve regional clinical care for infants with neonatal encephalopathy presumed to be due to hypoxic-ischemic encephalopathy (HIE) by developing a clinical data registry. STUDY DESIGN: We developed a regional multicenter registry of newborns at risk for HIE and leveraged data collected within the registry to design and monitor biannually quality improvement metrics in a collaborative effort among site investigators. RESULT: Over the 3 years since beginning our improvement efforts, we have demonstrated positive trends in all three initial metrics decided on by our investigators: i

Determinants of survival in extremely preterm infants born at 22-23 weeks: a retrospective cohort study.

BACKGROUND: This study investigated the risk factors associated with mortality in infants born at 22-23 weeks and assessed the impact of different variables on survival outcomes. METHODS: This retrospective study included 185 infants born at 22-23 weeks at a single tertiary care center (2006-2023). Univariate and logistic regression analyses identified independent predictors of mortality. RESULTS: Among 185 infants, 64 (34.6%) did not survive. Mortality was associated with a lower birth weight (509 g vs. 564 g, p = 0.0001) and higher rates of small for gestational age (SGA; 21.9% vs. 7.4%, p

Inter-center variations in inpatient outcomes for infants with gastroschisis from the Children's Hospitals Neonatal Consortium.

OBJECTIVE: To estimate inter-center variation (ICV) in hospital length of stay (LOS) and oral feeding at discharge among infants with gastroschisis. STUDY DESIGN: The Children's Hospitals Neonatal Consortium's (CHNC) database was used to identify hospitalized survivors with gastroschisis. Two outcomes were evaluated: LOS and discharge without tube feedings. Hospital outcomes were evaluated in unadjusted and adjusted analyses. Generalized linear models with log link for gamma distribution were used to model LOS and logistic regression for discharge without tube feeds. RESULTS: For 3987 surviv

The role of the placenta in perinatal asphyxia, neonatal encephalopathy, and neurodevelopmental outcome: A review.

Perinatal asphyxia contributes significantly to neonatal deaths globally. This may occur due to the effects of various phenomena like uterine rupture, infections, maternal hemodynamic compromise (amniotic fluid embolus), placenta, and umbilical cord (umbilical cord knot, placental abruption, or compression). Perinatal asphyxia is the term used for interrupted blood flow or the exchange of gases in the fetus during the prenatal period. The reduced oxygenation induces a cascade of brain injuries, resulting in long-term damage to the infant's brain. Some infants exposed to perinatal hypoxia-ische

Management of Acute Kidney Injury in Extremely Low Birth Weight Infants.

Acute kidney injury (AKI) is a common problem in the neonatal intensive care unit (NICU). Neonates born at <1,000 g (extremely low birth weight, ELBW) are at an increased risk of secondary associated comorbidities such as intrauterine growth restriction, prematurity, volume restriction, ischaemic injury, among others. Studies estimate up to 50% ELBW infants experience at least one episode of AKI during their NICU stay. Although no curative treatment for AKI currently exists, recognition is vital to reduce potential ongoing injury and mitigate long-term consequences of AKI. However, the definit

A LC-MS/MS method for the quantification of caffeine, betamethasone, clonidine and furosemide in cerebrospinal fluid of preterm infants.

BACKGROUND: Newborns, admitted to the Neonatal Intensive Care Unit (NICU), are exposed to a large number of medications, the majority of which are not labeled for use in infants, especially in preterm newborns, because clinical trials on their benefits and harms are lacking. There is a huge gap in knowledge on pharmacokinetic (PK) data in sick preterm infants, including that of drug penetration to cerebrospinal fluid (CSF). One of the issues is related to the lack of reliable analytical methods for the measurement of drugs in CSF. METHODS: In this paper we describe a specific and sensitive LC

The optic nerve sheath hemorrhage is a non-specific finding in cases of suspected child abuse.

In young infants, the triad consisting of acute encephalopathy, retinal hemorrhages, and a subdural hematoma is a nonspecific finding. It has traumatic and non-traumatic etiologies. The triad may be found among a vast spectrum of natural diseases. Optic nerve sheath hemorrhage in infants is typically detected at autopsy. It is a nonspecific finding that can be found in traumatic and non-traumatic etiologies. Neither the triad nor the ONSH are pathognomonic for an abusive head injury. Opposite to the triad, the spectrum of non-traumatic etiologies of ONSH is limited. In infants ONSH rarely occu
